Objective:To investigate the application value of sevoflurane combined with remifentanil in interventional thrombectomy of ischemic stroke patients and its influence on neurological and cognitive function.Methods:The clinical data of 84 patients with ischemic stroke treatment in the Xining Third People′s Hospital from September 2020 to September 2022 were selected retrospectively, and they were divided into the conventional group (42 cases) and experimental group (42 cases) according the treatment methods. Interventional thrombectomy was performed in both groups. Propofol combined with remifentanil was used in the conventional group, and sevoflurane combined with remifentanil was used in the experimental group. The excellent and good rate of anesthesia was compared between the two groups; the hemodynamic indexes before anesthesia induction (T 1), at the beginning of surgery (T 2), after thrombectomy and extubation (T 3) and at the end of surgery (T 4) were compared between the two groups; the brain injury indexes were compared before and 12, 24 and 48 h after surgery were compared between the two groups; the National Institutes of Health Stroke Scale (NIHSS) score, Simple Mental State Examination Scale (MMSE) score, postoperative cognitive dysfunction (POCD) and anesthesiation-related adverse reactions were compared between the two groups. Results:The excellent and good rate of anesthesia in the experimental group was higher than that in the conventional group:100.00% (42/42) vs. 83.33% (35/42), there was statistical difference ( χ2 = 5.61, P<0.05). The mean arterial pressure (MAP), heart rate (HR), and pulse oxygen saturation (SpO 2) in the two groups at T 2, T 3 and T 4 were higher than those at T 1 ( P<0.05); the MAP, HR and SpO 2 in the experimental group at T 2, T 3 and T 4 were lower than those in the conventional group ( P<0.05). The serum neuron-specific enolase (NSE) and S100β protein were significantly increased at 12 and 24 h after surgery in both groups ( P<0.05); serum NSE and S100β proteins in the experimental group at 12, 24 and 48 h were lower than those in the conventional group: (30.12 ± 4.26) μg/L vs. (35.64 ± 5.70) μg/L, (22.69 ± 4.01) μg/L vs. (26.81 ± 5.53) μg/L, (18.71 ± 2.92)μg/L vs. (21.34 ± 4.02) μg/L, (1.19 ± 0.20) μg/L vs. (1.37 ± 0.24) μg/L, (0.96 ± 0.13) μg/L vs. (1.13 ± 0.16) μg/L, (0.80 ± 0.11) μg/L vs. (0.94 ± 0.15) μg/L, there were statistical differences ( P<0.05). The NIHSS score was significantly higher and the MMSE score was significantly lower in both groups at 12 and 24 h after surgery ( P<0.05); the NIHSS score and MMSE score in both groups recovered to preoperative level at 48 h after the operation ( P>0.05). The NIHSS score in the experimental group at 12 and 24 h after surgery was lower than that in the conventional group, and the MMSE score was higher than that in the conventional group ( P<0.05). The incidence of anesthesia-related adverse reactions in the experimental groups was lower than that in the control group: 4.76%(2/42) vs. 21.43%(9/42); and the incidence of POCD was lower than that in the conventional group: 19.05%(8/42) vs. 2.38%(1/42), there were statistical differences ( χ2 = 5.13, 4.48, P<0.05). Conclusions:Sevoflurane combined with remifentanil has a significant anesthetic effect in interventional thrombectomy of ischemic stroke patients, which can stabilize hemodynamics, reduce the degree of brain injury, promote cognitive function recovery, and reduce POCD and adverse reactions.

Objective: To investigate the diagnostic value of immunoglobulin M (IgM) and immunoglobulin G(IgG) antibodies to 2019 Novel Coronavirus (2019-nCoV) in 2019-nCoV infection. Method: This is a retrospective study. Serum samples were collected from 284 patients including outpatients and inpatients in the Renmin Hospital of Wuhan University from January 20, 2020 to February 17, 2020. Among them 205 cases were 2019-nCoV infected patients, including 186 cases confirmed with nucleic acid test and 19 cases diagnosed by clinical symptoms and CT characteristics according to "the New Coronavirus Pneumonia Control Protocol (5th edition)" . A total of 79 subjects with other diseases but negative to 2019-nCoV infection were recruited as control group. Serum IgM and IgG antibodies to 2019-nCoV were measured with fully automated immunoassay technology for all subjects. Statistical significance between 2019-nCoV antibodies test and 2019-nCoV nucleic acid test was determined using the χ² tests. Result: The sensitivity of serum IgM and IgG antibodies to 2019-nCoV were 70.24%(144/205) and 96.10%(197/205) respectively and the specificity were 96.20%(76/79) and 92.41%(73/79) respectively. The positive and negative predictive values of 2019-nCoV antibodies were 95.63%(197/206) and 91.03% (71/78) respectively, and the positive and negative predictive values of 2019-nCoV nucleic acid test were 100%(186/186) and 80.61%(79/98) respectively. The total coincidence rate of diagnosing 2019-nCoV infection between antibody tests and nucleic acid test for 2019-nCoV were 88.03%(250/284). Conclusion Joint detection of serum IgM and IgG antibodies to 2019-nCoV is an effective screening and diagnostic indicators for 2019-nCoV infection, and an effective complement to the false negative results to nucleic acid test.

OBJECTIVE:To observe the effects of parecoxib preemptive analgesia combined with postoperative epidural analgesia on hip replacement and immune function. METHODS:Totally 86 patients undergoing unilateral hip replacement selected from Xining Third People's Hospital during Jan. 2014-Jun. 2016 were divided into observation group and control group according to random num-ber table,with 43 cases in each group. Two groups underwent unilateral hip replacement under spinal epidural anesthesia. Observation group was given Parecoxib sodium for injection 40 mg intravenously 30 min before the end of surgery. Control group was given con-stant volume of normal saline intravenously. Both groups were given epidural analgesia pump(Ropivacaine hydrochloride injection 150 mg+Ondansetron hydrochloride injection 16 mg+Dezocine injection 10 mg,added into normal saline to 100 mL)after surgery with dripping speed of 2 mL/h and patient-controlled time of 15 min. The hemodynamic indexes,analgesic effect,T lymphocyte subsets and NK cells levels were compared between 2 groups at different time points as well as the occurrence of ADR after surgery. RE-SULTS:Two patients of observation group and one patient of control group withdrew from the study. Before surgery,there was no sta-tistical significance in MAP and HR between 2 groups(P>0.05);12,24 h after surgery,MAP and HR of control group were signifi-cantly increased and higher than observation group,with statistical significance(P<0.05). There was no statistical significance in MAP and HR of observation group at different time points compared to before treatment;there was no statistical significance in MAP and HR between 2 groups immediately after surgery,48 h after surgery(P>0.05). There was no statistical significance in VAS score between 2 groups immediately after surgery(P>0.05);6,12,24,48 h after surgery,VAS scores of observation group were signifi-cantly lower than those of control group,with statistical significance(P<0.05). There was no statistical significance in CD3+,CD4+, CD4+/CD8+and NK cells levels between 2 groups before surgery(P>0.05). CD3+,CD4+and NK cells levels of 2 groups immediately after surgery,CD3+,CD4+,CD4+/CD8+ and NK cells levels of 2 groups at 12,24 h after surgery,CD3+and CD4+of control group at 48 h after surgery all changed significantly;CD3+,CD4+,CD4+/CD8+and NK cells levels of observation group at 12,24 h after sur-gery were significantly higher than those of control group,with statistical significantly(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:The parecoxib preemptive analgesia combined with postopera-tive epidural analgesia shows good analgesic effect for hip replacement patients,keeps their hemodynamics stable and protects immune function to certain extent with good safety.
