1.YOLOv8m-segmentation for detecting cervical burnout and caries in bitewing radiographs:A deep learning approach

Purpose:
This study evaluated the performance of the YOLOv8m-seg model in detecting and delineating interproximal caries and cervical burnout on bitewing radiographs and examined whether increasing the number of training epochs improved segmentation accuracy and consistency.
Materials and Methods:
In total, 1,410 bitewing radiographs were annotated using polygon-based masks by a trained dental clinician. The YOLOv8m-seg model was trained for 50, 100, and 150 epochs on 1,128 images and validated on 282 images using the Ultralytics segmentation framework. Model performance was assessed using precision, recall, and mean average precision at intersection-over-union thresholds of 0.5 and 0.5 to 0.95 (mAP0.5, mAP0.5-0.95) for both bounding box and mask outputs. Additional evaluation was conducted on a non-augmented validation subset.
Results:
Extended training duration was associated with improved segmentation performance. The highest mask mAP0.5-0.95 value was 0.828 at epoch 150. Both box-based precision and recall increased with longer training, whereas mask-based evaluation more accurately reflected the model’s ability to delineate the boundaries of caries and cervical burnout. Performance appeared consistent across both classes in the augmented validation split but was reduced in the non-augmented validation subset.
Conclusion
The YOLOv8m-seg model demonstrated high diagnostic accuracy in distinguishing proximal caries from cervical burnout on bitewing radiographs. Its mask-based outputs may assist clinicians in early lesion recognition and support improved diagnostic decision-making. Future studies should evaluate model generalizability across broader populations and diverse clinical environments and should prioritize assessment using non-augmented validation sets and independent test datasets.
2.Inhibition of the Arp2/3 Complex Attenuates Angiotensin Ⅱ-Induced Cardiomyocyte Hypertrophy

Pathological cardiac hypertrophy is an early and significant cardiac structural charac-teristic that contributes to the onset and progression of heart failure(HF).Its mainly structural feature is the abnormally enlarged cardiomyocyte.Effective intervention targets for abnormally en-larged cardiomyocyte remain to be identified.Previous studies have shown that the cellular shape and size can be regulated by the actin related protein 2/3(Arp2/3)complex,which is an actin-binding protein complex involved in the actin nucleation and assembly.However,the roles of the Arp2/3 complex in cardiomyocyte hypertrophy remain unknown.Here our study identifies its no-vel roles in the occurrence and development of cardiomyocyte hypertrophy.We found that mRNA levels of all subunits from the Arp2/3 complex are significantly upregulated(P<0.05)in the an-giotensin II(Ang Ⅱ)-induced neonatal rat primary and H9c2 cardiomyocyte hypertrophy.Fur-ther studies showed that siRNA-directed ARPC2 silencing inhibits the reactivation of fetal genes and enlargement of cardiomyocyte area induced by Ang Ⅱ in neonatal rat primary cardiomyocytes(NRCMs)and H9c2 cells(P<0.05).In addition,the upstream activators of the Arp2/3 com-plex including SH3 protein interacting with Nck,90 kD(SPIN90)and Ras-related C3 botulinum toxin substrate 1(Rac1)/WASp family Verprolin-homologous protein-2(WAVE-2)are upregu-lated(P<0.05)in Ang Ⅱ-induced neonatal rat primary and H9c2 cardiomyocyte hypertrophy,indicating the excessive activation of the Arp2/3 complex.We further show that CK666,a specif-ic Arp2/3 complex inhibitor,prevents the reactivation of fetal genes and the enlargement of car-diomyocyte area induced by Ang Ⅱ in NRCMs and H9c2 cells(P<0.05).Our results reveal that the Arp2/3 complex plays a crucial role in Ang Ⅱ-induced cardiomyocyte hypertrophy,which is beneficial to further studies about the molecular mechanisms by which the Arp2/3 complex regu-lates pathological cardiac hypertrophy.
3.Clinical study of laparoscopic high ligation of hernia sac combined with IPOM and local anesthesia Lichtenstein repair in the treatment of male patients with inguinal hernia after radical cystectomy

