1.Progress in Disease-Syndrome-Integrated Mouse Models of Lung Cancer

Lung cancer has the highest mortality rate among malignant tumors worldwide, posing a severe threat to human health. Traditional Chinese medicine (TCM) offers unique advantages in treating lung cancer, with disease-syndrome-integrated animal models serving as a critical component of TCM lung cancer research. According to TCM pathogenesis, lung cancer is classified as a pattern of underlying deficiency with superficial excess. Clinically, it is often categorized into syndromes such as qi and yin deficiency, lung-spleen qi deficiency, lung yin deficiency, qi stagnation and blood stasis, and phlegm-heat obstructing the lungs. Treatment should fundamentally adhere to the principle of "strengthening the healthy qi and expelling pathogenic factors". This paper systematically reviews literature from 2005 to 2025 on disease-syndrome-integrated animal models for lung cancer. It summarizes the theoretical basis and establishment methods of four core disease-syndrome-integrated models: qi-yin deficiency pattern, spleen qi deficiency pattern, qi stagnation and blood stasis pattern, and kidney deficiency pattern. Further discussion covers their evaluation systems and limitations. Additionally, modeling approaches for other patterns including yang deficiency, spleen deficiency, and qi deficiency with blood stasis are summarized, with evaluation methods categorized by behavioral characteristics and biochemical indicators. Analysis reveals that current models predominantly use C57BL/6 mice, exhibiting issues such as insufficient clinical relevance, complex modeling procedures, numerous confounding factors, and a low degree of standardization. Future improvements to disease-syndrome-integrated lung cancer animal models could include using clustered regularly interspaced short palindromic repeats (CRISPR) technology to establish organoid–patient-derived xenograft models; developing controllable composite stress systems and standardized stimulus models, such as gradient-controlled alternating cold/heat chambers and programmed dietary regulation systems, to reduce nonspecific interference; and developing multidimensional dynamic modeling techniques by integrating clinical big data, including metabolomics and imaging features, to design dynamic intervention protocols, thereby providing more precise and reliable experimental tools for elucidating the mechanisms of TCM in lung cancer prevention and treatment.
2.Impact of metabolic risk factor control on mortality risk in patients with metabolic dysfunction-associated steatotic liver disease

ObjectiveTo investigate the association of comprehensive control of multiple metabolic risk factors with the risk of all-cause mortality in patients with metabolic dysfunction-associated steatotic liver disease (MASLD) and its quantitative effect. MethodsA retrospective cohort study was conducted based on the data from National Health and Nutrition Examination Survey (NHANES) in 2003—2018. A total of 29 458 participants were enrolled, including 12 528 patients with MASLD and 16 930 non-MASLD controls. The control status of nine risk factors was assessed, i.e., hypertension, diabetes, liver fibrosis (assessed by fibrosis-4 index [FIB-4]), aspartate aminotransferase (AST)/alanine aminotransferase (ALT) ratio, serum albumin, renal function (assessed by serum creatinine), systemic inflammation (assessed by neutrophil count), smoking, and physical activity. According to the status of comprehensive risk factor control, the patients with MASLD were further divided into low-control group with 1 107 patients, moderate-control group with 7 042 patients, and high-control group with 4 379 patients. A total of 397 participants were enrolled in the external validation cohort, among whom there were 266 participants with MASLD and 131 participants without MASLD. The Kruskal-Wallis H test was used for comparison of continuous data between multiple groups, and the chi-square test was used for comparison of categorical data between groups. The Cox proportional hazards model incorporating complex sampling weights for NHANES data were used to investigate the association between the degree of risk factor control and all-cause mortality, and the Kaplan-Meier curve, the log-rank test, restricted mean survival time (RMST), and population attributable fraction (PAF) were used for further assessment. The robustness of the findings was validated in an independent clinical cohort (n=397). ResultsThe cohort study based on the NHANES database showed that during the median follow-up time of 6.7 years, 1 171 deaths occurred among the 12 528 patients with MASLD. There was a significant reduction in mortality risk with the increase in the number of risk factors controlled (trend test: χ2=65.06, P<0.001). Compared with the low-control