1.Current situation and countermeasure analysis of human resources allocation in urban non-profit nursing institutions

Objective:To understand the status of human resources allocation of non-profit pension institutions in representative cities of our country, analyze the existing problems and put forward countermeasures and suggestions, in order to provide reference for improving the service quality of non-profit pension institutions in cities.Methods:From October to December 2020, 5 sub-provincial capitals and 1 municipality directly under the central regions were selected by random sampling in Northeast, Northwest, Southeast, Southwest and Central Plains, and 60 non-profit pension institutions were selected as research objects by purpose sampling, and a questionnaire survey was conducted on the human resources allocation of pension institutions.Results:Among the elderly care institutions investigated, 48 were registered medical and nursing care institutions. There were 25 elderly care institutions with medical beds, accounting for 41.7%(25/60); there were 39 nursing institutions with doctors, accounting for 65.0%(39/60); there were 53 nursing institutions with nurses, accounting for 88.3%(53/60); from the perspective of relative indicators, the number of medical staff per bed in 60 nursing institutions was 0.12, and the number of medical staff per bed in medical nursing institutions was 0.14.Conclusions:The personnel allocation of non-profit elderly care institutions is not reasonable, and there is a lack of professional multidisciplinary professional teams to provide elderly care services. The personnel allocation and demand of non-profit pension institutions do not match, and the supply and application of human resources are unbalanced. Non-profit pension institutions should continue to promote the combined development of medical and nursing care to meet the diversified needs of elderly people in pension institutions. It is suggested that relevant departments jointly formulate the human resources allocation standards for elderly care institutions, and strengthen the practice access, grading and assessment of medical and nursing professionals.

3.Adrenal intravascular large B-cell lymphoma: report of 1 case and review of literature

Objective:To investigate the diagnosis and treatment of intravascular large B-cell lymphoma (IVLBCL).Methods:The clinical data of 1 patient with adrenal IVLBCL in Zhongda Hospital Southeast University in May 2020 were retrospectively analyzed, and the relevant literature was reviewed.Results:The patient was an elderly male with recurrent fever of unknown cause at initial stage, and was finally diagnosed as adrenal IVLBCL based on the results of laboratory, imaging and adrenal biopsy at different stages. After multiple courses of R-COP in combination with Bruton tyrosine kinase (BTK) inhibitor, the patient achieved complete remission.Conclusions:IVLBCL is rare and it lacks specific clinical symptoms. PET-CT and pathological biopsy can help in the diagnosis of it. R-COP combined with BTK inhibitor is effective in the treatment of biphenotype IVLBCL.
4.Application of uterine incision suture in multiple segment shrinkage in cesarean section

Objective To explore the application value of uterine incision suture in multiple segment shrinkage in cesarean section.Methods 85 pregnants with uterine scar who taken cesarean section were selected.In the single month,the pregnants were included in the observation group(43 cases):after pruning scars,uterine lower segment weak muscle tissue by wrinkling suture to reinforce the thickness of the lower segment again from both sides of the suture,intermediate knotted suture the uterine incision.In the bimonth,the pregnants were included in the control group(42 cases):after pruning scars by conventional suture incision of lower uterine segment.The duration of vaginal bleeding time after operation ,postoperative 42 days ,3 months ultrasonography of uterine incision healing were observed.Results The postoperative vaginal bleeding volume of the observation group was less than that of the control group[(480.55 ±53.75)mL vs.(550.14 ±50.45)mL],the bleeding time was less than that of the control group[(21.31 ±6.78)d vs.(30.45 ±5.16)d],the differences were statistically significant(t =3.65,2.28,all P <0.05).March B -ultrasonography uterine incision after surgery,the good healing rate of the observation group was 95.35%,which of the control group was 78.57%,the difference was statistically significant(χ2 =4.35,P <0.05). Conclusion Incision under section shrinkage joint is conducive to the incision structure and muscle layer mechanical recovery and can reduce the amount of vaginal bleeding and time and defect healing of incision morphology.
5.A Clinical Observation of Daidzein Capsules for Climacteric Syndrome

Objective To observe the clinical efficacy of Daidzein Capsules(DC) on female climacteric syndrome.Methods Sixty eight patients with climacteric syndrome were randomized into DCgroup(n=37) and Gengnian'an control group (n=31).The treatment group and the control group were orally administered DC 150 mg and Gengnian'an Tablet 4.8 g,three times a day,respectively.The course of treatment lasted three months.Results The total effective rate in DC group was 91.89 %,which was superior to that in the control group.Time of drug becoming effective was shorter in DC group than that in the control group.After treatment,the level of E2 was significantly increased and the content of LH decreased significantly in DC group.Conclusion DC is safe and effective for the treatment of female climacteric syndrome.
