Objective To explore the influence of hsa_circ_0004535 on type 2 diabetes(T2DM)combined with metabolic-associated fatty liver disease(MAFLD)model mice. Methods Forty-eight healthy SPF grade Balb/c mice were selected for modeling and divided into the following groups(n=6 per group):Control group:normal feed;T2DM group:diabetes model induced by high-glucose and high-fat diet;T2DM combined MAFLD group:non-alcoholic fatty liver high-glucose and high-fat diet-induced diabetes combined with fatty liver model;T2DM combined MAFLD+hsa_circ_NC group:after 4 weeks of modeling,10 nmol hsa_circ_NC injected into the tail vein;T2DM combined MAFLD+hsa_circ_0004535 group:after 4 weeks of modeling,10 nmol circ_0004535 injected into the tail vein;T2DM combined MAFLD+miRNA_NC group:after 4 weeks of modeling,10 nmol miRNA blank control injected into the tail vein;T2DM combined MAFLD+miR-1827 agomir group:after 4 weeks of modeling,10 nmol miR-1827 agomir injected into the tail vein;and T2DM combined MAFLD+miR-1827 antagomir group:after 4 weeks of modeling,10 nmol miR-1827 antagomir injected into the tail vein.Mouse body weight was measured after the interventions and recorded weekly.Glucose and insulin tolerance tests were performed, blood lipids and liver function were measured, the liver and insulin resistance indexes were calculated, and pathological tests(hematoxylin/eosin(HE),oil red O,and Masson staining, immunohistochemistry, terminal deoxynucleotidyl transferase dUTP nick end labeling(TUNEL))were performed to measure the degree of hepatic inflammation, fat deposition, and fibrosis.Results(1)Body weight, liver weight, liver index, insulin resistance index, and biochemical indexes were all significantly lower in the hsa_circ_0004535 injection group compared with the hsa_circ_NC injection group and the T2DM combined MAFLD group(both P<0.05).(2)Steatosis vacuoles were reduced and smaller and inflammatory cell infiltration was reduced in the T2DM combined MAFLD+circ_0004535 group,as shown by HE and oil red staining.(3)TUNEL-positive cells were significantly reduced in the T2DM combined MAFLD+hsa_circ_0004535 group(P<0.05).(4)Collagen fiber deposition was significantly reduced in the T2DM combined MAFLD+hsa_circ_0004535 group,as shown by Masson staining(P<0.05).Conclusions The expression of hsa_circ_0004535 and miRNA-1827 play important roles in regulating lipid metabolism, insulin sensitivity, inflammatory pathways, hepatocyte apoptosis, and hepatic fibrosis-related pathways in an animal model of T2DM combined with MAFLD.

Objective:To deeply understand the psychological experiences and needs of ward nurses caring for elderly patients with postoperative delirium, providing insights for improving their care capabilities. Methods:A descriptive qualitative study was conducted using purposive sampling to select nurses from the surgical wards of the Second Affiliated Hospital of Zhejiang University School of Medicine who had cared for elderly patients with postoperative delirium between January and March 2024. Semi-structured interviews were conducted, and traditional content analysis was used to extract themes. Results:Sixteen surgical ward nurses, aged 26-48 years old and all female, were interviewed. Four themes and nine sub-themes were identified: internal stress in caring for elderly patients with postoperative delirium (concerns about failing to recognize changes in condition, leading to delayed treatment; physical and psychological stress overwhelming nursing work); external challenges in caring for elderly patients with postoperative delirium (uncontrollable patient behavior; lack of close collaboration between medical staff; lack of awareness among patient families about postoperative delirium); lack of knowledge about postoperative delirium (difficulty in identifying subtypes of postoperative delirium; lack of knowledge about postoperative delirium management); and a desire for systematic professional support (urgent need for operational guidelines on postoperative delirium management in elderly patients; expectation for the establishment of a multidisciplinary team for postoperative delirium management in elderly patients). Conclusions:Nursing managers should pay attention to the physical and mental health of ward nurses, provide multifaceted support, strengthen their knowledge about postoperative delirium in elderly patients, optimize postoperative delirium management, and improve their care capabilities.

To summarize the nursing experience of an elderly postoperative laryngeal cancer patient who developed a Type Ⅳ hiatal hernia.Key nursing care points included the monitoring of shock index and MEWS to promptly activate the rapid response team and implement the Surviving Sepsis Campaign, active fluid resuscitation and continuous monitoring of central venous pressure to prevent cardiopulmonary failure, implementing a personalized postoperative respiratory rehabilitation plan to promote lung re-expansion and prevent pulmonary infection, monitoring and controlling intra-abdominal hypertension, and implementing an intra-abdominal pressure-guided enteral nutrition management plan, employing a restrictive fluid management strategy to reduce tissue edema and maintain fluid and electrolyte balance, implementing an integrated"hospital-community-home"health management plan to improve the patient's quality of life. The patient was hospitalized for 23 days and discharged smoothly, with good recovery after a 3-month follow-up.

Objective To explore the relationship between forkhead box P3(Fox P3)gene polymorphism and susceptibility to gastric cancer in Chinese Han population. Methods From January 2014 to December 2016,a total of 312 patients with primary gastric cancer and 548 age and gender matched controls were recruited.Polymorphisms of Fox P3 at rs3761548 and rs2232365 loci were analyzed by polymerase chain reaction-restriction fragment length polymorphism techniques(PCR-RFLP).Odd ratio(OR)and 95% confidence interval(CI)were used to analyze the correlation between different genotype and susceptibility to gastric cancer, clinical pathological characteristics of patients with gastric cancer. Results The frequency of AA,AC,and CC genotype of rs3761548 in gastric cancer group and control group were 22.8%(71/312),44.2%(138/312),33.0%(103/312)and 14.8%(81/548),48.5%(266/548),36.7%(201/548),respectively.AA genotype could increase the risk of gastric cancer(OR=1.711,95% CI 1.150 to 2.545,P=0.008).The frequency of A and G allele of gastric cancer group and control group were 44.9%(280/624),55.1%(344/624)and 39.1%(428/1 096),60.9%(668/1 096), respectively.A allele could increase the risk of gastric cancer(OR=1.270,95% CI 1.041 to 1.550,P=0.018).The frequency of AA,AG,and GG genotype of rs2232365 in gastric cancer group and control group were 19.2%(60/312),46.5%(145/312),34.3%(107/312)and 14.1%(77/548),48.7%(267/548),37.2%(204/548),respectively.In recessive model(AA vs AG+GG),AA genotype could increase the risk of gastric cancer(OR=1.456,95% CI 1.005 to 2.110,P=0.046).A allele of rs3761548 was associated with the depth of invasion in gastric wall(χ2 =12.710,P=0.001)and Helicobacter pylori infection(χ2 = 5.613,P= 0.018).A genotype could increase the risk of lymphatic metastasis(χ2 =4.878,P=0.027).Conclusion Fox P3 gene polymorphism is associated with the susceptibility to gastric cancer in Chinese Han population, and can be molecular-maker of poor prognosis of gastric cancer.
