1.Post-Orgasmic Illness Syndrome: Two Cases Treated with Autologous Seminal Plasma Subcutaneous Cluster Immunotherapy

Post-orgasmic illness syndrome (POIS) is a rare condition characterized by the rapid onset of extreme fatigue, flu-like symptoms, difficulty concentrating, depression, nasal congestion, rhinorrhea, itchy eyes, and other physical and psychological discomforts following ejaculation. This report presents the outcomes of two patients with POIS who underwent a two-year course of autologous seminal plasma subcutaneous cluster immunotherapy. Treatment efficacy was assessed using methods such as the symptom Visual Analogue Scale (VAS), the Union Physio-Psycho-Social Assessment Questionnaire (UPPSAQ)-70, and the Short Form 36 Health Survey (SF-36). The results suggest that autologous seminal plasma subcutaneous cluster immunother-apy may be a safe and effective therapeutic approach for POIS.
2.Clinical characteristics and influencing factors of vestibular migraine patients with sleep disorders.

Objective:To investigate the sleep characteristics and clinical features of patients with vestibular migraine(VM), and to explore the influencing factors of sleep disorder in VM patients. Methods:A cross-sectional study method was adopted to collect VM patients from Otolaryngology department and neurology department of our hospital from June 2022 to June 2024(divided into sleep disorder group and non-sleep disorder group according to whether there is sleep disorder) as the experimental group, and recruit non-VM volunteers with clinical characteristics matching with the experimental group during the same period as the control group. The clinical data of the subjects were collected, and the sleep quality of the subjects was assessed using the Pittsburgh Sleep Quality Index(PSQI). The influencing factors of sleep disorders in VM patients were analyzed by multivariate Logistic regression, and the correlation between sleep disorders and clinical features such as headache, vertigo and hearing in VM patients was analyzed by Spearman correlation coefficient. Results:A total of 530 individuals with VM were analyzed, including 332 with sleep disturbances(62.64%), 198 without sleep issues(37.36%), and 50 in the control group. The overall PSQI score and all its components were significantly higher in the VM group compared with the control group(P<0.05). A positive correlation was observed between PSQI and VAS, DHI-T, DHI-E, DHI-F and DHI-P(r=0.797, P<0.05; r=0.834, P<0.05; r=0.794, P<0.05; r=0.771, P<0.05; r=0.877, P<0.05), PSQI had no correlation with pure tone hearing(r=0.324, P=0.167). Multivariate logistic regression analysis showed that female, age ≥60 years, living alone, duration of disease ≥3 months, motion sickness history, and HADS-A were independent influencing factors for comorbidification of sleep disorder in VM patients(P<0.05). Conclusion:The prevalence of sleep disorders in patients with vestibular migraine(VM) was significantly higher compared to the control group. Moreover, the severity of sleep disorders was positively correlated with the intensity of headache and vertigo in VM patients. It is recommended that female VM patients aged 60 years or older, living alone, with a disease duration of three months or longer, a history of motion sickness, and anxiety symptoms undergo sleep assessments to determine the presence of sleep disorders. This approach provides a theoretical foundation for precise treatment and prevention strategies for VM.

Allergen-specific immunotherapy(AIT) remains the only therapeutic approach with the potential to modify the natural course of allergic rhinitis(AR). Nevertheless, considerable inter-individual variability exists in patients'responses to AIT. To facilitate more reliable assessment of treatment efficacy, the China Rhinopathy Research Cooperation Group(CRRCG) convened young and middle-aged nasal experts in China to formulate the present consensus. The recommended subjective outcome measures for AIT comprise symptom scores, medication scores, combined symptom and medication scores, quality-of-life assessments, evaluation of disease control, and assessment of comorbidities. Objective indicators may supplement these measures. Currently available objective approaches include skin prick testing, nasal provocation testing, and allergen exposure chambers. However, these methods remain constrained by practical limitations and are not yet appropriate for routine implementation in clinical efficacy evaluation. In addition, several biomarkers, including sIgE and the sIgE/tIgE ratio, sIgG4, serum IgE-blocking activity, IgA, cytokines and chemokines, as well as immune cell surface molecules and their functional activity, have been shown to have associations with AIT outcomes. While these biomarkers may complement subjective assessments, they are subject to significant limitations. Consequently, large-scale multicenter trials and real-world evidence are required to strengthen the evidence base. The present consensus underscores the necessity of integrating patients'subjective experiences with objective testing throughout the treatment process, thereby providing a more comprehensive and accurate framework for efficacy evaluation. Looking forward, future investigations should prioritize the incorporation of multi-omics data and artificial intelligence methodologies, which hold promise for overcoming current limitations in assessment strategies and for advancing both the standardization and personalization of AIT.

