1.Isolation,identification and genetic analysis of a three-lineage NADC30 PRRSV recombinant strain

In June 2018,sows at a pig farm in Jiamusi,Heilongjiang Province,suffered a large num-ber of miscarriages,and subsequently weaned piglets at the farm began to show persistent high fe-ver symptoms at around 35 days of age,with some pigs having a fever of more than 41.5 ℃.In or-der to determine the cause of this outbreak,63 clinical samples from this farm were tested.The re-sults showed that 60 out of 63 samples were positive for the porcine reproductive and respiratory syndrome virus(PRRSV)antigen.Subsequently,PRRSV antigen-positive plasmid was transfected into African green monkey embryonic kidney cells(Marc-145),and after three generations of blind transmission,indirect immunofluorescence assay(IFA)was performed.The results showed that one PRRSV strain,named HLJ38,was successfully isolated.Then the whole genome of HLJ38 strain was sequenced and then analyzed in detail by bioinformatics software.Sequence analysis showed that there were three deletions of 131 amino acids(323-433 aa,483 aa and 504-522 aa)in the derived sequence of Nsp2 gene of HLJ38 strain,which was consistent with the molecular ge-netic marker of NADC30 PRRSV.The phylogenetic tree analysis showed that HLJ38 and NADC30 PRRSV in GenBank belong to lineage 1 subgroup,and the nucleotide homology of HLJ38 and NADC30 PRRSV in GenBank was only 85.2%and 84.6%.Recombinant analysis showed that HLJ38 was a recombinant NADC30-like PRRSV,and the recombinant gene fragments were de-rived from multiple strains,among which the fragment of 1-201 nt was provided by VR2332 strain and fragment of 6 641-8 061 nt derived from the HP-PRRSV strain.In summary,the re-sults showed that the outbreak in this pig farm may be caused by the recombination of PRRSV strains among different lineages,and the recombinant circulating strains not only have certain pathogenicity but also suggest that the existing commercial vaccines provide limited cross-protec-tion against them.Recombination between different lineages increases the genetic diversity of PRRSV and aggravates the difficulty of prevention and control of PRRS in pig farms.Therefore,it is necessary to continuously monitor the epidemic dynamics of PRRSV in pig farms and take effec-tive measures in time to curb the spread of PRRS.
2.Clinical Evaluation of the Short-term Effects Assessment of the Condyle-stimulated Headband Bone Conduction Hearing Aids on Conductive,Mixed,and Sensorineural Hearing Loss

Objective To study the hearing intervention effects of the Shokz condyle-stimulated headband bone-conduction hearing aid on patients with conductive,mixed,and sensorineural hearing loss and to explore its clinical application prospects.Methods A total of 55 patients with hearing loss(age 18~82)participated in the study.Among them,9 had conductive hearing loss,15 had sensorineural hearing loss,and 31 had mixed hearing loss.Their bilat-eral bone conduction pure tone thresholds at 0.5,1,2,and 4 kHz were all ≤60 dB HL.The patients were fitted with the condyle-stimulated headband bone-conduction hearing aid.Hearing thresholds in sound field,single-syllable speech recogni-tion scores in quiet,and sentence recognition thresholds in quiet were assessed before fitting and on the day of 14±2 after fitting to compare differences in results.The effectiveness of the hearing aids on the day of 14±2 after fitting was also eval-uated using the IOI-HA questionnaire.Results After wearing the bone-conduction hearing aid,the average hearing thresh-old and sentence recognition threshold of the patients decreased significantly compared with before fitting(the average hear-ing threshold:56.5±8.2 dB HL before fitting,39.3±4.9 dB HL on the day of 14±2 after fitting;sentence recognition threshold:48.6±9.7 dB HL before fitting,34.3±5.6 dB HL on the day of 14±2 after fitting),and the difference was statistically significant(P<0.001).The single-syllable speech recognition score before fitting was 29.8%±11.4%,and on the day of 14±2 after fitting,it was 72.4%±14.4%,the difference was statistically significant(P<0.001).The av-erage total score of the IOI-HA questionnaire was 29.0±3.8 points.Conclusion Condyle-stimulated headband bone-con-duction hearing aids can significantly improve the hearing and speech recognition ability of patients with conductive,mixed and sensorineural hearing loss whose bone conduction pure tone thresholds at 0.5~4 kHz were ≤60 dB HL.It may poten-tially improve the quality of life for patients with hearing loss and holds substantial clinical application value.

