1.Epidemiological dynamics and spatiotemporal diffusion trend of brucellosis in China from 2010 to 2024

Objective:To investigate the epidemiological dynamics and spatiotemporal diffusion trend of brucellosis in China from 2010 to 2024.Methods:Data on reported human brucellosis cases in mainland China from January 1, 2010, to December 31, 2024, were collected via the"China Information System for Disease Control and Prevention", including detailed information on the date of onset, gender, age, occupation, and residential address of the cases. The Joinpoint regression and spatial interpolation techniques were used to investigate the spatiotemporal dynamics and population distribution characteristics of human brucellosis in pastoral/semi-pastoral areas and other regions, as well as urban and rural areas, and explore the epidemic trends of the disease.Results:From 2010 to 2024, pastoral/semi-pastoral regions reported 252 094 brucellosis cases, with a mean annual incidence rate of 36.57±7.28 per 100 000. In contrast, other regions cumulatively recorded 519 748 cases during the same period, demonstrating a significantly lower mean annual incidence rate of 2.54±0.74 per 100 000. The incidence rate of human brucellosis in pastoral/semi-pastoral regions exhibited a declining-rebounding-declining trend. Specifically, the incidence rate decreased significantly from 2010 to 2017 (APC=-7.20; P<0.001) and increased notably from 2017 to 2021 (APC=18.00; P=0.015) with a decline again from 2021 to 2024 (APC=-7.53; P=0.027). In other regions, the incidence rate showed a fluctuating upward trend. Specifically, the incidence rate increased significantly from 2010 to 2015 (APC=20.37; P<0.001) and decreased notably from 2015 to 2018 (APC=-21.78; P<0.001), followed by an increase again from 2018 to 2024, a significant upward trend in incidence rate from 2018 to 2021 (APC=26.73; P<0.001) and a non-significant decline from 2021 to 2024 (APC=-0.99; P=0.735), resulting in the maintenance of a relatively high incidence level. Rural areas demonstrated significantly higher brucellosis incidence rates than urban settings (all P<0.001). Brucellosis exhibited a diffusion trend from the northern epidemic areas of China to neighboring regions, along with sporadic diffusion in southern regions between 2010 and 2024. The age structure of patients in pastoral/semi-pastoral areas differed significantly from that in other regions. Specifically, in pastoral/semi-pastoral areas, the incidence rate was higher among the 35-49 age groups, while in other regions, the incidence rate was higher among those aged 55-64. Conclusion:There are notable disparities in the incidence of human brucellosis between pastoral/semi-pastoral areas and other regions in China. Human brucellosis exhibits a diffusion trend from the northern epidemic areas of China to neighboring regions, along with sporadic diffusion in southern regions.
2.Ultrasonic manifestations of pediatric lesser omentum torsion:a case report

Omental torsion represents a rare cause of acute abdomen,current literatures predominantly limited to case reports focusing on greater omentum torsion,by contrast,lesser omentum torsion is exceedingly uncommon. One case of male patient aged 8 years and 7 months with epigastric pain was admitted to the Children's Hospital of Zhejiang University School of Medicine. Initial abdominal ultrasound suggested omental pathology. To confirm the diagnosis,repeat ultrasonography and contrast-enhanced CT were performed,leading to a preliminary suspicion of greater omentum torsion. Following multidisciplinary collaborative consultation,surgical intervention was recommended. The patient underwent laparoscopic omentectomy with adhesiolysis,which intraoperatively confirmed lesser omentum torsion. Histopathological examination provided definitive diagnostic confirmation.Postoperative abdominal ultrasound revealed no significant abnormalities. This case underscored the critical value of dynamic multimodal imaging evaluation and multidisciplinary decision-making in the preoperative diagnosis and therapeutic management of lesser omentum torsion,aiming to establish ultrasonographic diagnostic references for lesser omentum torsion while simultaneously heightening clinical vigilance toward rare diseases.
3.Research progress of adipose-derived stem cells in the treatment of diabetic foot ulcers