Objective To compare the clinical efficacy of laparoscopic high ligation of the hernia sac combined with intraperitoneal onlay mesh(IPOM)and local anesthesia Lichtenstein repair for the treatment of inguinal hernia after total cystectomy in male patients.Methods A total of 59 male patients who underwent total cystectomy were included in this study and randomly divided into two groups:the laparoscopic high ligation of the hernia sac combined with IPOM group(IPOM group,30 cases)and the local anesthesia Lichtenstein repair group(local anesthesia group,29 cases).The patient age,surgical time,postoperative ambulation time,visual analogue scale score for pain on the 1st to 3rd days and 3 months after operation,time to first mobilization post-operation,pain scores on postoperative day 1,2 and 3,pain scores at 3 months post-operation were observed and compared between the two groups.Data of intraoperative blood loss,hospital stay,time to return to daily activities,total surgery costs,recurrence rates at 1 year after operation and complications were also observed and compared in two groups of patients.The results were analyzed to determine the optimal surgical method.Results The IPOM group demonstrated better outcomes in terms of surgical time,time to first mobilization,pain scores on postoperative days 1,2 and 3,pain scores at 3 months,intraoperative blood loss,hospital stay and time to return to daily activities compared to those of the local anesthesia group(P<0.01).However,the total cost of the operation was higher in the IPOM group than those of the local anesthesia group(P<0.01).There were no significant differences in postoperative complications between the two groups(P>0.05).The recurrence rate of the IPOM group one year after surgery was better than that of the local anesthesia group(P<0.05).Conclusion Laparoscopic IPOM with high ligation of the hernia sac is superior to the traditional Lichtenstein repair in reducing postoperative pain and recurrence rates.This surgical method should be the first choice when economic conditions permit.

Gut microbiota is the microbial community that is specifically planted in the human gut and interacts with the human body.It plays an important role in the human body by regulating the basal metabolism and immunity of the host to maintain the homeostasis of the human body.Once the intestinal flora is dysregulated,it can lead to multi-system diseases.Hashimoto's thyroiditis is an organ-specific autoimmune disease.The pathogenesis of Hashimoto's thyroiditis is complex and its incidence is increasing year by year,which is gradually becoming a serious public health disease affecting the world.In recent years,the relationship between gut microbiota and bridge onychitis has received extensive attention.A large number of studies have shown that the richness and diversity of intestinal flora in Hashimoto's thyroiditis patients are changed compared with healthy people.Studies have shown that traditional Chinese medicine therapy can play a role in treating this disease by regulating intestinal flora.This article systematically summarizes the relationship between the two and the effect of traditional Chinese medicine on the treatment of bridge onychitis by regulating intestinal flora,so as to provide some reference for research in this field.

6.Study on mechanism of Jiawei Shaofu Zhuyu decoction in treatment of endometriosis fibrosis based on mitophagy

Aim To explore the mechanism of Jiawei Shaofu Zhuyu decoction in antagonizing endometriosis fibrosis by regulating mitophagy.Methods After the animal model was constructed,the syndrome was evalu-ated by general condition,organ water content and ther-mal imaging.The curative effect was evaluated by the weight of ectopic focus and the degree of adhesion.The pathological changes were compared using HE stai-ning,transmission electron microscopy,Masson and Sir-ius red staining.The expression of PINK1 and Parkin was detected by immunohistochemistry.The expression of mRNA and protein was determined by qPCR and Western blot,and the level of serum ROS was detected by ELISA.Results The autonomic activity of model mice was weakened,the water content of organs rose,and the temperature of limbs and lower abdomen was reduced by thermal imaging.HE staining showed obvi-ous hyperplasia of ectopic epithelium and glands.Transmission electron microscopy showed mitochondrial and endoplasmic reticulum structure damage,and nor-mal autophagy structure disappeared.Masson and Siri-us red staining showed increased collagen deposition;immunohistochemistry showed decreased expression of PINK1 and Parkin in ectopic foci.qPCR and Western blot showed that the expression of PINK1,Parkin,Bec-lin1,LC3 mRNA and protein in ectopic foci of model mice decreased,the expression of p62 mRNA and pro-tein increased,and serum ROS increased.The syn-drome performance of model mice was improved after the intervention of Jiawei Shaofu Zhuyu decoction;the inflammatory infiltration of ectopic foci was relieved,the morphology of mitochondria and endoplasmic retic-ulum was restored,and normal autophagy structure ap-peared.The degree of collagen deposition and fibrosis was reduced;the mRNA and protein expression of PINK1,Parkin,Beclin1 and LC3 increased.The ex-pression of p62 mRNA and protein decreased,and the level of ROS decreased.Conclusions Jiawei Shaofu Zhuyu decoction can improve the fibrosis of ectopic le-sions in mice with endometriosis of cold-dampness sta-sis syndrome,which may be related to the regulation of mitophagy.

