1.Analysis of Neisseria gonorrhoeae, Chlamydia trachomatis and human papillomavirus coinfections in 111 patients with newly diagnosed syphilis at a hospital in Nanjing

Objective:To investigate the prevalence of and risk factors associated with Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT) and high-risk human papillomavirus (HPV) coinfections among patients with newly diagnosed syphilis at a hospital in Nanjing. Methods:A cross-sectional study was conducted on patients with newly diagnosed syphilis at the STD Clinic, Hospital of Dermatology, Chinese Academy of Medical Sciences in Nanjing, China from May 2023 to April 2024. Urethral, cervical or rectal swabs were collected according to the gender and types of sexual behavior of the patients. Screening tests for CT, NG and HPV infections were then performed by nucleic acid amplification testing. High-risk HPV testing was performed only in female patients. Univariate analysis was carried out to investigate risk factors associated with CT infection. The chi-square test, chi-square test with continuity correction, or Fisher's exact test was chosen based on the sample size and expected values.Results:A total of 111 newly diagnosed syphilis patients with test specimens were collected, including 71 males (64%) and 40 females (36%), and their ages ranged from 16 to 87 years; there were 82 patients with early syphilis and 29 with late latent syphilis; the number of sexual partners in the past 3 months ranged from 0 to 3. Among the 60 male patients with early syphilis, 1 (1.7%) was co-infected with NG, and 12 (20%) with CT; among the 16 men who have sex with men, 7 (43.8%) were co-infected with CT, while 5 (11.4%) of the 44 heterosexual patients were co-infected with CT, showing a significant difference in CT infection rates between the two groups ( χ2 = 5.80, P = 0.016). Among the 22 female patients with early syphilis, 1 (4.5%) was co-infected with NG, and 8 (36.4%) with CT; among the 12 female patients aged < 25 years, 8 (66.7%) were infected with CT, while none of the 7 patients aged 25-44 years or the 3 patients aged ≥ 45 years were infected with CT, showing a significant difference in CT infection rates among the 3 age groups ( P = 0.005) ; among the 16 female patients with 1 sexual partner in the past 3 months, 3 were infected with CT, while 5 were infected with CT in the 6 female patients with 2-3 sexual partners in the past 3 months, with a significant difference in CT infection rates between the two groups ( P = 0.011). Out of the 40 female patients with syphilis, 16 (40%) were co-infected with high-risk HPV; the HPV infection rates did not differ among different age groups (age groups of < 25 years, 25-44 years, ≥ 45 years: 8/14, 2/9, 6/17, respectively; P = 0.265) . Conclusion:The CT infection rate was relatively high in patients with newly diagnosed early syphilis, and a relatively high infection rate of high-risk HPV was observed in female patients with syphilis aged < 25 years.
2.A nomogram model based on clinical characteristics and immune indicators for predicting TKI treatment outcomes in CML patients

Objective To explore a nomogram model based on clinical characteristics and immune indicators for predicting the efficacy of tyrosine kinase inhibitor(TKI)against chronic myeloid leukemia(CML).Methods Clinical data was retrospectively collected from 100 patients with CML treated with TKI between January 2021 and January 2023 in Tangshan Gongren Hospital.Patients were divided into the best response and warning/treatment failure groups according to therapeutic efficacy.Factors affecting therapeutic efficacy were analyzed using logistic regression analysis,and a nomogram model was constructed.Results The best response and warning/treatment failure groups showed significant differences in red blood cell distribution width(RDW),platelet count(PLT),ELTS score,Th 1/CD4+,Treg/CD4+ratio,white blood cell count,and absolute value of natural killer cells(P<0.05).Logistic regression confirmed that the above indicators were influencing factors(P<0.05),indicating that the model was meaningful,and had a high goodness of fit as well as high predictive value.Conclusion The nomogram model constructed based on RDW,PLT,and other factors can effectively predict the thera-peutic efficacy of AKI in treating CML.

