Objective To explore the molecular mechanism by which curcumin enhances the anti-tuberculosis function of macrophages through immune metabolic regulation mediated by liver X receptor α(LXRα)/ABCA1.Methods A model was established by infecting THP-1-derived macrophages with attenuated strain of Mycobacterium bovis(M.bovis).The control group, curcumin group,M.pavis group,M.pavis+LXRα agonist(T0901317)group,M.pavis+LXRα inhibitor(GSK2033)group,M.pavis+curcumin group,M.pavis+curcumin+GSK2033 group and M.pavis+curcumin+T0901317 group were set up.The protein and gene expressions of LXRα/ABCA1 were detected by Western blotting and real-time fluorescence quantitative polymerase chain reaction(RT-qPCR).The accumulation of lipid droplets was analyzed by Oil Red O staining and micro-assay.The lipid content of the supernatant was determined by a biochemical analyzer, and cell proliferation was assessed by the MTT method.Bacterial clearance capacity was evaluated by measuring intracellular bacterial load. Results Curcumin significantly upregulated the protein and gene expression of LXRα/ABCA1 in M.Bovis-infected macrophages, reduced intracellular lipid accumulation and promoted lipid efflux, while enhancing cell proliferation and reducing intracellular bacterial load(P<0.05,P<0.01).LXRα inhibitors could reverse the effect of curcumin, while agonists synergistically enhanced its effect.Correlation analysis showed that the expression of LXRα/ABCA1 in cells was negatively correlated with the intracellular bacterial load, while the lipid level was positively correlated with the intracellular bacterial load(P<0.01).Conclusion Curcumin activates the LXRα/ABCA1 pathway, coordinates the metabolic remodeling of macrophages and the enhancement of immune function, and forms a synergistic effect against tuberculosis, providing an experimental basis for the development of a novel host-directed treatment strategy for tuberculosis based on immune-metabolic regulation.

Objective To evaluate the efficacy and safety of using stent semi-retrieval technique during mechanical thrombectomy(MT)for acute anterior cerebral artery(ACA)occlusion. Methods We made a retrospective analysis of 38 patients with acute ACA occlusion treated by stent semi-retrieval technique at Tangdu Hospital and Xi'an No.1 Hospital from June 2019 to September 2023.The successful recanalization,first-pass effect(FPE),perioperative complications and postoperative recovery of all the patients were analyzed.Modified thrombolytic test in cerebral infarction(mTICI)was used to evaluate the recanalization(unsuccessful recanalization,mTICI 0-2a;successful recanalization,mTICI 2b-3).FPE was defined as mTICI 3 after one thrombectomy. The clinical outcome was evaluated by modified Rankin Scale(mRS)3 months postoperatively.A good prognosis was defined as a mRS 0-2.Results Of the 38 patients,36(94.7%)got successful recanalization(mTICI 2b-3),with an FPE of 73.7%(28/38).Two cases of mild vascular spasm(5.3%)occurred during the mechanical thrombectomy, which completely relieved without further treatment.None of the 38 patients experienced vascular rupture or dissection, and no thrombus escaped during the thrombus removal process. There was no subarachnoid hemorrhage or parenchymal hemorrhage in the area related to ACA occlusion.Ninety days after operation,14 cases(36.8%)reached good outcome(mRS 0-2),18 cases(47.4%)reached mRS 3-5,and 6 cases(15.8%)died.Conclusion Our preliminary experience suggests that the stent semi-retrieval technique may be a safe and effective method for treating acute anterior cerebral artery occlusion.

Objective:To explore the establishment, implementation, and application effect of the annual progressive assessment mode for standardized training of resident physicians in a medical laboratory base. Methods:Since 2020, the base has improved the annual progressive assessment system through assessment system establishment, question bank construction, routine assessment, rotation examination, and annual examination. This new assessment mode was adopted for the residency trainees enrolled in the base after 2020. The control group consisted of seven residency trainees enrolled before 2020, while the experimental group comprised 13 trainees enrolled in 2020 and later. The two groups were compared based on the rotation and annual examination scores of second- and third-year trainees, as well as their academic achievements. Additionally, teaching outcomes of the base before and after 2020 were analyzed. The t test, rank-sum test, and Fisher's exact test were performed using SPSS 27.00. Results:There were no significant differences in age, sex, and education level between the two groups. The experimental group showed significantly higher scores than the control group in theory assessment ( P<0.001 for second-year trainees and P=0.008 for third-year trainees) and skill assessment ( P<0.001 for second-year trainees and P=0.038 for third-year trainees) in rotation examination, as well as in theory assessment (both P<0.001) and skill assessment (both P<0.001) in annual examination. The improvement was particularly pronounced among second-year trainees. The trainees of the experimental group submitted 17 case reports in three years, and won awards at national conferences, provincial conferences, and institutional case speech competitions. In a national academic conference, the trainees won the second prize of "Excellent Case". Since 2020, we have been granted with our first teaching reform project and published our first teaching article. To date, we have obtained five teaching reform projects, published six teaching articles, and won multiple awards in provincial and institutional teaching competitions. Conclusions:The establishment of the annual progressive assessment mode is critical for training high-quality laboratory physicians, and puts forward higher requirements for residency training teachers.
