Objective To evaluate the value of combined detection of serum fibrinogen/albumin ratio(FAR)and milk fat globule epidermal growth factor 8(MFG-E8)in predicting the fetal protection outcome of threatened abortion patients.Methods From May 2022 to May 2023,a total of 89 patients with threatened abortion who received fetal protection treatment in Jianli People's Hospital were regarded as the observation group.They were grouped into a miscarriage group(19 cases)and a successful group(70 cases)based on the fetal protection outcomes.89 normal pregnant women who underwent prenatal examinations were regarded as the control group.Enzyme-linked immunosorbent assay was applied to detect the levels of serum fibrinogen,albumin and MFG-E8.Multivariate Logistic regression was applied to analyze the influencing factors on the fe-tal protection outcome of patients with threatened abortion.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of serum FAR and MFG-E8 level on the fetal protection outcome of patients with threatened abortion.Results Compared with the control group,serum FAR was higher in the observation group(P<0.05)and serum MFG-E8 level was lower(P<0.05).The proportion of thyroid dys-function,serum C-reactive protein(CRP),and FAR in the miscarriage group were higher than those in the successful group(P<0.05),while serum MFG-E8 level was lower than that in the successful group(P<0.05).Serum FAR,CRP and thyroid dysfunction were risk factors for fetal protection outcome of patients with threatened abortion,while serum MFG-E8 was a protective factor for fetal protection outcome of patients with threatened abortion(P<0.05).The area under the curve(AUC)of serum FAR,MFG-E8,and their combination in predicting the fetal protection outcome of patients with threatened abortion was 0.787,0.853,and 0.954,respectively,and the efficacy of combination of the two in predicting the fetal protection outcome of patients with threatened abortion was better than that of serum FAR and MFG-E8 alone(Zcombination-FAR=2.805,Zcombination-MFG-E8=2.076,P=0.005,0.038),with sensitivity and specificity of 94.74%and 88.57%,re-spectively.Conclusion Serum FAR increases and MFG-E8 level decreases of patients with threatened abor-tion,and the combination of the two has higher predictive value for the fetal protection outcome of patients with threatened abortion.

Objective:To investigate the sleep characteristics and clinical features of patients with vestibular migraine（VM）, and to explore the influencing factors of sleep disorder in VM patients. Methods:A cross-sectional study method was adopted to collect VM patients from Otolaryngology department and neurology department of our hospital from June 2022 to June 2024（divided into sleep disorder group and non-sleep disorder group according to whether there is sleep disorder） as the experimental group, and recruit non-VM volunteers with clinical characteristics matching with the experimental group during the same period as the control group. The clinical data of the subjects were collected, and the sleep quality of the subjects was assessed using the Pittsburgh Sleep Quality Index（PSQI）. The influencing factors of sleep disorders in VM patients were analyzed by multivariate Logistic regression, and the correlation between sleep disorders and clinical features such as headache, vertigo and hearing in VM patients was analyzed by Spearman correlation coefficient. Results:A total of 530 individuals with VM were analyzed, including 332 with sleep disturbances（62.64%）, 198 without sleep issues（37.36%）, and 50 in the control group. The overall PSQI score and all its components were significantly higher in the VM group compared with the control group（P<0.05）. A positive correlation was observed between PSQI and VAS, DHI-T, DHI-E, DHI-F and DHI-P（r=0.797, P<0.05; r=0.834, P<0.05; r=0.794, P<0.05; r=0.771, P<0.05; r=0.877, P<0.05）, PSQI had no correlation with pure tone hearing（r=0.324, P=0.167）. Multivariate logistic regression analysis showed that female, age ≥60 years, living alone, duration of disease ≥3 months, motion sickness history, and HADS-A were independent influencing factors for comorbidification of sleep disorder in VM patients（P<0.05）. Conclusion:The prevalence of sleep disorders in patients with vestibular migraine（VM） was significantly higher compared to the control group. Moreover, the severity of sleep disorders was positively correlated with the intensity of headache and vertigo in VM patients. It is recommended that female VM patients aged 60 years or older, living alone, with a disease duration of three months or longer, a history of motion sickness, and anxiety symptoms undergo sleep assessments to determine the presence of sleep disorders. This approach provides a theoretical foundation for precise treatment and prevention strategies for VM.

