1.Comparison of automated versus manual blood pressure measurement among hospitalized medical patients: A crossover trial

BACKGROUND
Blood pressure is an important vital sign measured not only in hypertension but also among hospitalized patients for clinical evaluation of the actual hemodynamic status. In the digital era, mercury and aneroid sphygmomanometers are being replaced by automated monitors despite lacking validation and recommendations for their use, especially in acute illness.
OBJECTIVETo compare automated and manual blood pressure measurement among hospitalized medical patients with acute illness.
METHODSA crossover design was used in a single tertiary hospital. Blood pressure was recorded from 216 participants, with 432 observations from an automated monitor (Omron HBP1120) and a mercury sphygmomanometer. Automated and manual BP recordings were done twice following the same arm sequential method. The average of the two recordings was used for comparison.
RESULTSMost participants were female, elderly, obese, and had cardiac complaints. Comparing automated and manual methods, the mean difference for systolic was 1.47 ± 12.12 (p = 0.08) and 1.82 ± 10.99 (p=0.02) for diastolic. Subgroup analysis revealed that males had higher manual systolic BP than females (pairwise p-value= 0.017). Overweight and obese participants had higher automated systolic and diastolic BP (p=0.04). Overweight and obese participants had significantly higher systolic and diastolic BP regardless of the method. Significantly higher diastolic BP for different age groups and areas of admission (p=0.02) were observed from the automated method.
CONCLUSIONAutomated BP monitoring showed a significant difference in diastolic BP recordings. Automated BP monitors should be used with caution, especially in interpreting diastolic BP among hospitalized patients.

2.Needlestick injuries among obstetrician-gynecologist trainees: Knowledge, attitudes, and practices

INTRODUCTION
Needlestick injury is a major healthcare hazard. The burden of the problem is difficult to estimate due to underreporting by healthcare workers.
OBJECTIVESTo gain insight into the level of knowledge and perception, attitude and practices of obstetrician-gynecologist (OB-GYN) residents and subspecialty fellows-in-training in a tertiary hospital on blood-borne diseases from needle stick injuries and postexposure prophylaxis.
MATERIALS AND METHODSA cross-sectional study among all residents and fellows in training of the Department of Obstetrics and Gynecology of a tertiary hospital was conducted in December 2017. The explanatory sequential mixed method approach of data collection was utilized using a self-administered original pilot-tested questionnaire and focused group discussion.
RESULTSEighty-four OB-GYN s in training took part in the study. The prevalence of needle stick injury was 80.95%, but only about half of the incidents were reported. There was no statistically significant difference in the knowledge and attitudes toward management and policies on needlestick injuries between fellows and residents. The fellows appeared to be more frequently compliant with practices to prevent and manage needle stick injuries compared to the residents. There is weak but statistically significant positive relationship between the respondents’ knowledge and practice scores.
CONCLUSIONSAlthough the respondents have very good knowledge, adherence to the practices to prevent and manage needlestick injuries is low.

3.Blood transfusion in elective gynecological surgeries in the Philippines: A multicenter study

BACKGROUND
Intraoperative transfusion for gynecologic surgery, when appropriately used, can improve patient outcomes. However, when utilized incorrectly, blood transfusion can worsen patient outcomes and increase patient cost. This study aimed to evaluate the blood transfusion practices of tertiary hospitals in the Philippines.
METHODSThe study utilized a cross-sectional design wherein prospective data were gathered through multiple sources across seven tertiary-level hospitals. Women admitted to undergo gynecologic surgery were recruited based on a set of criteria. A chart review was conducted, and blood utilization indices were calculated. Outcomes were compared between public versus private facilities and transfused versus nontransfused patients.
RESULTSAmong 514 patients, 79.7% underwent cross-matching and 75.1% received transfusions. Adverse events were rare, with no transfusion-related deaths. The overall crossmatch-to-transfusion ratio (C/T ratio) was 2.8, exceeding the 2.5 optimal benchmark; all public hospitals recorded a C/T ratio >2.5, whereas private centers had more efficient usage. Six hospitals met acceptable benchmarks for transfusion probability and transfusion index. Open abdominal procedures, particularly hysterectomy, accounted for the most blood used. Transfused patients had longer operative times, greater blood loss, lower preoperative hemoglobin, and more frequently involved resident physicians in training. Public hospitals recorded higher cross-match and transfusion rates, greater resident physician participation, and broader use of general anesthesia.
CONCLUSIONResults of the study highlight the importance of monitoring blood transfusion parameters to optimize blood utilization. The observed differences between public and private institutions in the country highlight the urgent need for standardized and evidence-based practice to ensure efficient transfusion protocols nationwide.

