1.Clinical Study on Acupoint Injection of Methylcobalamin for the Treatment of Cervical Spondylotic Radiculopathy

OBJECTIVE To observe the clinical efficacy of acupoint injection of Methylcobalamin in the treatment of cervical spondylotic radiculopathy(CSR).METHODS A total of 90 patients with CSR in the outpatient department of Acupuncture-Moxi-bustion and Massage Department of Geriatric Hospital of Nanjing Medical University from April to October 2024 were selected and di-vided into a treatment group and a control group according to the random number table method,with 45 cases in each group.During the treatment,1 case dropped out of the control group and 2 cases dropped out of the treatment group.Both groups received conventional electroacupuncture treatment,and the control group was given conventional intramuscular injection of Methylcobalamin(Mecobalamin injection),and the treatment group was given acupoint injection of Methylcobalamin.The treatment was given once every other day,for a total of 20 times.The changes of visual analogue score(VAS)of pain,cervical dysfunction index(NDI)score and electromyography F wave velocity were observed before and after treatment.The clinical efficacy of the two groups was compared after treatment.RE-SULTS After treatment,the NDI and VAS scores of the two groups were significantly reduced(P<0.01),and the NDI score of the treatment group was better than that of the control group(P<0.01);the electromyography F wave velocity was significantly increased(P<0.05),and the treatment group was better than the control group(P<0.05,P<0.01);the clinical efficacy of the treatment group was better than that of the control group(P<0.01).CONCLUSION Compared with conventional non-acupoint intramuscular injec-tion,acupoint injection of Methylcobalamin can more effectively improve electromyography F wave velocity,and the clinical efficacy of CSR.

3.Risk factors for postoperative hypoxemia in patients undergoing Stanford type A aortic dissection surgery

Objective To explore the risk factors for postoperative hypoxemia in patients undergoing Stanford type A aortic dissection surgery.Methods The clinical data of 77 patients with Stanford type A aortic dissection surgery were analyzed retrospectively.Among the patients, 40 patients occurred hypoxemia(hypoxemia group),and 37 patients did not occur hypoxemia(non-hypoxemia group).The preoperative,intraoperative and postoperative clinical data were compared between 2 groups,and the independent risk factors for postoperative hypoxemia were analyzed by multiple Logistic regression analysis.Results The incidence of postoperative hypoxemia in patients with Stanford type A aortic dissection was 51.9% (40/77).The multiple Logistic regression analysis result showed that age (OR =1.088,95% CI 1.018-1.164,P=0.013),body mass index≥25 kg/m2(OR=6.495,95% CI 1.327-31.789,P=0.021),pericardial effusion(OR=6.384,95% CI 1.426-28.576,P=0.015),white blood cell count(OR=1.289,95% CI 1.033-1.609,P=0.024)and using recombinant human coagulationⅦa (OR = 23.757, 95% CI 2.849 - 198.085, P = 0.003) were the independent predictive factors for postoperative hypoxemia in patients with Stanford type A aortic dissection.Conclusions The postoperative hypoxemia in patients with Stanford type A aortic dissection is related with perioperative systemic inflammation, especially in obese patients who should be given anti-inflammatory treatment during perioperative period.Control of bleeding and reducing the recombinant human coagulationⅦa as far as possible can reduce the incidence of postoperative hypoxemia.
4.DNA barcoding research and its application on medicinal plants of Bletilla H. G. Reichenbach.

To identify adulterants from medicinal plants of Bletilla H. G. Reichenbach, the suitable candidate DNA barcoding of Bletilla was evaluated. In this study, the internal transcribed spacer (ITS) of nuclear ribosomal DNA, the LFY homologous gene intron 2 and chloroplast ycfl gene were amplified and sequenced from forty-one samples. The intra-specific and inter-specific divergences of Bletilla were calculated, and the identification efficiency was assessed using Barcoding Gap, NJ tree by K2P distance and BLAST1 method. The result showed the intra-specific divergence of nrDNA ITS and ycJfl (0.022-0.106 and 0.017-0.106) were obviously higher than the inter-specific divergence (0-0.012 and 0-0.015), and four species of Bletilla were also accurately distinguished in NJ trees. Whereas, there was no Barcoding Gap on LFY homologous gene intron 2, thus it cannot effectively identify species of Bletilla. Using NJ tree of nrDNA ITS and ycfl gene, powdery medicine and the adulterants of Bletilla were successfully unidentified. In conclusion, nrDNA ITS and ycfl can be used as a potential DNA barcoding to identify the medicinal plants in Bletilla and its adulterants. There were only three basic differences on nrDNA ITS between "Jujing baiji" and Bletilla striata of Lu'an in Anhui province, and two basic differences in ycfl. Based on morphological and molecular data, "Jujing baiji" could be recognized as the species of Bletilla striata.
