The surgical reconstruction of the anterior cruciate ligament (ACL) has shown continuous advances, but concerns persist regarding its instability, the occurrence of post-traumatic osteoarthritis, and the high frequency of re-ruptures. Despite the lack of consensus regarding the precise structure of the anterolateral ligament (ALL) complex, this structure is anatomically present, and it can be damaged concurrently with the ACL, providing resistance to rotational instability. The debate regarding the timing and necessity of additional surgeries during ACL reconstruction is ongoing. Some authors advocate for concurrent procedures because of various risk factors, despite the widely varying opinions on this issue. Common adjunctive surgical techniques include ALL reconstruction and lateral extra-articular tenodesis. These procedures typically use various grafts and generally produce favorable clinical outcomes. Nevertheless, additional surgeries should be considered in certain cases because standard ACL reconstruction techniques may not be suitable for all patients. These cases include revision reconstructions, patients under 20 years old with high activity levels, those experiencing significant rotational instability, and individuals with specific anatomical risk factors. In such cases, procedures targeting the ALL complex may offer added benefits.

BACKGROUND: The International Agency for Research on Cancer classifies asbestos as belonging to Carcinogen Group 2A for gastric cancer. We herein report a case of gastric cancer associated with asbestosis and describe the work-related and risk assessments of asbestos exposure for gastric cancer. CASE PRESENTATION: The 66-year-old male patient in our case worked in asbestos spinning factories. His level of cumulated asbestos fiber exposure was estimated to be 38.0-71.0 f-yr/cc. Thus, the Excess Life Cancer Risk for lung cancer associated with asbestos exposure was 9,648x10-5, almost 9,600 times the value recommended by the United States of America Environmental Protection Agency (1 x 10-5). The relative risk of developing lung cancer for this patient was more than 25 f-yr/cc, a well-known criterion for doubling the risk of lung cancer. CONCLUSION: The patient's exposure to high-dose asbestos was sufficient to increase his risk of gastric cancer because as the risk of lung cancer increased, the risk of gastric cancer was due to increase as well. Therefore, occupational asbestos fiber exposure might be associated with gastric cancer in this case.

BACKGROUND: We report cases of high lead exposure in a Korean steel manufacturing company and a likely cause for these cases. CASE REPORT: Neurological examinations, neurobehavioral tests, blood tests, and urine tests were performed to detect an association between high lead exposure and physical symptoms for workers in a steel manufacturing company. In order to determine the cause of high lead exposure, the work process and environment was assessed. The highest lead exposure was during the work process. We found that the interior of an oil storage tank was coated with lead-containing paint, leading to severe exposure during the cutting process. In two observations, 4 of 12 workers diagnosed with occupational disease were found to have high exposure to lead from the storage tank and three had increased beta-2 microglobulin levels in the blood (indicative of urinary tract damage). CONCLUSIONS: Lead management, including routine measurements of the working environment and examinations of lead concentrations in workers, are needed in the korean scrap metal industry. In addition, rules requiring workers to wear personal protective equipment and receive education about lead exposure should be strictly enforced.

Solitary Fibrous Tumors (SFTs) are uncommon spindle-cell neoplasms usually arising in the pleura of the thoracic cavity. Thirty percent of SFTs are found frequently in the extrapleural locations, but infrequently in the head and neck region, and very rarly in the suboccipital area. The treatment of choice for SFTs is a complete surgical excision of the tumor. Recurrent SFTs are more often associated with incomplete excision. In this article, we present a by surgical excision case of an 87-year-old woman, who was treated for SFT arising in the suboccipital area by surgical excision with no recurrence after 6 months.
