1.Influence of plasma infusion during orthotopic liver transplantation on the incidence of postoperative acute kidney injury

Objective To investigate the influence of plasma infusion during orthotopic liver transplantation on the incidence of acute kidney injury (AKI) in recipients. Methods Cinical data of 473 liver transplant recipients who underwent orthotopic liver transplantation at Beijing Chaoyang Hospital Affiliated to Capital Medical University from January 2016 to December 2020 were retrospectively collected. The study included 354 recipients who received plasma infusion during the operation (plasma group) and 119 recipients who did not receive plasma infusion during the operation (control group). Preoperative conditions, donor conditions, intraoperative conditions, main outcome indicators and secondary outcome indicators of the two groups were analyzed and compared. Receiver operating characteristic curve was drawn to calculate the maximum cut-off value of intraoperative plasma infusion volume that affected the occurrence of AKI within 7 days after surgery. Logistic regression analysis was performed to analyze the correlation between intraoperative plasma infusion volume and the incidence of AKI within 7 days after surgery. Results Before propensity score matching, the incidence of AKI within 7 days after surgery and the incidence of grade Ⅲ AKI in the plasma group were higher than those in the control group (both P<0.05). After propensity score matching, 62 recipients were included in each group. There was no statistically significant difference in the incidence of AKI within 7 days after surgery between the plasma group and the control group, but the incidence of grade Ⅲ AKI within 7 days after surgery in the plasma group was higher than that in the control group (P=0.041). Logistic regression analysis showed that intraoperative plasma infusion volume >900 mL was a potential risk factor for AKI within 7 days after surgery (odds ratio=1.936, 95% confidence interval 1.193-3.142, P=0.007). There were no statistically significant differences in the incidence of 365-day postoperative fatality, reperfusion syndrome, and postoperative 30-day complications between the two groups before and after propensity score matching. In addition, the postoperative albumin, fibrinogen levels, and international normalized ratio in the plasma group were better than those in the control group before and after matching (all P<0.05). Conclusions Large amount of intraoperative plasma infusion is associated with an increased risk of grade Ⅲ AKI after orthotopic liver transplantation. Intraoperative plasma infusion volume >900 mL may increase the risk of AKI within 7 days after surgery.
2.Analysis and prediction of the incidence trend of congenital syphilis in China in 2005 - 2020

Objective To analyze the incidence of congenital syphilis in four regions of China from 2005 to 2020 and predict its change trend, and to provide a scientific basis for the prevention and control of congenital syphilis. Methods The incidence data of congenital syphilis in eastern, western, central and northeastern China from 2005 to 2020 were collected. The annual change percentage (APC) and average annual change percentage (AAPC) were calculated by Joinpoint software, and the change trend was analyzed. The ARIMA model was established by SPSS26.0 software to predict the incidence from 2021 to 2025. Results From 2005 to 2020, there were 107 504 cases of congenital syphilis, and the incidence reached the peak in 2011, which was 0.90/100 000, and decreased to 0.09/100 000 in 2020. The incidence of congenital syphilis in the eastern, central, western, and northeastern regions of China decreased to 0.07/100 000, 0.06/100 000, 0.12/100 000, and 0.15/100 000 in 2020, respectively. Joinpoint regression analysis found that from 2005 to 2020, the incidence of congenital syphilis showed a downward trend in the whole country (AAPC=-8.68%, t=-5.18, P<0.05), the eastern region (AAPC=-12.01%, t=-6.81, P<0.05), the central region (AAPC=-5.45%, t=-2.09, P<0.05), and the western region (AAPC=-6.05%, t=-3.00, P<0.05), while the incidence in the northeast region was relatively stable (AAPC=-1.66%, t=-1.18,P>0.05). The ARIMA (2,2,0) model was constructed to predict that the annual incidence of congenital syphilis from 2021 to 2025 would be 0.06/100,000, 0.03/100,000, 0.02/100,000, 0.01/100,000, and 0.01/100,000, respectively. Conclusion From 2005 to 2020, the incidence of congenital syphilis in China shows an overall downward trend. The western and northeastern regions are the key prevention and control areas, and the work of "eliminating mother to child transmission of syphilis" needs to be continuously promoted.
3.Research progress on the influencing factors of psychological distress in patients with coronary heart disease