group, the mortality risk decreased successively in the moderate-control group (hazard ratio [HR]=0.59, 95% confidence interval [CI]: 0.51 — 0.69) and the high-control group (HR=0.36, 95%CI: 0.27 — 0.48), and the mortality risk was reduced by 22% for each additional risk factor controlled (HR=0.78, 95%CI: 0.74 — 0.82). The PAF analysis showed that AST/ALT ratio control (PAF=19.2%) and physical activity (PAF=18.9%) were the largest contributors. The survival analysis of the sub-cohort constructed based on nearest neighbor matching (with a maximum follow-up time of 13.3 years) showed that there was a significant difference in cumulative survival rate between the MASLD groups with different control levels and the non-MASLD group (P<0.001). The RMST analysis further quantified the survival benefit of each group, and the high-control group had the longest 10-year RMST (9.88 years), which was superior to that of the non-MASLD group (9.43 years), the moderate-control group (9.33 years), and the low-control group (7.96 years). The multivariable-adjusted model analysis of the complete cohort showed that compared with the non-MASLD population, the MASLD patients in the low-control group had a significant increase in mortality risk, the moderate-control group had a comparable mortality risk, and the high-control group had a significantly lower mortality risk. The subgroup analysis showed a significant protective effect of comprehensive control in both male and female individuals (P<0.05), and the interaction analysis suggested a potentially stronger protective effect in female individuals (Pfor interaction=0.031). The protective effect was statistically significant in patients aged ≥60 years (P<0.05). There was a significant difference in protective effect across the groups stratified based on waist circumference (P<0.05), but there was no significant interaction between groups (Pfor interaction=0.821). In the external validation cohort, among the 397 participants, there were 72 cases of all-cause mortality (18.1%). The survival analysis showed a significant gradient increase in the survival rate of MASLD patients with the increase in the level of control (P=0.022). The RMST analysis showed that the moderate- and high-control groups had a better RMST than the non-MASLD group at 5 years (4.47 years vs 4.56 years vs 4.40 years), with a more significant survival advantage at 10 years (8.67 years vs 8.95 years vs 8.44 years). The Cox regression analysis showed that, with the low-control MASLD group as the reference, there was a significant reduction in mortality risk in the moderate-control group (HR=0.41, 95%CI: 0.21 — 0.81) and the high-control group (HR=0.32, 95%CI: 0.14 — 0.71); with the non-MASLD population as the reference, there was a significant increase in mortality risk in the low-control group (HR=2.02, 95%CI: 1.03 — 3.95), with a comparable mortality risk in the moderate-control group (HR=0.83, 95%CI: 0.48 — 1.44) and the high-control group (HR=0.66, 95%CI: 0.30 — 1.38), and the point estimates of the effects were highly consistent with the main study. ConclusionAmong MASLD patients, better control of metabolic risk factors is associated with a lower mortality risk. Achieving optimal control can eliminate the excess mortality risk associated with MASLD, thereby helping patients achieve a better survival prognosis than the general population.
3.Development of DUS Test Guidelines for New Pinellia ternata

Pinellia ternata, belonging to the Pinellia genus within the Araceae family, is a medicinal plant due to its tubers. There are severe issues with unclear germplasm and mixed varieties in its cultivation, necessitating urgent new variety protection efforts. The distinctness, uniformity, and stability (DUS) testing of the plant variety is the basis for protecting new plant varieties, and the DUS test guidelines are the technical basis for DUS testing. To develop the DUS test guidelines for P. ternata, agronomic traits of 229 germplasm of P. ternata were observed and measured during its two growth stages over the years, and each character was graded and described. A total of 38 traits were selected as the test traits of the DUS test guideline for P. ternata. There were three plant traits, 19 leaf traits, six flower traits, two fruit traits, two tuber traits, five bulbil traits, and one ploidy trait. These traits could be divided into 22 quality characters, 12 quantitative characters, and four pseudo-quantitative characters, as well as seven groups, including plants, leaves, flowers, fruit, tubers, bulbils, and ploidy. By searching for standard traits, 10 standard varieties were ultimately determined. Preparing these guidelines will have great significance for reviewing and protecting P. ternata varieties, safeguarding breeders' rights, and promoting the development of the P. ternata industry.