Diabetic foot ulcers (DFUs) are a common and serious complication of diabetes. Current treatment methods typically focus on controlling blood glucose levels, routine care, anti-infection, debridement, and other aspects. However, these approaches have not consistently yielded satisfactory results. Adipose-derived stem cells (ADSCs) are characterized by their diverse sources, ease of acquisition, and low immunogenicity, making them a prominent topic of research area in the field of stem cells. ADSCs have emerged as a new strategy for the treatment of DFUs. This review aims to describe the pathophysiological characteristics of diabetic foot ulcers, the advantages and biological functions of ADSCs, and their mechanisms for promoting wound healing in DFUs, thereby providing a theoretical basis for the clinical application of ADSCs in the treatment of DFUs.

Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.
5.Predictive value of triglyceride-glucose index on microvascular obstruction after emergency PCI in patients with ST-segment elevation myocardial infarction

Aim To investigate the association of triglyceride-glucose(TyG)index with microvascular obstruction(MVO)after percutaneous coronary intervention(PCI)in patients with ST-segment elevated myocardial infarction(STEMI).Methods Individual patient-data were pooled from 310 patients with STEMI underwent emergency PCI in Nanjing Drum Tower Hospital,the Affiliated Hospital of Nanjing University Medical School from June 2018 to June 2021 for a prospective analysis.A week following the operation,cardiac magnetic resonance imaging was used to evaluate the MVO region and the patients were divided into two groups based on whether MVO occured after PCI:the MVO group(n=183)and the non-MVO group(n=127).The clinical data of the two groups were compared,and the linear relationship between TyG index and MVO was described using restricted cubic splines(RCS).Univariate and multivariate Logistic regression analysis were used to correct for confounding factors and identify independent risk factors for MVO occurrence.Results After adjustment for confounding factors,TyG index was an independent risk factor for MVO after emergency PCI in STEMI patients,and every 1-unit increased in TyG index,the risk of MVO increased by 1.24 times(OR=2.24,95%CI:1.07~4.71,P=0.033).The RCS curve analysis results showed that there was a linear re-lationship between the TyG index and the occurrence of MVO after emergency PCI(non-linear correlation test P=0.47).When the TyG index was greater than 9.5,the risk of MVO after emergency PCI significantly increased.Conclusion An increased TyG index is postively associated with the incidence of MVO in STEMI patients who have undergone PCI,and has clinical significantce for early prevention and risk stratification of MVO in STEMI patients.
6.Three-dimensional finite element analysis of the influence of different surgical methods on the stress of meniscus and patellofemoral joint in the treatment of episodic patellar dislocation

Objective To explore the influence of different surgical methods on the stress of knee me-niscus and patellofemoral joint surface of simulated episodic patellar dislocation using three-dimension-al finite element software under different flexion conditions.Methods The knee joint CT and MRI of pa-tients with episodic patellar dislocation were acquired and imported into Mimics and Geomagic to ex-tract the model,followed by the 0°,30°,60°,90° and 120° buckling construction using the Soild-works software.For each model,medial patellofemoral ligament reconstruction,medial retinaculum pli-cation,lateral retinaculum release,medial patellofemoral and patellotibial ligament reconstruction,as well as medial patellofemoral ligament reconstruction and lateral retinaculum release were conducted re-spectively.Finally,they were imported into the mechanical software Ansys for biomechanical analysis,and the stress results of knee meniscus and patellofemoral joint surface were output.Results The medi-al patellofemoral and patellotibial ligament reconstruction had the smallest meniscus peak stress com-pared with other methods.Moreover,it also exerted the smaller peak stress on the patellofemoral joint surface,along with the medial patellofemoral ligament reconstruction and the lateral retinaculum re-lease.Conclusion The medial patellofemoral and patellotibial ligament reconstruction can provide good patellofemoral contact stress and meniscus stress,effectively restore the stability of the knee joint.How-ever,medial patellofemoral ligament reconstruction and lateral retinaculum release can also provide bet-ter patellofemoral joint stress,but the meniscus stress is slightly higher,so we should be alert to the complications related to the meniscus after surgery.

