Objective To explore the mechanism underlying the role of disulfidptosis-related genes(DRGs)in the disease progression of metabolically associated fatty liver disease(MAFLD)based on bioinformatics. Methods In this study, the GEO database was utilized to screen for eligible MAFLD expression data, conduct differential gene analysis, and identify DRGs through consistent clustering to subtype MAFLD patients. The immune infiltration status among subtypes was further evaluated, and the infiltration of immune cells was analyzed using the CIBERSORT algorithm. The gene modules related to the disease were selected through weighted gene co-expression network analysis(WGCNA).Subsequently,a diagnostic model was constructed based on DRGs using machine learning models, and the performance of the model was verified. Finally, the stability of DRGs among different subtypes was evaluated using an external dataset, and the significance of the results was analyzed using statistical tests. Results Through the analysis of the dataset GSE31803,six disulfide death genes,namely,SLC3A2,NCKAP1,CYFIP1,FLNA,MYL6 and MYH10,which were closely related to the clinical characteristics of MAFLD,were screened out.MAFLD patients were classified into two subtypes, with subtype 1 having a higher level of immune cell infiltration.Key gene modules were identified through WGCNA.Through machine learning screening, the support vector machine(SVM)model was determined as the optimal classification model.External validation confirmed the stability and effectiveness of the key genes in different subtypes of MAFLD.Conclusion Based on DRGs,two highly heterogeneous subtypes of MAFLD were identified, which exhibited significant differences in clinical characteristics, biological processes and immune status, indicating that DRGs play a crucial role in the occurrence and development of MAFLD.

To investigate the phenotype and genomic characterization of a mucoid-type Salmonella Saintpaul ST50 isolate from a urinary tract infection patient, promoting clinical diagnosis and treatment for urinary tract infections caused by Salmo-nella spp.Culture-based quantitative counts of midstream urine sample from the patient were conducted, and further biochemi-cal identification, mass spectrometry detection, serum agglutination test and antimicrobial susceptibility test(AST)were con-ducted on Salmonella isolate(2024JD5).Whole-genome sequencing(WGS)was performed on isolate 2024JD5 to predict sero-type,multilocus sequence type(MLST),resistance genes, and virulence genes. Two smooth-type of Salmonella Saintpaul ST50 were selected as comparative genomic reference strains from the Chinese local Salmonella genome database. The literature reviews of global Salmonella serotype of urinary tract infection were summarized.Specific serum agglutination confir-mation of isolate 2024JD5 failed due to characterization of the mucus type. The strain 2024JD5 was predicted as Salmonella Saintpaul(4,5,12:e,h:1,2)ST50 using WGS,and was resistant to ciprofloxacin, nalidixic acid, chloramphenicol and tetracy-cline with carrying aminoglycoside resistance genes aac(6')-Ⅰaa and aph(3)-Ⅱa,chloramphenicol resistance gene floR,tetra-cycline resistance gene tet, quinolone resistance gene qnrS1,and S83Y substitution in the gyrA gene was found in the quinolo-ne resistance determination region(QRDR).In addition, the strain 2024JD4 carried six types of non-plasmid-based mobile ge-netic elements and 144 virulence genes, including 71 secretion transporter genes and 58 fimbriae adhesion genes, respectively.Four types of fimbriae regulatory genes(csgB,csgC,fimW,fimY)were absent in comparison with smooth-type Salmonella Saintpaul. The literature reviews showed Salmonella Saintpaul was currently a rare Salmonella serotype in cases of urinary tract infections worldwide.Salmonella Saintpaul ST50 with mucoid-type is the pathogen of urinary tract infection with multi-drug resistant phenotypic and genotypic characteristics, and the high mucoid expression may be related to the compensatory mechanism of fimbriae regulatory genes absence in urinary tract colonization and adaptation.WGS combined with the Chinese local Salmonella genome database can effectively solve the diagnosis and biosafety assessments of rare Salmonella phenotypes.

Objective:To compare the clinical and imaging characteristics of eyes with idiopathic focal choroidal excavation (FCE) and acquired FCE, and to preliminarily analyze the association of FCE with different fundus diseases. Methods:A retrospective clinical study. A total of 90 patients (93 eyes) diagnosed with FCE at the Ophthalmology Department of The First Affiliated Hospital of Zhengzhou University from March 2021 to May 2024 were included in the study. All affected eyes underwent best-corrected visual acuity (BCVA) testing, intraocular pressure (IOP) measurement, ophthalmoscope, fundus color photography, and swept-source optical coherence tomography (SS-OCT). SS-OCT was used to measure subfoveal choroidal thickness (SFCT), choroidal thickness at the temporal, nasal, and inferior margins of the FCE, as well as the maximum width and depth of the FCE. Based on the presence of concomitant fundus diseases, FCE cases were divided into an idiopathic FCE group (51 eyes) and an acquired FCE group (42 eyes). Clinical characteristics at the initial and final visits were compared between the two groups. Clinical features were also analyzed for acquired FCE eyes with different associated fundus diseases. A logistic regression model was used to identify potential risk factors for FCE coexisting with other fundus diseases. Results:Compared to the idiopathic FCE group, the acquired FCE group had significantly decreased BCVA ( Z=?8.290), significantly increased FCE width ( Z=?2.762), and significantly higher incidence rates of saucer-shaped FCE ( χ2=8.352) and ellipsoid zone disruption ( χ2=7.999). These differences were all statistically significant ( P<0.05). No significant differences were found between the two groups in age, gender distribution, IOP, foveal involvement, proportion of conforming FCE, retinal pigment epithelium-Bruch’s membrane rupture, presence of hyperreflective material under the excavation, presence of surrounding thick vessels, FCE depth, SFCT, or choroidal thickness at the inferior, temporal, and nasal margins of the FCE ( P>0.05). Logistic regression analysis revealed that FCE width (odds ratio=1.002, 95% confidence interval: 1.001-1.004, P=0.005) was the sole risk factor for FCE coexisting with other fundus complications. In both the idiopathic and acquired FCE groups, FCE depth and width remained relatively stable during follow-up. BCVA improved in the acquired FCE group after treatment. Conclusions:FCE can coexist with various fundus diseases. FCE width is the only risk factor for the development of other fundus complications in FCE-affected eyes. Treating retinal or choroidal diseases associated with acquired FCE can provide visual benefits to patients.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