Allergen-specific immunotherapy（AIT） remains the only therapeutic approach with the potential to modify the natural course of allergic rhinitis（AR）. Nevertheless, considerable inter-individual variability exists in patients'responses to AIT. To facilitate more reliable assessment of treatment efficacy, the China Rhinopathy Research Cooperation Group（CRRCG） convened young and middle-aged nasal experts in China to formulate the present consensus. The recommended subjective outcome measures for AIT comprise symptom scores, medication scores, combined symptom and medication scores, quality-of-life assessments, evaluation of disease control, and assessment of comorbidities. Objective indicators may supplement these measures. Currently available objective approaches include skin prick testing, nasal provocation testing, and allergen exposure chambers. However, these methods remain constrained by practical limitations and are not yet appropriate for routine implementation in clinical efficacy evaluation. In addition, several biomarkers, including sIgE and the sIgE/tIgE ratio, sIgG4, serum IgE-blocking activity, IgA, cytokines and chemokines, as well as immune cell surface molecules and their functional activity, have been shown to have associations with AIT outcomes. While these biomarkers may complement subjective assessments, they are subject to significant limitations. Consequently, large-scale multicenter trials and real-world evidence are required to strengthen the evidence base. The present consensus underscores the necessity of integrating patients'subjective experiences with objective testing throughout the treatment process, thereby providing a more comprehensive and accurate framework for efficacy evaluation. Looking forward, future investigations should prioritize the incorporation of multi-omics data and artificial intelligence methodologies, which hold promise for overcoming current limitations in assessment strategies and for advancing both the standardization and personalization of AIT.

Post-orgasmic illness syndrome (POIS) is a rare condition characterized by the rapid onset of extreme fatigue, flu-like symptoms, difficulty concentrating, depression, nasal congestion, rhinorrhea, itchy eyes, and other physical and psychological discomforts following ejaculation. This report presents the outcomes of two patients with POIS who underwent a two-year course of autologous seminal plasma subcutaneous cluster immunotherapy. Treatment efficacy was assessed using methods such as the symptom Visual Analogue Scale (VAS), the Union Physio-Psycho-Social Assessment Questionnaire (UPPSAQ)-70, and the Short Form 36 Health Survey (SF-36). The results suggest that autologous seminal plasma subcutaneous cluster immunother-apy may be a safe and effective therapeutic approach for POIS.

The ventral anterior (VA) nucleus of the thalamus is a major target of the basal ganglia and is closely associated with the pathogenesis of Parkinson's disease (PD). Notably, the VA receives direct innervation from the hypothalamic histaminergic system. However, its role in PD remains unknown. Here, we assessed the contribution of histamine to VA neuronal activity and PD motor deficits. Functional magnetic resonance imaging showed reduced VA activity in PD patients. Optogenetic activation of VA neurons or histaminergic afferents significantly alleviated motor deficits in 6-OHDA-induced PD rats. Furthermore, histamine excited VA neurons via H1 and H2 receptors and their coupled hyperpolarization-activated cyclic nucleotide-gated channels, inward-rectifier K⁺ channels, or Ca²⁺-activated K⁺ channels. These results demonstrate that histaminergic afferents actively compensate for Parkinsonian motor deficits by biasing VA activity. These findings suggest that targeting VA histamine receptors and downstream ion channels may be a potential therapeutic strategy for PD motor dysfunction.

Multi-rehabilitation treatment and early nutritional support are closely related to the recovery of critical ill patients.At present,a number of nutritional and rehabilitation interventions have been applied to critically ill patients to aid in their treatment and improve their nutritional status and motor dysfunction..In recent years,certain progress has been made in related research.This paper aims to summarize the latest literature on the application of multidisciplinary rehabilitation therapy in patients with CI under early nutritional support,provide comprehensive,systematic and practically valuable reference for clinical medical staff when formulating comprehensive treatment plans for CI patients under early nutritional support,and improve the treatment effect and quality of life of CI patients.