4.Apoptosis Regulation by Buzhong Yiqitang via PERK/eIF2α/ATF4/CHOP Pathway to Enhance Pulmonary Surfactant-associated Protein C Expression and Ameliorate Lung Injury in CIH Mice

ObjectiveTo investigate the effects of Buzhong Yiqitang on the abnormal expression of pulmonary surfactant-associated protein C (SFTPC) and lung injury induced by chronic intermittent hypoxia (CIH) and the mechanism of action. MethodsForty healthy adult male SPF-grade C57BL/6 mice were randomly allocated into five experimental groups: a normoxia group, a CIH group, and low-, medium-, and high-dose Buzhong Yiqitang groups, with eight mice in each group. During the modeling, mice in the normoxia group were housed under standard oxygen concentrations, while the CIH and all Buzhong Yiqitang groups were placed in a hypoxic chamber for 8 h daily over 35 d. Prior to each chamber session, mice in the low-, medium-, and high-dose Buzhong Yiqitang groups were administered decoctions by gavage at corresponding doses (8.1, 16.2, 32.4 g·kg-1·d-1 of crude drug, respectively), while those in normoxia and CIH groups received an equivalent volume of saline by gavage. The general conditions of the mice were recorded before and after the experiment. Pulmonary function was assessed using a non-invasive detection system. Serum SFTPC levels were measured using enzyme-linked immunosorbent assay (ELISA). Histopathological changes in lung tissue were evaluated using hematoxylin-eosin (HE) staining. Apoptosis in lung tissue was detected using terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL). Protein expression of SFTPC, B-cell lymphoma-2 (Bcl-2), Bcl-2-associated X protein (Bax), protein kinase R-like endoplasmic reticulum kinase (PERK), phosphorylated PERK (p-PERK), eukaryotic initiation factor 2α (eIF2α), phosphorylated eIF2α (p-eIF2α), activating transcript factor 4 (ATF4), and CCAAT/enhancer-binding protein homologous protein (CHOP) in lung tissue was analyzed by Western blot. Immunofluorescence staining was employed to assess the expression of SFTPC and CHOP proteins in lung tissue. ResultsCompared to those in the normoxia group, mice in the CIH group showed significantly impaired pulmonary function and increased histopathological lung injury scores (P<0.05, P<0.01). Serum SFTPC levels increased, while SFTPC expression in lung tissue was reduced (P<0.05, P<0.01). The rate of apoptotic cells in lung tissue increased, and the expression of endoplasmic reticulum stress markers p-PERK, p-eIF2α, ATF4, and CHOP was upregulated (P<0.05, P<0.01). Compared with the CIH group, Buzhong Yiqitang intervention improved pulmonary function indicators and decreased the histopathological lung injury scores (P<0.05, P<0.01). Serum SFTPC levels were decreased, and lung tissue SFTPC expression was recovered (P<0.05, P<0.01). The apoptotic rate of lung tissue cells was significantly reduced, with downregulation of pro-apoptotic Bax and upregulation of anti-apoptotic Bcl-2 expression (P<0.05, P<0.01). Activation and expression of p-PERK, p-eIF2α, ATF4, and CHOP were also decreased (P<0.05, P<0.01). ConclusionBuzhong Yiqitang can alleviate lung injury and improve pulmonary function by reducing lung cell apoptosis and enhancing alveolar surfactant secretion, which may be related to the modulation of the PERK/eIF2α/ATF4/CHOP signaling pathway.
5.Mechanisms of bone marrow mesenchymal stem cells in counteracting D-galactose-induced brain aging

Objective To investigate the effect and potential mechanism of rat mesenchymal stem cells(MSC)on D-galactose-induced brain-tissue aging.Methods A rat brain-aging model was established by injecting D-galactose,and rats in the treatment group received MSC injections via the tail vein.Superoxide dismutase(SOD)activity and malondialdehyde(MDA)levels were assessed in rat brain tissue at the end of the experiment,and pathological changes in brain tissue were observed by hematoxylin-eosin(HE)staining.Expression levels of the inflammatory factors interleukin(IL)-1 and IL-6,the pathway proteins brain-derived neurotrophic factor(BDNF)-tropomyosin receptor kinase B(TrkB),the negative growth regulators p53 and p16,as well as vascular endothelial growth factor(VEGF)and basic fibroblast growth factor(bFGF)were observed by polymerase chain reaction(PCR)and Western Blot.Results Brain levels of SOD activity were significantly increased and MDA levels were significantly decreased in rats in the modle group compared with the treatment group(P<0.05).The pathological state of the cerebral cortex and hippocampus were improved and the number of neurons and nucleus pulposus ratio in the brain were increased in the treatment group,as shown by HE staining.Expression levels of IL-1,IL-6,p53,and p16 were significantly decreased,while BDNF,TrkB,VEGF,and bFGF were significantly increased in the treatment group compared with the model group,as shown by PCR and Western Blot(P<0.05).Conclusions These result suggest that MSCs potentially mitigate D-galactose-induced cerebral senescence by concurrently modulating the BDNF-TrkB axis to attenuate oxidative/inflammatory damage,while enhancing the secretion of vasculotrophic(VEGF)and neurotrophic(bFGF)factors for neuronal maintenance.
