1.Mechanism of Number 2 Feibi Recipe in Ameliorating Pulmonary Fibrosis in Mice by Modulating Endoplasmic Reticulum Stress in AT2 Cells to Attenuate Apoptosis and Promote Alveolar Repair

ObjectiveTo investigate the intervention mechanism of the traditional Chinese medicine Number 2 Feibi recipe (N2FBR) in idiopathic pulmonary fibrosis (IPF), focusing on its effects on endoplasmic reticulum (ER) stress, apoptosis, stemness maintenance, and regenerative capacity of alveolar type Ⅱ epithelial cells (AT2 cells), and to validate the modern translational pathway of the theory of "deficiency of Zong Qi leading to pulmonary atelectasis and atrophy". MethodsA mouse model of pulmonary fibrosis was induced by bleomycin (BLM). Mice were randomly divided into blank control, model, low-, and high-dose N2FBR intervention groups (9.1, 18.2 g·kg-1), and prednisolone intervention group (6.5 mg·kg-1). Pulmonary histopathological changes and collagen deposition were evaluated using hematoxylin-eosin (HE) and Masson's trichrome staining. Hydroxyproline (HYP) content was measured by the alkaline hydrolysis method. Lung coefficient and pulmonary function parameters were evaluated. The mRNA expression levels of fibrosis-related factors, including collagen type Ⅰ alpha 1 chain (ColIa1), alpha-smooth muscle actin (α-SMA), and tissue inhibitor of metalloproteinase 1 (Timp1), were detected by real-time polymerase chain reaction (Real-time PCR). Cell apoptosis was assessed using the terminal deoxynucleotidyl transferase dUTP nick-end labeling (TUNEL) assay. Apoptosis of AT2 cells was further evaluated by double immunofluorescence staining for surfactant protein C (SPC) and cysteine-aspartic protease-3 (Caspase-3). Endoplasmic reticulum (ER) stress in AT2 cells was examined by double staining for SPC and protein kinase R-like endoplasmic reticulum kinase (PERK). Ultrastructural changes of ER and lamellar bodies in AT2 cells were observed by transmission electron microscopy (TEM). The expression levels of key proteins involved in ER stress and apoptosis pathways, including PERK, activating transcription factor 4 (ATF4), and Caspase-3, were detected by Western blot. Double immunofluorescence staining of SPC and Ki-67 antigen (Ki-67) was performed to evaluate the proliferative capacity of AT2 cells. Lineage tracing technology (labeling AT2 cells with GFP) combined with Krt8 labeling was used to evaluate intermediate differentiation states, and morphological transformation of AT2 cells into alveolar type Ⅰ epithelial cells (AT1) was observed. ResultsBLM-induced mice exhibited significant structural disruption of lung tissue, increased collagen deposition, elevated lung coefficient, decreased pulmonary function, and upregulation of fibrosis-related factors (P<0.01). High-dose N2FBR treatment significantly ameliorated lung tissue damage and dysfunction, significantly reduced HYP content (P<0.01), and significantly downregulated ColIa1, α-SMA, and Timp1 expression (P<0.01). Apoptosis analysis showed increased TUNEL-positive and Caspase-3-positive AT2 cells in the model group, which was significantly reduced by high-dose N2FBR treatment. TEM revealed swollen ER structures in AT2 cells of the model group, which tended to return to normal following treatment. PERK protein staining analysis showed evident ER stress in AT2 cells of the model group, which were markedly alleviated in the treatment group. The expression levels of ER stress-related proteins PERK and ATF4, as well as the apoptosis-related protein Caspase-3, were elevated in the model group and significantly reduced after treatment. TEM also revealed disrupted lamellar body structures in the model group, which tended to recover in the treatment group. Regarding the proliferative capacity of AT2 cells, the proportion of Ki-67⁺SPC⁺ AT2 cells significantly increased in the treatment group (P<0.01). Lineage tracing showed that the proportion of keratin 8-positive green fluorescent protein-positive (Krt8⁺GFP⁺) cells increased in the model group, indicating differentiation arrest. This proportion was significantly reduced in the treatment group, and the morphology of GFP⁺ cells exhibited a flattened, extended shape, suggesting restored differentiation toward AT1 cells. ConclusionN2FBR alleviates ER stress in AT2 cells, reduces AT2 cell apoptosis, restores lamellar body structure and function, enhances proliferation activity, and alleviates differentiation arrest to promote differentiation into AT1 cells, thereby repairing the alveolar epithelium and effectively blocking the progression of pulmonary fibrosis. Its traditional Chinese medicine mechanism of "replenishing Zong Qi, harmonizing Qi and blood, and unblocking pulmonary meridians" closely aligns with the modern regulatory pathway of AT2 stem cells, providing a novel theoretical basis and experimental evidence for the intervention of IPF with traditional Chinese medicine.