Objective To develop and validate risk prediction models utilizing five machine learning algorithms for assessing postoperative pulmonary infection(PPI)risk in lung cancer patients undergoing grade Ⅳ thoracoscopic surgery.Methods A retrospective cohort study included 2,380 lung cancer patients who underwent grade Ⅳ thoracoscopic surgery at a tertiary hospital in Shanghai(January 2022 to June 2024).Patients were stratified into training(n=1,665)and validation(n=715)cohorts.Five machine learning algorithms—Logistic regression(LR),artificial neural network(ANN),support vector machine(S VM),random forest(RF),and extreme gradient boosting(XGB)—were employed to construct predictive models.A nomogram was developed for clinical utility.Results Among 2,380 patients,226(9.5％)developed PPI.The Least Absolute Shrinkage and Se-lection Operator(LASSO)regression identified eight predictive variables:daily cigarette consumption,diabetes history,preoperative diffusing capacity,maximal tumor diameter,24-hour postoperative chest drainage volume,perioperative oral nutritional supplementation(ONS),postoperative urinary cathe-terization,and intraoperative pleural adhesion severity.All models demonstrated robust discrimina-tion,with area under the curve(AUC)values ranging from 0.862 to 0.947.The XGB model a-chieved superior performance(AUC=0.947,95％CI,0.937 to 0.962),followed closely by the LR model(AUC=0.926,95％CI,0.918 to 0.933).Conclusion Machine learning-based algo-rithms models effectively stratify PPI risk in lung cancer patients following grade Ⅳ thoracoscopic surgery.The derived nomogram provides a practical tool for perioperative risk management by healthcare providers.

Pituitary thyroid-stimulating hormone(TSH)adenomas is a rare pituitary disorder,accounting for less than 2%of pituitary adenomas.The clinical manifestations primarily include mild to moderate symptoms of hyperthyroidism,corresponding symptoms caused by other anterior pituitary hormone secretion disorders,and symptoms resulting from the mass effect of pituitary tumors.Pituitary TSH adenomas need to be differentiated from primary hyperthyroidism(Graves'disease)and resistance to thyroid hormone(RTH),as misdiagnosis can lead to tumor growth and aggravation of the condition.Currently,with the help of sensitive laboratory tests,imaging examinations,and targeted functional tests,pituitary TSH adenomas can be diagnosed relatively accurately.The preferred treatment is surgical resection.In cases where surgery is not feasible or unsuccessful,radiotherapy or medical therapy can be considered.Long-acting somatostatin analogs can effectively reduce tumor volume and decrease TSH secretion,thereby normalizing free 3,5,3',5'-tetraiodothyronine(FT4)and free 3,5,3'-triiodothyronine(FT3).Early identification and effective treatment are significant for patients with pituitary TSH adenomas.This review summarizes the epidemiology,pathological characteristics,screening objects,clinical manifestations,auxiliary examinations,diagnosis and treatment,follow-up and evaluation of pituitary TSH adenoma,aiming to provide guidance for the clinical diagnosis and treatment of this condition.

Disorders of sex development(DSD)are a group of conditions with strong clinical phenotype heterogeneity,and the incidence of DSD in the population is 1/5 000 to 1/4 500.According to the international classification standards,DSD is divided into sex chromosome DSD,46,XY DSD and 46,XX DSD according to chromosomal karyotype.Only 35%?45%of patients with 46,XY DSD and 10%of those with 46,XX DSD have definite etiologies.So far,the phenotype and pathogenic mechanism of DSD are still the focus of research.DEAH-box helicase 37(DHX37)is a novel candidate pathogenic gene for DSD,first identified in 2019.In recent years,researchers have confirmed that DHX37 gene variants are closely related to 46,XY DSD.DHX37 gene is one of the most conserved genes across genomes,making its genetic diagnosis difficult,and the molecular mechanism in causing 46,XY DSD is still unclear.In recent years,many researchers have screened the variation sites of the DHX37 gene by whole exome sequencing(WES)technology and explored its pathogenic mechanisms,which has expanded the genetic map of DSD.This article reviews the relationship between the structure and function of the DHX37 gene,the pathogenic mechanisms of clinical phenotypes and DSD caused by DHX37 gene variants.It also discusses the pathogenic mechanisms of human diseases in the context of ribosomal-related diseases caused by DHX37 gene variants.

Objective:This study aims to explore the synergistic effects of DIP and other medical insurance supply-side policies.Method:City A that has piloted DIP reform was set as the treatment group,and City B without reform was set as the control group.A total of 1 120 public medical institution samples from 2019 to 2022 were collected.The total medical expenses during hospitalization and some structural expenses were analyzed using DID method.Result:DIP had a significant inhibitory effect on the medical expenses,and the expenses of checkups and examinations during hospitalization in city A,but had no impact on the drug and the material expenses during hospitalization.Conclusion:DIP played a significant cost control role and effectively controlled the total medical expenses during hospitalization.The synergistic effects of price adjustment of medical services policy and national centralized drug/material procurement policy on cost control were insufficient.DIP synergized with other supply-side policies to promote rational medical cost structure.It is suggested that medical insurance departments should focus on the synergistic effects of medical insurance supply-side policies to jointly improve the efficiency of medical insurance fund utilization.