5.Analysis of related factors of nursing dependence in bladder cancer patients with urinary diversion and abdominal wall stoma

Objective To investigate the related factors of nursing dependence in bladder cancer patients with urinary diversion and abdominal wall stoma.Methods The clinical data of 120 patients with bladder cancer undergoing abdominal ostomy with urinary diversion in the hospital from June 2020 to June 2022 were retrospectively analyzed.The self-esteem scale,the civilian version of the Post-traumatic Stress Disorder Scale,the Personal Sense of Control Scale,and the Nursing Care Dependence Scale were used to evaluate the self-esteem level,post-traumatic stress disorder level,personal sense of control level and nursing dependence degree of patients,and carry out univariate and multivariate regression analysis on the influencing factors of nursing dependence in patients with bladder cancer urinary diversion and abdominal stoma.Results The total score of nursing dependence in 120 bladder cancer patients with urinary diversion and abdominal wall stoma was 56.95±7.94.The lowest score was activity,followed by excretion and cleanliness.Univariate analysis showed that there were statistically significant differences in the nursing dependence scores of patients with different age,marital status,educational level,work status,per capita monthly income,and comorbidities(P<0.05);the patients'self-esteem,the civilian version of the Traumatic Stress Disorder Scale,and the Personal Sense of Control Scale scores were 25.08±2.59,46.04±5.72,24.18±2.95,respectively;after multi-factor a-nalysis,it can be seen that age,education level,comorbidities,self-esteem level,post-traumatic stress obstacles and personal control finally entered the regression equation as factors influencing dependence on care for pa-tients with urinary diversion abdominal wall stoma for bladder cancer(P<0.05).Conclusion Bladder cancer patients with urinary diversion abdominal wall stoma have the highest degree of dependence on nursing care for mobility,excretion and cleaning.Age,education level,complications,self-esteem,post-traumatic stress dis-order,and personal sense of control are related factors that affect the degree of dependence on nursing care.
6.Epidermal growth factor receptor signaling pathway activation mediating inflammatory damage in central nervous system

Cerebral ischemia,brain tumor,and spinal cord injury (SCI) are the central nerve disease which usually cause irreversible,severe neurologic deficits or death.Inflammation plays a key role in the associated secondary damage after central nervous system (CNS) injury.Epidermal growth factor receptor (EGFR) excessive activation has been closely implicated in the initiation and progression of inflammation.Previous studies have shown that through depressing CNS inflammation,EGFR inhibitors may play a role in the therapeutic treatment of CNS disorders.We mainly reviewed the mechanisms of EGFR signaling pathway activation mediating inflammatory damage in CNS.
7.Acceptability and associated factors of pre-exposure prophylaxis based on different sexual purpose among human immunodeficiency virus male sero-discordant couples

Objective To study the acceptability and associated factors of pre-exposure prophylaxis (PrEP) based on different sexual purpose among human immunodeficiency virus (HIV) male serodiscordant couples.Methods HIV-negative female spouses of HIV-positive male in Guigang,Guiping and Pingnan county in Guangxi Zhuang Autonomous Region were recruited by the existing epidemic database and the method of cluster random sampling.A face-to-face questionnaire survey was conducted during June 2013 to November 2016.Information including the general condition and acceptability of PrEP was surveyed.x2 test and Logistic regression were used to analyze the data.Results A total of 526 qualified questionnaires were collected.The intention rate of PrEP based on physiological sexual purpose was 66.9% (352/526),the intention rate of PrEP based on fertility purpose was 93.3% (491/526).The difference was statistically significant (x =139.48,P<0.01).The reasons of refusal included worrying about its safety,effectiveness poor confidentiality and low self-assessment risk of infection.Logistic analysis showed that the associated factors of the intention rate of PrEP based on physiological sexual purpose included cognition of PrEP (OR=0.70,P=0.014),the number of lifetime partners (OR=0.55,P<0.01),frequency of sexual behavior (OR=1.33,P=0.016),willingness of condom use (OR=0.16,P<0.01),infection risk self-assessment (OR =0.22,P<0.01),male viral load (OR=1.25,P=0.035),male CD4+ T lymphocyte count (OR=0.37,P<0.01) and male usefulness of medication or not (OR=1.59,P<0.01).The influencing factors of the intention rate of PrEP based on physiological sexual purpose included with or without children (OR =1.70,P<0.01),cognition of PrEP (OR=0.72,P=0.026),willingness of using condom (OR=0.18,P<0.01),infection risk self-assessment (OR=0.14,P<0.01) and male CD4+ T lymphocyte count (OR=0.45,P<0.01).Conclusions The acceptability of PrEP based on fertility purpose is significantly higher than that on the physiological purpose.It is more feasible to conducting clinical trials of PrEP in peri-conceptional period HIV serodiscordant couples.The main influencing factors include the uninfected individual behavior characteristics and infection features of HIV infected men,and have nothing to do with the general social demographic characteristics.
8. Investigation and analysis impact factors of distress in HIV-infected pregnant women

