BACKGROUND:Ovarian tissue vitrification cryopreservation is one of the important methods for preserving fertility.Tanshinone ⅡA has various pharmacological activities, including anti-oxidation,inhibition of inflammatory response, and reduction of apoptosis, but its role as an additive for vitrification cryoprotection of ovarian tissue is still unclear.OBJECTIVE:To explore the protective effect of tanshinone ⅡA on vitrification cryopreservation of mouse ovarian tissue.METHODS:Twenty-five 6-week-old female KM mice were randomly selected and their ovarian tissues were randomly divided into five groups, with 10 ovaries per group. The fresh group was not cryopreserved. The frozen control group used vitrification cryoprotectant. The 0.5,2.5,and 5 μmol/L tanshinone ⅡA groups used vitrification cryoprotectant containing 0.5,2.5,and 5 μmol/L tanshinone ⅡA,respectively, and were cryopreserved in liquid nitrogen. After 3 days of storage, the cryopreserved tubes were taken out and thawed. The ovarian tissue and follicle morphology of each group were observed by hematoxylin-eosin staining, and the normal follicle morphology and survival rate were analyzed. The levels of superoxide dismutase, catalase, malondialdehyde, tumor necrosis factor-α,interleukin-1β,and interleukin-17 in the ovary were detected by enzyme-linked immunosorbent assay.RT-qPCR and western blot assay were used to detect the mRNA and protein expressions of nuclear factor E2-related factor 2(Nrf2)and heme oxygenase 1(HO-1)in the mouse ovary.RESULTS AND CONCLUSION:(1)Compared with the fresh group, the frozen control group had abnormal morphology of follicles at all levels in the ovary, decreased follicle survival rate(P<0.05),decreased superoxide dismutase and catalase activities(P<0.05);the levels of malondialdehyde, and tumor necrosis factor α,interleukin 1β,and interleukin 17 were all increased(P<0.05),and the mRNA and protein expressions of Nrf2 and HO-1 were decreased(P<0.05).(2)Compared with the frozen control group, different concentrations of tanshinone ⅡA could improve the morphology of follicles at all levels in the ovary, increase the survival rate of follicles, enhance the activities of superoxide dismutase and catalase, and reduce the levels of malondialdehyde, tumor necrosis factor α,interleukin 1β,and interleukin 17,increased the mRNA and protein expression of Nrf2 and HO-1 in a concentration-dependent manner, with 5 μmol/L tanshinone ⅡA having the most significant effect.(3)The results show that tanshinone ⅡA may reduce the oxidative stress level and inflammatory response of mouse ovarian tissue by mediating the Nrf2/HO-1 signaling pathway, thereby alleviating the reproductive damage caused by vitrification cryopreservation of mouse ovaries.

Objective:To explore the therapeutic law of moxibustion in Professor Zhou Meisheng's medical manuscripts for epidemic hemorrhagic fever (EHF) based on data mining and knowledge map technology. Methods:The manuscript data of Professor Zhou Meisheng's moxibustion treatment of EHFwere collected from Infectious Diseases Department of Dangshan County People's Hospital from December 16, 1985 to December 25, 1987. Graphpad Grism 8.0 software was used for descriptive analysis. PHP 5.4 program code was used for association rule analysis. SPSS Statistics 26.0 was used for clustering analysis. Neo4j Community 3.5.25 database was used to analyze the syndrome-weight graph.Results:205 prescriptions were included. There were 21 symptoms with frequency>40, in which the frequency of aversion to cold, fever, rash and irritability was 100%. The main types of moxibustion methods used in the treatment included moxibustion frame fumigation moxibustion, Wanying acupoint moxibustion pen moxibustion, and fire needle instead of moxibustion. There were 29 acupoints with a frequency of >25, including Zhongwan (CV12), Shenshu (BL23) and Mingmen (DU4), etc. Association rules showed that Sanyinjiao (SP6)-Zhongwan (CV12)-Feishu (BL13)-Shenshu (BL23)-Zhiyang (DU9) had the highest correlation. Six effective clustering combinations of moxibustion for EHF were summarized by clustering analysis. The weight graph can obtained the first 30 relationships with high correlation of target syndromes. Conclusions:Professor Zhou applied the idea of "moxibustion for heat syndrome" to the treatment of EHF, and took the method of "acupoint selection according to symptoms" as the main acupoint selection idea for moxibustion treatment of EHF. In clinical practice, moxibustion combined with auxiliary operation of TCM is often used to treat EHF, which can achieve good results.

