1.Genetic diversity analysis of oxacillinase in 241 clinical isolates of Pseudomonas aeruginosa

Objective:To analyze the carriage status, subtype distribution and flanking gene sequence characteristics of oxacillinases (OXA enzyme) in 241 clinical strains of Pseudomonas aeruginosa, and assess their roles in the drug resistance of Pseudomonas aeruginosa and ability to horizontally transfer across species. Methods:Clinical P. aeruginosa isolates were collected from four hospitals in Sanya, Tangshan, Zhangjiakou, and Beijing. The prevalence of oxacillinases and their flanking gene sequences was analyzed by whole-genome sequencing (NGS) and bioinformatic approaches. Results:A total of 241 isolates of P. aeruginosa were gathered, and 35 blaOXA subtypes were identified through screening of 252 blaOXA genes. These genes were classified into three subfamilies: blaOXA-50-like (241, 95.6%), blaOXA-1-like (9, 3.6%) and blaOXA-10-like (2, 0.8%). Among these, 11 subtypes (11, 31.4%) were novel blaOXA subtypes. Nine of these belonged to the blaOXA-50-like subfamily and were designated as blaOXA-1244, blaOXA-1245, blaOXA-1246, blaOXA-1250, blaOXA-1252, blaOXA-1253, blaOXA-1254, blaOXA-1255, and blaOXA-1256. The remaining two belonged to the blaOXA-10-like subfamily and were named blaOXA-1247 and blaOXA-1248. Compared to the amino acid sequence of OXA-10, the newly identified subtype OXA-1247 exhibited a mutation at position 117, where a valine was replaced by a leucine. This change was thought to improve the enzyme′s ability to hydrolyze carbapenems. In the analysis of the flanking sequences of the blaOXA genes, Class I integrons were identified in four bacterial strains. The variable regions of these integrons carried three distinct patterns of resistance gene cassettes: aac( 6′) -Ib-blaOXA-1247-ant( 3′′) -Ia, aac( 6′) -Ib-blaOXA-1248 and aac( 6′) -Ib- blaIMP-45-blaOXA-1-catB3. Among these, the strain BJ2326 carried a class I integron that was connected to the downstream IS CR1 element to form a composite class I integron structure, additionally carrying the resistance gene blaPER-1. Out of the 223 non-wild-type P. aeruginosa strains, 127 strains exhibited non-wild-type profiles to the four beta-lactam antibiotics MEM, CAZ, FEP, and TZP, with the combination of MEM+CAZ+FEP being the most prevalent, representing 57.0% of the total. Conclusions:The blaOXA genes in 241 clinical P. aeruginosa strains showed diversity. Some blaOXA genes had a co-transfer risk with the metallo-β-lactamase resistance gene blaIMP-45. Among the 11 newly discovered blaOXA subtypes, the new subtype OXA-1247 may have carbapenemase activity and potential for horizontal transfer.

Objective:To explore the relationship between parenting styles of toddlers aged 30 months and attention deficit hyperactivity disorder (ADHD) symptom trajectories in preschool children.Methods:The subjects were 2 111 children and their parents in Ma'anshan Birth Cohort. Parenting styles were assessed at 30 months of age using the Brief Toddler Parenting Style Questionnaire, which defines parenting styles on four dimensions: warmth and interaction, attention and acceptance, outdoor and social activities, and neglect and restrictions. The Chinese version of the Conners Abbreviated Symptom Questionnaire was used to examine preschooler's ADHD symptoms aged 3, 5, and 6 years. ADHD symptom trajectories were fitted using group-based trajectory modeling. The multinomial logistic regression model was used to evaluate the relationship between parenting styles at 30 months of age and ADHD symptom trajectories in children. The additive and multiplicative models were applied to analyze the interaction effect of negative parenting styles in children's ADHD symptom trajectories.Results:ADHD symptom trajectories in children included the low-score (48.3%), moderate-score (47.7%), and high-score groups (4.0%). After adjusting for the confounding factors, the results of the multinomial logistic regression analysis showed that negative parenting styles of the father/mother were associated with ADHD symptom trajectories in children ( P<0.05). The father/mother's low warmth and interaction (father: OR=2.07,95% CI:1.68-2.54; mother: OR=2.02,95% CI:1.64-2.48), low attention and acceptance (father: OR=1.94,95% CI:1.57-2.39; mother: OR=1.47,95% CI:1.20-1.81), low outdoor and social (father: OR=1.74,95% CI:1.43-2.13; mother: OR=1.60,95% CI:1.32-1.94), and high neglect and restriction (father: OR=2.09,95% CI:1.67-2.62; mother: OR=2.06,95% CI:1.65-2.56) were associated with ADHD symptom moderate-score trajectories compared to the low-score trajectory group, mother's low warmth and interaction, low outdoor and social, and high neglect and restrictions. Statistically significant association was seen in negative parenting styles of fathers with the high-score group. There was a multiplicative interaction for hostile parenting ( P<0.05). The additive interaction results showed that there was a synergistic effect of hostile parenting, which increased the risk of the high-score trajectories of ADHD symptoms in children. Conclusions:Both parents' parenting styles influenced ADHD symptom change the trajectories in preschoolers. Negative parenting styles of the father/mother increase the risk of the moderate-score and high-score trajectories of ADHD symptoms in children. Hostile parenting by both parents has a synergistic impact on the risk of ADHD symptom trajectories in children, and the simultaneous presence of both increases the risk of the high-score trajectory of ADHD symptoms.
3.Latent profile analysis of health activation among community-dwelling older patients with chronic diseases

