BACKGROUND:Currently, numerous experiments delve into the intricate anatomy and biomechanical behavior of distinct segments of the supraspinatus muscle. However, the impact of shoulder joint stress resulting from damage to various regions of this muscle remains a scarcely explored domain.Understanding the repercussions of supraspinatus muscle injuries across different regions on the stress distribution and magnitude of articular cartilage and the glenoid is crucial for providing some theoretical support for clinical diagnosis and treatment. OBJECTIVE:To ascertain the maximum stress values by simulating different degrees of supraspinatus muscle rupture on the humeral cartilage surface, glenoid lip,and glenoid cartilage joint surface using three-dimensional finite element software. METHODS:Normal and healthy shoulder joint CT or MRI scans were processed through Mimics and Geomagic to extract molds.Subsequently, models were constructed via Solidworks.Varying degrees of supraspinatus muscle damage were simulated for each model to mimic fractures in different regions. Finally,Ansys, mechanical software, was employed for three-dimensional finite element biomechanical analysis, calculating stress values for the humeral cartilage surface, glenoid lip,and glenoid cartilage joint surface. RESULTS AND CONCLUSION:(1)With worsening degrees of supraspinatus muscle injury, the stress on the shoulder joint cartilage surface and glenoid lip escalated.(2)Among various regions, the anterior part of the supraspinatus muscle exhibited paramount significance.(3)While supraspinatus muscle fractures of differing degrees impacted the magnitude of cartilage stress on the glenoid labial surface, the stress distribution remained constant.(4)It is indicated that during the initial stages of horizontal abduction of the shoulder joint, the anterior region assumes a pivotal role, followed by the posterior deep region.Injury to the anterior part of the supraspinatus muscle leads to a significant surge in stress within the shoulder joint's soft tissue, potentially causing damage to the top of the glenoid lip and the anterior part of the glenoid cartilage.

Ultra performance liquid chromatography-quadrupole-time-of-flight-mass spectrometry/mass spectrometry(UPLC-Q-TOF-MS/MS), network pharmacology, and animal experiments were integrated o explore the blood glucose-lowering effects and mechanisms of Poria aqueous extract. Firstly, the active components of Poria aqueous extract were identified by UPLC-Q-TOF-MS/MS. Subsequently, network pharmacology was employed to predict the blood glucose-lowering components and mechanisms of Poria aqueous extract. Finally, a rat model of diabetes mellitus, 16S rDNA sequencing, and Western blot were employed to investigate the blood glucose-lowering effect and mechanism of Poria aqueous extract. A total of 39 triterpenoids were identified in the Poria aqueous extract, among them, 25-hydroxypachymic acid, 25α-hydroxytumulosic acid, 16α-hydroxytrametenolic acid, polyporenic acid C, and tumulosic acid may be the main active ingredients for treating diabetes. The Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analysis revealed that Poria might exert its therapeutic effects through multiple pathways such as NOD-like receptor signaling pathway, nuclear factor-kappa B(NF-κB) signaling pathway, and tumor necrosis factor(TNF) signaling pathway. The results of animal experiments demonstrated that Poria aqueous extract significantly reduced the levels of blood glucose and lipids and regulated the intestinal flora in diabetic rats. The main affected taxa included g_Escherichia-Shigella, g_Corynebacterium, g_Prevotella_9, g_Prevotellaceae_UCG-001, and g_Bacteroidota_unclassified. In addition, Poria aqueous extract lowered the levels of D-lactic acid and lipopolysaccharide, alleviated colonic mucosal damage, significantly down-regulated the protein levels of NOD-like receptor pyrin domain-containing protein 3(NLRP3), NF-κB, and TNF-α, and significantly up-regulated the protein levels of zonula occludens 1 and occludin in diabetic rates. Poria aqueous extract may play a role in treating diabetes mellitus by repairing the intestinal flora disturbance, protecting the intestinal barrier function, and inhibiting the NF-κB/NLRP3 signaling pathway. The results provide a scientific basis for clinical application and expansion of indications of Poria.

