1.Association between daytime outdoor physical activity and common oral diseases among primary school students

Objective:
To explore the association between daytime outdoor physical activity (OPA) and oral health indicators among primary school students, aiming to provide evidence for developing oral health intervention strategies based on natural exposure.
Methods:
In October 2023, based on the "Tianchang Children s Light Exposure and Growth Development Cohort", 799 second and third grades children were recruited from two primary schools in Tianchang, Chuzhou City, Anhui Province. Physical activity intensity and light exposure were objectively monitored for 24 hours over 5 consecutive days using triaxial accelerometers synchronized with portable illuminance meters. Standardized oral examinations were performed to record dental caries, gingivitis, and malocclusion. Demographics, lifestyle variables, and household socioeconomic data were collected via questionnaires. Multiple linear regression (for the number of carious teeth) and Logistic regression (for gingivitis risk) were used to analyze the relationship between daytime outdoor moderate to vigorous physical activity (MVPA) and oral health outcomes.
Results:
The average daytime outdoor MVPA was (0.76±0.35)h, with (0.95±0.40)h on weekdays and (0.49±0.47)h on weekends. The detection rates for dental caries, gingivitis, and malocclusion were 31.0%, 4.6%, and 59.7%, respectively. Compared with children with good oral health, the duration of outdoor MVPA on school days was reduced in children with caries or gingivitis ( Z =-11.4, -5.01, both P <0.01). Multiple regression analysis showed that after adjusting for factors such as gender, age, body mass index, oral hygiene behaviors, an increase in daytime outdoor MVPA duration was associated with a decrease in the number of dental caries ( β=-0.64, 95%CI =-0.93 to -0.35) and a reduced risk of gingivitis ( OR= 0.58 , 95%CI =0.34-0.98) in primary school students (both P <0.05). The association was primarily observed on school days ( β=-0.72, 95%CI = -1.07 to -0.37; OR=0.42, 95%CI =0.21-0.85) (both P < 0.05).
Conclusions
Daytime outdoor MVPA on weekdays is significantly associated with a lower number of carious teeth and a reduced risk of gingivitis in primary school students. Increasing daytime outdoor activities on weekdays may serve as a promising and potential strategy for promoting children s oral health.
2.Value of multimodal imaging-based precise evaluation and standardized monitoring in reducing perioperative complications of carotid endarterectomy

Severe carotid artery stenosis is an independent risk factor for stroke, and carotid endarterectomy (CEA) can effectively prevent ischemic stroke by alleviating such stenosis. However, CEA carries certain risks of adverse events, including perioperative death, ischemic stroke, and hyperperfusion injury, and these adverse events not only compromise perioperative safety but also diminish the efficacy of the procedure in preventing cerebral ischemic events. A multimodal imaging-based comprehensive management mode (i.e., preoperative assessment with carotid ultrasound, transcranial Doppler (TCD), and CT angiography, intraoperative monitoring with TCD and cerebral oximetry, and standardized postoperative follow-up with carotid ultrasound and TCD) can significantly reduce perioperative mortality and the incidence rate of stroke and avoid hypoperfusion, hyperperfusion syndrome, and arterial restenosis or occlusion, thereby enhancing the perioperative safety of CEA.
3.Recent advances in RNA-targeted antiviral agents against SARS-CoV-2

The development of antiviral drugs targeting SARS-CoV-2 remains a major strategy for the prevention and treatment of COVID-19 infection. Current anti-SARS-CoV-2 agents mainly act on key viral protein targets involved in the viral life cycle, such as the main protease (Mpro), papain-like protease (PLpro), and RNA-dependent RNA polymerase (RdRp). However, these drugs are insufficient to address the challenges posed by the rapid mutation rate and drug resistance of SARS-CoV-2. In recent years, viral RNA has demonstrated to be a highly promising therapeutic target. Small-molecule drugs that target viral RNA possess distinct advantages, featuring novel mechanisms of action and the potential for broad-spectrum antiviral activity as well as the ability to overcome drug resistance. Such agents provide a new avenue for combating coronavirus threats. This review systematically summarizes recent advances in the development of small-molecule inhibitors targeting conserved SARS-CoV-2 RNA structures, including the frameshift stimulation element (FSE), stem-loops (SL), G-quadruplexes (G-4), and RNA-targeting chimeras (RNATAC), aiming to provide some insight to guide future.
4.Investigation of the role of HSF1 in HBx-driven progression of hepatocellular carcinoma