4.Histaminergic Innervation of the Ventral Anterior Thalamic Nucleus Alleviates Motor Deficits in a 6-OHDA-Induced Rat Model of Parkinson's Disease.

The ventral anterior (VA) nucleus of the thalamus is a major target of the basal ganglia and is closely associated with the pathogenesis of Parkinson's disease (PD). Notably, the VA receives direct innervation from the hypothalamic histaminergic system. However, its role in PD remains unknown. Here, we assessed the contribution of histamine to VA neuronal activity and PD motor deficits. Functional magnetic resonance imaging showed reduced VA activity in PD patients. Optogenetic activation of VA neurons or histaminergic afferents significantly alleviated motor deficits in 6-OHDA-induced PD rats. Furthermore, histamine excited VA neurons via H1 and H2 receptors and their coupled hyperpolarization-activated cyclic nucleotide-gated channels, inward-rectifier K+ channels, or Ca2+-activated K+ channels. These results demonstrate that histaminergic afferents actively compensate for Parkinsonian motor deficits by biasing VA activity. These findings suggest that targeting VA histamine receptors and downstream ion channels may be a potential therapeutic strategy for PD motor dysfunction.

5.A survey of common eye behaviors and influencing factors of myopia in pri-mary and secondary school students in Tianjin

Objective To explore the eye behaviors and myopia-related risk factors among primary and secondary school students in Tianjin,in order to carry out targeted eye health education and guidance.Methods A cross-sectional study was conducted.In December 2021,an eye health questionnaire was distributed to primary and secondary school students in Tianjin to investigate their eye habits;combined with the campus vision screening data in the second half of 2021,a multivariate logistic regression analysis model was constructed to explore the risk factors related to myopia.Re-sults A total of 821 459 questionnaires were collected,including 701 104 valid questionnaires.The usage time of electron-ic learning products and electronic entertainment products for primary school students was mostly less than 1 hour(ac-counting for 57.74%and 40.98%,respectively).The population of junior high school students who spend more than 2 hours using electronic learning products and electronic entertainment products(accounting for 26.61%and 35.05%,re-spectively)significantly increased compared to primary school students(accounting for 11.21%and 20.93%,respective-ly).Most senior high school students spend more than 2 hours daily using electronic devices for learning and entertainment purposes,as well as reading paper materials.51.59%of primary school students do not meet the standard for sleep dura-tion,while the rate of reaching the standard sleep duration of junior high and senior high school students was 60.09%and 64.99%,respectively.Most(55.74%)primary and secondary school students have an average outdoor activity duration of 1-2 hours daily.The results of multivariate logistic regression analysis showed that the risk of myopia in primary school students was correlated with the time spent using electronic products(including learning and entertainment),the time spent reading paper materials at a close range,outdoor activities,time to fall asleep,and sleep duration(all P<0.05);the risk of myopia among junior high school students was related to the time spent using electronic entertainment products,reading paper materials at a close range,outdoor activities,and sleep duration(all P<0.05);the risk of myopia among senior high school students was only related to the time spent reading paper materials at a close range and the duration of outdoor activities(both P<0.05).Conclusion The proportion of primary school students,junior high school students and senior high school students in Tianjin who use electronic products for more than 2 hours is gradually increasing,and the rate of reaching the standard sleep duration is gradually increasing.The risk of myopia among these students is related to the time spent reading paper materials at a close range and outdoor activities.Science popularization should be carried out and management of eye behaviors should be strengthened in the whole society to reduce myopia rates in children and adolescents in China.
6.Clinical observation of Zishen Ningxin capsule combined with telmisartan in the treatment of perimenopausal hypertension