OBJECTIVE:
To explore clinical efficacy of percutaneous endoscopic discectomy with a lateral approach and dual-channel method in treating highly free lumbar disc herniation(LDH).
METHODS:
A retrospective analysis was conducted on 54 patients with highly free LDH who were treated with spinal endoscopic techniques from January 2021 to December 2022. Twenty-seven patients were treated with lateral approach dual-channel(lateral approach dual-channel group), including 16 males and 11 females, with an average age of (54.6±10.5) years old. Twenty-seven patients were treated with unilateral biportal endoscopic (UBE group), including 17 males and 10 females, with an average age of (52.9±12.3) years old. The number of intraoperative fluoroscopy, operation time and hospital stay, as well as visual analogue scale (VAS) and Oswestry diability index (ODI) of low back and leg pain between two patients before operation, 1 day, 1, 3, and 12 months after operation, and the efficacy was evaluated by the modified MacNab criteria at 12 mohths after operation.
RESULTS:
All patients were successfully completed surgical and were followed up, the time raged from 12 to 22 months with an average of (13.57±4.12) months. There was no statistically significant difference in operation time between two groups (P>0.05). The hospital stay of lateral approach dual-channel group was (3.9±1.1) days, which was shorter than that of UBE group (6.5±1.4) days, the number of intraoperative fluoroscopy in lateral approach dual-channel group was (12.7±2.1) times, which was more than that in UBE group (6.6±1.3) times, the differences were statistically significant (t=5.197, -7.532;P<0.05). VAS and ODI for low back pain at 1 day and 1 month after operation, and VAS for leg pain at 1 day after operation of lateral approach dual-channel group were superior to those of UBE group, and the differences were statistically significant (P<0.05). However, there were no statistically significant differences in VAS and ODI for low back and leg pain between two groups before operation and 3 and 12 months after operation (P>0.05). VAS and ODI of low back and leg pain were significantly improved at each time point before and after operation in both groups, and the difference were statistically significant (P<0.05). At 12 months after operation, according to the modified MacNab criteria, the excellent and good rates of therapeutic effects between lateral approach dual-channel group and UBE group were 92.6% (25/27) and 88.9% (24/27), respectively, and the difference was not statistically significant (χ2=0.22, P>0.05).
CONCLUSION
For patients with highly free lumbar intervertebral disc protrusion, both of lateral approach dual-channel method and UBE endoscopic surgery are safe and effective. Endoscopic surgery with lateral approach and dual-channel method could be performed under local anesthesia, allowing for the removal of the nucleus pulposus under direct vision. It is simpler, more efficient.

International Journal of Laboratory Medicine 2025;46(5):610-615
Glucose-6-phosphate dehydrogenase(G6PD)deficiency is one of the most common monogenic genetic diseases in the world.China is the high incidence area of this disease.G6PD heterozygote is the main form of G6PD deficiency in females, which belongs to X-linked incomplete dominant inheritance. Therefore, the enzyme activities of female heterozygotes vary widely, showing normal to mild to moderate enzyme activi-ties.There is no unified calibrators for G6PD enzymatic detection reagents commonly used in clinical practice, and the results of the same sample detected by different kits from different manufacturers are different,so there is a phenomenon of missed detection in clinical screening of female G6PD heterozygotes.Although G6PD gene detection can effectively improve the detection rate of female heterozygotes, its clinical application is lim-ited due to cost and other reasons. Therefore, the development of detection methods that can improve the de-tection rate of female heterozygotes and are suitable for large-scale screening and clinical application will be an urgent need to be solved in clinical practice. This article reviews the epidemiology of G6PD deficiency, genetic characteristics of female G6PD gene, clinical manifestations and detection methods of G6PD female heterozy-gotes.