6.Application effect of multidisciplinary collaborative nursing based on the Information-Knowledge-Attitude-Practice theory in patients undergoing lobectomy via thoracoscopy

Objective To analyze the application effect of multidisciplinary collaborative nursing based on the Information-Knowledge-Attitude-Practice(IKAP)theory in patients undergoing lobectomy via thoracoscopy.Methods A total of 168 patients undergoing lobectomy via thoracoscopy were se-lected,and randomly divided into control group and observation group using a random number table,with 84 patients in each group.The control group received routine nursing,while the observation group received multidisciplinary collaborative nursing based on the IKAP theory in addition to routine nurs-ing.The postoperative recovery,pulmonary function indicators[forced vital capacity(FVC),maxi-mum voluntary ventilation(MVV)and ratio of forced expiratory volume in the first second to forced vi-tal capacity(FEV1/FVC)],self-efficacy[General Self-Efficacy Scale(GSES)score]and self-man-agement ability[Chronic Disease Self-Management Questionnaire(CDSMQ)score]before and after intervention were observed and compared between the two groups.Results The observation group had shorter first ambulation time,first flatus time and length of hospital stay compared with the control group(P<0.05).Before intervention,there were no statistically significant differences in FVC,MVV,FEV1/FVC,CDSMQ scores and GSES scores between the two groups(P>0.05).After inter-vention,the observation group had higher FVC,MVV,FEV1/FVC,CDSMQ scores and GSES scores compared with the control group(P<0.05).Conclusion Multidisciplinary collaborative nursing based on the IKAP theory can effectively promote the recovery process in patients undergoing lobec-tomy via thoracoscopy,improve postoperative pulmonary function,self-management ability,and self-efficacy,thereby enhancing the quality of rehabilitation.
7.Mechanism of "olfactory three needles" in regulating microglia and promoting remyelination in vascular dementia rats.

OBJECTIVE:
To observe the effects of "olfactory three needles" on cognition, learning and memory abilities, as well as hippocampal microglia (MG) phagocytic activity in vascular dementia (VD) rats, and explore the mechanisms of acupuncture in regulating MG activation and improving remyelination, so as to ameliorate VD.
METHODS:
Among 38 SD rats meeting experimental requirements, 9 rats were randomly assigned to a sham-operation group, and the remaining rats underwent permanent bilateral common carotid artery ligation to establish VD model. Eighteen successfully modeled rats were randomly divided into a model group and an electroacupuncture (EA) group, with 9 rats in each one. In the EA group, EA was performed at "olfactory three needles" ("Yintang" [GV24+] and bilateral "Yingxiang" [LI20]), at disperse-dense wave, the frequency of 2 Hz/15 Hz and the current intensity of 1 mA, for 15 min per intervention, once daily. One course was composed of 7 days, and 2 courses were required, with the interval of 2 days. The novel object recognition test was employed to assess the cognition of rats, and the Morris water maze was adopted to observe learning and memory abilities. Luxol fast blue (LFB) staining was performed to evaluate myelin sheath loss in the hippocampus, the Western blot was used to detect the protein expression of triggering receptor expressed on myeloid cells-2 (TREM2) and proteolipid protein (PLP) in the hippocampus; and the immunofluorescence staining was used to detect the positive expression of PLP, sex determining region Y-box 10 (SOX10), ionized calcium binding adaptor molecule 1 (Iba1)+ TREM2+ and Iba1+ lysosome-associated membrane protein 1 (LAMP1)+ in the hippocampus.