Objective To understand the current situation of healthcare-associated infection(HAI)in China,pro-vide data support and decision-making basis for formulating scientific and effective strategies for HAI prevention and control.Methods A nationwide cross-sectional survey on HAI was conducted among various types and levels of medical institutions in China according to a unified protocol of bedside surveys and case investigations.Results In 2024,a total of 5 736 medical institutions and 2 751 765 patients were surveyed.Among them,34 889 HAI cases were identified,with a prevalence rate of 1.27%.The number of HAI episodes was 38 032,and case prevalence rate was 1.38%.The prevalence rate of HAI in medical institutions in different regions of China ranged from 0.66%to 2.35%.Among medical institutions of different scales,those with a bed capacity of ≥900 had the high-est incidence of HAI,reaching 1.65%.The most common infection site was the lower respiratory tract(44.66%),followed by the urinary tract(12.94%),surgical site(9.32%),upper respiratory tract(7.02%),and bloodstream infection(5.78%).The top 3 departments with the highest HAI rates were the general intensive care unit(10.02%),department of neurosurgery(5.51%),and department(group)of hematology(5.34%).A total of 23 238 strains of HAI pathogens were detected,with 10 714 strains(46.10%)from lower respiratory tract speci-mens.The top 5 detected strains were Klebsiella pneumoniae(14.76%),Pseudomonas aeruginosa(13.33%),Escherichia coli(12.79%),Acinetobacter baumannii(9.23%),and Staphylococcus aureus(7.88%).231 944 pa-tients underwent class Ⅰ incision surgery were monitored,with 1 647 cases experienced surgical site infection,and the prevalence rate of surgical site infection was 0.71%.The number of patients who should undergo pathogen de-tection(patients receiving therapeutic and therapeutic combined prophylactic antimicrobial agents)was 715 179,while the actual number was 480 492,with a pathogen detection rate of 67.18%.425 225 patients received patho-genic detection before treatment,with a detection rate of 59.46%.Conclusion The overall HAI prevalence in Chi-na is lower,showing disparities among medical institutions of different regions and scales.Therefore,precise imple-mentation of measures is necessary for HAI prevention and control,with a focus on high-risk institutions and high-risk departments,key areas,and critical procedures.All levels of medical institutions should continuously reduce the incidence of HAI by strengthening monitoring,standardizing the use of antimicrobial agents,and reinforcing basic HAI prevention and control measures.
8.Efficacy and Safety of Inclisiran in Aisa Atherosclerotic Cardiovascular Disease Patients or High-risk Population:Chinese Mainland Sub-population Analysis of ORION-18 Study

