1.Application of a refined early diet plan in patients with gastric cancer after laparoscopic radical surgery

Objective To investigate the application effect of a refined early diet plan in patients with distal gastric cancer after laparoscopic radical surgery.Methods By convenience sampling method,46 patients with laparoscopic radical gastrectomy for distal gastric cancer admitted to a tertiary hospital in Zhejiang Province from July to December 2023 were selected as an experimental group,while 49 patients admitted from January to June 2023 as a control group.The experimental group was administered with the refined early diet plan,and the control group was administered with conventional methods.The intervention period was from the first day after surgery to discharge.The differences of the first postoperative defecation time,perioperative thirst and hunger score,postoperative hospitalization time,hospitalization cost,postoperative complications and the incidence of readmission 30 days after surgery were compared between the 2 groups.Results 42 patients in the experimental group and 49 patients in the control group completed the study.First postoperative exhaust time(t=4.922,P<0.001),first postopera-tive defecation time(Z=-2.440,P=0.015),perioperative thirst score(Z=-8.024,P<0.001),perioperative hunger score(Z=-8.192,P<0.00 1),postoperative hospitalization time(Z=-7.622,P<0.001)and hospitalization cost(Z=-4.522,P<0.001)were lower than those of the control group,and the difference was statistically significant.There was no significant difference in complication rate and 30-day readmission rate between the experimental group and the control group(P>0.05).Conclusion The refined early diet plan is safe and effective for early recovery of patients after laparoscopic radical gastrectomy for distal gastric cancer,promoting intestinal function recovery,shortening hospital stay,reducing hospital costs,and improving patients'subjective comfort.
2.The 70th anniversary academic conference of the Chinese Journal of Psychiatry and the first plenary meeting of the seventh editorial committee

This article introduces the 70th anniversary academic conference of the Chinese Journal of Psychiatry and the first plenary session of the 7th Editorial Committee. At the conference, the approval document of the Chinese Medical Association on the composition of the 7th Editorial Committee was read out, and appointment letter were issued to the new members. The work of the 6th Editorial Committee was summarized, and the work plan and prospects of the 7th Editorial Committee were introduced. The academic conference included three academic symposiums focused on the full course management of schizophrenia, bipolar disorder, and depression, anxiety and insomnia.
3.Prediction of PD-1 monoclonal antibody human pharmacokinetic characteristics based on PK in cynomolgus monkeys

OBJECTIVE To establish a population pharmacokinetic(PopPK)model to predict the PK characteristics of GLS-010 in humans.METHODS Fifty-eight cynomolgus monkeys were used,18 of which were randomly divided into three groups and received a single intravenous infusion of GLS-010 at doses of 2,6,and 18 mg·kg-1,respectively.The rest were randomly assigned to four groups and received multiple intravenous infusions of GLS-010 at doses of 0,5,25,and 100 mg·kg-1,respectively,once a week(quaque week,qw)for five consecutive weeks.Blood samples were collected before and after administration.The concentrations of GLS-010 in the monkey serum were measured using a validated enzyme-linked immunosorbent assay,while those of anti-drug antibodies(ADA)in the cynomolgus monkey serum were determined by ultra-sensitive electrochemiluminescence immunoassay.The PK data on GLS-010 in cynomolgus monkeys was obtained,and the drug-time curves were plotted.A PopPK model was constructed using non-compartmental analysis and evaluated by goodness-of-fit plots and visual predictive checks.The constructed PopPK model was used to predict the PK characteristics in humans,which were finally compared with actual Phase Ⅰ clinical study results for validation.RESULTS The predictive results of the PopPK model were highly consistent with the actual Phase Ⅰ clinical study results.The model was able to predict the human PK characteristics under various dosing regimens,including 1 mg·kg-1 quaque 2 weeks(q2w),4 mg·kg-1(q2w),240 mg(q2w),240 mg(q3w),and 10 mg·kg-1(q2w).The predicted maximum plasma concentrations(Cmax)were 24.8,99.1,85.0,85.0,and 247.8 mg·L-1,respectively,and the AUC0-336h was 4 902.0,20 060.0,17 147.7,22 145.7(AUC0-504h),and 50 817.6 mg·h·L-1,respectively.The safety risks for the corresponding dosing regimens were 47.3,11.6,13.5,10.5,and 4.6,respectively.The predicted receptor occupancy at steady state(ROss)at Cmax,average plasma concentration(Cavg),and minimum plasma concentration(Cmin)were 38.8%,72.7%,69.4%,64.1%and 87.2%,29.1%,63.8%,60.0%,49.8%and 82.1%,21.9%,55.5%,51.3%,36.3%and 76.7%,respectively.CONCLUSION The PopPK model can effectively predict the human PK characteristics under different dosing regimens with high consistency with actual Phase Ⅰ clinical study results,which can serve as an important reference for selection of safe and effective doses for first-in-human research.

7.Clinical features and prognosis of anterior circulation cerebral Infarction secondary to cervical artery dissection

Objective To investigate the clinical features and prognosis of anterior circulation cerebral infarction secondary to cervical artery dissection.Methods Sixty one patients with cervical artery dissection (CAD) secondary to anterior circulation cerebral infarction treated in Department of Neurology of North Jiangsu People’s Hospital from January 2016 to December 2020 were included consecutively.And 67 patients with large artery atherosclerosis (LAA) cerebral infarction complicated with moderate to severe carotid artery stenosis who were hospitalized at the same time were enrolled for comparison.The clinical data and prognosis between the two groups were compared.Results Unifactorial analysis showed that the average age of CAD cerebral infarction group was younger,the incidence of head and neck pain and monocular amaurosis was higher,the proportion of massage trauma fatigue history was higher,the incidence of hypertension and smoking history was higher in LAA cerebral infarction group.The comparison of the two hematologic examinations showed that the erythrocyte count was higher in the CAD cerebral infarction group,and the D-dimer,cholesterol,glycated hemoglobin,and uric acid values were higher in the LAA cerebral infarction group.Binary logistic regression analysis showed that the history of massage trauma fatigue was more correlated with CAD cerebral infarction,while older age and smoking history were more correlated with LAA cerebral infarction.The improvement of NIHSS scores during hospitalization in the CAD cerebral infarction group was more significant than that in the LAA cerebral infarction group,and the difference was statistically significant.There were no significant differences in 90 day prognosis,mortality,and reinfarction rate within 6 months between the two groups.Conclusion The correlation between the history of massage trauma and fatigue and CAD cerebral infarction was greater,and the correlation between the older age and smoking history and LAA cerebral infarction was greater.The improvement of NIHSS score in CAD cerebral infarction group during hospitalization was more obvious than that in LAA cerebral infarction group,which was statistically significant.There were no significant differences in 90 days prognosis,mortality and 6 month reinfarct rate between the two groups.
