1.Inhibitory effect and mechanism of asiaticoside on hypertrophic scar formation in rabbit ears

OBJECTIVE To explore the inhibitory effect and its mechanism of asiaticoside on hypertrophic scar formation in rabbit ears based on the Wnt/ β -catenin pathway. METHODS The hypertrophic scar model in rabbit ears was established. Model rabbits were divided into the model group (normal saline), asiaticoside low-dose group (12 mg/kg), asiaticoside high-dose group (24 mg/kg), and positive control group (tri amcinolone acetonide acetate 40 mg/kg); additionally, a non-modeled control group (normal saline) was set up. There were 8 rabbits in each group. Rabbits in each group were injected with the corresponding drug solution/normal saline at the base within the scar area once daily for 28 consecutive days. After the last medication, the appearance of hypertrophic scars in rabbit ears was observed, and the hypertrophic scar index was measured; the levels of inflammatory factors [interleukin-6 (IL-6) and IL-10 ] in hypertrophic scar tissue of rabbits were determined; histopathological morphology of hypertrophic scar tissues in rabbit ears was observed, and the protein expression levels of collagen type Ⅰ (COLⅠ), COLⅢ, Wnt4 and β -catenin in the hypertrophic scar tissues were detected. RESULTS Compared with control group, in the model group, the symptoms of hypertrophic scars were observed, the stratum corneum was thicker, and there was abnormal accumulation and disorganized arrangement of collagen. The hypertrophic scar index, IL-6 level, fibroblast density, area density of collagen fibers, mean optical density values of COLⅠ and COLⅢ, as well as the expression levels of Wnt4 and β -catenin proteins, were all significantly elevated ( P <0.05), while the level of IL-10 was significantly decreased ( P <0.05). Compared with the model group, the hypertrophic scars in asiaticoside low- and high-dose groups were significantly softened, with a thinned stratum corneum, and the above quantitative indicators were all significantly reversed ( P <0.05). CONCLUSIONS Asiaticoside can inhibit the formation of hypertrophic scars in rabbit ears and alleviate the inflammatory response in scar tissue. Its mechanism of action may be related to the inhibition of activation of the Wnt/ β -catenin pathway.

Objective:To retrospectively analyze the treatment status of tyrosine kinase inhibitors (TKI) in newly diagnosed patients with chronic myeloid leukemia (CML) in China.Methods:Data of chronic phase (CP) and accelerated phase (AP) CML patients diagnosed from January 2006 to December 2022 from 77 centers, ≥18 years old, and receiving initial imatinib, nilotinib, dasatinib or flumatinib-therapy within 6 months after diagnosis in China with complete data were retrospectively interrogated. The choice of initial TKI, current TKI medications, treatment switch and reasons, treatment responses and outcomes as well as the variables associated with them were analyzed.Results:6 893 patients in CP ( n=6 453, 93.6%) or AP ( n=440, 6.4%) receiving initial imatinib ( n=4 906, 71.2%), nilotinib ( n=1 157, 16.8%), dasatinib ( n=298, 4.3%) or flumatinib ( n=532, 7.2%) -therapy. With the median follow-up of 43 ( IQR 22-75) months, 1 581 (22.9%) patients switched TKI due to resistance ( n=1 055, 15.3%), intolerance ( n=248, 3.6%), pursuit of better efficacy ( n=168, 2.4%), economic or other reasons ( n=110, 1.6%). The frequency of switching TKI in AP patients was significantly-higher than that in CP patients (44.1% vs 21.5%, P<0.001), and more AP patients switched TKI due to resistance than CP patients (75.3% vs 66.1%, P=0.011). Multi-variable analyses showed that male, lower HGB concentration and ELTS intermediate/high-risk cohort were associated with lower cytogenetic and molecular responses rate and poor outcomes in CP patients; higher WBC count and initial the second-generation TKI treatment, the higher response rates; Ph + ACA at diagnosis, poor PFS. However, Sokal intermediate/high-risk cohort was only significantly-associated with lower CCyR and MMR rates and the poor PFS. Lower HGB concentration and larger spleen size were significantly-associated with the lower cytogenetic and molecular response rates in AP patients; initial the second-generation TKI treatment, the higher treatment response rates; lower PLT count, higher blasts and Ph + ACA, poorer TFS; Ph + ACA, poorer OS. Conclusion:At present, the vast majority of newly-diagnosed CML-CP or AP patients could benefit from TKI treatment in the long term with the good treatment responses and survival outcomes.
3.Observational study of chronic myeloid leukemia Chinese patients who discontinued tyrosine kinase inhibitors in the real-world