3.Mechanistic Interpretation of Zheng’s San Qi San Powder in Treating Skeletal Muscle Injury via Bioinformatics Prediction, Chemical Analysis and Experimental Verification

ObjectiveZheng’s San Qi San (ZSQS) power, a classic traditional Chinese medicine (TCM) formula, is used for treating soft tissue injuries involving muscles, tendons, and ligaments. However, its underlying therapeutic mechanisms remain unclear. This study aimed to screen and identify pharmaceutically active ingredients and their candidate biomolecule targets, and further elucidate the molecular mechanism of ZSQS in the treatment of skeletal muscle injury. MethodsNetwork pharmacology was employed to construct “ZSQS-component-target”, “protein-protein interaction (PPI)” and “active ingredient-core protein-pathway” networks to predict the key active ingredients and potential core targets of ZSQS for skeletal muscle injury. The predicted results were then validated via microarray data from the GEO database. Molecular docking was then performed to assess the binding ability between the screened active ingredients of ZSQS and the candidate core targets. Moreover, liquid chromatography-mass spectrometry (LC-MS) was used for qualitative and quantitative analysis to verify the active components of the drug and ZSQS serum. Finally, an animal model of eccentric exercise-induced skeletal muscle injury and a myotube cell model of oxidative stress-induced injury were established to validate the effects of ZSQS and its interventional effects on the biological functions of critical targets, thereby demonstrating the potential therapeutic mechanism of ZSQS. ResultsAmong the 111 active components identified in ZSQS and their corresponding 204 targets related to the skeletal muscle injury repair process, 14 core targets (including AKT1) and 4 core active components (quercetin, luteolin, kaempferol, and β‑sitosterol) were screened out, while the corresponding metabolites of quercetin, luteolin and kaempferol were detected in the ZSQS serum. Among these targets, 5 candidate genes (IL-6, CASP3, HIF1A, STAT3, and JUN) overlapped with the differential expression screening results with GEO data, and IL-6 was confirmed to be enriched in the PI3K/AKT pathway. Combined with the prediction results of the AKT expression levels, these findings suggest that the phosphorylation level of AKT1 plays a core role in the therapeutic mechanism of ZSQS. Molecular docking analysis further revealed that the PH domain of AKT1 had high binding energy with all 4 core active components, as verified by LC-MS. Finally, animal model studies have shown the promoting effect of ZSQS administration on skeletal muscle injury repair and its possible antioxidant damage mechanism. Cell model studies further demonstrated that ZSQS-containing serum, core active ingredient combination therapy, and quercetin monomer could increase the phosphorylation level of AKT, promote the nuclear translocation of Nrf2, upregulate the expression of downstream antioxidant enzymes (SOD, GPx, and GR), and inhibit the expression of inflammatory factors (IL-6 and TNF-α), thereby alleviating oxidative stress and the inflammatory response. ConclusionZSQS alleviates skeletal muscle injury mainly by activating the AKT/Nrf2 signaling pathway, enhancing cellular antioxidant and anti-inflammatory capabilities. The results of this study provide a scientific basis for the clinical application and modernized development of ZSQS.