Objective:
To explore the psychological distress status of HIV-infected pregnant women and analyze the possible influencing factors.
Methods:
A total of 483 HIV-infected pregnant women were enrolled for this study by a cluster random sampling method from Sept. 2014 to Apr. 2017. Participants completed questionnaires including Distress Thermometer (DT), Berger HIV Stigma Scale (BHSS), HIV/AIDS Stress Scale (SS-HIV), Social Support Rating Scale (SSRS) and general questionnaire.
Results:
The detection rate of psychological distress was 68.1%, the detection rates of moderate, severe and extreme psychological pain were 49.7%, 17.6% and 0.8% respectively. The detection rate of continuing pregnancy (75.2%) was higher than the termination pregnancy (56.4%), the difference was statistically significant (χ2=18.44,
9.Effect of cerebral hemodynamic changes on white matter damage in premature infant with patent ductus arteriosus.

OBJECTIVE:
To investigate the effect of cerebral hemodynamic changes on white matter damage in premature infant with patent ductus arteriosus(PDA).
METHODS:
A total of 106 premature infants were enrolled in the study, including 35 PDA infants with hemodynamic changes (hsPDA group), 35 PDA infants without hemodynamic changes (non-hsPDA group) and 36 non-PDA infants (control group). Serum level of neuron-specific enolase (NES) was detected and craniocerebral ultrasound examination was performed on d3, d7 and d14 after birth. The correlation between blood flow rate of PDA and gray scale value of lateral ventricle was analyzed.
RESULTS:
Gray scale values of lateral ventricle and serum levels of NES in hsPDA group were higher than those in control group on d3, d7 and d14 (P<0.01), but no significant difference was observed between non-hsPDA group and control group (P>0.05). There was a positive correlation between the blood flow rate of PDA and gray scale value of lateral ventricle (r=0.876, P<0.01) in premature infants.
CONCLUSION
Patent ductus arteriosus with hemodynamic changes is closely related to white matter damage in premature infants, and early intervention is necessary.

10.Regular follow-up on physical growth and mental development of small and appropriate for gestational age preterm infants

Objective To investigate the physical and mental development of small and appropriate for gestational age preterm infants in their early life. Methods This study recruited 220 preterm infants, who were discharged from our hospital and visited preterm following-up clinic at regular intervals from February 2009 to December 2012. All of those infants were divided into two groups based on whether their birth weight below 10th percentile for their gestational ages or not. Weights, lengths and head circumferences were measured up to seventh month age adjusted by gestational age. Meanwhile, mental tests were conducted by the professional staffs working on the children developmental assessment at their adjusted months of 5th, 6th or 7th. All of physical and mental scores were compared between the two groups. Results The SGA group was statistically less than the AGA group on the Z-score of weights from the ifrst to sixth month adjusted by gestational age (P<0.05). The difference disappeared on the seventh adjusted month age (P>0.05). The SGA group was statistically less than the AGA group on the Z-score of lengths from the ifrst to iffth month adjusted by gestational age (P<0.05). The difference disappeared on the sixth and seventh adjusted month age (P>0.05). The SGA group was statistically less than the AGA group on the Z-score of head circumferences from the ifrst to seventh month adjusted by gestational age (P<0.05). The SGA babies scored statistically less than the AGA babies with a mean development quotient score of 96.7 and 102.9, respectively (P<0.05). The scores of movement, cognitive, language in the SGA group were statistically less than those in the AGA group(P<0.05). Conclusions Preterm SGA could achieve satisfactory weight catch-up gain, with a decreasing difference from preterm AGA while they were getting older. But the length catch-up growth of preterm SGA seemed unsatisfactory with a big differece from preterm AGA. There was the worst catch-up on head circumference in those preterm SGA, backward in mental development, particularly in their movement, cognitive and language capacity.