RESULTS:
Compared with the sham-operation group, the rats in the model group exhibited the prolonged escape latency on day 3 and 4 (P<0.05, P<0.01), the increase of the total distance traveling (P<0.01) and the decrease of the recognition index (RI) and platform crossing frequency (P<0.01). Compared with the model group, the rats in the EA group showed the shortened escape latency on day 3 and 4 (P<0.05), the decrease of total distance traveling (P<0.01) and the increase of RI and platform crossing frequency (P<0.05, P<0.01). When compared with the sham-operation group, the rats of the model group presented uneven staining, sparse arrangement of myelin sheath fibers, unclear contours, and prominent vacuole-like changes in the hippocampal CA1 region. When compared with the model group, the EA group showed more dense staining, the increase of myelin sheath fibers with more orderly alignment, and fewer vacuolar changes in the hippocampal CA1 region. Compared with the sham-operation group, the model group exhibited the increase of TREM2 protein expression and the decrease of PLP protein expression in the hippocampus (P<0.01), whereas the EA group showed the up-regulation of TREM2 and PLP protein expression when compared with the model group (P<0.01, P<0.05). The positive expression of the hippocampal PLP, SOX10, and Iba1+LAMP1+ in the model group was reduced in comparison with the sham-operation group (P<0.05, P<0.01), and the positive expression of Iba1+ TREM2+ was elevated (P<0.05). In the EA group, the positive expression of PLP, SOX10, Iba1+TREM2+, and Iba1+ LAMP1+ was higher compared with that in the model group (P<0.05, P<0.01).
CONCLUSION
"Olfactory three needles" can improve the learning and memory, and cognitive functions of VD rats, and its mechanism may be associated with the up-regulation of TREM2 and LAMP1 to adjust MG phagocytic activity and intracellular degradation, and promote remyelination.

8.The diagnostic value of CT imaging evaluation for lymph node metastasis in gallbladder cancer and its correlation with prognosis

Objective:To assess the accuracy of preoperative enhanced CT in evaluating the degree of lymph node metastasis in gallbladder cancer.Methods:A retrospective analysis was performed on the enhanced CT imaging data of 124 gallbladder cancer patients who underwent surgical treatment at Henan Provincial People's Hospital from Jan 2017 to Dec 2018. Imaging staging was used to classify lymph node metastasis. Pathological and imaging data of 70 patients with confirmed postoperative lymph node pathology were compared to evaluate the accuracy of imaging methods in detecting lymph node involvement.Results:Lymph node metastasis in the 124 surgical patients was categorized into three groups using imaging evaluation methods. The overall accuracy of determining lymph node positivity and negativity was 63%, with a sensitivity of 64% and specificity of 62%. The accuracy of detecting lymph node metastasis in gallbladder cancer was higher when lymph node fusion and internal necrosis were observed. The overall survival rate differed significantly among gallbladder cancer patients at different lymph node imaging stages ( P<0.05). Conclusion:CT imaging evaluation has diagnostic value for lymph node metastasis in gallbladder cancer and has a certain predictive effect on the prognosis of patients.
9.Acupuncture for treatment of postmenopausal osteoporosis:meta-analysis,systematic evaluation and trial sequential analysis

OBJECTIVE:To assess the efficacy and safety of acupuncture in treating postmenopausal osteoporosis.METHODS:Clinical randomized controlled trials on acupuncture for postmenopausal osteoporosis were retrieved from databases including CNKI,VIP,WanFang,China Biomedical Literature Service System,Web of Science,PubMed,Embase,and Cochrane Library.The search period was from the inception of the database to July 23,2024.The quality of evidence and risk of bias of the included studies were assessed using the GRADEprofiler and the revised Cochrane risk-of-bias tool for randomized trials(RoB 2.0).Meta-analysis was performed using StataSE 15 and TSA 0.9.5,along with heterogeneity analysis,sensitivity analysis,subgroup analysis,publication bias analysis,trim-and-fill analysis,and trial sequential analysis.RESULTS:The study included 19 randomized controlled trials involving a total of 1 464 patients.(1)The meta-analysis revealed that compared with conventional therapies,acupuncture significantly increased the total effective rate(RR=1.25,95%confidence interval[1.19,1.32],P<0.001),lumbar spine L2-L4 bone mineral density(mean difference=0.08,95%confidence interval[0.05,0.11],P<0.001),and estrogen levels(standard mean difference=0.55,95%confidence interval[0.30,0.81],P<0.001).Additionally,acupuncture increased bone mineral density and gonadotropin levels in the femoral neck,greater trochanter,and Ward's triangle,reduced visual analogue scale scores,and improved bone metabolism without significantly increasing the incidence of adverse events.(2)Subgroup analysis of the main outcome indicators showed no significant difference between the treatment and control groups when the body mass index was greater than 23.9 kg/m2 and the total treatment duration was 6 months.All other treatment subgroups were significantly superior to the control group.(3)Publication bias analysis indicated a significant risk of publication bias in the overall efficacy;however,a trim-and-fill analysis showed that this risk did not significantly impact the reliability of the conclusions.Publication bias analysis showed no significant risk of publication bias in the bone mineral density values of lumbar spine L2-L4 and estradiol levels.(4)The sequential analysis of the trial further confirmed that acupuncture significantly increased the overall effective rate,bone mineral density values at the lumbar spine L1-L4,and estradiol levels.CONCLUSION:Acupuncture is an effective and safe treatment for postmenopausal osteoporosis.More rigorously designed and high-quality studies are needed in the future to verify this conclusion.