5.Regulation of Suaeda salsa in renal AIM and macrophage polarization in rats with diabetes kidney disease

Objective:To explore the regulatory effect of Suaeda salsa on renal apoprosis inhibitor of macrophage (AIM) and macrophage polarization in diabetes kidney disease (DKD) model rats.Methods:A DKD rat model was established using a high-sugar and high-fat diet combined with intraperitoneal injection of streptozotocin (STZ). The rats were divided into model group, metformin group, and Suaeda salsa high-, medium-, and low-dosage groups using a random number table method, with 8 mice in each group. The normal group was set with 8 rats in this group. The metformin group was given 85.71 mg/kg of metformin solution by gavage, while the Suaeda salsa high-, medium-, and low-dosage groups were given 3.08, 1.54, and 0.77 g/kg of Suaeda salsa suspension by gavage (raw dosage). The normal group and model group were given equal volumes of saline by gavage, once a day, administered by gavage for 12 weeks of intervention. Fasting blood glucose (FBG) and glucose tolerance (OGTT) were measured, and the area under the curve (AUC) was calculated; the levels of glycosylated hemoglobin (HbA1c) and glycosylated serum protein (GSP) were detected; urea nitrogen (BUN), serum creatinine (SCr), and 24-hour urine protein (24 hUP) were detected; HE staining was used to observe the pathological morphology of the kidneys; Masson and PAS staining were used to observe renal tissue fibrosis; Western blot method was used for detecting AIM, CD206, CD86, TNF-α and IL-10 protein levels in renal tissue; Immunofluorescence was used to detect the average optical density values of AIM, CD206, and CD86 proteins in renal tissue.Results:Compared with the model group, the FBG, OGTT AUC, HbA1c, GSP of each dosage group of Suaeda salsa decreased ( P<0.01); the expression levels of AIM, CD206, and IL-10 proteins in renal tissue increased ( P<0.01 or P<0.05), while the expression levels of CD86 and TNF-α protein significantly decreased ( P<0.01 or P<0.05); HE, Masson, and PAS staining results showed that compared with the model group, the changes in renal microvasculature and renal fibrosis of rats in each dosage group of Suaeda salsa were improved. Conclusion:Suaeda salsa may regulate AIM, promote polarization of M2 macrophages, improve the inflammatory microenvironment of macrophages, thereby lowering blood lipids of DKD rats, and improving renal pathological damage.
6.The predictive value of NK cells combined with Treg cells for TKI discontinuation in patients with chronic myeloid leukemia

Objective To investigate the dynamics of NK cells and Treg cells,as well as their potential prognostic significance in relation to TKI discontinuation among patients diagnosed with chronic myeloid leukemia(CML).Methods In this study,a total of 200 patients diagnosed with CML were randomly selected and divided into two groups:the discontinuation group(n=100)and the non-discontinuation group(n=100).Within the discontinuation group,patients were further categorized into a recurrence subgroup(n=41)and a non-recurrence subgroup(n=59).Clinical data and follow-up information of these patients were retrospectively analyzed.Logistic regression analysis was performed to investigate the impact of various variables on patient outcomes following drug discontinuation,as well as to explore independent factors influencing recurrence in these individuals.Receiver operating characteristic(ROC)curve analysis was employed to assess the predictive value of NK cells and Treg cells for TKI discontinuation outcomes.A significance level of P<0.05 was considered statistically significant.Results The proportion of patients treated with interferon in the discontinuation group was significantly higher than that in the non-discontinuation group(P<0.05).Moreover,the former group exhibited a significantly higher number of NK cells(P<0.05)and Treg cells(P<0.01)compared to the latter group.Compared to the recurrence group,there was a significant increase in the proportion of patients using interferon in the non-recurrence group(P<0.05),along with longer durations of TKI treatment and deep molecular response(DMR)duration(P<0.05).The number of NK cells and Treg cells in the non-recurrence group was significantly higher than that in the recurrence group(P<0.01).Logistic regression analysis found that the use of interferon(OR=1.25,95%CI:1.11~2.03,P<0.001),duration of DMR(OR=1.16,95%CI:1.08~1.92,P<0.05),NK cells(OR=1.64,95%CI:1.14~2.28,P<0.01),and Treg cells(OR=1.83,95%CI:1.15~2.42,P<0.01)were all influencing factors for the recurrence of patients after drug discontinuation.The results of ROC curve analysis showed that the AUC of NK cells combined with Treg cells for predicting the recurrence of TKI after discontinuation was 0.892(95%CI:0.857~0.927,P<0.001).Conclusion The frequencies of NK cells and Treg cells were significantly elevated in patients who remained recurrence-free following TKI discontinuation,highlighting the potential predictive value of combined NK cell and Treg cell analysis for drug cessation in CML patients.