Objective To observe the efficacy of Zishen Ningxin capsule combined with telmisartan in the treatment of perimenopausal hypertension,and its influence on the patients'blood pressure,biochemical indexes and TCM symptom scores.Methods 300 patients with perimenopausal hypertension were randomly divided into control group and observation group with 150 patients in each group.The control group was given telmisartan treatment,and the observation group was given Zishen Ningxin capsule on the basis of the control group.The treatment course of both groups was 8 weeks.The dynamic blood pressure and coefficient of variation of blood pressure at 24 h before and after treatment were observed and compared between two groups(24 h systolic blood pressure and coefficient of variation,24 h diastolic blood pressure and its coefficient of variation),sex hormone levels(serum estradiol,follicle stimulating hormone,testosterone,progesterone),blood lipid levels(triacylglycerol,total cholesterol,low density lipoprotein cholesterol,high density lipoprotein cholesterol),renin and hypersensitive C-reactive protein levels,as well as the changes of TCM symptom score were evaluated for the efficacy of the two groups.Results After treatment,the 24 h systolic blood pressure and its coefficient of variation,24 h diastolic blood pressure and its coefficient of variation,the levels of sex hormones,blood lipids,renin and hypersensitive C-reactive protein,as well as TCM symptom scores in two groups were significantly improved(P<0.05),and all indexes in the observation group were better than those in the control group(P<0.05).The total effective rate of observation group was significantly higher than that of control group(P<0.05).There was no significant difference of the incidence of adverse reactions in the two groups(P>0.05).Conclusion Zishen Ningxin capsule combined with telmisartan has significant clinical efficacy in the treatment of perimenopausal hypertension,which can effectively reduce blood pressure,relieve symptoms,improve the levels of sex hormones,blood lipids,renin and hypersensitive C-reactive protein.
7.Correlation of CD200-CD200R axis and diseases and its research progress

CD200 and its receptor CD200R constitute an endogenous inhibitory signal. The binding of CD200 and CD200R can regulate the immune response to pathogenic stimuli, which has received much attention in recent years. It has been found that CD200-CD200R is involved in the regulation of many kinds of pathological inflammation, including autoimmune diseases, cardiac cerebrovascular disease, infection and tumor. This paper reviews the protein structure, distribution, expression, biological function of CD200-CD200R and the correlation with diseases, and analyses the current status and development ideas of CD200-CD200R as drug targets. It aims to provide theoretical support for new drug research and development based on this target.
8.Protective effect of quercetin mediated ferroptosis pathway on hypertonic stimulation induced dry eye cell model