Objective:To explore the latent categories and influencing factors of health activation among community-dwelling older adults with chronic diseases.Methods:Older patients with chronic diseases attending three community health centers in Hefei City from August to December 2023 were selected for the study using the convenience sampling method. The survey utilized the General Information Questionnaire, Consumer Health Activation Index, Chinese version of the Life-Space Assessment, Social Support Rating Scale, and Simplified Coping Style Questionnaire. Mplus 8.3 software was applied for latent profile analysis to examine patients' health activation, and unordered multicategory Logistic regression was used to explore the factors influencing different categories of health activation.Results:A total of 382 questionnaires were distributed, and 368 valid questionnaires were recovered, with a valid recovery rate of 96.34%. Three latent categories of health activation existed in community-dwelling older adults with chronic diseases, which were named as low health activation-high efficiency-coping inability group (16.85%, 62/368) , medium health activation group (27.99%, 103/368) , and high activation perception-action group (55.16%, 203/368) . Multicategory logistic regression analysis indicated that place of residence, age, self-rated health status, life space, social support, and active coping strategies were significant factors influencing the health activation of different latent categories of community-dwelling older adults with chronic diseases (all P<0.05) . Conclusions:The health activation of community-dwelling older adults with chronic diseases can be classified into three latent categories. Primary healthcare providers should identify the characteristics of patients in different categories and implement targeted nursing interventions to enhance their health activation levels.

Diabetic kidney disease(DKD),a prevalent complication of diabetes mellitus,remains a leading cause of end-stage renal disease.Recent research has identified lipophagy,a novel mechanism in DKD pathogenesis,drawing increasing attention in the field.This paper explores the biological connotation of the"internal heat leading to Zheng"pathogenesis based on lipophagy.The study proposes that lipophagy represents the microscopic biological correlation of liver-spleen coordination in regulating spleen transport and the ascending-descending dynamics of the middle jiao.Under persistent hyperglycemia,the suppression of lipophagic activity mirrors the traditional Chinese medicine(TCM)pathophysiological process described as"excessive fire consuming healthy qi,"whereas aberrant lipid accumulation in the kidney corresponds to the dynamic aggregation and dispersion of micro-zhengjia.Lipotoxicity,a key driver of DKD progression,is interpreted as the biological manifestation of accumulated turbidity transforming into toxicity,resulting in progressive impairment of renal essence and function.The dynamic process of lipophagy dysfunction under hyperglycemia,marked by renal microangiopathy,glomerular and tubular dysfunction,and renal fibrosis,closely mirrors the pathological evolution of"micro-zhengjia"and"internal heat leading to Zheng."Consequently,TCM strategies for DKD prevention and treatment should emphasize heat regulation,stage-specific interventions,liver-spleen harmonization,metabolic modulation,early collateral protection,and blood-activating approaches.
5.Clinical Observation of Guben Quyu Jiedu Prescription in Treating Nocturnal Hypoxemia of COPD Combined with OSAHS