4.Effect of Xiaoqinglong Decoction (小青龙汤) on Th2 Immune Responses and Nasal Mucosal TSLP/OX40L Pathway in Allergic Rhinitis Model Rats

ObjectiveTo explore the potential mechanism of Xiaoqinglong Decoction (小青龙汤, XD) in the treatment of allergic rhinitis. MethodsForty-five rats were randomly assigned to a control group, a model group, a loratadine group, low-, medium- and high-dose XD groups, and low-, medium- and high-dose Mahuang Decoction and Cang'erzi Powder (麻黄汤合苍耳子散, MDCP) groups. Except for the control group, rats were administered with ovalbumin (OVA) and aluminum hydroxide via intraperitoneal injection for 14 days to establish an allergic rhinitis model. After the 14th-day injection, nasal stimulation was continued with 20 μl of 10% OVA solution to maintain the model. Rats in the control group and the model group received 10 ml/(kg·d) of saline, whereas those in the loratadine group were administered with 0.9 mg/(kg·d) of loratadine. The low-, medium- and high-dose XD groups were administered XD at the dose of 2.7, 5.4, and 10.8 g/(kg·d), respectively. The low-, medium- and high-dose MDCP groups were administered MDCP at the dose of 2.43, 4.86, and 9.72 g/(kg·d), respectively. All treatments were administered by gavage once daily for 7 consecutive days. One hour after the final gavage, nasal symptom scores were recorded for all group of rats. The next day, serum levels of immunoglobulin E (IgE), interleukin-4 (IL-4), and interleukin-13 (IL-13) were measured. HE staining was used to observe the pathological morphology of the nasal mucosal tissue. Quantitative reverse transcription PCR (RT-qPCR) and Western Blot were performed to assess mRNA and protein expression of thymic stromal lymphopoietin (TSLP) and OX40 ligand (OX40L) in the nasal mucosa. ResultsCompared to the control group, total nasal symptom score in the model group significantly increased (P<0.01). HE staining revealed disrupted and adhered cilia, thickened basement membranes, and extensive inflammatory cell infiltration in the nasal mucosa. Serum levels of total IgE, IL-4, and IL-13, as well as TSLP and OX40L mRNA and protein expression in the nasal mucosa, were significantly elevated in the model group (P<0.05 or P<0.01). Compared to the model group, the total nasal symptom scores in all drug intervention groups were significantly reduced; the serum total IgE levels in the loratadine group, the low- and medium-dose XD groups, and the low- and high-dose MDCP groups were significantly reduced; and the serum levels of IL-4 and IL-13 in the high-dose XD group and the high-dose MDCP group decreased (P<0.05 or P<0.01). Nasal mucosal structure was improved. Except for the low-dose MDCP group, all other intervention groups showed a significant reduction in TSLP and OX40L mRNA expression in the nasal mucosa (P<0.01). All doses of XD and the medium- and high-dose MDCP groups significantly decreased the protein levels of TSLP and OX40L (P<0.05). The medium-dose XD group exhibited more improvement of nasal symptom scores and greater suppression of expression of TSLP and OX40L mRNA, and TSLP protein levels compared to the loratadine group (P<0.05). ConclusionXD may protect nasal mucosa of rats and alleviate allergic rhinitis by suppressing the TSLP/OX40L pathway, thereby attenuating Th2-mediated immune responses.