Objective:To investigate the role of heat shock transcription factor 1(HSF1)in hepatitis B virus X protein(HBx)-driven migration and invasion of hepatocellular carcinoma(HCC)cells,and to preliminarily explore the mechanism of HSF1 mediating HBx-driven HCC progression.Methods:4D label-free quantitative proteomics and Western blot were used to analyze the effect of HBx on HSF1 expression.HBx was overexpressed in the HCC cell lines Huh7 and MHCC97H,and its impact on the mRNA and protein levels and stability of HSF1 was assessed by RT-PCR and Western blot.The Cancer Genome Atlas database was used to analyze the expres-sion of HSF1 in hepatitis B virus(HBV)-associated HCC tissues and its relationship with tumor stage/grade and patient prognosis,and Western blot was used to measure the expression of HSF1 in HBV-associated HCC tissues.HBx was overexpressed in HCC cells,fol-lowed by HSF1 knockdown or cell treatment with the HSF1 inhibitor KRIBB11,and Transwell migration and invasion assay,scratch as-say,and F-actin staining experiment were performed to analyze the role of HSF1 in HBx-driven HCC cell migration and invasion.GEO and HCMDB datasets were used to identify the downstream target of HSF1,and the role of downstream target c-Myb in HSF1-me-diated HBx-driven HCC progression.Results:HBx upregulated HSF1 protein levels without significantly affecting its mRNA expres-sion,through enhancing HSF1 protein stability.HSF1 was highly ex-pressed in HBV-associated HCC tissues,and its elevated expres-sion correlated with tumor stage/grade and poor prognosis.HBx overexpression significantly promoted the migration,invasion,wound-healing capacity,and pseudopodia formation capacity of Huh7 and MHCC97H cells,while HSF1 knockdown or KRIBB11 treatment significantly attenuated the HBx-driven migration and in-vasion of HCC.HSF1 promoted the expression of the metastasis-associated protein c-Myb,and c-Myb overexpression in HSF1-knock-down HCC cells restored the promotive effect of HBx on HCC cell migration and invasion.Conclusion:HBx enhances HSF1 protein stability to promote its expression.Upregulation of c-Myb by HSF1 plays a pivotal role in HBx-driven HCC cell migration and inva-sion.Targeting HSF1 may help to delay the progression of HBV-associated HCC.
5.Research advances in the mechanism of clemastine regulating oligodendrocytes and remyelination

Oligodendrocytes(OLs)play a crucial role in the central nervous system,and the myelin sheath formed by them is essential for maintaining the normal function of the nervous system.Damage to the myelin sheath can trigger various nervous system diseases,which seriously affect the quality of life of patients.Clemastine,a traditional antihistamine,has shown certain potential in"repurposing old drugs"in recent years and can promote the repair of myelin sheath.This article summarizes the role of clemastine in regulating the differentiation,maturation,senescence,and apoptosis of OLs at multiple levels by acting on receptors on the OL membrane and regulat-ing the key signaling pathways in cells and epigenetic modifications in the nucleus.A deeper understanding of the mechanism of action of clemastine in OLs can help to provide new targets and new approaches for drug treatment of myelin-degenerative diseases.
6.Perioperative anastomotic management strategies for tracheal reconstruction surgery

In tracheal resection and reconstruction, a technically demanding, complex, and high-risk procedure, management of the anastomotic site significantly impacts postoperative outcomes and long-term quality of life. However, comprehensive studies detailing perioperative anastomotic management strategies in tracheal reconstruction remain scarce. This review summarizes perioperative management strategies for tracheal reconstruction, covering preoperative assessment, surgical techniques, and other key aspects. It also highlights future research directions and challenges, aiming to provide clinicians with a systematic guide to perioperative management in tracheal reconstruction.
7.Randomized controlled trial of enhanced cognitive behavioral therapy for chronic insomnia with comorbid anxiety/depression