Objective:This study aimed to observe whether the treatment-free remission (TFR) of second-generation tyrosine kinase inhibitors (TKI) in chronic myeloid leukemia (CML) is better than imatinib (IM) .Methods:The clinical data of 274 CML patients who discontinued treatment and with complete clinical data were retrospectively studied from June 2013 to March 2021. Using both univariate and multivariate Cox proportional hazards regression models, risk factors influencing TFR outcomes after drug withdrawal in CML patients were assessed.Results:A total of 274 patients were enrolled, 140 patients were women (51.1%) , with a median age of 48 (9-84) years at the time of TKI discontinuation. Prior to TKI discontinuation, 172 (62.8%) patients were treated with IM, and 102 (37.2%) had received second-generation TKI treatment, including 73 patients who had shifted from IM to a second-generation TKI and 29 patients who used second-generation TKI as the first-line treatment. The rationale for converting to a second-generation TKI are as follows: 37 patients aimed deep molecular response (DMR) to achieve TFR, seven patients changed due to IM intolerance, and 29 patients changed because of failure to achieve the optimal treatment response. The use of the last type of TKI included 96 patients (94.1%) with nilotinib, three patients (2.9%) with dasatinib, and two patients (2%) with flumatinib, including one patient who changed to IM due to second-generation TKI intolerance. No statistical differences were found in the median age at diagnosis and TKI discontinuation, sex, Sokal score, IFN treatment before TKI, median time of TKI treatment to achieve DMR, and the reasons for TKI discontinuation between the second TKI and IM ( P>0.05) .The median cumulative treatment time of TKI (71.5 months vs 88 months, P<0.001) , the last TKI median treatment time (60 months vs 88 months, P<0.001) , and the median duration of DMR (58 months vs 66 months, P=0.002) were significantly shorter in the second-generation TKI compared with IM. In the median follow-up of 22 (6-118) months after TKI discontinuation, 88 patients (32.1%) had lost their MMR at a median of 6 (1-91) months; of the 53 patients (60.2%) who lost MMR within 6 months, the overall TFR rate was 67.9%, and the cumulative TFR rates at 12 and 24 months were 70.5% and 67.5%, respectively. Withdrawal syndrome occurred in 26 patients (9.5%) . For patients who restarted TKI treatment, 72 patients (83.7%) achieved DMR again at a median treatment of 4 (1 to 18) months. The univariate analysis showed that the TFR rate of patients treated with second-generation TKI was significantly higher than those who were treated with IM (77.5% vs 62.2%, P=0.041) . A further subgroup analysis found that the TFR rate of the second-generation TKI patients was significantly higher than those treated with IM (80.8% vs 62.2%, P=0.026) . No significant difference was found in the second-generation TKI used as the first line treatment compared with those who were treated with IM (69.0% vs 62.2%, P=0.599) . The multivariate analysis results showed that second-generation TKI treatment was an independent prognostic factor affecting TFR in patients who discontinued TKI ( RR=1.827, 95% CI 1.015-3.288, P=0.044) . Conclusion:In the clinical setting, more CML patients rapidly achieved TFR using second-generation TKI than IM treatment.
4.Allele genetypes and homology analysis of MSP-1 and CSP gene of Plasmodi-um vivax in Shandong Province

Objective To analyze the genotypes and homology of MSP?1 and CSP gene of Plasmodium vivax in Shandong Province,so as to provide the evidence for case traceability. Methods A total of 12 blood samples were collected from P. vivax?infected cases in Shandong Province in 2011. Parasite genomic DNA was extracted. Primers were designed according to MSP?1 and CSP gene sequences of P. vivax. Then Nested PCR,enzyme digestion,sequencing and sequence alignment,and homolo?gous analysis were performed. Results The MSP?1 gene of all the 12 samples from P. vivax?infected cases were detected with a 470 bp PCR amplification band,and 350 bp and 120 bp enzyme digestion fragments,which were identified as type Sal?1. An analysis of phylogenetic tree of MSP?1 gene showed that the sequences of 9 indigenous case samples in Shandong Province were located in the same branch,one case sample infected from India was located in the same branch with India strains. All the 12 P. vivax?infected samples covered GDRA(D/A)GQPA sequences in CSP gene,which were identified as type PV?Ⅰ. Of the CSP gene among 12 P. vivax?infected samples,10 samples of indigenous case in Shandong Province and one sample of the case in?fected in Guangdong Province were detected with both 560-840 bp and 150-230 bp PCR amplification bands,which were iden?tified as temperate zone family strain of type PV?Ⅰ. However,one sample from the case infected in India was detected only with a 560-840 bp band,which was identified as tropical zone family strain of PV?Ⅰ. An analysis of phylogenetic tree of CSP gene showed that the sequences of 10 samples from the indigenous cases in Shandong Province and one sample from the case infected in Guangdong Province were located in the same branch,one sample from the case infected in India was located in the same branch with India and Indonesia strains. Conclusion Of all the indigenous isolates in Shandong Province,MSP?1 gene is geno?typed type Sal?1,CSP gene is genotyped temperate zone family strain of type PV?Ⅰ,with a high homology found among the in?digenous isolates.
5.Laboratory Study on the Effect of Moxibustion on the Proliferation of Neural Stem Cells after Cerebral Ischemia