6.Value of combined detection of serum FAR and MFG-E8 in predicting the fetal protection outcome of threatened abortion patients

Objective To evaluate the value of combined detection of serum fibrinogen/albumin ratio(FAR)and milk fat globule epidermal growth factor 8(MFG-E8)in predicting the fetal protection outcome of threatened abortion patients.Methods From May 2022 to May 2023,a total of 89 patients with threatened abortion who received fetal protection treatment in Jianli People's Hospital were regarded as the observation group.They were grouped into a miscarriage group(19 cases)and a successful group(70 cases)based on the fetal protection outcomes.89 normal pregnant women who underwent prenatal examinations were regarded as the control group.Enzyme-linked immunosorbent assay was applied to detect the levels of serum fibrinogen,albumin and MFG-E8.Multivariate Logistic regression was applied to analyze the influencing factors on the fe-tal protection outcome of patients with threatened abortion.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum FAR and MFG-E8 level on the fetal protection outcome of patients with threatened abortion.Results Compared with the control group,serum FAR was higher in the observation group(P<0.05)and serum MFG-E8 level was lower(P<0.05).The proportion of thyroid dys-function,serum C-reactive protein(CRP),and FAR in the miscarriage group were higher than those in the successful group(P<0.05),while serum MFG-E8 level was lower than that in the successful group(P<0.05).Serum FAR,CRP and thyroid dysfunction were risk factors for fetal protection outcome of patients with threatened abortion,while serum MFG-E8 was a protective factor for fetal protection outcome of patients with threatened abortion(P<0.05).The area under the curve(AUC)of serum FAR,MFG-E8,and their combination in predicting the fetal protection outcome of patients with threatened abortion was 0.787,0.853,and 0.954,respectively,and the efficacy of combination of the two in predicting the fetal protection outcome of patients with threatened abortion was better than that of serum FAR and MFG-E8 alone(Zcombination-FAR=2.805,Zcombination-MFG-E8=2.076,P=0.005,0.038),with sensitivity and specificity of 94.74%and 88.57%,re-spectively.Conclusion Serum FAR increases and MFG-E8 level decreases of patients with threatened abor-tion,and the combination of the two has higher predictive value for the fetal protection outcome of patients with threatened abortion.

Objective To explore the relationship between serum CXC chemokine ligand 12(CXCL12),an-giotensin Ⅱ receptor-like 1 endogenous ligand 13(Apelin-13)and carotid atherosclerosis(CAS)in patients with type 2 diabetes mellitus(T2DM).Methods From October 2022 to September 2024,a total of 105 T2DM patients in Rugao People's Hospital(the hospital)were included as the T2DM group,and healthy volunteers who experienced physical check ups in the hospital were regarded as the control group.According to whether T2DM patients developed CAS,they were assigned into non CAS group(n=61)and CAS group(n=44).Fully automatic biochemical analyzer was applied to detect the levels of blood lipid indicators total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C)and low-density lipoprotein choles-terol(LDL-C).Fully automatic glycated hemoglobin analyzer was used to detect glycated hemoglobin(HbA1c)level.Enzyme-linked immunosorbent assay(ELISA)was applied to detect the levels of CXCL12 and Apelin-13.Multivariate Logistic regression was applied to analyze the influencing factors of concurrent CAS in T2DM.Pearson correlation was used to analyze the correlation between CXCL12,Apelin-13 and the course of diabetes,blood glucose and lipid indicators.Receiver operating characteristic(ROC)curve was applied to ana-lyze the predictive value of CXCL12 and Apelin-13 for T2DM complicated with CAS,and the Z-test was ap-plied to compare the differences in the area under the curve(AUC).Results The level of CXCL12 in the T2DM group was higher than that in the control group(P<0.05),and Apelin-13 level was lower than that in the control group(P<0.05).The course of T2DM,HbA1c,TC,TG,LDL-C,and CXCL12 levels in the CAS group were higher than those in the non CAS group(P<0.05),while HDLC and Apelin-13 levels were lower than those in the non CAS group(P<0.05).The level of CXCL12 in the CAS group was positively correlated with the course of T2DM,HbA1c,TC,TG,and LDL-C levels(P<0.05),and negatively correlated with HDL-C levels(P<0.05),while the correlation of Apelin-13 was opposite(P<0.05).CXCL12 and Apelin-13 were independent influencing factors for concurrent CAS in T2DM patients(P<0.05).The AUC predicted by CX-CL12 and Apelin-13 in T2DM patients complicated with CAS was 0.916,which was better than 0.783 and 0.788 predicted separately(P<0.05).Conclusion The high levels of CXCL12 and low levels of Apelin-13 in the serum of T2DM patients are independent influencing factors for concurrent CAS in T2DM.The combined detection of CXCL12 and Apelin-13 to predict CAS in T2DM patients has certain clinical significance and pro-vides a basis for disease assessment and treatment.