8.Clinical effect analysis of rigid laryngoscopy performed in two body positions

Objective To explore the optimal body position of patients during rigid laryngoscopy.Methods 120 outpatients who underwent rigid laryngoscopy from December 2023 to March 2024 were divided into the conventional body position group(58 cases)and the anterior tilted position group(62 cases)by the random number table method.Patients in the conventional body position group were examined in the conventional body position(upright lower head position)and patients in the anterior tilted position group were examined in the anterior tilted position(body tilted forward by 60° and jaw lifted).The vocal folds were scored on the Friedman scale,the visual analogue scale(VAS)score was used to evaluate the comfortable degree of patients during the examination,the incidence of gag reflex sensitivity and the rate of complete pharyngeal exposure were compared between the two groups of patients.Results In the conventional body position group,there were 26 cases of Friedman's difficult exposure and 32 cases of easy exposure.The VAS scores of difficult exposure and easy exposure patients were 2.5(1.0,5.8)and 1.0(1.0,3.0),respectively;The incidence of gag reflex sensitivity was 42.3%and 34.4%,respectively;and the rates of complete pharyngeal exposure were 76.9%and 100.0%,respectively.In the anterior tilted position group,there were 28 cases of Friedman difficult exposure and 34 cases of easy exposure.The VAS scores of patients with difficult and easy exposure were 1.5(1.0,7.0)and 2.0(1.0,4.0),respectively;The incidence of gag reflex sensitivity was 46.4%and 29.4%,respectively;And the rates of complete pharyngeal exposure were 82.1%and 91.2%,respectively;The differences were not statistically significant(P>0.05).Conclusion When rigid laryngoscopy is performed in the upright lower head position or anteriorly tilted position,there is no significant difference in the comfortable degree scores,incidence of gag reflex sensitivity,and the rate of complete pharyngeal exposure in the patients in the two positions,and the examiners can choose the appropriate position according to their own examination habits.

10.Analysis on the synergy degree between financing and payment of basic medical insurance for urban workers in China under the background of aging

Objective:To analyze the synergistic relationship between financing and payment systems in China's urban employee basic medical insurance under the background of aging,providing decision-making basis for promoting coordinated development of these two systems.Methods:Based on statistical yearbook data from medical security systems,the Delphi method was employed to identify collaborative analysis indicators for urban employee medical insurance financing and payment under the aging population context.A collaborative theory-based model was constructed to measure the coordination degree of China's urban employee medical insurance financing and payment composite system,while simultaneously assessing the orderliness of the financing and payment system and the coordination degree of the composite system.Results:The order degree of the financing system ranged from 0.324 in 2019 to 0.517 in 2023;the order degree of the payment system ranged from 0.454 in 2019 to 0.517 in 2023;the synergy degree of both systems reached-0.084 in 2023.Conclusions:Influenced by aging,the development speed and scale of China's urban employee basic medical insurance financing system and payment system are asynchronous,with the payment system being more affected than the financing system,resulting in a non-coordinated and disordered development state of the composite financing-payment system.