From January 2020 to January 2022, 60 hospitalized patients with severe pneumonia complicated with gastrointestinal dysfunction (syndrome of lung heat and fu-organ excess) in the Intensive Care Department and Emergency Intensive Care Unit of Nanjing Hospital of Chinese Medicine Affiliated to Nanjing University of Chinese Medicine were selected as the observation subjects. They were divided into experimental group and control group according to random number table method, with 30 cases in each group. Patients in the control group were treated with conventional Western medicine, while patients in the experimental group received Xuanbai Chengqi Decoction combined with conventional Western medicine. Both groups were treated continuously for 7 days and followed up for 28 days. TCM syndrome scores were evaluated before and after treatment. Clinical Pulmonary Infection Score (CPIS) was used to assess pulmonary infection, Gastrointestinal Dysfunction Score (GIDS) was used to assess the degree of gastrointestinal dysfunction, and acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score was used to assess the prognosis. ELISA was used to detect levels of procalcitonin (PCT), IL-6, CRP, D-lactate (D-LA), short chain fatty acids (SCFA), and G protein coupled receptor 43 (GPR43). The fully automated blood analyzer (flow cytometry) was used to detect white blood cells (WBC) and the proportion of neutrophils (N%). Indirect bladder pressure measurement method was used to measure the intra-abdominal pressure of patients. During treatment, the adverse reactions or events were recorded. Patients were followed up for 28 days and the 28-day mortality rate was recorded. Results:The total effective rate was 73.33% (22/30) in the experimental group and 40.00% (12/30) in the control group, with statistical significance ( χ2=6.79, P<0.05). After treatment, TCM syndrome scores, CPIS, GIDS, and APACHEⅡ score in the experimental group were lower than those in the control group ( t values were 2.84,3.34,2.75,3.05,respectively, P<0.01), serum PCT [(0.35 ± 0.11) μg/L vs. (0.64 ± 0.10) μg/L, t=2.45], IL-6 [(19.33 ± 3.54) ng/L vs. (60.13 ± 15.01) ng/L, t=2.98], N% [(78.84 ± 2.09)% vs. (83.30±2.31)%, t=3.43], and CRP [(28.43 ± 6.38) mg/L vs. (54.48 ± 9.03) mg/L, t=4.02], intra-abdominal pressure [(9.11 ± 2.55) mmHg vs.(11.70 ± 3.02) mmHg, t=7.78] and D-LA [(0.11±0.05) mmol/L vs. (0.18±0.12) mmol/L, t=6.45] in the experimental group were lower than those in the control group ( P<0.05 or P<0.01). After treatment, serum SCFA [(48.18 ± 36.31) μmol/L vs. (35.10 ± 19.32)μmol/L, t=1.95] and GPR43 [(1 254.61 ± 437.40) ng/L vs. (990.15 ± 403.03) ng/L, t=2.13] in the experimental group were higher than those in the control group ( P<0.05). The 28-day mortality rates of the experimental group and the control group were 20.00% (6/30) and 46.67% (14/30) respectively, with statistical significance ( χ2=4.80, P<0.05). During the trial, there were no serious adverse reactions or adverse events in either group. Conclusion:Xuanbai Chengqi Decoction can effectively treat severe pneumonia complicated with gastrointestinal dysfunction. The mechanism may involve the up-regulation of SCFA/GPR43 axis.

