1.A panel study on association of short-term air pollution exposure and peripheral blood microparticles in healthy adults

Background Microparticles (MPs) are one of the main medium of inflammatory reaction with an important role in atherosclerotic progression. Studies on association of air pollution exposure and levels of peripheral blood MPs are limited among human. Objective To evaluate the effects of short-term exposure to air pollution on levels of peripheral blood MPs. Method A panel of 73 healthy adults was followed with 4 repeated follow-ups in Beijing, China, from November 2014 to January 2016. During each visit, we collected questionnaire information, fasting venous blood, urine, and exposures to fine particulate matter (PM2.5), black carbon, nitric oxide, nitrogen dioxide, nitrogen oxide, sulfur dioxide, carbon monoxide, and ozone. We used linear mixed-effect models to analyze associations of air pollution exposure with levels of total MPs (TMPs) and MPs derived from various cells. Stratified analysis was conducted by levels of C-reactive protein (CRP) and malondialdehyde (MDA). Results The results showed significant associations between air pollution exposure and peripheral blood TMPs at 2 h-6 d prior to the follow-ups (P<0.05), while no statistical associations were found for MPs derived from different cell types. Significant increases in TMPs of 7.8% (95%CI: 0.7%, 15.3%) and 14.3% (95%CI: 2.8%, 27.2%) were observed with each interquartile range (IQR) increase in PM2.5 (IQR=64.9 μg·m−3) at prior 18 h and NO (IQR=40.5 μg·m−3) at prior 48 h. Among participants with low levels of CRP and MDA, significantly positive associations were observed between air pollution exposure and levels of TMPs (P<0.05). Conclusion Short-term exposure to air pollution is significantly associated with increased levels of circulating MPs in healthy adults, and in people with lower systemic inflammation, peripheral blood MPs levels are more easily affected after exposure to air pollutants.

A unified consensus statement on medication-related osteonecrosis of the jaw (MRONJ) has not yet been established among the Asian member countries or regions of the Asian Federation of Osteoporosis Societies (AFOS). This study aimed to develop a consensus on MRONJ in patients with osteoporosis across these countries and regions. In this study, the term “Asia-Pacific” refers specifically to the Asian member countries and regions of AFOS. A structured survey consisting of nine MRONJ-related questions was distributed across 10 countries and regions to assess the level of agreement and summarize regional perspectives. In addition, a manual literature review and voting were conducted to evaluate the current evidence on MRONJ. The key aspects of MRONJ, including definition, staging, diagnosis, pathogenesis, risk factors, management, and prevention, were generally consistent among the AFOS countries and regions. The annual incidence and incidence rate of MRONJ associated with low-dose antiresorptive therapy in patients with osteoporosis ranged from 0.025% to 0.136% and 21 to 283 cases per 100,000 person-years, respectively. However, evidence regarding the benefits of drug discontinuation before dental surgery, such as tooth extraction, remains insufficient. Large-scale, multinational studies across AFOS countries and regions are warranted to determine the incidence of MRONJ better and evaluate the impact of antiresorptive drug discontinuation before dental procedures. These findings may contribute to the devel opment of effective evidence-based strategies for preventing MRONJ in patients with osteoporosis.

Background/Aims:
Liver transplantation (LT) following total hepatectomy is a life-saving treatment for hepatocellular carcinoma (HCC). The HCC recurrence after LT hinders the effectiveness of the procedure. The objective of this study is to develop a pre-operative risk stratification model based on a liquid biopsy.
Methods:
We conducted a comprehensive multi-omics study of 260 HCC patients from three centers, including clinical data, low-coverage whole-genome sequencing of cell-free DNA (cfDNA) from plasma, as well as whole-exome, single-nucleus RNA, and spatial transcriptomics from matched tumor and non-tumor tissues.
Results:
We identified cfDNA-derived copy number alteration (CNA) signatures associated with post-transplant recurrence. By integrating cfDNA-derived CNA profiles with single-cell transcriptomic data, we traced recurrence-associated cfDNA to a distinct subpopulation of malignant cells within the primary tumor. These cells were embedded in a pro-metastatic microenvironment of specialized endothelial subtypes and cancer-associated fibroblasts. Notably, most recurrence-associated lesions were detectable in cfDNA prior to liver transplantation (LT). Building on these insights, we developed the ZJU Criteria based on CNA fragments and tumor markers, a pre-LT risk prediction tool that integrates conventional clinical factors with cfDNA-derived CNA signatures, and validated it using internal and independent external cohorts.
Conclusion
Our findings suggest that post-transplant recurrence commonly originates from advanced subclones that emerge late during tumor evolution. The ZJU Criteria provides an accurate, non-invasive strategy that significantly improves pre-LT risk stratification and clinical decision-making for patients with HCC.