ObjectiveTo observe the clinical efficacy and safety of Guben Quyu Jiedu prescription in treating nocturnal hypoxemia of chronic obstructive pulmonary disease (COPD) combined with Obstructive sleep apnea hypopnea syndrome ( OSAHS ) (deficiency of lung, spleen and kidney with blood stasis and toxicity). MethodsThe paper used a forward-looking, random double-blind, placebo-controlled design method to select 96 patients with COPD combined with OSAHS, and their traditional Chinese medicines (TCM) syndrome differentiation was deficiency of lung, spleen and kidney with blood stasis and toxicity. These patients were randomly divided into the observation group and the control group, with 48 cases in each group. Based on conventional Western medicine treatment, the observation group was treated with Guben Quyu Jiedu prescription and the control group was treated with traditional Chinese medicine placebo. Both courses of treatment were 90 days. Then the paper compared the changes in minimum pulse oxygen saturation (SpO2) during the night, apnea index (AHI), OSAHS efficacy evaluation, TCM syndrome efficacy evaluation, and TCM symptom score before and after treatment between the two groups. ResultsThere were 5 withdrawals in the observation group and 8 withdrawals in the control group, so 43 cases in the observation group and 40 cases in the control group completed the trial. Compared with the condition before treatment, the minimum SpO2 during the night and AHI in the observation group were significantly improved at night (P<0.01) and were better than those in the control group (P<0.01). OSAHS efficacy in the observation group was better than in the control group (χ2=7.085, P<0.05). In terms of TCM syndrome efficacy, the total effective rate was 81.40% (35/43) in the observation group, significantly higher than that in the control group, which was 15.00% (6/40) (χ2=36.78, P<0.01). The TCM symptom scores of the two groups were improved compared with the condition before treatment, and the effect of the two groups was similar in the four main symptoms of snoring, choking, lethargy, and cough. However, the observation group was better than the control group in 10 details such as dizziness, headache, chest tightness, chest pain, wheezing, dry mouth, and thirst (P<0.05). ConclusionUsing Guben Quyu Jiedu prescription combined with conventional Western medicine can treat COPD combined with OSAHS hypoxemia at night (deficiency of lung, spleen and kidney with blood stasis and toxicity). In this way, the minimum pulse oxygen saturation (SpO2) of patients, the level of disease control, and the quality of life of patients can be improved, and the clinical symptoms can be relieved.
6.The relationship between demoralization syndrome and death anxiety in stroke patients undergoing neuro-endovascular procedures based on latent profile analysis

Objective To identify potential subtypes of demoralization syndrome among stroke patients undergoing neuroendovascular procedures using latent profile analysis,and to examine differences in death anxiety among these subtypes.Method This cross-sectional study used convenience sampling to recruit 202 stroke patients undergoing neuroendovascular procedures at a hospital from November 2024 to March 2025.Participants completed the General Information Questionnaire,Despair Scale(DS),and the Chinese version of the Templer's Death Anxiety Scale(CT-DAS).Using R software,we estimated a series of latent profile models(ranging from 2 to 6 profiles)based on four manifestations of demoralization syndrome:loss of meaning and purpose,dysphoria,helplessness and hopelessness,and sense of failure.Starting from the 2-profile model,we incrementally increased the number of profiles and compared model fit indices to identify the optimal solution.Subsequently,logistic regression analysis was conducted to identify the factors influencing DS scores according to the latent profile classifications.We compared DS scores across subgroups and performed bivariate correlation analysis between DS and CT-DAS subdimensions(Affective,Stress&Pain,Time Awareness,and Cognitive).Results The DS scores classified stroke patients into two distinct subtypes of demoralization syndrome:A Meaninglessness-Dysphoria Group(51.00%,103/202)and a Helplessness-Hopelessness Group(49.00%,99/202).The HHG demonstrated significantly higher proportions(P<0.05)of female patients,those hospitalized for 6~10 days,recipients of arterial stenosis/occlusion surgery,individuals with junior high school or higher education,employed subjects,and rural residents compared to the MDG.Binary logistic regression analysis revealed:Lower probability of HHG membership for urban(OR=0.159,P<0.001)and city(OR=0.224,P=0.007)residents versus rural counterparts;Higher HHG probability in patients hospitalized 6-10 days versus 1~5 days(OR=2.311,P=0.017);Elevated HHG risk among junior high school(OR=4.956,P<0.001)and high school or above graduates(OR=5.102,P=0.001)relative to those with primary education or less;Increased HHG probability in aneurysm embolization(OR=2.419,P=0.040)and arterial stenosis/occlusion surgery(OR=2.733,P=0.014)recipients compared to cerebrovascular angiography patients.The HHG scored significantly lower on the cognitive dimension of death anxiety than the MDG(t=2.421,P=0.016),while showing no differences in affective,stress&pain,or time awareness dimensions(P>0.05).Bivariate Pearson correlations indicated dysphoria(DS)positively correlated with Affective(r=0.192,P=0.006),Time Awareness(r=0.172,P=0.015),and Cognitive(r=0.139,P=0.049)of death anxiety.Conclusions Stroke patients undergoing neuroendovascular procedures exhibit elevated levels of death anxiety,with significant varia-tions observed across different subtypes of demoralization syndrome.Residential location,hospitalization duration,anxiety severity,and surgical type constitute significant determinants of demoralization syndrome.Targeted clinical interventions should be implemented to alleviate demoralization severity and mitigate death anxiety.