8.Exploration of the predictive value of high-resolution CT imaging features combined with Ki-67 expression for poorly differentiated invasive non-mucinous lung adenocarcinoma

Objective:To investigate the predictive value of high-resolution CT (HRCT) imaging features and Ki-67 expression levels for poorly differentiated invasive non-mucinous lung adenocarcinoma (INMA), and to construct and validate a nomogram prediction model based on these factors.Methods:A total of 210 INMA patients who underwent radical surgery at Shaanxi Provincial Cancer Hospital from July 2020 to October 2023 and obtained histopathological results were retrospectively included. Based on the degree of lesion differentiation, they were divided into well/moderately differentiated INMA group ( n=152) and poorly differentiated INMA group ( n=58). The general clinical data, HRCT imaging features and Ki-67 expression of the two groups of patients were compared. Multivariate logistic regression analysis was used to analyze independent influencing factors for poorly differentiated INMA, and a nomogram prediction model was constructed. The diagnostic efficacy of the prediction model was evaluated by the receiver operator characteristic (ROC) curve, and the prediction model was validated by consistency index (C-index) and calibration curve. Results:There were no statistically significant differences in terms of age, location, margin, lobulation, vascular convergence, and cavitation between the well/moderately differentiated INMA group and poorly differentiated INMA group (all P>0.05). There were statistically significant differences in sex ( χ2=6.65, P=0.010), Ki-67 expression ( U=2.33, P=0.021), nodule size ( t=-3.34, P=0.010), spiculation ( χ2=5.22, P=0.022), pleural indentation ( χ2=17.02, P<0.001), air bronchogram ( χ2=15.54, P<0.001) and nodule type ( χ2=59.67, P<0.001) between the two groups. Multivariate analysis showed that nodule size ( OR=1.07, 95% CI: 1.01-1.14, P=0.025), nodule type ( OR=8.23, 95% CI: 3.04-22.32, P<0.001) and Ki-67 expression ( OR=1.07, 95% CI: 1.03-1.11, P<0.001) were independent influencing factors for the occurrence of poorly differentiated INMA. A nomogram prediction model for poorly differentiated INMA was constructed based on the above indicators. ROC curve analysis showed that the area under curve of the prediction model to predict the occurrence of poorly differentiated INMA was 0.893, and the sensitivity and specificity were 89.70% and 77.60%, respectively. The C-index value of the model was 0.893. The calibration curve showed that the predicted probability was in good agreement with the actual probability. Conclusions:Nodule size, nodule type in HRCT imaging features and Ki-67 expression are independent influencing factors for the occurrence of low differentiation in INMA patients. The nomogram prediction model constructed based on the above indicators has good predictive performance.
9.Clinical study on the treatment of chronic atrophic gastritis with spleen and stomach weakness syndrome by Piwei Peiyuan Pill combined with moxibustion

Objective:
To determine the clinical efficacy and mechanism of Piwei Peiyuan Pill (PPP) combined with moxibustion for treating patients with chronic atrophic gastritis (CAG) with spleen and stomach weakness syndrome.
Methods:
Ninety-six CAG patients with spleen and stomach weakness syndrome who met the inclusion and exclusion criteria were enrolled at the Department of Spleen and Stomach Diseases of the Second Affiliated Hospital of Anhui University of Chinese Medicine from June 2022 to December 2023. The patients were randomly divided into a control, a Chinese medicine, and a combined group using a random number table method, with 32 cases in each group (two cases per group were excluded). The control group was treated with rabeprazole combined with folic acid tablets (both thrice daily), the Chinese medicine group was treated with PPP (8 g, thrice daily), and the combined group was treated with moxa stick moxibustion (once daily) on the basis of the Chinese medicine group for 12 consecutive weeks. Gastric mucosa atrophy in the three groups was observed before and after treatment. The gastric mucosal pathological score was evaluated. The Patient Reported Outcome (PRO) scale was used to evaluate the patients′ physical and mental health status and quality of life.An enzyme-linked immunosorbent assay was used to detect serum tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-4, IL-10, IL-37, and transforming growth factor (TGF)-β levels in each group. Real-time fluorescence PCR was used to detect the relative expression levels of signal transducer and activator of transcription 3 (STAT3) and mammalian target of rapamycin (mTOR) mRNA in each group. Western blotting was used to detect the relative expression levels of proteins related to the STAT3/mTOR signaling pathway, and the adverse drug reactions and events were recorded and compared.