BACKGROUND:The incidence of medial collateral ligament injuries in the knee joint is easy to lead to secondary meniscus and cartilage damage, and long-term chronic damage can lead to the occurrence of osteoarthritis. At present, there are few studies on the mechanics of meniscus and articular cartilage injury caused by medial collateral ligament rupture. OBJECTIVE:To investigate the effect of different degrees of medial collateral ligament injury on the biomechanics of meniscus and cartilage of knee joint. METHODS:The CT and MRI examinations of the knee joint of a healthy volunteer were performed to obtain the image data. The scanning data were imported into Mimics,Geomagic, and Solidworks software in turn. After registration and fusion,a 3D model of normal knee joint was established. On this basis, models of medial collateral ligament injury in different degrees of knee joint were simulated, which were divided into four groups,including:(1)medial collateral ligament was intact;(2)deep medial collateral ligament fracture;(3)superficial medial collateral ligament fracture;(4)complete rupture of medial collateral ligament. Finally,Ansys software was introduced to apply three modes of loads to the knee joint:(1)10 N·m valvaration torque was applied to the top of the femur.(2)A 4 N·m internal torque was applied to the top of the femur.(3)A 4 N·m external torque was applied to the top of the femur. The effects of four groups of models on knee biomechanics under different loads were analyzed. RESULTS AND CONCLUSION:(1)In the extension position of the knee joint, when a 10 N·m valgus torque was applied to the knee joint, the overall stress of the posterolateral meniscus increased with different degrees of medial collateral ligament injuries, while the stress of the articular cartilage did not change significantly. The peak stress of the posterolateral meniscus increased significantly with superficial medial collateral ligament rupture.(2)In the knee extension position, when a 4 N·m internal rotation torque was applied to the knee joint, the overall stress of the medial and lateral meniscus increased after different degrees of medial collateral ligament injury. When superficial medial collateral ligament rupture occurred, the peak stress of the meniscus shifted from the anterior horn of the medial meniscus to the anterior horn of the lateral meniscus.(3)In the knee extension position, applying a 4 N·m external rotation torque to the knee joint, the peak stress of the posterolateral meniscus increased more significantly than that of the medial meniscus, and the stress of the articular cartilage changed less.(4)These results show that the risk of meniscus injury secondary to superficial medial collateral ligament rupture is much higher than that of deep medial collateral ligament rupture when the knee is in extension, and the lateral meniscus is more vulnerable to injury than the medial meniscus. Both superficial medial collateral ligament and deep medial collateral ligament play an important role in the rotational stability of the knee joint.

Objective:To investigate the alterations of multilayer network and dynamic causal connectivity in patients with bilateral sudden sensorineural hearing loss. Methods:The hearing ability, neural-scale data and resting-state functional magnetic resonance imaging(fMRI)data of 60 patients with bilateral sudden sensorineural hearing loss from the Department of Otolaryngology and 60 healthy controls from Physical Examination Center, Nanjing First Hospital were collected from January 2022 to January 2024. Multilayer brain network analysis was computed to identify the nodes with abnormal switching rate. Then these abnormal nodes were used as regions of interest (ROIs) in the subsequent dynamic causal model analysis. The SPSS 26.0 software was used to analyze causal connectivity between two groups based on independent-sample Mann-Whitney U test. And Pearson correlation coefficients between fMRI results and clinical parameters were calculated. Results:Pure tone audiometry test showed that the left and right hearing thresholds of bilateral sudden sensorineural hearing loss were significantly higher than those of healthy controls( Z=-9.460, -9.462, both P<0.01). The score of depressive emotion in patients with bilateral sudden hearing loss was 6.00(2.00, 9.00).Multilayer brain network analysis showed that the switching rates in the left superior frontal gyrus (0.073(0.049, 0.102), Z=3.603, P<0.001), left anterior cingulate gyrus (0.077(0.044, 0.105), Z=3.189, P=0.001), right hippocampus (0.080(0.045, 0.116), Z=3.616, P<0.001), left para-hippocampal gyrus (0.080(0.043, 0.108), Z=3.577, P<0.001), left superior parietal gyrus (0.079(0.047, 0.103), Z=3.160, P=0.002), and right inferior parietal gyrus (0.078(0.043, 0.105), Z=3.396, P<0.001) in patients with bilateral sudden sensorineural hearing loss were lower that those in healthy controls. These six brain areas were used as ROIs to compute dynamic causal model analysis, and the results showed that the connection strength from left superior frontal gyrus to the left para-hippocampal gyrus in patients with bilateral sudden sensorineural hearing loss was significantly higher than that in healthy controls ( Z=-2.593, P<0.05). Additionally, enhanced connectivity from left superior frontal gyrus to the left para-hippocampal gyrus was positively correlated with duration of hearing loss ( r=0.376, P=0.003). Conclusions:Bilateral sudden sensorineural hearing loss patients have decreased switching rates in many brain areas, and the causal connectivity from left superior frontal gyrus to the left para-hippocampal gyrus is enhanced.