Objective To explore the preoperative assessment of the differentiation degree of hepatocellular carcinoma(HCC),based on the imaging features observed on gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced MRI HCC.Methods A retrospective analysis was conducted on the data of 198 patients with HCC confirmed by surgery or biopsy pathology,all of whom underwent Gd-EOB-DTPA enhanced examination before operation.According to pathological results,HCC patients were divided into poorly-differentiated group(group A,69 cases),moderately-differentiated group(group B,97 cases)and well-differentiated group(group C,32 cases).The signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),relative intensity ratio(RIR,,RIR2),and enhancement ratio(ER)of hepatobiliary phase HCC were calculated and analyzed.Results The CNR was the high-est in the group A and the lowest in the group C.Conversely,the SIHCC unenhanced,SIHCC hepatobiliary phase,and RIR1,RIR2 all were the highest in the group C and the lowest in the group A.There were statistically significant differences between the group C and the group B,as well as the group C and the group A(P<0.05).Regarding signal characteristics during the hepatobiliary phase,the group A had the highest proportion of low/slightly low signal,while the group C had the lowest proportion.A statistically significant difference was observed between the group C and the group A(P<0.05).Conclusion The Gd-EOB-DTPA enhanced MRI is helpful to preoperative assessment the differentiation degree of HCC.The higher the signal in the hepatobiliary phase,the smaller the CNR,the larger val-ues for the SIHCCunenhanced,SIHCC hepatobiliaryphase and RIR1,RIR2 are associated with the better the differentiation degree of HCC.

6.Mining mitophagy-related target genes in diabetic foot ulcers and analysing their immune relevance based on bioinformatics analysis and machine learning

Objective To investigate mitophagy in diabetic foot ulcer(DFU)target genes and analyze the immune correlation.Methods DFU related datasets were obtained from the Gene Expression Omnibus(GEO)and screened for their differential expression genes(DEGs).Mitophagy related genes(MRGs)were obtained from the Genecards database.DEGs and MRGs were intersected and analysed using gene ontology(GO),Kyoto Encyclopedia of Genes and Genomes(KEGG)and protein-protein interaction(PPI).Target genes were mined using four machine learning models:Support vector machine-recursive feature eliminatio,least absolute shrinkage and selection operator,random forest,extreme gradient boosting.DEGs were analysed for immune infiltration and the correlation of target genes with immune infiltration.Results GO enrichment results showed that it was mainly enriched in regulation of autophagy of mitochondrion.KEGG enrichment results showed that hypoxia inducible factor-1 signalling pathway,p53 signalling pathway and AMPK signalling pathway were mainly enriched.Machine learning results showed TP53 and CYCS as target genes.Immune infiltration results showed a clear correlation of TP53 with plasma cells,regulatory T cells,follicular helper T cells,CD8+T cells and monocytes.CYCS showed a clear correlation with B cell memory,T cells CD8+T cells,monocytes,plasma cells and regulatory T cells.Conclusion TP53,CYCS may be the target genes regulating mitophagy in DFU and have a key role in reducing the level of oxidative stress.At the same time,the dysregulation of plasma cells,T cells and other immune cells may also have some effect on mitophagy.
7.Theme identification and evolution analysis of compensation mechanism reform policy in public hospitals since the new healthcare reform

Objective:To analyze the thematic distribution characteristics and evolution trends of the compensation mechanism reform policies for public hospitals in China since the new healthcare reform,and to provide references for correctly understanding the basic connotation and extension of the reform of compensation mechanism.Methods:A total of 75 documents issued at the national level were collected.The BERTopic topic model was used for thematic identification.The dynamic topic modeling method of BERTopic was adopted to conduct policy evolution analysis.Result:Since the new medical reform,the reform policy of compensation mechanism in public hospitals can be divided into eight themes,such as strengthening the operation management of public hospitals and adjusting the price of medical services.It shows an evolutionary trend of emphasizing the control of excessive growth of medical expenses and the implementation of government investment responsibilities in the early stage,focusing on the elimination of drug additions and the adjustment of medical service prices in the middle stage,and strengthening the reform of medical insurance payment methods in the later stage.Conclusions:The reform of the compensation mechanism for public hospitals is essentially a systematic reconstruction of the operation mechanism of public hospitals.Adjusting and optimizing medical service prices and reforming the medical insurance payment method are the key and focus of improving the compensation mechanism for public hospitals.
8.Effect of Ba Duan Jin Combined with Du Mai Fumigation on Improving Sleep Quality and Negative Emotions in Perimenopausal Insomnia Patients

