OBJECTIVE To investigate the differences in audiological characteristics and speech recognition rates between vestibular schwannoma(VS)patients presenting with sudden sensorineural hearing loss(SSNHL)as first symptom and patients with idiopathic sudden deafness(ISD),in order to provide a reference for clinical diagnosis and differential diagnosis.METHODS A retrospective analysis was conducted on 60 patients with VS presenting as SSNHL(VS group),and 60 patients with unilateral ISD(SD group).Pure-tone thresholds, audiogram configurations, and speech recognition scores were compared between the two groups.Statistical analyses were performed using t-test,Mann-Whitney U test, and chi-square test.RESULTS Hearing loss in the VS group was mainly distributed in the moderate to profound range, and the proportion of descending-type audiograms was significantly higher than that in the SD group(χ2=13.97,P=0.002 9).In contrast, the SD group was predominantly characterized by mild to moderate hearing loss and flat-type audiograms.Regarding speech recognition, the VS group showed significantly poorer monosyllabic recognition(38.6%±40.4%)and sentence recognition(53.4%±42.0%)compared with the SD group(59.0%±37.8%,75.8%±36.0%,P<0.01).Notably,some VS patients exhibited complete loss of speech recognition even before the pure-tone average reached the level of total deafness.CONCLUSION VS patients presenting with SSNHL showed significant differences in audiogram configurations and speech recognition compared with those with ISD.A marked decline in speech recognition, combined with a typical descending-type audiogram, may serve as important clinical indicators, suggesting that early imaging examinations should be performed to confirm the diagnosis.

OBJECTIVE:
This study aimed to identify high-risk areas for type 2 diabetes mellitus (T2DM) mortality to provide relevant evidence for interventions in emerging economies.
METHODS:
Empirical Bayesian Kriging and a discrete Poisson space-time scan statistic were applied to identify the spatiotemporal clusters of T2DM mortality. The relationships between economic factors, air pollutants, and the mortality risk of T2DM were assessed using regression analysis and the Poisson Log-linear Model.
RESULTS:
A coastal district in East Guangdong, China, had the highest risk (Relative Risk [RR] = 4.58, P < 0.01), followed by the 10 coastal districts/counties in West Guangdong, China (RR = 2.88, P < 0.01). The coastal county in the Pearl River Delta, China (RR = 2.24, P < 0.01), had the third-highest risk. The remaining risk areas were two coastal counties in East Guangdong, 16 districts/counties in the Pearl River Delta, and two counties in North Guangdong, China. Mortality due to T2DM was associated with gross domestic product per capita (GDP per capita). In pilot assessments, T2DM mortality was significantly associated with carbon monoxide.
CONCLUSION
High mortality from T2DM occurred in the coastal areas of East and West Guangdong, especially where the economy was progressing towards the upper middle-income level.

In most types of erectile dysfunction, particularly in advanced stages, typical pathological features observed are reduced parenchymal cells coupled with increased tissue fibrosis. However, the current treatment methods have shown limited success in reversing these pathologic changes. Recent research has revealed that changes in autophagy levels, along with alterations in apoptosis and fibrosis-related proteins, are linked to the progression of erectile dysfunction, suggesting a significant association. Autophagy, known to significantly affect cell fate and tissue fibrosis, is currently being explored as a potential treatment modality for erectile dysfunction. However, these present studies are still in their nascent stage, and there are limited experimental data available. This review analyzes erectile dysfunction from a pathological perspective. It provides an in-depth overview of how autophagy is involved in the apoptotic processes of smooth muscle and endothelial cells and its role in the fibrotic processes occurring in the cavernosum. This study aimed to develop a theoretical framework for the potential effectiveness of autophagy in preventing and treating erectile dysfunction, thus encouraging further investigation among researchers in this area.