Objective To investigate the effect of quercetin on HCE-2 injury of human corneal epithelial cells induced by high osmotic pressure and its mechanism.Methods HCE-2 cells were randomly divided into control group(normal osmotic pressure),model group(high osmotic pressure),experimental-L group(high osmotic pressure+31.25 pg·mL-1 quercetin),experimental-M group(high osmotic pressure+62.50 μg·mL-1 quercetin),experimental-H group(high osmotic pressure+125.00 μg·mL-1 quercetin),erastin group(high osmotic pressure+125.00 μg·mL-1 quercetin+30.00 μmol·L-1 iron death inducer erastin).Cell survival rate was detected by cell counting kit 8;reactive oxygen species(ROS)levels was detected by C11-BODIPY 581/591 probe staining;glutathione(GSH)and malondialdehyde(MDA)levels were determined by kit method;the expression levels of glutathione peroxidase 4(GPX4),dihydrolactate dehydrogenase(DHODH)and ferroptosis suppressor protein 1(FSP1)were detected by real-time quantitative polymerase chain reaction and Western blot.Results The cell survival rates of control group,model group,experimental-H group and erastin group were(100.00±3.97)%,(50.05±5.83)%,(86.35±7.35)%and(58.32±4.66)%,respectively;ROS levels were 1.00±0.09,2.45±0.16,1.19±0.05 and 2.09±0.30,respectively;GPX4 protein levels were 1.09±0.11,0.34±0.03,0.91±0.12 and 0.30±0.04,respectively;FSP1 protein levels were 0.92±0.06,0.25±0.03,0.89±0.07 and 0.39±0.07,respectively;DHODH protein levels were 0.89±0.11,0.31±0.04,0.86±0.11,0.41±0.04,respectively.Compared with model group,the above indexes in control group were statistically significant(all P<0.05);the differences between experimental-H group and model group were statistically significant(all P<0.05);the above indexes in erastin group were significantly different from those in experimental-H group(all P<0.05).Conclusion Quercetin can ameliorate HCE-2 cell damage induced by high osmotic pressure by inhibiting iron death pathway.
9.Chain mediation of psychological resilience and coping styles between social support and psychological distress in elderly stroke patients

Objective:To investigate the chain mediating role of psychological resilience and coping styles between social support and psychological distress in elderly stroke patients.Methods:Using convenience sampling, 245 elderly stroke patients with their first episode, admitted to the Neurology Department of the Affiliated Hospital of Hebei University from June to July 2023, were recruited as study subjects. A questionnaire survey was conducted using a General Information Questionnaire, Distress Thermometer for Stroke Patients, Perceived Social Support Scale, Connor-Davidson Resilience Scale Short Form, and Medical Coping Modes Questionnaire. Pearson correlation analysis was used to examine the relationships between psychological distress, social support, psychological resilience, and coping styles in elderly stroke patients. Harman's single-factor test was employed to detect common method bias among variables. The PROCESS macro in SPSS software was utilized to test the chain mediation effects.Results:A total of 245 questionnaires were distributed, with 230 valid responses collected, yielding a response rate of 93.9%. Among the 230 elderly stroke patients, the incidence of psychological distress was 23.9% (55/230). Significant correlations were observed among psychological distress, social support, psychological resilience, and coping styles ( P<0.05). Social support influenced psychological distress through the mediation of psychological resilience and confrontational coping, with a total indirect effect of -0.098. The same relationship existed for social support through psychological resilience and avoidant coping, with a total indirect effect of -0.058. Additionally, social support influenced psychological distress through psychological resilience and submissive coping, with a total indirect effect of -0.113. Avoidant coping had a suppressing effect on the influence of social support on psychological distress. Conclusions:Elderly stroke patients experienced moderate to low levels of psychological distress. Psychological resilience and coping styles played a chain-mediating role between social support and psychological distress. Special attention should be given to elderly stroke patients with low levels of social support.
10.Research on the impact of supply side policy coordination of medical insurance on cost control under DIP payment method

Objective:This study aims to explore the synergistic effects of DIP and other medical insurance supply-side policies.Method:City A that has piloted DIP reform was set as the treatment group,and City B without reform was set as the control group.A total of 1 120 public medical institution samples from 2019 to 2022 were collected.The total medical expenses during hospitalization and some structural expenses were analyzed using DID method.Result:DIP had a significant inhibitory effect on the medical expenses,and the expenses of checkups and examinations during hospitalization in city A,but had no impact on the drug and the material expenses during hospitalization.Conclusion:DIP played a significant cost control role and effectively controlled the total medical expenses during hospitalization.The synergistic effects of price adjustment of medical services policy and national centralized drug/material procurement policy on cost control were insufficient.DIP synergized with other supply-side policies to promote rational medical cost structure.It is suggested that medical insurance departments should focus on the synergistic effects of medical insurance supply-side policies to jointly improve the efficiency of medical insurance fund utilization.
