Objective To explore the feasibility,effectiveness,and safety of balloon catheter-assisted total hepatic vascular exclusion.Methods We designed and manufactured an endovascular catheter with three lumens and double balloons,which can be inserted into the retrahepatic inferior vena cava through the femoral vein.The superior and inferior vena cava of the liver can be blocked by filling balloon,and the total hepatic vascular exclusion was achieved by combining with pringle method.In animal experiments,total hepatic vascular exclusion was performed by balloon catheter-assist method(experimental group)or traditional methods(control group),and the complete time was compared between the two groups.Blood flow blocking effect was observed by angiography and incision of retrahepatic inferior vena cava under direct vision.The complications were recorded.Results Total hepatic vascular exclusion was successfully completed in both groups.The completion time in the experimental group was significantly shorter than that in the control group([12.5±1.2]min vs.[35.8±4.9]min,P<0.05).CT angiography,DSA,and direct vision of blood vessels all confirmed the effectiveness of balloon catheter-assisted hepatic blood flow exclusion.No catheter displacement,balloon rupture,or air embolism occurred.Conclusion The balloon catheter-assisted hepatic total vascular exclusion is simpler and more feasible than traditional method.

Objective To re-evaluate systematic reviews on the application of virtual reality(VR)technology in limb rehabilitation for stroke patients,providing evidence to support the implementation of VR.Methods Systematic searches were conducted in Web of Science,PubMed,CINAHL,Cochrane Library,Embase,CNKI,CBM,WanFang,and VIP databases from their inception until March 2025,identifying systematic reviews/meta-analyses investigating VR for limb rehabilitation in stroke patients.2 researchers independently screened the literature.Methodological quality was evaluated using the Assessment of Mutiple System Reviews 2(AMSTAR 2)tool;reporting quality was evaluated according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines;evidence quality was graded using the Grades of Recommendations Assessment,Development,and Evaluation(GRADE)system;literature overlap was assessed using the Corrected Covered Area(CCA)method.Results A total of 15 articles were included.According to AMSTAR 2,totally 3 reviews were of moderate quality,while the remainder were rated as low or very low quality.PRISMA scores ranged from 19.5 to 27.0 points.Based on GRADE,a review was classified as high quality,6 as moderate quality,and the remainder as low or very low quality.The CCA was 5.3％,indicating slight overlap.Conclusion VR contributes to the improvement of indicators such as upper limb motor function/mobility and lower limb balance in stroke patients.However,methodological limitations,including weak control of bias and inadequate handling of heterogeneity among included studies,restrict the overall quality of evidence.Further standardized,high-quality studies are needed to provide more reliable evidence to support clinical decision-making.

Myelin is an essential structure that facilitates rapid saltatory conduction in the nervous system. Discrepancies in myelin microstructure are a hallmark of numerous neurological disorders, rendering the assessment of myelin integrity and content an indispensable tool in clinical diagnostics and neuroscience research. Extensive research has been dedicated to scrutinizing its biochemical makeup and morphology under normal, pathological, and experimental conditions over the years. In this review, we present an updated summary of the myelin sheath's structure, composition, and developmental trajectory. We systematically enumerate and contrast eight prevalent myelin staining techniques across dimensions of sensitivity, specificity, and resolution, delving into their underlying staining principles. With an initial application of myelin histology on the mouse demyelination model, our review accentuates the accurate delineation of myelination and the microstructural analysis of the myelin sheath. Such insights are anticipated to significantly contribute to the evaluation and understanding of white matter pathologies.

OBJECTIVE: Therapeutic angiogenesis has become a promising approach for treating ischemic heart disease (IHD). The present study aims to investigate the effects of Qishen Granule (QSG) on angiogenesis in myocardial ischemia (MI) and the potential mechanism. METHODS: In vivo study was conducted on rat model of myocardial infarction. QSG was performed daily at a dose of 2.352 g/kg for four weeks. Cardiac function was assessed by echocardiogram and pro-angiogenic effects were evaluated by Laser Doppler and CD31 expression. Oxygen-glucose deprivation (OGD) was applied in cultured human umbilical vein endothelial cells (HUVECs). Cell viability, wound healing and tube formation assay were used to test functions of HUVECs. ELISA and Western blots were used to assess protein expressions of bone morphogenetic protein 2-delta-like 4-notch homolog 1 (BMP2-Dll4-Notch1) signaling pathway. RESULTS: The results showed that QSG improved heart function, cardiac blood flow and microvessel density in myocardial ischemic rats. In vitro, QSG protected HUVECs by promoting the cell viability and tube formation. QSG upregulated bone morphogenetic protein-2 (BMP2) and downregulated delta-like 4 (Dll4) and notch homolog 1 (Notch1) expressions both in rats and HUVECs. CONCLUSION QSG protected against MI by promoting angiogenesis through BMP2-Dll4-Notch1 pathway. BMP2 might be a promising therapeutic target for IHD.

