Objective:To investigate the changes of serum complement C1q/tumor necrosis factor-associated protein 3 (CTRP3) in patients with rheumatoid arthritis (RA) and its clinical significance.Methods:A total of 60 RA patients admitted to Chongming Hospital affiliated to Shanghai University of Medicine & Health Sciences (Chongming Branch of Xinhua Hospital) from January 2023 to February 2024 were collected.They were divided into a plaque group (38 cases) and a plaque free group (22 cases) according to the results of carotid intima-media thickness (cIMT) by carotid artery ultrasonography. ESR, CRP, blood lipid, HOMA-IR, RF, CCP antibody and CTRP3 levels were detected, and the relationship between CTRP3 levels and disease activity and atherosclerosis in RA patients was analyzed. The statistical analysis was carried out with independent t-test, analysis of variance, Pearson correlation analysis and logistic regression. Results:Serum CTRP3 level in RA patients was lower than that in healthy control group [(116±44)ng/ml and (184±63)ng/ml, t=-6.54, P=0.004]. The CTRP3 level in RA group with plaque was lower than that in RA group without plaque [(98±28) ng/ml and (123±38)ng/ml, t=-5.57, P=0.008]. Serum CTRP3 levels in RA patients were correlated with LDL-C ( r=-0.68, P=0.011), HOMA-IR ( r=-0.74, P=0.001), RF ( r=-0.46, P=0.042), anti-CCP antibody( r=-0.54, P=0.037), DAS28 ( r=-0.66, P=0.024) were negatively correlated with cIMT ( r=-0.76, P=0.001), and positively correlated with DMARDs duration ( r=0.51, P=0.040) and flow-mediated di latatiton ( r=0.70, P=0.004). The CTRP3 level [( OR(95% CI)=0.683(0.355, 0.807), P=0.023] was an independent correlation factor affecting cIMT. Conclusion:CTRP3 level in RA patients is significantly lower than that in healthy control group, and is negatively correlated with insulin resistance, autoantibody level and disease activity, and has a protective effect on early atherosclerosis in RA patients.

Objective:To observe the influence of long-term exercise training on carotid intima-media thickness (IMT)in patients with mild-to-moderate hypertension.Methods:A total of 92 patients with mild-to-moderate es-sential hypertension (EH)were randomly and equally divided into exercise group and routine treatment group.Exer-cise group received exercise training based on routine treatment.All patients were followed up one year.Influence of long-term exercise training on carotid artery diameter and carotid IMT was observed in EH patients.Results:Af-ter one year,there were significant reductions in systolic blood pressure (SBP),diastolic blood pressure (DBP),ca-rotid artery diameter and carotid IMT in both groups,P<0.05 all;compared with routine treatment group,there were significant reductions in SBP [(145.72±11.31)mmHg vs.(130.89±13.01)mmHg],DBP [(88.49±7.32) mmHg vs.(81.71±8.45)mmHg],carotid artery diameter [(6.34±1.23)mm vs.(6.22±1.01)mm]and carotid IMT [(0.89±0.21)mm vs.(0.84±0.11)mm]in exercise group after treatment,P<0.05 all.Conclusion:Long-term exercise training can effectively reverse early arteriosclerosis lesion of carotid wall and effectively control blood pressure.

Objective: To observe influence of long-term exercise training on carotid intima-media thickness (IMT) in patients with mild-moderate hypertension. Methods: A total of 92 patients with essential mild-moderate hypertension were randomly divided into routine treatment group (n=46) and exercise group (n=46, received exercise training based on routine treatment) according to number table. All patients were followed up for one year, and the blood pressure, carotid artery diameter and carotid IMT were measured, compared and analyzed in two groups before and after treatment. Results: There was no significant difference in systolic and diastolic blood pressure before treatment between two groups (P＞0.05). Compared with baseline value, there were significant decrease in systolic blood pressure [(176.66±l1.78)mmHg vs. (130.89±13.01)mmHg], diastolic blood pressure [(101.43±6.41)mmHg vs. (81.71±8.45)mmHg], carotid artery diameter [(6.62±0.97)mm vs. (6.22±1.01)mm] and carotid IMT [(0.98±0.12)mm vs. (0.84±0.11)mm] in exercise group after one-year training (P<0.05 all), and they were all significantly lower than those of routine treatment group (P<0.05 all ). Conclusion: Long-term exercise training can effectively control blood pressure, decrease carotid artery diameter and carotid intima-media thickness.

OBJECTIVE: To investigate the relationship between the prevalent species and the seasonal drift law of airborne fungi and respiratory allergic disease in the city of Wuhan. METHOD: Airborne fungi was investigated by exposed sides from 2007 to 2008, and 1674 patients with respiratory allergic disease that were used to do allergens skin test simultaneously, in order to analyze the simultaneous. RESULT: Airborne fungi could be detected in air all over the year. In 2007, the exposed films collected 26 734 fungi spores, and 686 other fungi (hypha and not well known fungi), to the sum of 27420. Otherwise, in 2008, the exposed films collected 26 531 fungi spores, and 730 other fungi, to the sum of 27 261. In the survey, 17 types of species of the collected fungi spores were identified, and the predominant species were alternaria sp, ustilaginales, deightoniella sp, uredinales, and the quantity of Fungi alternaria was most of all . And the peak period mainly concentrated from April to June, and from September to October. Otherwise, the positive rate of fungi skin test in patients with respiratory allergic disease was 10.48% in 2007, but 10.07% in 2008. The main period of onset of respiratory allergic disease was from April to June, and from September to October, similar as the seasonal drift law of airborne fungi. CONCLUSION The period of onset of respiratory allergic disease was consistent with the seasonal drift law of airborne fungi.

OBJECTIVE: To explore the effects of specific immunotherapy by the nasal spray and/or sublingual contain on the regulation the balance of serum Th1/Th2 cell ratio, and the expression of total IgE (tIgE) level in perennial allergic rhinitis patients. METHOD: Thirty-six cases of allergic rhinitis of perennial (PAR) patients with nasal specific immunotherapy (nasal spray group) were chosen as the object of study, 36 PAR patients with sublingual specific immunotherapy (sublingual group) were chosen for the efficacy comparison group, and 32 cases of healthy adults as control (normal control group). The levels of serum IL-2, IL-4, IL-8 and tIgE in PAR nasal spray group and sublingual group were examined by IRMA and double-antibody sandwich assay before and after the treatment of specific immunotherapy (SIT); The content of IFN-gamma was determined by enzyme-linked immunosorbent assay; The infiltration and changes of eosinophilic granulocyte were observed by the smear of nasal secretions. RESULT: In nasal spray group and sublingual group, the contents of serum Th1 cytokines IL-2 and IFN-gamma were reduced levels significantly, while the contents of Th2 cytokines IL-4, IL-8 and tIgE were significantly increased before the treatment of SIT. After the SIT to the maintenance dose, the levels of IL-2 and IFN-gamma in serum were significantly higher than that before treatment, while IL-4, IL-8 and tIgE content were significantly lower in AR patients (P < 0.01 or P < 0.05). The clinical efficacy of nasal spray group and sublingual group were 97.22% and 94.44%, while the two groups was no statistical difference in efficacy (P > 0.05). CONCLUSION With the presence of Th1/Th2 cell ratio and cytokine imbalance, the AR patients manifested as Th2 cell function in accentuation. Nasal mucosa and/or sublingual in the local SIT can have changed no-balance Th1/Th2 cell of by regulating the balance of serum Th1/Th2 cell ratio, the expression level of cytokine expression and the level of tIgE.