Objective To observe the effect of moxibustion on the proliferation of neural stem cells after cerebral ischemia- reperfusion in rat models.Method The middle cerebral artery occlusion (MCAO) model was developed by using modified Zea-Longa method. Seventy-two Wistar rats were randomized into group A (sham-operation group), group B (model group) and group C (moxibustion group). Each group was then further divided into 4 sub-groups according to different time points: 3 d, 7 d, 14 d, and 21 d, 6 rats in each sub-group. For each group, Baihui (GV 20) and bilateral Taiyang (EX-HN 5) were selected to study. The proliferated cells were marked by intraperitoneal injection of BrdU, and TTC staining and HE staining were adopted to detect the pathological changes of brain tissues after cerebral ischemia-reperfusion. Immunohistochemical method was used to dynamically detect the positive expressions of BrdU and Nestin cells in DG and SVZ zones at different time points, and microscope image analysis was taken to study the positive expressions.Result The neurological deficit score in group C was significantly lower than that in group B at each time point (P<0.05). The neurological deficit score in group C was significantly different from that in group B at the same time point (P<0.05). Moxibustion can up-regulate the expression of BrdU in SVZ and DG, and the increase started from the 3rd day and reached the peak on the 7th day. The increase of BrdU expression in group C was more significant than that in group B (P<0.05). Moxibustion can up-regulate the expression of Nestin in SVZ and DG, and the increase started from the 3rd day and reached the peak on the 7th day. The increase of Nestin expression in group C was more significant than that in group B (P<0.05).Conclusion Moxibustion can remarkably promote the recovery of nerve function and the proliferation of BrdU and Nestin after cerebral ischemia-reperfusion in rats.
6.Survey of intestinal parasitic infections and related knowledge and behavior of residents in Jiaodong area of Shandong Province

Objective To understand the status of intestinal parasitic infections and the related knowledge and behavior in residents of Jiaodong area of Shandong Province,so as to provide the evidence for making an appropriate preventive and control strategy. Methods A total of 18 villages from 6 counties in Jiaodong area were selected as investigation sites according to the stratified sampling method. The feces samples of the permanent residents aged above 3 years were collected and examined by Kato-Katz technique to find the intestinal parasite eggs,and the children under 12 years old were examined by the method of cellophane anal swab to detect the Enterobius vermicularis eggs. In addition,50 households in each survey sites were randomly selected to in-vestigate the basic family situation and the condition of awareness on prevention knowledge and formation of correct behavior of res-idents by using a structured questionnaire. Results Totally 6 163 residents involved in the feces examinations,and the total in-fection rate of intestinal parasites was 6.91%. The infection rates of Trichuris trichiura,Ascaris lumbricoides and hookworm were 6.56%,0.62%and 0.21%,respectively. The infection rate of E. vermicularis in children under 12 years old was 0.51%. The eggs of Clonorchis sinensis and Taenia solium were not found in this survey. The awareness rate of knowledge about preventing parasitic diseases was 49.54%. The formation rates of washing hands before eating,washing hands after using the toilet,never eating raw fruit and vegetable without washing clean,never working in the field with bare feet,and never drinking unboiled water were 97.78%,91.95%,88.81%,92.42%and 86.48%respectively. Conclusions The infection rate of intestinal parasites is low in Ji-aodong area,but there is a significant difference among different counties. The awareness rate of knowledge about preventing para-sitic diseases is low,but the formation rate of healthy behavior is high. In the future,the health education and the strategy of tak- ing medicine among the key population should be enhanced,and the project of reconstructing safe water supply and lavatory should be advanced.
7.Construction and identification of pcDNA3-HBsAg-p30-ROP2 expression vec-tor

Objective To construct a multi-gene recombinant pcDNA3-HBsAg-p30-ROP2 expression vector and identify it preliminarily. Methods According to recombinant pcDNA3-p30-ROP2 restriction sites,HBV HBsAg gene sequences of primers were designed and synthesized to amplify target fragment,and then cloned into pcDNA3-HbsAg-p30-ROP2 expression vector. Af-ter sequencing,it was identified finally by restriction enzyme digestion and other molecular biology techniques. Results HBV HBsAg gene segment was amplified by PCR and the multi-gene recombinant pcDNA3-HBsAg-p30-ROP2 expression vector was constructed and identified to be correct as theoretical values. The PCR and restriction enzyme digestion results showed that HBsAg and p30-ROP2 gene in recombinant plasmid were confirmed by DNA sequencing. Conclusion The multi-gene recombinant pcD-NA3-HBsAg-p30-ROP2 expression vector is successfully constructed.
8.Large trophozoites in blood smear of falciparum malaria:one case report

This paper reports one case of atypical falciparum malaria imported from Africa,whose blood smear contains many large trophozoites,with punctiform or massive brown pigment granules,the body shape of the plasmodium is similar to that of Plas-modium vivax and Plasmodium ovale. After the gene detection by PCR,the case was diagnosed as falciparum malaria. As large tro-phozoites were rarely seen in the peripheral blood of non-severe falciparum malaria cases,much attention should be paid to the identification of Plasmodium falciparum and other plasmodia in microscopic examinations.