Results:
There was no statistical difference in age, gender, disease duration, family history of gastrointestinal tumors, alcohol consumption history, and body mass index among the three groups of patients.The total therapeutic efficacy rates of the control, Chinese medicine, and combined groups in treating gastric mucosal atrophy were 66.67% (20/30), 86.67% (26/30), and 90.00% (27/30), respectively (P<0.05). Compared to before treatment, the pathological and PRO scale scores of gastric mucosa in each group decreased after treatment, and TNF-α, IL-1β, IL-37, and TGF-β levels decreased. The relative STAT3 and mTOR mRNA expression levels, as well as the relative STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels decreased (P<0.05), whereas the IL-4 and IL-10 levels increased (P<0.05). After treatment, compared to the control group, the pathological score of gastric mucosa, PRO scale score, TNF-α, IL-1β, IL-37, TGF-β content, relative STAT3 and mTOR mRNA expression levels, and relative STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels in the Chinese medicine and combined groups after treatment were reduced (P<0.05), whereas the IL-4 and IL-10 levels increased (P<0.05). After treatment, compared to the Chinese medicine group, the combined group showed a decrease in relative STAT3, mTOR mRNA expression levels, and STAT3, p-STAT3, mTOR, and p-mTOR protein expression levels (P<0.05).
Conclusion
The combination of PPP and moxibustion may regulate the inflammatory mechanism of the body by inhibiting the abnormal activation of the STAT3/mTOR signaling pathway, upregulating related anti-inflammatory factor levels, downregulating pro-inflammatory factor expression, and increasing related repair factor expression, thereby promoting the recovery of atrophic gastric mucosa, reducing discomfort symptoms, and improving the physical and mental state of CAG patients with spleen and stomach weakness syndrome.
10.Mechanism of Colquhounia Root Tablets against diabetic kidney disease via RAGE-ROS-PI3K-AKT-NF-κB-NLRP3 signaling axis.

This study aimed to explore the therapeutic mechanisms of Colquhounia Root Tablets(CRT) in treating diabetic kidney disease(DKD) by integrating biomolecular network mining with animal model verification. By analyzing clinical transcriptomics data, an interaction network was constructed between candidate targets of CRT and DKD-related genes. Based on the topological eigenvalues of network nodes, 101 core network targets of CRT against DKD were identified. These targets were found to be closely related to multiple pathways associated with type 2 diabetes, immune response, and metabolic reprogramming. Given that immune-inflammatory imbalance driven by metabolic reprogramming is one of the key pathogenic mechanisms of DKD, and that many core network targets of CRT are involved in this pathological process, receptor for advanced glycation end products(RAGE)-reactive oxygen species(ROS)-phosphatidylinositol 3-kinase(PI3K)-protein kinase B(AKT)-nuclear factor-κB(NF-κB)-NOD-like receptor family pyrin domain containing 3(NLRP3) signaling axis was selected as a candidate target for in-depth research. Further, a rat model of DKD induced by a high-sugar, high-fat diet and streptozotocin was established to evaluate the pharmacological effects of CRT and verify the expression of related targets. The experimental results showed that CRT could effectively correct metabolic disturbances in DKD, restore immune-inflammatory balance, and improve renal function and its pathological changes by inhibiting the activation of the RAGE-ROS-PI3K-AKT-NF-κB-NLRP3 signaling axis. In conclusion, this study reveals that CRT alleviates the progression of DKD through dual regulation of metabolic reprogramming and immune-inflammatory responses, providing strong experimental evidence for its clinical application in DKD.
