1.Relative Tumor Density of Soft-Tissue Sarcoma in Korean Population:An Institutional Review

Background:
Comprehensive studies on the tumor burden of soft-tissue sarcoma (STS) by anatomical site are lacking in Asian populations.
Objective:
To investigate the anatomical distribution of STS via relative tumor density (RTD) in a Korean cohort.
Methods:
The RTDs of patients with STS at a single-institution from 2007–2022 were retrospectively analyzed. To describe the STS locations, the body was divided into 4 anatomical sites, and the RTD of each was calculated to the compare topographic tumor burden.
Results:
Fifty-nine cases in 58 individuals, 35 male (60.3%) and 23 female (39.7%), with a mean age of 56.5±20.4 were analyzed. Overall, the most frequent STS site was the lower extremity (LE, n=22, 37.3%), and the highest RTD was in the head and neck (H&N, 2.44; 95% confidence interval, 1.39–3.77). Dermatofibrosarcoma protuberans (DFSP), Kaposi’s sarcoma (KS), and angiosarcoma (AS) accounted for 76.3% of all the cases. DFSP, KS, and AS showed significantly higher RTD on the trunk (2.55, p=0.025), LE (3.88, p<0.001), and H&N (7.42, p<0.001), respectively, than elsewhere.
Conclusion
Each STS displays topographic variability and produces different topographic tumor burdens by body site in an Asian population.
2.Comparative analysis of novel damping capacity analysis devices for assessing implant stability in clinical practice

Purpose:
Implant stability testing is crucial for verifying osseointegration before prosthetic loading. Several methods have been developed to assess osseointegration. Among these, damping capacity analysis (DCA) devices offer a user-friendly and non-invasive approach.In this study, we evaluated the accuracy and reliability of newly available DCA devices in assessing dental implant stability.
Methods:
This study included 58 implants from 37 patients over a 1-month period. Three measurements per implant were obtained with healing abutments in place, following the manufacturers’ guidelines, using the DCA-P (Periotest M, Medizintechnik Gulden), DCA-A (Anycheck, Neobiotech), and DCA-T (The Trust, Dentium) devices. Factors such as healing abutment height, time since placement, bone grafting, fixture diameter, and fixture length were evaluated. Accuracy was assessed using DCA-P as the reference, and reproducibility was statistically analyzed using 3 measurements per implant. Statistical analyses were performed with SPSS Statistics 23.0 (IBM Corp.).
Results:
All implants that passed the stability tests using DCA devices were restored with definitive prostheses and showed no signs of early failure. The DCA-A value demonstrated a very strong correlation with the DCA-P value (DCAV-P), whereas the DCA-T value exhibited only a moderate correlation with DCAV-P. DCA-P also showed the highest reliability, followed by DCA-A and then DCA-T. The reliability of DCA-A and DCA-T was not significantly affected by any of the assessed factors; in contrast, DCA-P’s reliability was significantly influenced by arch location and specific quadrant position.
Conclusions
With DCA-P as the reference, DCA-A demonstrated superior accuracy compared to DCA-T. Although DCA-P exhibited the highest reliability, its performance was significantly affected by the positional factors of the target implant.
3.Pompholyx and Eczematous Reaction After Intravenous Immunoglobulin Therapy for Patient with Guillain-Barré Syndrome

Cutaneous adverse events associated with intravenous immunoglobulin (IVIG) have a relatively low significance, ranging from 0.4% to 6%. In Korean dermatology literature, eczematous reactions associated with the use of IVIG have been underestimated. A 34-year-old male presented with a 2-week history of multiple vesicles on both hands and diffuse eczematous lesions on the trunk and both arms. He had been diagnosed with Guillain-Barré syndrome and had received IVIG at a dosage of 0.4 g/kg/d for 5 days during hospitalization. One week after discharge, cutaneous manifestations developed, starting with tiny blisters on both hands and progressing to eczematous patches on the trunk and arms.Histopathologic examination revealed spongiotic dermatitis with intracorneal bullae formation. A diagnosis of palmar pompholyx and systemic eczematous reaction associated with the administration of IVIG was made. Treatment with systemic corticosteroid, antihistamines and topical steroid was initiated. At a 2-week follow-up, significant improvement was observed.
5.Successful emergency medical service training with virtual field trips using video during the COVID-19 pandemic: the Official Development Assistance Project in Uzbekistan

Purpose:
The coronavirus disease 2019 pandemic saw many restrictions on the provision of emergency medical service (EMS) training through actual field trips (AFTs), requiring a search for alternatives. This study aimed to assess trainees’ reactions to virtual field trips (VFTs) and determine the characteristics of instructional design for successful VFTs using edited videos and expert interviews.
Methods:
This study evaluated Uzbekistan trainees’ reactions to the VFT of EMS training using questionnaires in three categories: satisfaction, relevance, and engagement. Factors of satisfaction and dissatisfaction were identified through open-ended questions.
Results:
A total of 286 trainees responded to the survey during 15 educational sessions conducted from 2020 to 2022. The trainees’ responses to the VFT were positive. Overall mean scores were 4.65±0.49, 4.63±0.50, and 4.63±0.50 out of 5 points for satisfaction, relevance, and engagement, respectively. The trainees reported that the most interesting and helpful videos concerned the introduction of an EMS training curriculum and the observation of training facilities, such as the simulation centers of educational institutes. The leading causes of satisfaction were (1) authenticity of the VFTs, (2) easy-to-understand content, and (3) relevance to the job. The trainees suggested that Uzbek or Russian voice-overs would be better than subtitles in the video clip for focusing on VFT.
Conclusion
In situations where AFTs are not available, VFTs using edited videos and expert interviews are a good alternative to EMS education. Based on these results, it is possible that AFTs could be replaced by VFTs using qualified videos with designed instructions as a distance learning method under specific conditions.
6.Comparison of fit and trueness of zirconia crowns fabricated by different combinations of open CAD-CAM systems