Coronary heart disease is a chronic and lifelong disease, which places a dual burden on the physiological and psychological well-being of patients, and can easily lead to psychological distress and affect their prognosis and quality of life. This article provides a systematic review, in which the current status, evaluation tools, influencing factors and intervention methods of psychological distress in patients with coronary heart disease are explored, aiming to provide key information beneficial for identifying and preventing psychological distress, and to improve the overall management and treatment effectiveness of coronary heart disease patients. In this paper, 18 articles were included, and the demographic, physiological, psychological and social factors affecting the psychological distress of patients with coronary heart disease were systematically analyzed, thus to provide a deeper understanding of psychological distress and offering references for formulating targeted intervention strategies.
4.Langerhans Cell Histiocytosis of Bone:Report of Eight Cases and Review of the Literature.

Langerhans cell histiocytosis of bone is a rare tumor disease characterized by the large accumulation of CD1a+ and CD207+ dendritic cells in tissues of unknown cause.It mainly occurs in children aged 1-4 years old,with incidences of 4-6 per million in children and 1-2 per million in adults.Due to its low incidence,diverse clinical manifestations,and no obvious specificity of imaging manifestations,the definitive diagnosis and early treatment of this type of tumor are challenging.In this paper,we report 8 cases of Langerhans cell histiocytosis of bone and review the relevant literature published in the past five years to summarize the clinical characteristics,pathological features,diagnosis,treatment,and prognosis of this disease.

5.Improvement effect of ginseng alcohol extract on sleep of aged drosophila and its mechanism

Objective:To investigate the impact of ginseng alcohol extract(GEE)on improving sleep quality in the aged Drosophila model by regulating the redox balance,and to elucidate its associated mechanism.Methods:Thirty-two male drosophila melanogaster(7-days-old)were randomly selected as young group,while 64 male Drosophila melanogaster flies(35-days-old)were randomly assigned to aged model group(n=32)and GEE group(n=32).The sleep parameters,including total sleep duration,daytime sleep duration,night sleep duration,0-4 h of sleep duration after lights off(ZT0-4 sleep duration),deep sleep duration,sleep episodetimes,sleep fragmentation,and the activity parameters such as the total number of locomotor activity daytime locomotor activity amount and nighttime locomotor activity amount were analyzed using the DAM2 Drosophila behavioral analysis system 7 d after administration.The grouping of the drosophila was as above,and there were 100 drosophila ineach group.The differentially expressed proteins in drosophila brain tissue were screened,identified,and functionally analyzed using two-dimensional fluorescence difference gel electrophoresis(2D-DIGE)and matrix-assisted laser desorption/ionization time of flight mass spectrometry(MALDI-TOF/TOF-MS)proteomic methods.The grouping of the drosophila was as above,and there were 100 drosophila in each group.The activities of superoxide dismutase(SOD),catalase(CAT),and glutathione peroxidase(GSH-Px)and the levels of lipid peroxidation product(MDA)in brain tissue of the drosophila were determined using assay kits.Results:Compared with young group,the total sleep duration daytime sleep duration and night sleep cluration of the drosophila in agaed group were decreased(P<0.05 or P<0.01);and the sleep rhythm amplitude was shortened.Compared with aged group,the total sleep duration and daytime and nighttime sleep durations of the drosphila in GEE group were lengthened(P<0.01).Compared with young group,the ZT0-4 sleep duration deep sleep duration and sleep fragment of the drosophila in aged group were decreased(P<0.05 or P<0.01),and the sleep rhythm amplitude was shortened.Compared with young group,the ZT0-4 sleep duration,deep sleep duration,and single sleep fragment of the drosphila in GEE group were significantly prolonged(P<0.01),and the sleep amplitude was increased.Compared with young group,there was no significant difference in diurnal spontaneous activity or total spontaneous activity of the drosophila in aged group(P>0.05),while the nocturnal spontaneous activity was significantly increased(P<0.05).Compared with aged group,the diurnal spontaneous activity,nocturnal spontaneous activity,and total spontaneous activity of the drosophila in GEE group were significantly decreased(P<0.05 or P<0.01).A total of 47 differentially expressed proteins were selected in the 2D-DIGE electrophoretic mapping.Compared with young group,the expressions of 47 differentially expressed protein sites in aged group were down-regulated mainly including glutathione S-transferase,peroxiredoxin 1 and dihydrolipoic dehydrogenase,which were related to redox balance.Compared with young group,the activities of SOD,CAT and GSH-Px in brain tissue of the drosophila in aged group were decreased(P<0.05 or P<0.01),and the level of MDA was increased(P<0.01);compared with aged group,the activities of SOD,CAT and GSH-Px in brain tissue of the drosphila in GEE group were increased(P<0.05 or P<0.01),and the MDA level was decreased(P<0.05).Conclusion:GEE has improvement effect on the sleep quality of aged drosophila,and its possible mechanism may be related to upregulating the activities of antioxidant enzymes,inhibiting the accumulation of lipid peroxidation products,and maintaining redox balance.
6.The 3D finite element study on implant restoration in the maxillary anterior region with different palatal bone plate thicknes-ses