The pathogenesis of depression is complex and treatment method are currently limited,and it is therefore important to clarify its pathogenesis and develop new therapeutic drugs.Hormones and hormone-modifying compounds have been used to treat depression,and their therapeutic effects are attributed to regulation of the peripheral endocrine system and their effects on non-endocrine circuits in the central nervous system.The hypothalamic-pituitary-adrenal(HPA)/hypothalamic-pituitary-thyroid(HPT)/hypothalamic-pituitary-gonadal(HPG)axes are closely related to the pathogenesis of depression.The mechanism of action of traditional Chinese medicines in the treatment of depression is extensive,and the three axes are regulated in a variety of ways.Based on new evidence of hormones and hormone operon compounds involved in the pathology and treatment of depression,this review summarizes the relevant mechanisms of current traditional Chinese medicine in the treatment of depression,to provide new ideas and references for traditional Chinese medicine research.

9.Doxycycline-moxifloxacin sequential therapy for Mycoplasma genitalium urethritis/cervicitis: a clinical observation

Objective:To investigate outcomes and safety of doxycycline-moxifloxacin sequential regimen in the treatment of Mycoplasma genitalium urethritis/cervicitis. Methods:From June 2019 to December 2020, patients with Mycoplasma genitalium urethritis/cervicitis confirmed by nucleic acid amplification testing were successively recruited at Department of Sexually Transmitted Diseases, Hospital of Dermatology, Chinese Academy of Medical Sciences, and received sequential therapy with oral doxycycline for 7 days followed by oral moxifloxacin for 7 days. Clinical and/or etiological assessment was conducted 2 to 3 weeks after the end of treatment. Fisher′s exact test was used to analyze factors influencing the treatment outcome. Results:Totally, 36 eligible subjects were enrolled, including 30 males and 6 females. Among them, 18 (50%) patients completed post-treatment etiological assessment, which showed that 12 achieved microbiological cure, and treatment failures occurred in 6; another 18 patients achieved clinical cure. The overall response rate to doxycycline-moxifloacin sequential therapy was 83.3% (30/36, 95% confidence interval[ CI]: 70.5%, 96.1%) . The treatment outcome showed no significant association with the patients′ age, gender, marital status, number of sexual partners in the past 1 month, history of sexually transmitted diseases, history of antibiotic use in the past 1 month, or co-infections (all P > 0.05) . Conclusion:The efficacy of doxycycline-moxifloacin sequential regimen is limited in the treatment of Mycoplasma genitalium infections in Nanjing area, and clinicians should be alerted to the possibility of treatment failure in clinical practice.
10.Progress in research on the relationship between Helicobacter pylori infection and cardiovascular diseases and its risk factors

In recent years, the high infection rate of Helicobacter pylori and its impact on cardiovascular disease have attracted public attention. It may directly affect coronary heart disease, stroke, etc. through various mechanisms such as inflammation, immune response, and damage to endothelial cells. It could also play an important role in the formation of cardiovascular disease risk factors such as atherosclerosis, hypertension, hyperhomocysteinemia, and dyslipidemia. However, domestic and international research results are still inconsistent, and a large number of experiments are still required to confirm it to take effective measures to control the incidence of cardiovascular disease. This article reviews the prevalence of Helicobacter pylori and cardiovascular disease, the interaction mechanisms and the status of relevant domestic and international researches.