Objectives:The ORION-18 study has demonstrated that inclisiran can significantly reduce low-density lipoprotein cholesterol(LDL-C)and has good safety in Asian atherosclerotic cardiovascular disease(ASCVD)patients or ASCVD high-risk population.This subgroup analysis aims to further evaluate the efficacy and safety of inclisiran in Chinese mainland population.Methods:ORION-18 study is a multi-center,randomized,double-blind,placebo-controlled,phase Ⅲ clinical trial among Asian subjects,Chinese mainland subgroup included 232 ASCVD patients or ASCVD high-risk subjects who had already been treated with diet control and maximum tolerated doses of statins treatment(with or without other lipid-lowering treatments)but still had elevated LDL-C levels.Subjects were randomized in a 1:1 ratio to the inclisiran group and the placebo group(n=116 each),and received 300 mg of inclisiran or placebo respectively on day 0,90 and 270.The primary endpoint was the percentage change in LDL-C from baseline to day 330.The secondary endpoints included the time-adjusted percentage change and absolute change in LDL-C from baseline after day 90 and up to day 360,the absolute change in LDL-C from baseline to day 330,and the percentage changes from baseline to day 330 in proprotein convertase subtilisin/kexin type 9(PCSK9),total cholesterol,apolipoprotein B(ApoB),non-high-density lipoprotein cholesterol(non-HDL-C).Other secondary endpoints included the proportion of participants reaching LDL-C levels of<1.8 mmol/L at day 330,the proportion of participants with≥50%LDL-C reduction from baseline to day 330 and the proportion of participants who attained global lipid targets(the LDL-C target was<1.4 mmol/L for ASCVD patients and<1.8 mmol/L for ASCVD high-risk subjects)at day 330.Safety endpoints included adverse events during treatment,aboratory test abnormalities during treatment,serious adverse events,and assessed their severity and relation to treatment.Results:The inclisiran group showed a placebo-corrected percentage change in LDL-C from baseline to day 330 of-61.16%,and an absolute change of-1.73 mmol/L(both P<0.0001).Compared to the placebo group,the inclisiran group's time-adjusted percentage change in LDL-C from baseline between day 90 and day 360 was-58.51%,and an absolute change of was-1.64 mmol/L(both P<0.0001).At day 330,reductions from baseline were observed in the inclisiran group for PCSK9,total cholesterol,ApoB,non-HDL-C,with placebo-corrected percentage changes of-77.44%,-35.65%,-43.43%,-50.90%(all P<0.0001),respectively.At day 330,79.6%(74/93)of patients in the inclisiran group and 7.8%(6/77)in the placebo group achieved LDL-C levels<1.8 mmol/L,69.9%(65/93)of patients in the inclisiran group and 0%(0/77)in the placebo group achieved≥50%LDL-C reduction from baseline,66.7%(62/93)of patients in the inclisiran group and 2.6%(2/77)in the placebo group achieved their global LDL-C targets.The safety profile of inclisiran treatment over 12 months was comparable to that of the placebo,with no occurrence of treatment-related serious adverse events.Conclusions:In ASCVD patients or ASCVD high-risk subjects in Chinese mainland who have received diet control and maximum tolerable dose statins treatment(with or without other lipid-lowering treatments)and still have elevated LDL-C,inclisiran has a definite efficacy and good safety in reducing LDL-C.The efficacy and safety results of inclisiran assessed in Chinese mainland population are consistent with those of the general Asia population.
9.Effect of dihydroquercetin on the expression of inflammatory response markers in rats with spinal cord injury

BACKGROUND:The"inflammatory storm"induced by microglia is an important pathological factor causing the death of neuronal cells after spinal cord injury,which is extremely unfavorable for the recovery after spinal cord injury.Dihydroquercetin belongs to the vitamin P group and has excellent biological activity.It has good anti-inflammatory,anti-lipid peroxidation and neuroprotective effects on spinal cord injury.The mechanism by which it promotes the repair of spinal cord injury needs to be further clarified.OBJECTIVE:To investigate whether dihydroquercetin can ameliorate the inflammatory state induced after spinal cord injury through its anti-inflammatory and antioxidant properties for the purpose of protection against spinal cord injury.METHODS:Forty-eight Sprague-Dawley rats were divided into sham operation group,spinal cord injury group,low-dose dihydroquercetin group,and high-dose dihydroquercetin group according to the random number table method.The sham operation group only underwent laminectomy and was given intraperitoneal injection of normal saline after the operation.Animal models of spinal cord injury were made in the latter three groups.After the operation,normal saline,30 mg/kg,and 50 mg/kg dihydroquercetin were given by intraperitoneal injection respectively,and samples were collected 4 weeks after the operation.The Basso-Beatte-Bresnahan(BBB)limb motor function score and mechanical paw withdrawal threshold were used to evaluate the limb motor function and pain recovery of rats.Hematoxylin-eosin staining was used to observe the pathological changes of spinal cord tissue;Nissl staining was used to observe the number of Nissl bodies and the morphology of neurons.The activity of superoxide dismutase in spinal cord tissue was detected.Immunofluorescence was used to detect the expression of reactive oxygen species and microglia Iba1.Western blot was used to detect the protein expression of Iba1 and interleukin-1β,interleukin-6,interleukin-10,tumor necrosis factor-α,and p38 mitogen-activated protein kinase in the spinal cord.RESULTS AND CONCLUSION:(1)In the behavioral test,the BBB limb motor function score showed that since 14 days after the operation,the BBB limb motor function score of the high-dose dihydroquercetin group was significantly better than that of the spinal cord injury group(P<0.01);the mechanical paw withdrawal threshold was significantly lower in the high-dose dihydroquercetin than the spinal cord injury group at 21 days after the operation(P<0.05),and the former group showed better motor function.(2)Hematoxylin-eosin staining and Nissl staining showed that compared with the spinal cord injury group,the high-dose dihydroquercetin group showed slightly swollen but structurally intact and clearer neuron morphology in the spinal cord tissue,and the number of Nissl bodies increased.(3)Superoxide dismutase detection:Compared with the spinal cord injury group,the activity of superoxide dismutase in the spinal cord tissue of the high-dose dihydroquercetin increased significantly(P<0.05).(4)Reactive oxygen species immunofluorescence detection:Compared with the spinal cord injury group,the high-dose dihydroquercetin could significantly reduce the accumulation of reactive oxygen species after spinal cord injury(P<0.05).(5)Detection of microglial marker Iba1:Compared with the spinal cord injury group,the high-dose dihydroquercetin could significantly reduce the expression of microglial marker Iba1 after spinal cord injury(P<0.01).(6)Western blot detection results showed that compared with the spinal cord injury group,the expressions of inflammatory factors(interleukin-1β,interleukin-6,and tumor necrosis factor-α)and phosphorylated p38 mitogen-activated protein kinase in the high-dose dihydroquercetin group decreased significantly(P<0.01),while the expression of anti-inflammatory factor interleukin-10 increased significantly(P<0.01).These findings indicate that dihydroquercetin can effectively reverse the expression changes of multiple inflammatory markers induced after spinal cord injury in rats,thereby exerting neuroprotective effects and improving motor function.