4.Effects of local infiltration of analgesia and tranexamic acid in total knee replacements: safety and efficacy in reducing blood loss and comparability to intra-articular tranexamic acid.

INTRODUCTION:
The use of periarticular (PA) tranexamic acid (TXA) and its efficacy in comparison with intra-articular (IA) TXA have not been well explored in the literature. This retrospective cohort study aimed to compare the effects of IA and PA TXA with analgesic components in reducing blood loss and improving immediate postoperative pain relief and functional outcomes in patients after unilateral primary total knee arthroplasty (TKA).
METHODS:
A total of 63 patients underwent TKA, and they were divided into the IA TXA delivery group ( n = 42) and PA TXA delivery group ( n = 21). All patients were administered 1 g of TXA. They also received pericapsular infiltration consisting of 0.5 mL of adrenaline, 0.4 mL of morphine, 1 g of vancomycin, 1 mL of ketorolac and 15 mL of ropivacaine. Outcomes for blood loss and surrogate markers for immediate functional recovery were measured.
RESULTS:
Of the 63 patients, 54% were female and 46% male. The mean drop in postoperative haemoglobin levels in the PA and IA groups was 2.0 g/dL and 1.6 g/dL, respectively, and this was not statistically significant ( P = 0.10). The mean haematocrit drop in the PA and IA groups was 6.1% and 5.3%, respectively, and this was also not statistically significant ( P = 0.58). The postoperative day (POD) 1 and discharge day flexion angles, POD 1 and POD 2 visual analogue scale (VAS) scores, gait distance on discharge and length of hospitalisation stay were largely similar in the two groups.
CONCLUSION
Our study showed that both IA and PA TXA with analgesic components were equally efficient in reducing blood loss and improving immediate postoperative pain relief and functional outcomes.

10.Efficacy and safety of platelet-rich plasma using acid citrate dextrose solution A versus sodium citrate PRP kit in treating adult pattern hair loss: A single blind, randomized, placebo-controlled, equivalence trial

BACKGROUND
Despite the widespread prevalence of pattern hair loss, treatment options remain limited. Platelet-rich plasma (PRP) is a promising alternative but is hindered by high costs and a lack of standardized protocols. In the Philippines, only one FDA-approved PRP kit is available, leading to interest in whether Acid Citrate Dextrose Solution A (ACD-A) tube could provide equivalent results. Additionally, international research on different anticoagulant preparations is lacking, with no studies conducted in the Philippines.
OBJECTIVETo determine if the efficacy and safety of PRP therapy using ACD-A and Sodium Citrate (SC) PRP Kit are equivalent in the treatment of adult pattern hair loss.
METHODSA single-blind, randomized, placebo-controlled, equivalence trial. Participants were randomly assigned to either a control group or a treatment group receiving ACD-A or SC PRP Kit preparations. Treatments were administered monthly for six sessions. Hair growth was assessed at baseline and after each session using global photography, hair classification system, and trichoscopy.
RESULTSA total of 48 participants completed the study, divided into three groups of 16 participants each. Mean hair density scores for the ACD-A and SC KIT groups, along with 95% confidence intervals for mean differences at various timeframes, fell within the equivalence margin of ±16 hair follicles/cm2. Minimal adverse effects were observed throughout the study.
CONCLUSIONACD-A produces results equivalent to the SC PRP Kit in terms of hair growth and patient satisfaction. Both preparations are safe, with only minor adverse effects, making ACD-A a viable alternative for PRP treatments of pattern hair loss.