AIM:To investigate whether the expression of perilipin 2(PLIN2)in renal tubular cells could predict a decline in renal function in diabetic kidney disease(DKD)patients, and to explore the potential mechanisms in-volved in renal tubular cell injury induced by PLIN2 during the progression of DKD.METHODS:Control individuals(n=12)and DKD patients(n=51)were enrolled in this retrospective cohort study.Demographic and laboratory data were col-lected.A simplified linear mixed-effects model was applied to assess the estimated glomerular filtration rate(eGFR)slope. The relationship between PLIN2 and renal function decline in DKD patients was predicted by Spearman correlation analysis and a generalized linear model.BKS-db/db diabetic mice and streptozotocin-induced diabetic mice were used.Primary renal tubular cells were treated with glucose and transfected with small interfering RNA or plasmid.Western blot-ting and immunofluorescence staining were used to detect PLIN2 expression.Lipid droplets were stained with oil red O.The oxygen consumption rate(OCR)of mitochondria was measured using an extracellular flux analyser.RESULTS:The expression of PLIN2 was markedly higher in the tubules of DKD patients than in those of control subjects. After 24(12,39)months of follow-up,the eGFR slope of DKD patients was-7.42(-19.77,-2.09)mL/(min·1.73 m2·year).An in-crease in the baseline percentage of PLIN2-positive tubules was significantly associated with the eGFR slope during the fol-low-up period[hazard ratio(HR)=1.90,95%confidence interval(CI):1.00~3.58],indicating that tubular PLIN2 could predict a decrease in renal function in DKD patients. Both the accumulation of lipid droplets and the expression of PLIN2 were markedly greater in the tubules of diabetic mice than in those of control mice.Glucose treatment induced lipid droplet accumulation and PLIN2 expression in renal tubular cells.Knockdown of PLIN2 significantly alleviated glucose-in-duced lipid droplet accumulation, whereas PLIN2 overexpression aggravated glucose-induced lipid droplet accumulation. The decrease in mitochondrial OCR in renal tubular cells induced by glucose treatment was alleviated after PLIN2 knock-down.However,overexpression of PLIN2 directly decreased the mitochondrial OCR.CONCLUSION:The PLIN2 ex-pression in tubules predicts a decline in renal function in patients with DKD.The PLIN2 suppresses mitochondrial aerobic respiration and contributes to the accumulation of lipid droplets in renal tubular cells to promote the progression of DKD.

Objective:To explore the clinical effect of reconstruction of soft tissue defect on the palmar side of 2 adjacent fingers with bilobed perforator flap of medial foot artery. Methods:From September 2020 to September 2023, 6 patients with soft tissue defects on the palmar side of 2 adjacent fingers were treated in the Department of Hand and Foot Microsurgery, the Affiliated Hospital of Qingdao University. After admission, Doppler ultrasound scan was performed to locate the perforating branches of the deep and superficial branches of medial foot artery, as well as the course of medial foot artery. A bilobed flap with two perforating branches of medial foot artery was used to reconstruct the soft tissue defects on the palmar side of 2 adjacent fingers. The perforators of medial foot artery were anastomosed with the proper palmar digital arteries, and the accompanying veins was anastomosed with the dorsal digital veins. The branches of saphenous nerve were anastomosed with the proper palmar digital nerves. The sizes of soft tissue defect were 4.5 cm×2.0 cm-3.0 cm×1.5 cm, and the flaps sized 5.0 cm×2.5 cm-3.5 cm×2.0 cm. All donor sites for the perforator flaps were covered by artificial dermis (4 patients) or autologous skin grafts (2 patients). Short and long-term follow-up for flap survival, flap shape, scar, and functions of interphalangeal joint and metacarpophalangeal joint function were evaluated in outpatient clinic and WeChat reviews. Results:All the bilobed perforator flaps of medial foot artery survived in 6 patients. There was no infection in the flap and donor area. Wounds in fingers and dornor sites in foot all healed well. Postoperative follow-up ranged 4-12 months, with an average of 9.3 months. The sensation of the palmar fingers recovered well with TPD at 7-10 mm and at 8.5 mm in average. The sensation recoveries were up to S 3-S 4. According to the Evaluation Trial Standards of Upper Limb Partial Function of Hand Surgery of the Chinese Medical Association, 5 patients (10 fingers) were rated as excellent and 1 patient (2 fingers) as good. The donor sites healed well and walking, running, jumping, mountaineering and other food motions were not affected, and without any obvious discomfort. According to the American Orthopedic Foot and Ankle Societ (AOFAS) ankle-hindfoot score, the ankle functions of 6 patients were evaluated as excellent. Conclusion:A bilobed perforator flap of medial foot artery is applied to reconstruct the soft tissue defects on the palmar side of 2 adjacent fingers. It can simultaneously reconstruct 2 defects in a single surgery, and offer a fast functional recovery and aesthetical appearance.