Immune checkpoint inhibitors(ICIs)represent the most widely used immunotherapeutic approach for antitumor treatment, yet the understanding of their associated hepatotoxicity remains incomplete. This article delves into and analyzes the similarities and differences among the management guidelines on ICI-related hepatotoxicity issued by the Chinese Society of Clinical Oncology(CSCO),the National Comprehensive Cancer Network(NCCN)of the United States, and the American Society of Clinical Oncology(ASCO),aiming to provide a more comprehensive management strategy for clinical practice.By reviewing and analyzing the latest guidelines, this study compares the differences and similarities in the diagnosis, assessment, grading criteria, and treatment strategies for ICI-related liver toxicity among these guidelines. The definitions and diagnostic criteria for ICI-related liver toxicity are generally consistent across different guidelines, primarily relying on the elevated levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),bilirubin, and alkaline phosphatase(ALP)for grading.Notably, the ASCO guidelines place a stronger emphasis on the assessment of symptoms of hepatic dysfunction. In terms of treatment strategies, all guidelines recommend using corticosteroids or immunosuppressants based on the toxicity grade. However, there are discrepancies in management strategies among the guidelines.Clinicians should tailor management strategies by considering the specific conditions of patients and integrating the recommendations from various guidelines.Additionally, given the current inadequate understanding of ICI-induced hepatotoxicity primarily manifested as cirrhosis in the existing guidelines,it is imperative to continuously update and refine these management guidelines as research progresses and clinical experience accumulates.

Objective:To analyze the effect of rapid division of left Glisson pedicle and Arantius tube plane in laparoscopic anatomical left hemihepatectomy (LALH).Methods:Clinical data of 25 patients (15 with intrahepatic bile duct calculus and 10 with liver tumor) undergoing LALH in the Department of Hepatobiliary and Pancreatic Surgery, Jiangyin Hospital Affiliated to Nantong University from June 2020 to November 2024 were retrospectively analyzed, including 14 males and 11 females, aged (66.6±11.9) years. Among the patients, 15 received LALH with rapid division of left Glisson pedicle and Arantius tube plane in " one-clamp" fashion, and the others received LALH after traditional dissection of left Glisson pedicle. Age, sex, body mass index, time of left Glisson pedicle dissection, whether the MHV exposure, the time of liver transection, whether MHV and its important branches or bile duct injury occurred, intraoperative blood loss, the incidence of postoperative biliary leakage, the time of abdominal drainage remove, the hospital stay, and alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin, albumin 3 days after postoperative were compared between the groups. Results:The times of left Glisson pedicle dissection in the new-fasion and traditional group were (6.1±1.6) min and (13.8±3.0) min, and the time of liver transection was (24.9±3.5) min and (33.4±3.3) min, respectively ( t=-8.34, 6.08, P<0.001 for both). After division of left Glisson pedicle, the MHV was well exposed in 14 cases of new-fashion group and none of traditional group ( P<0.001). All the patients successfully completed the operation without conversion to laparotomy. Intraoperative blood loss, incidence of postoperative bile leakage, time of peritoneal drainage tube removal, postoperative hospital stay, AST, ALT, total bilirubin and albumin 3 days after surgery between the two groups were no significant differences (all P>0.05). Conclusion:LALH using the rapid division of left Glisson pedicle and Arantius tube plane in " one-clamp" fashion could be safe and feasible, the time of left Glisson pedicle and liver transection was short.

Objective: To explore the effects of Cldn14 gene knockout on renal metabolism and stone formation in rats,so as to provide reference for research in the field of urinary calium metabolism and stone formation. Methods: Cldn14 gene knockout homozygous rats and wild-type rats of the same age were randomly divided into 4 groups:wild-type control (WC) group,wild-type ethylene glycol (WE) group, gene knockout control (KC) group and gene knockout ethylene glycol (KE) group, with 10 rats in each group. The WE and KE groups were induced with ethylene glycol + ammonium chloride to form kidney stones, while the WC and KC groups received normal saline gavage. After 4 weeks of standard maintenance feeding, the urine samples were collected to detect the venous blood. The kidneys were collected for HE,Pizzolatto's staining and transmission electron microscopy. The protein in renal tissues was extracted to detect the expressions of Claudin16 and Claudin19. Results: Crystal deposition was observed in the renal tubular lumen of the WE and the KE groups, and more crystals were detected in the KE group. The WE group had a large number of intracytoplasmic black crystalline inclusions observed in renal tubular epithelial cells under transmission electron microscope, followed by the KE and KC groups.Compared with WC and WE groups,KC and KE groups had significantly decreased serum calcium and magnesium levels but significantly increased urinary calcium level. In addition, the urinary calcium level was higher in the WE group than in the WC group and higher in the KE group than in the KC group. The KE group had lower level of Claudin16,but there was no significant difference in the level of Claudin19 among the 4 groups(P>0.05). Conclusion: Knockout of Cldn14 gene alone cannot effectively reduce urinary calcium excretion or reduce the risk of stone formation in rats, which may be related to the decrease of Claudin16 level.
