1.Treatment principle of invigorating the spleen and kidney and regulating menstrual cycle combined with Dong's extra-point acupuncture for the patients with polycystic ovary syndrome

Objective:To evaluate the clinical curative effect of invigorating the spleen and kidney and regulating menstrual cycle treament combined with Dong's extra-point acupuncture on polycystic ovary syndrome (PCOS).Methods:According to random number table method, 70 patients with PCOS meeting inclusion criteria in the hospital were divided into western medicine group (23 cases), TCM group (23 cases) and combination group (24 cases) between January and December 2020. The western medicine was treated with ethinylestradiol and cyproterone acetate tablets, TCM group was treated with invigorating the spleen and kidney and regulating menstrual cycle treatment, and combination group was treated with invigorating the spleen and kidney and regulating menstrual cycle treatment and Dong's extra-point acupuncture. All the three groups were treated continuously for 3 menstrual cycles. Before and after treatment, levels of testosterone, estradiol, FSH and LH were detected by full-automatic immunoluminescence analyzer. The ovaries were detected by color Doppler ultrasound diagnostic apparatus. The ovarian volume, number of follicles and adverse reactions were recorded. The clinical curative effect was evaluated.Results:The differences in total response rate among western medicine group, TCM group and combination group were statistically significant [52.17% (12/23), 60.87% (14/23) vs. 87.50% (21/24), P<0.05]. After treatment, levels of testosterone [(1.02±0.12) μg/L vs. (1.23±0.25) μg/L, (1.46±0.19) μg/L, F=30.47], estradiol [(40.18±12.31) ng/L vs. (48.62±12.58) ng/L, (50.73±12.37) ng/L, F=4.78] and LH [(6.87±2.45) U/L vs. (8.28±2.29) U/L, (8.54±2.51) U/L, F=3.26] in combination group were significantly lower than those in the TCM group and western medicine group ( P<0.05). After treatment, ovarian volume [(7.05±2.34) m 3vs. (8.79±2.11) m 3, (9.12±2.18) m 3, F=5.97] in combination group was significantly lower than that in TCM group and western medicine group ( P<0.05), and number of follicles (<10 mm) (8.28±2.24 vs. 9.67±2.40, 10.15±2.64, F=3.77) was significantly lower than that of TCM group and western medicine group ( P<0.05). During treatment, difference in incidence of adverse reactions among western medicine group, Tiaozhou group and combination group was not statistically significant [21.7% (5/23), 13.0% (3/23) vs. 8.3% (2/24), P>0.05]. Conclusion:The invigorating the spleen and kidney and regulating menstrual cycle treatment combined with Dong's extra-point acupuncture can regulate levels of sex hormones, promote follicles development and ovulation, and improve ovarian morphology of PCOS patients safely.
2. Effect of different frequency hyperbaric oxygen preconditioning on ischemia-reperfusion injury of rat flap after transplantation

Objective:
To explore the effect of hyperbaric oxygen preconditioning with different frequency on the survival rate of flap and ischemia-reperfusion injury in rats after transplantation, and to explore the best preconditioning conditions to improve the survival rate of rat flaps after transplantation.
Methods:
Thirty-six Sprague Dawley rats were randomly divided into four groups according to the random number table method, 9 groups in each group.Four groups of rats were pretreated with hyperbaric oxygen pretreatment for 0, 2, 4, and 6 days before the operation, control group, pretreatment 2 d group, pretreatment 4 d group, and pretreatment 6 d group. Taking the midline of the back of the rat as the axis, an ultra-long random flap with a pedicle at the tail end and about 1 cm from the superior iliac spine was designed and cut to a size of 10.0 cm×2.5 cm. The survival of the flaps in each group was observed and the final survival area and survival rate of the flaps were measured on the 7th day after surgery. On the 7th day after operation, the tissue was taken at a distance of 5 cm from the pedicle, and the histopathology was observed; The content of superoxide dismutase (SOD) and malondialdehyde (MDA) in flap tissue was detected by immunohistochemistry, and the expression rate of positive cells in each group was calculated. Immunofluorescence was used to detect the expression of interleukin-6 (IL-6) in the flap tissue.
Results:
On the 7th day after the operation, the survival area and survival rate of the transplanted flaps in the hyperbaric oxygen pretreatment group were significantly higher than those in the control group (
3.Efficacy of posterior lumbar interbody fusion and internal fixation in treatment of lumbar spinal stenosis with redundant nerve roots

Objective To investigate the clinical efficacy of posterior lumbar interbody fusion and internal fixation for the treatment of lumbar spinal stenosis with redundant nerve roots (RNRs).Methods A retrospective case series study was performed on the clinical data of 23 cases of lumbar spinal stenosis with RNRs from January 2009 to December 2014.This study involved 10 males and 13 females,with an average age of 48.4 years(range,38-58 years).The course of disease averaged 28.1 months(range,2 months-7 years).There were seven patients with single segment of stenosis,12 patients with two segments of stenosis,and four patients with three segments of stenosis.RNRs diagnostic criteria:in the sagittal section of the MRIT2 image of lumbar spine,the cauda equina nerve root in the dural sac was circular,tortuous and twining.MRI information was collected and analyzed by three double-blind radiologists,respectively.RNRs were determined based on the unanimous consent of all the three doctors.All patients underwent posterior discectomy,spinal canal decompression,interbody fusion,and internal fixation after admission.The operation time,intraoperative blood loss,and postoperative complications were recorded.The visual analogue scale (VAS),Oswestry disability index (ODI),and Japanese Orthopedic Association (JOA) score were used to evaluate the back pain and leg pain before operation and 3 months after operation.Fischgrund standard was used for the overall efficacy evaluation.Results All patients were followed up for 12-30 months (mean,23.3 months).The operation time was (130.0 ± 23.2) minutes,and the intraoperative blood loss was (513.0 ± 165.0) ml.MRI was reviewed 1 week after operation,and the result showed that redundancy of cauda equina disappeared.Preoperatively,the scores of back pain VAS,leg pain VAS,ODI and JOA were (6.3± 0.8) points,(6.8 ±0.9)points,(46.7±2.5)points and (10.3 ±2.8)points,respectively.At 3 months postoperatively,the scores were (1.4 ± 0.5) points,(1.8 ± 0.7) points,(11.9 ± 2.1) points,and (25.3 ± 1.8) points,respectively (P < 0.05).In terms of the efficacy,17 cases were excellent,four good,and two fair,with an excellent and good rate of 91%.At 12 months after operation,the CT scan showed no screw rupture or cage dislocation,with bony union seen between the vertebrae.Conclusion Posterior lumbar interbody fusion and internal fixation can help relieve the back and leg pain and eliminate redundant nerve roots in patients with lumbar spinal stenosis with RNRs.