Objective:To compare the effects of standard cognitive behavioral therapy for insomnia (CBT-I) and enhanced cognitive behavioral therapy for insomnia(CBT-I Plus) in patients with chronic insomnia disorder comorbid anxiety or depressive symptoms.Methods:This prospective study included 148 patients with chronic insomnia disorder and anxiety/depression symptoms who were treated at the Sleep Disorder clinic of Shanghai Mental Health Center between July 2020 and August 2023. Participants (56 males, 92 females; aged 18-65 years, mean age 35.08±10.30 years) were randomly assigned in a 1∶2 ratio to the CBT-I group ( n=54) or CBT-I Plus group ( n=94). The CBT-I Plus group received additional treatments targeting anxiety and depressive symptoms. Treatment lasted 8 weeks, with assessment conducted at baseline, weeks 2, 4, and 8. Depression severity was measured using the 17-item Hamilton Depression Rating Scale (HAMD 17), anxiety severity with the Hamilton Anxiety Scale (HAMA), and sleep quality with the Pittsburgh Sleep Quality Index (PSQI). Paired sample t-tests were used to evaluate within-group changes, repeated-measures ANOVA compared treatment effects between groups, and ANCOVA was employed to adjust for confounding variables. Results:Significant reductions in PSQI, HAMD 17, and HAMA scores were observed in both groups after treatment: CBT-I group: PSQI ((14.15±2.54) vs. (7.50±3.35), t=13.25), HAMD 17 ((14.70±4.09) vs. (7.40±4.61), t=9.33), and HAMA ((14.94±4.11) vs. (5.56±3.67), t=12.38) (all P<0.001).CBT-I Plus group: PSQI ((14.87±3.01) vs. (7.19±3.86), t=18.75), HAMD 17 ((16.84±3.91) vs. (6.84±4.79), t=17.42), and HAMA ((15.57±3.93) vs. (6.10±4.57), t=18.39) (all P<0.001). After adjusting for HAMD 17 scores and medication use, no statistically significant between-group differences were observed in changes in PSQI, HAMD 17, and HAMA scores ( P>0.05). A significant time-by-group interaction was found for the PSQI daytime dysfunction subscale ( F=4.87, P<0.01). Conclusion:Both CBT-I and CBT-I Plus improve sleep and emotional symptoms in patients with chronic insomnia disorder and comorbid anxiety/depression symptoms. However, CBT-I Plus has no significant advantages over standard CBT-I. Further studies are needed to refine the timing and content of interventions.
8.Major histocompatibility complex regulates immune responses in Parkinson's disease

BACKGROUND:The immune response is strongly associated with the pathological development of Parkinson's disease.Studies have shown that the major histocompatibility complex plays a key role in immune response.OBJECTIVE:To summarize the mechanism of major histocompatibility complex regulation of immune response and the effect on the pathological markerα-synuclein of Parkinson's disease.METHODS:"Parkinson's disease,the major histocompatibility complex,innate immunity,adaptive immunity,microglia,T cell,B cell,α-syn,inflammation,MHC-Ⅰ,MHC-Ⅱ"were used as search terms to search the literature in PubMed database.Finally,92 articles were included for reading analysis.RESULTS AND CONCLUSION:(1)Congenital immune response is involved in the occurrence and development of Parkinson's disease,and the change of microglia's pro-inflammatory and anti-inflammatory phenotypes may aggravate the degenerative changes of Parkinson's disease.(2)The phenotype and function of T cells are related to the progression of Parkinson's disease.Regulatory T cells promote the activation of anti-inflammatory microglia and inhibit Th subgroup.B-cell-mediated humoral immunity can clear pathological α-synuclein,and its specific mechanism needs further study.(3)The major histocompatibility complex is closely related to the occurrence of innate and adaptive immunity,and thus affects the inflammation of Parkinson's disease.(4)α-Synuclein can regulate the activation of microglia and the expression of major histocompatibility complex,which leads to inflammatory changes in Parkinson's disease.(5)α-Synuclein is closely related to the immune response of Parkinson's disease and has become an important target for the treatment of Parkinson's disease.
9.Recent advance in application of amide proton transfer-weighted imaging in common central nervous system diseases

Amide proton transfer-weighted (APTw) imaging is a kind of new magnetic resonance molecular imaging that rapidly developed in recent years. APTw imaging reflects the protein concentration and acid-base changes in tissues by detecting amide protons in free proteins and peptides in tissues. APTw imaging is a complementary technique to the existing magnetic resonance techniques, which can provide new molecular biological information for central nervous system diseases. In this article, the basic principle of APTw imaging and its application in common central nervous system diseases such as brain tumor, ischemic stroke, and neurodegenerative diseases are reviewed to provide new methods for promoting the transformation of APTw imaging from the research stage to routine clinical application, and facilitating the precise diagnosis and treatment of central nervous system diseases.
10.A case report of ischemic bowel disease caused by Shengxuexiaoban capsules

A 76-year-old male patient with lung cancer developed thrombocytopenia due to chemotherapy.Three days after taking the Shengxuexiaoban capsules orally,he experienced abdominal pain and bloody stool and was admitted to the hospital.The blood test showed white blood cell count of 11.54×109·L-1,neutrophil percentage of 82.4%,platelet count of 93×109·L-1,and C-reactive protein level of 23.50 mg·L-1.Fecal occult blood test positive,and pathology indicatesd ischemic-like changes in the rectal mucosa.The patient was diagnosed with ischemic bowel disease.After treatment with fluid replacement and anti-infection,the patient's abdominal pain disappeared and his stool turned yellow.The Naranjo's Assessment Scale was used to evaluate the association between adverse effects of ischemic bowel disease and Shengxuexiaoban capsules,and the result was very likely related.This article analysed the clinical characteristics and drug treatment process of ischemic bowel disease caused by Shengxuexiaoban capsule through this case,aiming to provide references for the safe use of drugs in the clinically.