9.Optimization of inferior vena cava imaging quality using spectral CT virtual monoenergetic images combined with multiphase scanning

Objective:To investigate the optimization of inferior vena cava imaging using dual-layer spectral detector CT (DLCT) virtual monoenergetic images (VMI) combined with multiphase scanning.Methods:A retrospective analysis was conducted on the imaging data of 184 patients who underwent inferior vena cava imaging using dual-layer detector spectral CT at Union Hospital, Tongji Medical College, Huazhong University of Science and Technology, from January 2021 to October 2024. Each patient underwent multiphase scanning (60, 80, and 120 s after contrast injection were referred to as the first, second, and third phases, respectively). The images were reconstructed into conventional 120 kVp polyenergetic image (PI) and VMIs at 40, 50, 60, 70, and 80 keV. Image quality of 120 kVp PI and VMI for each phase was evaluated. The objective image quality indicators included CT value, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), and noise. Comparisons of the above indictors within the same phase were performed using repeated measures ANOVA or the Friedman test, while comparisons between different phases were conducted using one-way ANOVA or the Kruskal-Wallis test.Results:At the same phase, the CT value, SNR, and CNR of the 40 keV VMI were higher than those of other energy level VMIs and PI (all P<0.001). The SNR of the 40 keV VMI in the third phase was significantly higher than in the first phase ( P<0.05), while there was no significant difference between the first and second phases ( P>0.05). The standard deviation (SD) of the 40 keV VMI in the third phase was significantly lower than that in the first and second phases (all P<0.05). The subjective scores for the 40 keV VMI were higher than those for other energy level VMIs and PI at the same phase ( P<0.001). The subjective scores for the 40 keV VMI in the third phase were higher than those in the second and first phases ( P<0.001). The percentage of scores≥4 in the third phase (77.17%,142/184) was significantly higher than those in the first phase (28.26%,52/184) and second phase (61.96%,114/184) ( P<0.001). Conclusion:In inferior vena cava imaging, the 40 keV VMI, combined with the optimal phase (120 s delay), effectively optimizes image quality.

Objective To investigate the application effect of a refined early diet plan in patients with distal gastric cancer after laparoscopic radical surgery.Methods By convenience sampling method,46 patients with laparoscopic radical gastrectomy for distal gastric cancer admitted to a tertiary hospital in Zhejiang Province from July to December 2023 were selected as an experimental group,while 49 patients admitted from January to June 2023 as a control group.The experimental group was administered with the refined early diet plan,and the control group was administered with conventional methods.The intervention period was from the first day after surgery to discharge.The differences of the first postoperative defecation time,perioperative thirst and hunger score,postoperative hospitalization time,hospitalization cost,postoperative complications and the incidence of readmission 30 days after surgery were compared between the 2 groups.Results 42 patients in the experimental group and 49 patients in the control group completed the study.First postoperative exhaust time(t=4.922,P<0.001),first postopera-tive defecation time(Z=-2.440,P=0.015),perioperative thirst score(Z=-8.024,P<0.001),perioperative hunger score(Z=-8.192,P<0.00 1),postoperative hospitalization time(Z=-7.622,P<0.001)and hospitalization cost(Z=-4.522,P<0.001)were lower than those of the control group,and the difference was statistically significant.There was no significant difference in complication rate and 30-day readmission rate between the experimental group and the control group(P>0.05).Conclusion The refined early diet plan is safe and effective for early recovery of patients after laparoscopic radical gastrectomy for distal gastric cancer,promoting intestinal function recovery,shortening hospital stay,reducing hospital costs,and improving patients'subjective comfort.
