1.Short-term clinical outcomes of different courses of antenatal corticosteroids for preterm twins

Objective:To study the short-term clinical outcomes of different courses of antenatal corticosteroids (ACS) for preterm twins.Methods:From January 2017 to December 2021, preterm twins with gestational age (GA) 24-34 weeks admitted to the neonatal ward of our hospital and received ACS were retrospectively studied. The infants were assigned into single-course group, partial-course group and multiple-course group according to ACS courses. The short-term clinical outcomes were compared among the groups. SPSS software version 25.0 was used for statistical analysis.Results:A total of 286 infants were enrolled in this study, including 128 in single-course group, 89 in partial-course group and 69 in multiple-course group. Compared with single-course group, the risks of neonatal respiratory distress syndrome (RDS) in both partial-course group ( OR=2.332, 95% CI 1.028-5.293, P=0.043) and multiple-course group ( OR=3.872, 95% CI 1.104-13.584, P=0.034) were higher. The birth length in multiple-course group ( β=-0.016, 95% CI -0.029 - -0.002, P=0.024) was lower than single-course group. Conclusions:The risks of neonatal RDS in preterm twins are higher in partial-course and multiple-course of ACS. A full course of ACS should be used to prevent neonatal RDS until further evidence of effectiveness is available.
2.Influential factors of adverse pregnancy outcome in women with subchorionic hematoma

Objective:To investigate the influential factors of adverse pregnancy outcomes in women with subchorionic hematoma.Methods:A total of 101 women with subchorionic hematoma who received treatment in the People's Hospital of Xinjiang Uygur Autonomous Region from January 2017 to June 2020 were included in this study. They were divided into a control group and an adverse pregnancy outcome group according to whether there was an adverse pregnancy outcome. The epidemiological characteristics, hematoma characteristics observed on ultrasound images, and pregnancy outcomes were compared between the two groups.Results:There was no significant difference in the number of women who used assisted reproductive technology between the control and adverse pregnancy outcome groups [6 (8.0%) vs. 8 (30.7%), χ2 = 8.38, P = 0.004]. There was a significant difference in hematoma volume between adverse pregnancy outcome and control groups [(4.12 ± 0.61) mL vs. (6.36 ± 0.87) mL, t = 6.73, P = 0.009]. There was a significant difference in the number of patients who had obstetric complications between control and adverse pregnancy outcome groups [11 (14.7%) vs. 16 (61.5%), χ2 = 21.66, P = 0.001]. There was a significant difference in the number of patients who had hematomas located at the edge of the placenta between the control and adverse pregnancy outcome groups [15 (20.0%) vs. 12 (46.2%), χ2 = -4.81, P = 0.001]. Conclusion:Women who use assisted reproductive technology for pregnancy, have obstetric complications, or have a subchorionic hematoma with hematoma at the edge of the placenta are more likely to experience a miscarriage. Therefore, women of childbearing age should actively treat the primary disease and be alert to the occurrence of placental abruption.
3.Effects of sufentanil and fentanyl on glucose and insulin in patients of gallbladder surgery using metabonomics technique

Objective To compare the effects of sufentanil and fentanyl anesthesia on blood glucose and insulin levels in patients undergoing laparoscopic cholecystectomy,and explore the mechanism using metabonomics technique.Methods Total 60 patients,37 males and 23 females,ASA physical status Ⅰ or Ⅱ undergoing laparoscopic cholecystectomy were randomly divided into sufentanil and fentanyl groups,and 30 cases in each group.Venous blood was collected before anesthesia and 5 min,40 min and 2 h after anesthesia,which was used to detect blood glucose,insulin levels and metabonomics analysis.Results Blood glucose were significantly lower in sufentanil group than in fentanyl group at 5 min after intubation,40 min in surgery,and postoperative 2 h (P<0.05).After 2 h,insulin in sufentanil group was significantly lower than that in fentanyl group (P<0.05).Metabolic analysis showed that in the positive ion mode,using orthogonal partial least squares discriminant analysis,the patients in the two groups were classified according to the serum metabolites at postoperative 2 h,which showed that the serum metabolites in patients of different groups were significantly different,and 5 biomarkers closely related to blood glucose and insulin were found.Conclusion Sufentanil showed slighter effects on blood glucose and insulin in patients,and more suitable for cholecystectomy patients.The mechanism may be related with the glucose and insulin metabolism.
4.Thin-thicknessversus blade-thickness micro-skin pulping covered with heterogeneous skin for repair of burn wounds

BACKGROUND:Autologous micro-skin graft covered with alogeneic skin has become a major means for repair of extremely severe burn wounds. Due to the limitation of the source of alogeneic skin, heterogeneous skin (pigskin) or artificial skin serves as a vector to cover the wound on the clinical use. OBJECTIVE: To observe the curative effect of thin-thickness micro-skin pulping covered with heterogeneous skin for repair of burn wounds in comparison with blade-thickness micro-skin pulping covered with heterogeneous skin. METHODS:A self-control study was performed in 36 patients with severe burns of bilateral limbs. There were 42 pairs of wounds, 32 of which were at joint sites. One or two pairs of III-degree burns treated with escharectomy were selected from each patient for self-control comparison. Covered with heterogeneous skin, thin-thickness micro-skin pulping and blade-thickness micro-skin pulping were used in the trial and control groups, respectively. The expansion ratio was controled at 1:8-1:12 in al patients. Wound healing time, degree of scar hyperplasia within 6 months to 2 years after transplantation, and ratio of reconstruction surgery at joint sites were observed and compared. The removed scars by surgery were analyzed based on the total score of the Vancouver Scar Scale. RESULTS AND CONCLUSION: The average wound healing time was (44.7±1.24) days in the trial group and (49.6±1.41) days in the control group, and there was a significant difference between the two groups (P < 0.05). The trial group exhibited less scar hyperplasia than the control group at 6 months to 2 years after transplantation. Compared with the control group, the ratio of severe scar hyperplasia was significantly less in the trial group (P < 0.05), while the ratio of mild scar hyperplasia was obviously greater in the trial group (P < 0.01). However, there was no difference in the ratio of moderate scar hyperplasia between the two groups (P > 0.05). The reconstructive surgery ratio of the trial group was 38%, significantly lower than that of the control group (59.38%;P < 0.01). The total score on the Vancouver Scar Scale was less in the trial group than the control group (P < 0.05). These findings suggest that the thin-thickness micro-skin covered with heterogeneous skin is likely to be a better treatment to repair large ful-thickness skin burn by increasing the thickness of micro-skin, and it can obtain better wound healing quality.
5.MicroRNA target predicition based on SVM and the optimized feature set.

MicroRNA (miRNA) is a family of endogenous single-stranded RNA about 22 nucleotides in length. Through targeting 3' UTR of message RNA (mRNA), they play important roles in post-transcriptional regulatory functions. For further research of miRNA function, the identification of more miRNA positive targets is needed urgently. Aiming at the high-dimensional small sample data sets in miRNA target prediction, an algorithm of eliminating redundant features is proposed based on v-SVM in this paper, and classification and features selection are also fused. The algorithm of eliminating redundant features optimizes the combination of features, and then constructs the best features combination which can represent miRNA and targets interaction model. The prior parameter v (0 < u < or = 1) controls the compression proportion of data set and selects more distinguishing support vectors. Finally, the classifier model of miRNA target prediction is built. The unbiased assessment of the classifier is achieved with a completely independent test dataset. Experiment results indicated that in both classification recognition and generalization performance of miRNA targets predicition, this model was superior to the present machine learning algorithms such as miTarget, NBmiRTar and TargetMiner, etc.