10.Exploring the mechanism of Xiaoqinglong decoction on endothelial-mesenchymal transition in pulmonary arterial hypertension based on the principle of resolving the exterior and expanding the collaterals'principle

Objective To analyze the role and mechanism of Xiaoqinglong decoction in alleviating the pathological process of pulmonary arterial hypertension(PAH),and to observe the effect of Xiaoqinglong decoction on endothelial-mesenchymal transition(EMT)in human pulmonary arterial endothelial cell(HPAEC)and the involvement of the Toll-like receptor/nuclear factor-κB/hypoxia-inducible factor-1α(TLR/NF-κB/HIF-1α)pathway in this mechanism.Methods Thirty-six male Sprague Dawley(SD)rats and HPAEC were randomly divided into control group,model group,Xiaoqinglong decoction plus Earthworm group,Bosentan tablet group,dimethyl sulfoxide(DMSO)group,and monophosphoryl lipid A(MPL)group.PAH rat models and HPAEC models were established by hypoxic exposure.The Xiaoqinglong decoction plus Earthworm group received intragastric administration Xiaoqinglong decoction plus Earthworm(5 g·kg-1·d-1)or cultured with 10%corresponding drug serum,the Bosentan group received Bosentan(100 mg·kg-1·d-1)by gavage or cultured with 10%corresponding drug serum,the MPL group received 1 μg MPL,and the DMSO group received an equivalent volume of the DMSO and corn oil mixed solvent as a negative control for the MPL group.The hemodynamic parameters,including mean pulmonary arterial pressure(mPAP),right ventricular systolic pressure(RVSP),and the maximum rate of right ventricular pressure(+dp/dt max),were measured via right heart catheterization.After euthanasia,lung and heart tissues were collected to assess the right ventricular hypertrophy index(RVHI);hematoxylin-eosin(HE)staining was used to observe the degree of right ventricular cardiomyocyte hypertrophy and to calculate the average intima-media thickness(IMT)in small pulmonary arteries;Western blotting was used to detect the protein expression levels of proliferating cell nuclear antigen(PCNA),CD68,TLR4,NF-κB,HIF-1α,vascular endothelial cadherin,and vimentin;cell counting kit-8(CCK-8),Transwell,and scratch assays were used to observe cell proliferation and migration;Enzyme-linked immunosorbent assay(ELISA)was used to measure the levels of interleukins(IL-8,IL-6),tumor necrosis factor-α(TNF-α),endothelin-1(ET-1),and nitric oxide(NO).Results Compared with the model group,the Xiaoqinglong decoction plus Earthworm group showed significant reductions in mPAP,RVSP,RVHI,and IMT in PAH rats[mPAP(mmHg,1 mmHg≈0.133 kPa):22.17±1.94 vs.42.00±4.90,RVSP(mmHg):34.67±3.20 vs.52.83±3.76,RVHI:0.402±0.057 vs.0.822±0.101,IMT:(37.85±2.49)%vs.(62.06±4.52)%,all P<0.05],and a significant increase in+dP/dT max(mmHg/s:2 730.83±137.89 vs.1 718.33±148.36,P<0.05).Western blotting and ELISA results showed that compared with the model group,the Xiaoqinglong decoction plus Earthworm group had significantly lower protein expression of PCNA and CD68 in lung tissue,and levels of inflammatory factors(IL-6,IL-8,TNF-α)in rat serum[lung tissue:PCNA protein expression(PCNA/GAPDH)was 1.56±0.08 vs.2.20±0.26,CD68 protein expression(CD68/GAPDH):1.46±0.09 