OBJECTIVE To understand the clinical characteristics of patients with Mycoplasma pneumoniae(MP)infection,and to explore the related factors of mixed infection,to provide theoretical support for the diag-nosis and treatment of MP infection.METHODS The clinical data of patients diagnosed with MP infection in the 305th hospital of PLA from Dec.2023 to Apr.2024 were collected,and the clinical characteristics of patients in-fected with MP were analyzed.Additionally,the impacts of MP mixed infections with other pathogens on the se-verity of the disease were compared,while the relevant influencing factors of MP mixed with other pathogens were analyzed by the logistic regression model.RESULTS A total of 114 patients with MP infection were collected,both genders had 57 cases(50.00％,respectively),60(52.63％)were adults,73(64.04％)had simple MP infection and 41(35.96％)had mixed infection.The clinical manifestations of MP infection primarily included fever and cough,with most fevers being low to medium intensity.Additionally,the biochemical indexes were predominantly within normal ranges,however,the interleukin 6(IL-6)level was relatively high.The abnormalities of chest X-ray examination accounted for 84.21％of the cases.In mixed infection cases,fever,dizziness,fatigue,headache and muscle pain were more prevalent than single MP infection cases(P＜0.05),and CRP level elevated in the mixed infection group(P=0.001),whereas white blood cell level and lymphocyte count decreased compared to the single MP infection group(P＜0.05).Multivariate logistic regression analysis showed that fever(OR=4.112,95％CI:1.046-16.161)and above 46 years old(OR=8.555,95％CI:1.394-52.493)were the rele-vant influencing factors for MP mixed with other pathogens infection.CONCLUSION Clinical features of MP-infected patients are atypical,but IL-6 levels are relatively high.MP is often mixed with viral infections,and fever and above 46 years old are the risk factors for mixed MP infection.

OBJECTIVE To understand the epidemiological characteristics and change of drug resistance of Klebsiel-la pneumoniae strains from a hospital so as to adjust the use of antibiotics and control the transmission of hospital-associated infection caused by the K.pneumoniae.METHODS The K.pneumoniae strains were isolated from the patients who were hospitalized in The 305 Hospital of PLA from 2018 to 2022.The data of drug susceptibility tes-ting were exported by using WHONET 5.6,the clinical data and changing trend of drug resistance were retro-spectively analyzed with the use of SPSS 25.0 software.RESULTS Totally 9157 strains of bacteria were isolated from the patients who were hospitalized from 2018 to 2022,1502(16.40％)of which were K.pneumoniae,and the isolation rate showed a downward trend(x2=15.054,P＜0.001).Sputum(77.16％)was the major type of specimen;the strains were mainly isolated from intensive care unit(55.26％).The patients aged more than 79 years old were dominant,accounting for 65.58％.The result of drug susceptibility testing showed that there were significant differences in the drug resistance rates to the commonly used antibiotics from 2018 to 2022(P＜0.05).The drug resistance rates to aztreonam,ertapenem and tigecycline showed upward trends with the years(P＜0.05),while the drug resistance rates to the rest of drugs such as cefoperazone-sulbactam,ceftazidime and amika-cin showed downward trends(P＜0.05).CONCLUSIONS There is significant difference in the clinical distribution of the K.pneumoniae strains in recent 5 years.The key populations and departments should be attached great im-portance to.The drug resistance rates of the K.pneumoniae strains to many types of antibiotics show downward trends,but the situation of drug resistance is still serious.It is necessary to continue to implement the prevention and control policies for drug resistance and reasonably use antibiotics based on the result of drug susceptibility testing.

ObjectiveTo observe the efficacy and safety of Uyghur medicine Yangxin Dawayimixike Honey Paste （养心达瓦依米西克蜜膏， YDMHP） in the treatment of stable angina pectoris （SAP） of qi stagnation and blood stasis syndrome. MethodsA randomized ， double-blind， positive-controlled，multi-center clinical trial was conducted， in which 370 patients with SAP of qi stagnation and blood stasis syndrome were randomly divided into treatment group（279 cases）and control group（91cases）at a ratio of 3∶1. The treatment group was orally administered with YDMHP， 3 g each time， and placebo of Xuefu Zhuyu Capsule （血府逐瘀胶囊）， 2.4 g each time， while the control group was treated with Xuefu Zhuyu Capsule， 2.4 g each time， and placebo of YDMHP， 3 g each time， both twice a day for a course of 12 weeks. The primary outcome was the effect of angina pectoris symptom. The secondary outcomes include single angina symptom scores such as number of attacks， duration of attacks， pain intensity and usae of nitroglycerin scores， the total angina symptom score before and after the treatment， the usage of nitroglycerin， the exercise duration in treadmill exercise test （TET） and the Duck treadmill score among patients，the scores of Seattle Angina Questionnaire （SAQ） on five dimensions including physical limitations， anginal stability， anginal frequency， treatment satisfaction， and disease perception， and efficacy of TCM syndrome and of each single TCM symptom after treatment. The safety were evaluated by examine blood routine， urine routine， liver and kidney function， fasting blood sugar， electrocardiogram， adverse events. ResultsThe total effective rate of angina symptom in the treatment group was 71.69% （200/279）， significantly higher than 51.64% （47/91） in the control group （P＜0.01）. The curative and markedly effective rate of TCM syndrome in the treatment group was 53.05% （148/279）， which was significantly higher than 25.27% （23/91） in the control group （P＜0.01）. After treatment， scores of the number as well as duration of angina attacks and pain severity， the total score of angina symptoms， and the usage of nitroglycerin significantly decreased in both groups， and more changes were seen in the treatment group than in the control group； the scores of physical limitations， anginal stability， anginal frequency， treatment satisfaction， and disease perception in both groups significantly increased， and more improvement were shown in the experimental group regarding the anginal stability， anginal frequency and treatment satisfaction （P＜0.05 or P＜0.01）. The effects of chest pain， chest tightness， palpitation， shortness of breath and fatigue in experimental group were significantly higher than those in control group （P＜0.05 or P＜0.01）. There was no significant difference in the exercise duration of treadmill test and Duke score among patients between the two groups either before or after treatment （P＞0.05）. Adverse events occurred in 66 cases （23.66%） of the experimental group and 16 cases （17.58%） of the control group， with no statistical significance between the two groups （P＞0.05）. ConclusionThe Uyghur medicine YDMHP can effectively improve symptoms of angina pectoris， reduce the number， duration， and intensity of attacks， decrease the dosage of nitrogly-cerin and improve the individual TCM symptoms and has good safety in the treatment of SAP patients of qi stagnation and blood stasis.