Aim To analyze the incidence of major adverse cardiovascular and cerebrovascular events(MACCE)in patients with different types of acute coronary syndrome(ACS)undergoing coronary artery rotational atherec-tomy(RA)within two years.Methods 268 patients with ACS who underwent RA in the Department of Cardiology,Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School of Nanjing University,between November 2011 and December 2022 were retrospectively included.According to whether ST-segment elevation myocardial infarction(STEMI)occurred,they were divided into 25 cases in the ST-segment elevation myocardial infarction(STEMI)group and 243 cases in the non-ST-segment elevation acute coronary syndrome(NSTE-ACS)group.The NSTE-ACS group included unstable angina pectoris(UAP)and non-STEMI(NSTEMI).The basic information and intraoperative data related to percutaneous coronary intervention(PCI)in the two groups were collected,and the occurrence of MACCE(including car-diovascular death,non fatal myocardial infarction,worsening heart failure,ischemic stroke and target vessel revasculariza-tion)within two years after RA was followed up and analyzed.Results Compared with the NSTE-ACS group,the STEMI group had a higher incidence of MACCE and cardiovascular mortality during the two-year follow-up period(10.3%and 0.4%vs.28.0%and 8.0%;P<0.05).There was no statistical difference between the incidence of target vessel revascularization,nonfatal infarction,ischemic stroke and worsening heart failure between the two groups(P>0.05).According to subgroup analysis based on enrollment periods,the results showed that over time(2011-2017 compared to 2018-2022),the incidence of MACCE in all patients within two years after RA showed a decreasing trend(18.97%vs.6.58%).Combined with previous studies,gender,hypertension,diabetes,renal insufficiency,smoking and left ven-tricular ejection fraction(LVEF)were included in the Cox regression model.It was found that the use of intravascular ul-trasound(IVUS)was an independent factor to reduce the incidence of MACCE in ACS patients within two years after RA(HR=0.333,95%CI:0.153~0.723,P<0.01).Kaplan-Meier analysis showed that among ACS patients undergoing RA,the cumulative incidence of MACCE events was higher in the STEMI group than that in the NSTE-ACS group(P<0.05).Conclusion STEMI patients have a higher incidence of MACCE and cardiovascular mortality within two years after RA compared to NSTE-ACS patients,and the use of IVUS during RA surgery can reduce the incidence of MACCE in ACS patients after RA.