vs.2.60±0.23;serum:IL-8(ng/L)was 39.67±6.28 vs.149.17±7.49,IL-6(ng/L):81.00±6.63 vs.211.00±25.31,TNF-α(ng/L):213.17±24.86 vs.799.50±43.51,all P<0.05].In vitro experiments,compared with the model group,Xiaoqinglong decoction plus Earthworm inhibited abnormal proliferation(A value:2.052±0.087 vs.2.242±0.057,P<0.05)and migration[number of migrating cells(per field):101.33±12.01 vs.226.67±17.56,P<0.05]of HPAEC,and reversed the EMT process,manifested as upregulation of vascular endothelial cadherin protein expression levels(vascular endothelial cadherin/GAPDH:0.39±0.06 vs.0.12±0.03,P<0.05)and downregulation of vimentin protein expression(vimentin/GAPDH:4.96±0.33 vs.7.89±0.44,P<0.05).Western blotting results indicated that compared with the model group,the protein expression levels of TLR4,the ratio of phosphorylated p65 to total p65,and HIF-1α in both lung tissue and HPAEC were significantly reduced in the Xiaoqinglong decoction plus Earthworm group[lung tissue:TLR4 protein expression(TLR4/GAPDH)was 3.13±0.20 vs.4.38±0.30,p-p65/p65 ratio:7.11±0.81 vs.12.73±1.80,HIF-1α protein expression(HIF-1α/GAPDH):2.37±0.32 vs.4.45±0.34;HPAEC:TLR4 protein expression(TLR4/GAPDH)was 1.42±0.03 vs.2.43±0.05,p-p65/p65 ratio:6.01±1.84 vs.11.28±1.06,HIF-1α protein expression(HIF-1α/GAPDH)was 3.24±0.17 vs.5.50±0.44,all P<0.05],accompanied by upregulated vascular endothelial cadherin protein expression(vascular endothelial cadherin/GAPDH:0.66±0.03 vs.0.49±0.03,P<0.05)and downregulated vimentin protein expression(vimentin/GAPDH:1.81±0.12 vs.2.47±0.10,P<0.05),indicating that Xiaoqinglong decoction plus Earthworm inhibits the EMT process in endothelial cells by suppressing the activation of the TLR/NF-κB/HIF-1α pathway.Experiments with a TLR agonist further confirmed that activation of the TLR pathway reverses the protective effects of Xiaoqinglong decoction plus Earthworm,as shown by the MPL group compared to the DMSO group having significantly increased protein expression of the p-p65/p65 ratio and HIF-1α[p-p65/p65 ratio:2.17±0.35 vs.1.08±0.14,HIF-1α/GAPDH:3.96±0.25 vs.1.03±0.10,both P<0.05],further decreased vascular endothelial cadherin protein expression(vascular endothelial cadherin/GAPDH:0.66±0.04 vs.0.99±0.02,P<0.05),further increased vimentin protein expression(vimentin/GAPDH:1.53±0.12 vs.0.93±0.07,P<0.05),along with enhanced cell migration capacity[number of migrating cells(per field):176.67±17.50 vs.107.00±11.14;cell migration rate in scratch assay:(34.32±2.82)%vs.(22.71±2.49)%,both P<0.05]and increased proliferation activity(48 hours A value:2.156±0.044 vs.1.810±0.088,P<0.05).Conclusions Xiaoqinglong decoction combined with Pheretima not only significantly reduces pulmonary artery pressure,improves cardiac function and mitigates pulmonary vascular fibrosis in PAH rats,but also alleviates pulmonary vascular remodeling by inhibiting inflammatory responses and EMT.It can further decrease the content of ET-1,increase the level of NO,and ameliorate vascular stenosis.This result further indicates that exterior-relieving medicines exert a significant dilating and supporting effect on the narrowed meridians and collaterals.