Objective To compare the differences in stress on maxillary anterior implants and labial cortical bone under varying thicknesses of palatal bone plates through three-dimensional finite element analysis.Methods Using CBCT scan data and finite ele-ment software,a three-dimensional finite element model of maxillary central incisor implant restoration was constructed.The thickness of the palatal bone plate at the cervical region of the implant was set to 0,0.5,1.0 mm,respectively.The effects of different palatal bone plate thicknesses on the equivalent stress of the implant and the minimum principal stress(compressive stress)of the labial corti-cal bone under normal occlusal conditions were simulated.Results Under normal occlusion,when the palatal cervical bone plate thickness was 0,0.5,1.0 mm,the maximum equivalent stress of the implant was consistently located at the midline of the labial cervi-cal region at the implant-abutment interface,with values of 106.8,103.5,99.71 MPa,respectively.Meanwhile,the minimum princi-pal stress of the cortical bone appeared at the alveolar crest at the junction of the implant-abutment and labial alveolar bone,measuring 107.8,103.2,95.55 MPa,respectively.The results indicated that as the palatal cervical bone plate thickness decreased,both the maximum equivalent stress of the implant and the minimum principal stress of the labial cortical bone exhibited an increasing trend,though they remained below their respective yield strengths.Conclusion From a biomechanical perspective,maxillary anterior implant restoration remains feasible even when the thickness of the palatal bone plate at the cervical region of the implant is 0 mm.
7.Study on the TCM drug-symptom patterns in the treatment of idiopathic pulmonary fibrosis based on the " three-stage differentiated management" principle

Objective:To explore the therapeutic methods and drug-symptom patterns of Traditional Chinese Medicine (TCM) for idiopathic pulmonary fibrosis (IPF) at different stages under the guidance of the " three-stage differentiated management" principle.Methods:Under the three-stage principle, 133 IPF prescriptions treated by prof. Yan Jun from 2017.2-2023.12 were analyzed. Disease and category information of each eligible case was entered into the " FangNet" platform. Association and cluster analyses were performed for core herbs, key herb pairs, and symptom-drug relationships, aiming to uncover the clinical medication characteristics and rules in the three stages.Results:(1) Early inflammatory stage: phlegm-stasis harassment, incipient but unformed masses, superficial disease location; priority was dispelling wind, dispersing pathogen, clearing lung and resolving phlegm-commonly used: ephedra, stemona, processed pinellia, peucedanum, armeniaca. (2) Mid injury stage: formed phlegm-stasis, struggle of pathogen and healthy Qi, impaired lung collaterals, obstructive phlegm-stasis, excess pathogen with intact healthy Qi; emphasis on expelling pathogen, resolving phlegm, activating blood and dredging collaterals-e.g., angelica, pheretima, bombyx, scorpio. (3) Late fibrotic stage: healthy Qi deficiency with excess pathogen, phlegm-stasis masses entrenched in collaterals; treatment focuses on tonifying lung and dissolving masses, combining dredging and tonifying to regulate Qi and blood-e.g., pinellia, rhodiola, ginseng, gecko.Conclusions:Phlegm-stasis as excess pathogen and dual deficiency of lung-kidney run through all three stages of IPF. The rise and fall of healthy Qi vs pathogen and the degree of mass formation determine stage-specific therapeutic strategies and drug-pattern adjustments.

Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.
9.Analysis of prostate cancer screening results and exploration of screening model for elderly males in Songjiang Rural Areas of Shanghai City based on PSA preliminary screening under the community linkage mode

This study aims to analyze the screening results and epidemiological characteristics of prostate cancer (PCa) among elderly males in the rural areas of Songjiang, Shanghai City, through the implementation of a preliminary prostate-specific antigen (PSA) screening based on a community-linkage model, and to explore an effective screening approach. A retrospective observational study design was employed to collect data from residents who underwent PSA screening at Songjiang Hospital affiliated to Shanghai Jiao Tong University School of Medicine, in collaboration with multiple community health service centers in Songjiang District, Shanghai City, between June 2022 and June 2024, through free clinics and annual health examinations. Prostate biopsy was recommended for individuals with total PSA (tPSA) levels >10 ng/ml and those with 4 ng/ml≤tPSA≤10 ng/ml and abnormal free-to-total PSA (f/tPSA) ratios. Clinical characteristics of detected PCa patients were analyzed. Follow-up was conducted through phone calls and home visits by family doctors, coupled with enhanced health education. The results indicated that a total of 17 198 residents participated in the screening, among which 2 234 (12.99%) had tPSA levels between 4 ng/ml and 10 ng/ml, and 257 (1.49%) had tPSA levels >10 ng/ml. Ultimately, 417 residents underwent prostate biopsy, with 171 being diagnosed with PCa, yielding a positive biopsy rate of 41.00% and a PCa detection rate of 0.99%. The predominant pathological subtype among PCa patients was adenocarcinoma (168 cases, 98.24%). Of the 146 PCa patients who received treatment, the majority were classified as intermediate or high-risk (124 cases, 84.93%). Furthermore, with the optimization of the screening model, there was a significant increase in the proportion of subsequent outpatient visits. In conclusion, the community-linkage-based PSA screening model demonstrated high effectiveness in screening for PCa among elderly males in the rural areas of Songjiang, Shanghai City. Epidemiological findings revealed that PCa patients in this region are primarily composed of intermediate and high-risk groups, highlighting the need for intensified early screening and health education.
10.Finite element study on the stress of hole sealing resin in screw approach of dental implant

Objective To compare the stress difference of the resin with different thicknesses in the screw access hole using three-dimensional finite element method.Methods A three-dimensional finite element model of the implant at the site of the mandibular first molar was established by computer aided design(CAD)software.A static load of 200 N was applied to the resin models with different thicknesses(1,2,3,4,5 mm)to analyze the effect of thickness on the von Mises stress and shear stress of the resin.Results In a certain range,the maximum von Mises stress and the maximum shear stress of the resin decreased with the increase of the thickness.At the thickness of 1mm,the maximum von Mises stress and shear stress of the resin were 23.85 MPa and 11.82 MPa,respectively.When the thickness was 2 mm,the maximum von Mises stress and shear stress of the resin were 18.75 MPa and 9.73 MPa respectively.At the thickness of 3 mm,the maximum von Mises stress and shear stress of the resin were 17.46 MPa and 9.04 MPa,respectively.When the thickness of the resin was more than 3 mm,the stress on it was in a stable level trend.When the thickness was 4 mm,the maximum von Mises stress and shear stress of the resin were 17.38 MPa and 9.04 MPa,respectively.When the thickness was 5 mm,the maxi-mum von Mises stress and maximum shear stress of the resin were 17.18 MPa and 8.85 MPa,respectively.Conclusion When the res-in reaches a certain thickness,the stress is small and stable.This study provides a new design strategy for reducing the complications after implant restoration caused by stress fatigue of the resin.