Objective:To observe the effect of Ba Duan Jin combined with Du Mai fumigation on improving sleep quality and negative emotions in perimenopausal insomnia patients.Methods:This study was a single-center prospective randomized controlled trial,80 perimenopausal insomnia patients who were admitted to Zhuzhou Maternal and Child Health Hospital from December 2023 to August 2024 were selected.Patients were randomly divided into control group(treated with Du Mai fumigation)and study group(treated with Ba Duan Jin combined with Du Mai fumigation)used random number table method,with 40 cases in each group.The clinical efficacy,sleep quality[Pittsburgh Sleep Quality Index(PSQI)],negative emotions[Self Rating Anxiety Scale(SAS),Self Rating Depression Scale(SDS)],clinical symptoms(Kupperman score),improvement in quality of life[the short form-36 health survey(SF-36)Score],and incidence of adverse reactions between two groups were compared before and after treatment.Results:The clinical total efficiency rate in the study group was higher than that in the control group(P<0.05).2 weeks and 4 weeks after treatment,PSQI,SAS,SDS and Kupperman scores in the study group were lower than those in the control group,and SF-36 score was higher than that in the control group(P<0.05).Neither group showed significant adverse reactions.Conclusion:Ba Duan Jin combined with Du Mai fumigation on improving sleep quality and negative emotions in perimenopausal insomnia patients,can effectively improve sleep quality,alleviate negative emotions,and enhance quality of life,without any significant adverse reactions.
9.Correlation Study between TCM Susceptibility of Pulmonary Nodules and Methylation of SHOX2/RASSF1A/PTGER4 Genes

Objective To investigate the influencing factors associated with the risk of positive methylation levels of SHOX2/RASSF1A/PTGER4 genes in patients with pulmonary nodules with TCM susceptibility.Methods Using a cross-sectional study method,totally 165 patients with pulmonary nodules from the Respiratory Department of Wangjing Hospital,China Academy of Chinese Medical Sciences were included from May 2024 to December 2024.Clinical data such as patients'general medical history,nodule imaging reports,lung cancer gene methylation levels,and TCM constitution assessments were organized and statistically analyzed.On the basis of exploring the correlation between various variables and the risk of positive methylation of lung cancer genes through univariate correlation analysis,a binary logistic regression model was used for independent risk factor analysis.Results Univariate correlation analysis showed that age,gender,TCM susceptibility score,diameter of pulmonary nodules and upper lobe of the right lung were correlated with positive methylation levels of the SHOX2/RASSF1A/PTGER4 genes(P=0.03,P=0.02,P=0.01,P<0.01,P=0.03);and binary Logistic regression analysis showed that yang-deficiency quality,qi deficiency,qi depression and phlegm-dampness,the four pulmonary nodules susceptibility body scores were significantly and positively correlated with the risk of SHOX2/RASSF1A/PTGER4 gene methylation positivity.Compared with yang deficiency,phlegm-dampness and qi depression had a higher risk of gene methylation positivity for patients with pulmonary nodules(P=0.04,P=0.02).Conclusion Older age,males,and TCM susceptibility body correlated with the lung cancer gene methylation positive risk are correlated,in which phlegm dampness and qi depression are independent risk factors.
10.Optimization of 90Y PET/CT imaging based on the block-sequential regularized expectation maximization reconstruction algorithm

Objective:To optimize the image quality of PET/CT following 90Y-selective internal radiation therapy ( 90Y-SIRT) using block-sequential regularized expectation maximization (BSREM) reconstruction algorithm, and to evaluate its impact of different β values on image quality and quantitative analysis. Methods:A retrospective study was conducted on 8 male patients with hepatic tumors (age: 62(52, 71) years) treated at Beijing Tsinghua Changgung Hospital, School of Clinical Medicine, Tsinghua Medicine Tsinghua University, between June 2024 and January 2025. All patients were treated with 90Y resin microspheres (2.6(0.9, 3.6)GBq) and underwent post-treatment 90Y PET/CT liver imaging. Imaging data were reconstructed using BSREM with different noise penalty weighting factors ( β values: 0, 300, 1000, 1500, 2500, 3500, 4000, 6000, 8000, 10000). Visual assessment was independently performed by two nuclear medicine physicians, using a 4-point scale (1=worst, 4=best). The mean score was considered as the final score. The consistency of the 2 reviewers was calculated and analyzed by Kappa test. Visual scores of different β value groups were compared by Friedman test. The β value demonstrating highest mean score and optimal consistency was selected as the optimal. Quantitative analysis was performed using MIM software to calculate the maximum absorbed dose ( Dmax) and the mean absorbed dose ( Dmean) for tumor, normal liver, and whole liver regions, and the CV was used to evaluate the impact of β values. Results:The visual assessment consistency of reviewers in 3 β value groups (0, 3500, 6000) were the highest (7/8) (all kappa=0.88, all P<0.05). Visual scores of the 10 β value groups were significantly different ( χ2=28.74, P<0.001), and the visual scores of 2 β value groups (3500, 4000) were the highest, both of which were 4.0(4.0, 4.0). Overall, visual assessment identified β=3500 as the optimal. Quantitative analysis revealed that, (1) Dmax in all regions (tumor, normal liver, whole liver) decreased with the increasing β values, stabilizing when β>1000 ( CV 56%-67%); (2) Dmean remained stable across different β values ( CV 0.04%-5.00%). Conclusions:In BSREM reconstruction, β=3500 is the optimal parameter for improving 90Y-PET image quality. β values significantly affect Dmax (stabilizing at β > 1000), but have no significant impact on Dmean, suggesting that reconstruction parameters primarily influence dose distribution morphology rather than average dose assessments.
