Based on the theory of "yang governs and yin follows"， it is believed that pediatric obesity is primarily involves the key pathogenesis of constraint and stagnation due to yang deficiency， and the excessive accumulation of yin turbid. The root cause lies in the spleen-stomach yang deficiency and impaired digestion and transformation， while the branch manifestation is characterized by constraint and stagnation in sanjiao （三焦） and excessive accumulation of yin turbid. This emphasizes the imbalance between the governing function of yang qi and the transformation mechanism of yin essence in the overall pathological process. Accordingly， the treatment approach of reinforcing yang and unblocking stagnation， inhibiting yin and resolving turbid has been proposed. And a comprehensive treatment plan is suggested， including abdominal tuina combined with spinal manipulation， timed acupuncture according to the the eightfold method of the sacred tortoise， syndrome differentiation-based acupuncture， and mild moxibustion. This approach aims to provide an integrated approach for the prevention and treatment of pediatric obesity.

Solitary cervical submental nodule is a relatively rare case in clinical procedure and prone to miss diagnosis.Differential diagnosis with various head and neck diseases is necessary.This article reported a case of solitary cervical submental metastasis of papillary thyroid carcinoma received in the department of surgery,Civil Aviation Shanghai Hospital,Ruijin Gubei Branch,Shanghai Jiao Tong University School of Medicine.The patient came to the outpatient clinic for treatment due to"consciously larger submental tubercle than before".Ultrasound examination revealed suspicious lesions in both the thyroid and submental regions.Ultrasound-guided final needle aspiration biopsy diagnosed as malignant tumor.Surgical resection was performed and the central group lymph nodes dissected Pathological examination confirmed papillary thyroid carcinoma with solitary submental metastasis.This article reported the clinical diagnosis and treatment of this case,in order to improve the disease recognition for clinicians,and make differential diagnosis with other rare neck diseases,and avoid missing diagnoses.

Objective:To explore the prognostic factors of patients with Vibrio vulnificus sepsis. Methods:The clinical data of 67 patients with Vibrio vulnificus sepsis from January 2008 to December 2019 in the First Affiliated Hospital of Wenzhou Medical University were retrospectively analyzed. Univariate analysis was used to compare the differences in general information, clinical manifestations, admission laboratory indicators, antibiotics and surgery between the death group and the cured group. Then the factors with significant difference in univariate analysis were included in multivariate analysis, and the factors of prognosis were obtained. Results:Univariate analysis showed that there were significant difference in liver disease, admission with hypotension shock, multiple limb injuries; admission leukocytes, platelets, pH value, albumin, lactic acid, aspartate aminotransferase, creatinine, procalcitonin, creatine kinase, activated partial thromboplastin time, prothrombin time between the death group and the cured group (all P <0.05). Multivariate analysis showed that admission lactate ( OR=0.628, 95% CI: 0.461-0.855, P=0.003), albumin ( OR=1.330, 95% CI:1.062-1.667, P=0.013), creatine kinase ( OR=0.999, 95% CI: 0.998-1.000, P=0.016) and admission surgery time ( OR=0.118, 95% CI: 0.015-0.938, P=0.043) were risk factors of the prognosis. Patients with high lactate, creatine kinase and low albumin at admission indicate poor prognosis; patients with admission surgery time≤ 12 h have better prognosis. Conclusion:For the treatment of patients with Vibrio vulnificus sepsis, medical staff should dynamically evaluate these prognostic factors in the early stage, and early surgical treatment should be adopted to improve the prognosis of patients.

Objective Comparing the performance of the two nonparametric Bayesian methods for benchmark dose estimating under different dose response data,then comparing them with traditional parametric methods.Methods Introduce the basic principle of the nonparametric Bayesian method based on weighted process and stochastic process separately,then compared the estimations through simulate study and instance analysis.Results The simulate study shows that the posterior estimates were reasonably close to the target true BMD value for the two nonparametric methods,and NBP2 is more desirable compared to NPB1.The nine examples indicate that the BMD estimates from the nonparametric approaches generally fall into or very near the interval of those obtained from BMDS and nonparametric approaches tend to produce lower BMDLs than the parametric modeling approaches.Conclusion The posterior estimates were reasonably close to the target true BMD value for the two nonparametric methods,especially when standard parametric models fail to fit to the data adequately.The NPB2 method is slightly bet-ter than the NPB1 method in the aspect of estimation result and the software operation speed.