PURPOSE:
. This study aims to clinically compare the fitness and trueness of zirconia crowns fabricated by different combinations of open CAD-CAM systems.
MATERIALS AND METHODS:
. Total of 40 patients were enrolled in this study, and 9 different zirconia crowns were prepared per patient. Each crown was made through the cross-application of 3 different design software (EZIS VR, 3Shape Dental System, Exocad) with 3 different processing devices (Aegis HM, Trione Z, Motion 2). The marginal gap, absolute marginal discrepancy, internal gap(axial, line angle, occlusal) by a silicone replica technique were measured to compare the fit of the crown. The scanned inner and outer surfaces of the crowns were compared to CAD data using 3D metrology software to evaluate trueness.
RESULTS:
. There were significant differences in the marginal gap, absolute marginal discrepancy, axial and line angle internal gap among the groups (P < .05) in the comparison of fit. There was no statistically significant difference among the groups in terms of occlusal internal gap. The trueness ranged from 36.19 to 43.78 μm but there was no statistically significant difference within the groups (P > .05).
CONCLUSION
. All 9 groups showed clinically acceptable level of marginal gaps ranging from 74.26 to 112.20 μm in terms of fit comparison. In the comparison of trueness, no significant difference within each group was spotted. Within the limitation of this study, open CAD-CAM systems used in this study can be assembled properly to fabricate zirconia crown. [J Adv Prosthodont 2023;15:155-70]

Background/Aims:
The short- and long-term effects of adalimumab (ADA) on Korean patients with intestinal Behcet’s disease (BD) for remain unclear. Therefore, a multicenter study was performed to evaluate the efficacy and safety of ADA in Korean patients with intestinal BD in a real-world setting.
Methods:
The medical records of 67 patients with BD prescribed ADA between January 2012 and December 2020 at five referral centers in Korea were retrospectively analyzed and the safety and efficacy of ADA within 52 weeks were assessed. To evaluate the clinical efficacy of ADA, the Disease Activity Index for Intestinal BD (DAIBD) and representative blood biochemical markers were compared at 0, 12, 24, and 52 weeks of ADA treatment.
Results:
During the follow-up period of 52 weeks, 46 patients continued ADA treatment. The cumulative drug survival rate was 83.5%. The DAIBD score decreased over the study period (p < 0.001). Moreover, the erythrocyte sedimentation rate, serum C-reactive protein levels, and serum albumin levels significantly improved at 12, 24, and 52 weeks of ADA treatment (all, p <0.05).
Conclusions
As ADA is effective for refractory intestinal BD with few safety concerns in real-world situations, it is a potential treatment option for Korean patients with intestinal BD.

The aim of this study was to develop evidence-based recommendations for determining the surgical extent in patients with locally invasive differentiated thyroid cancer (DTC). Locally invasive DTC with gross extrathyroidal extension invading surrounding anatomical structures may lead to several functional deficits and poor oncological outcomes. At present, the optimal extent of surgery in locally invasive DTC remains a matter of debate, and there are no adequate guidelines. On October 8, 2021, four experts searched the PubMed, Embase, and Cochrane Library databases; the identified papers were reviewed by 39 experts in thyroid and head and neck surgery. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to assess the quality of evidence, and to develop and report recommendations. The strength of a recommendation reflects the confidence of a guideline panel that the desirable effects of an intervention outweigh any undesirable effects, across all patients for whom the recommendation is applicable. After completing the draft guidelines, Delphi questionnaires were completed by members of the Korean Society of Head and Neck Surgery. Twenty-seven evidence-based recommendations were made for several factors, including the preoperative workup; surgical extent of thyroidectomy; surgery for cancer invading the strap muscles, recurrent laryngeal nerve, laryngeal framework, trachea, or esophagus; and surgery for patients with central and lateral cervical lymph node involvement. Evidence-based guidelines were devised to help clinicians make safer and more efficient clinical decisions for the optimal surgical treatment of patients with locally invasive DTC.

Purpose:
Acute promyelocytic leukemia (APL) is a rare disease in children and there are some different characteristics between children and adult. We aimed to evaluate incidence, clinical characteristics and treatment outcomes of pediatric APL in Korea.
Materials and Methods:
Seventy-nine pediatric APL patients diagnosed from January 2009 to December 2016 in 16 tertiary medical centers in Korea were reviewed retrospectively.
Results:
Of 801 acute myeloid leukemia children, 79 (9.9%) were diagnosed with APL. The median age at diagnosis was 10.6 years (range, 1.3 to 18.0). Male and female ratio was 1:0.93. Thirty patients (38.0%) had white blood cell (WBC) count greater than 10×109/L at diagnosis. All patients received induction therapy consisting of all-trans retinoic acid and chemotherapy. Five patients (6.6%) died during induction chemotherapy and 66 patients (86.8%) achieved complete remission (CR) after induction chemotherapy. The causes of death were three intracranial hemorrhage, one cerebral infarction, and one sepsis. Five patients (7.1%) suffered a relapse during or after maintenance chemotherapy. The estimated 4-year event-free survival and overall survival (OS) rates were 82.1%±4.4%, 89.7%±5.1%, respectively. The 4-year OS was significantly higher in patients with initial WBC < 10×109/L than in those with initial WBC ≥ 10×109/L (p=0.020).
Conclusion
This study showed that the CR rates and survival outcomes in Korean pediatric APL patients were relatively good. The initial WBC count was the most important prognostic factor and most causes of death were related to serious bleeding in the early stage of treatment.
