1.Clinical efficacy analysis of hemorrhoidal artery ligation combined with ultrasonic knife excision suture tethering suspension for the treatment of circumferential prolapsed mixed hemorrhoids

Objective To investigate the clinical effect of hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligature and suspension in the treatment of mixed hemorrhoids with annular prolapse.Methods A retrospective analysis was conducted to select 80 patients with annular prolapse mixed hemorrhoids admitted to the anorectal Department of our hospital from January 2022 to April 2023.According to different treatment plans,they were divided into control group(n=40 cases)who received external stripping and internal ligation combined with tape ligation,and study group(n=40 cases)who received hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligation and suspension.The operative effect,operative status,postoperative anal pain,anal edema,stool blood score and quality of life were evaluated.Results The effective rate of the study group was 97.50%,which was higher than that of the control group(77.50%)(P<0.05).The operation time,intraoperative blood loss,hospital stay and wound healing time of the research group were(25.03±10.00)minutes,(10.00±5.32)ml,(7.25±1.54)days and(12.21±2.00)days respectively.[is better than that of control group(29.85±14.00)minutes,ml(17.85±10.25),(11.87±2.35),(18.89±4.85)days],two groups of comparison,the difference was statistically significant(P<0.05).The VAS scores of anal pain in the study group on 1 day,3 days and 7 days after the operation were(4.25±0.85)points,(2.89±0.54)points and(2.00±0.30)points,respectively.The scores of perianal edema were(1.87±0.36)points,(1.41±0.30)points,and(1.00±0.20)points,respectively.The hematochezia scores were(1.85±0.21)points,(1.34±0.18)points,and(0.85±0.13)points,respectively.Which were better than that of control group[(7.56±1.10),(6.54±1.03),(4.87±1.00)][(2.40±0.58),(2.03±0.49),(1.87±0.45)][(2.45±0.55),(2.03±0.47),(1.88±0.25)],and the differences were statistically significant(P<0.05).The recurrence rate was 2.50%in the study group and 17.50%in the control group(P<0.05).The scores of SF,MH,PF and GH of the Modified Health Survey Summary Form(SF-36)in the study group were(88.54±10.22,87.87±10.58,88.97±10.00,89.95±10.05)higher than those of the control group(67.52±10.00,70.10±11.25,71.10±9.85,70.00±10.00)(P<0.05).Conclusion Hemorrhoidal artery ligation combined with ultrasonic knife resection,suture,ligature and suspension in the treatment of patients with annular prolapse mixed hemorrhoids has ideal surgical effect and little trauma,which is beneficial to promoting postoperative rehabilitation,alleviating anal pain,improving anal edema and hematostoecium,and improving quality of life.

3.Establishment and validation of a mouse liver injury model induced by chronic low-dose exposure to atrazine

Objective To establish a model of long-term atrazine(ATR)-induced liver injury in mice and to evaluate the hepatotoxic effects induced by ATR.Methods C57BL/6-N male mice were randomly divided into a control group and 1.5 mg/L and 150 mg/L ATR dose(ATR-L,ATR-H)groups.After 35 and 63 days,serum liver function biochemical indexes and inflammatory factors were detected,the hepatosomatic ratio was calculated,and the histopathology and ultrastructure of the liver were observed.Lipid peroxidation levels and antioxidant capacity,the activities of major phase I metabolic enzymes and phase Ⅱ detoxification enzymes,and the expression of related proteins in liver tissues were detected.Results Compared with the control group,the ATR groups showed significant changes in the AST/ALT ratio,levels of pro-inflammatory factors CCL2,TNF-α and IL-6,H2O2 content and activities of the metabolic enzymes NCR,CYTb5,and UDPGT(P<0.05).In the 150 mg/L ATR group,GGT content,peroxide levels(as indicated by malondialdehyde),and CYP1A2 expression were significantly increased(P<0.01),while GSH content was significantly decreased(P<0.05),and hepatocyte injury and mitochondrial vacuolation were more serious when compared to control and 1.5 mg/L groups.Conclusions In a mouse model of low-dose ATR liver injury,both 1.5 mg/L and 150 mg/L ATR exposure induced liver injury in mice,with 150 mg/L ATR inducing the maximum metabolic toxicity in the liver after 63 days.

Traumatic rib fractures are the most common injury in thoracic trauma. Previously,the patients with traumatic rib fractures were mostly treated non-surgically,of which 50%,especially those combined with flail chest presented chronic pain or chest wall deformities and over 30% had long-term disabilities,being unable to retain a full-time job. In the past two decades,thanks to the development of internal fixation material technology,the surgical treatment of rib fractures has achieved good outcomes. However,there are still some problems in clinical treatment,including inconsistency in surgical treatment and quality control in medical services. The current consensuses on the management of regional traumatic rib fractures published at home and abroad mainly focus on the guidance of the overall treatment decisions and plans,and relevant clinical guidelines abroad lacks progress in surgical treatment of rib fractures in recent years. Therefore,the Chinese Society of Traumatology affiliated to Chinese Medical Association and Chinese College of Trauma Surgeons affiliated to Chinese Medical Doctor Association,in conjunction with national multidisciplinary experts,formulate the Chinese Consensus for Surgical Treatment of Traumatic Rib Fractures(2021)following the principle of evidence-based medicine,scientific nature and practicality. This expert consensus puts forward some clear,applicable,and graded recommendations from aspects of preoperative imaging evaluation,surgical indications,timing of surgery,surgical methods,rib fracture sites for surgical fixation,internal fixation methods and material selections,treatment of combined injuries in rib fractures,in order to provide references for surgical treatment of traumatic rib fractures.
5.Stability and pathophysiology of volume-controlled hemorrhagic shock model in rat

Objective:To establish a rat model of volume-controlled hemorrhage and observe the pathophysiological changes that ensued.Methods:Sprague Dawley (SD) rats were subjected to femoral arterial cannulation and hemorrhage for 40% or 50% of total volume. The hemodynamics data and arterial blood gas were collected, and survival after hemorrhage was monitored for 24 hours ( n=8/group). Then lung, liver, intestine and kidney were collected for HE staining after 40% hemorrhage at 4, 8 and 12 hours ( n=6/group). Results:Mean arterial pressure of 40% and 50% blood loss group decreased significantly from (90±5)mmHg and (93±4)mmHg to (40±4)mmHg and (34±4)mmHg ( P<0.05). Heart rate of 40% and 50% blood loss group increased significantly from (330±35) bpm and (336±32) bpm to (478±36) bpm and (490±21) bpm ( P<0.05). Hemoglobin and pH decreased significantly following hemorrhage, while lactate increased. The survival rate of 50% blood loss group was just 12.5% and 40% blood loss group was 100% ( P<0.05). Compared with sham group, the most significant injury was observed in lung and small intestine after 4 hours, in liver after 8 hours and in kidney after 12 hours following hemorrhage. Conclusion:This study established a stable and repeatable volume-controlled hemorrhagic shock model in rat.

The National Health Commission recently has released a notice about further promoting the trauma rescue and treatment in China, which includes four main contents as follows: ( 1 ) to strengthen the establishment of regional trauma rescue and treatment system based on trauma center;(2) to lift the specialized medical service ability related to trauma rescue and treatment; ( 3 ) to further develop the radiant and stimulating role of National Trauma Medical Centre and National Regional Trauma Medical Center;(4) to enhance the trauma-related professional training and public health education. In this article, we interpret the third aspect regarding medical quality control and maintenance of trauma treatment, providing reference for better understanding the specific requirements of the document.

Objective To investigate the inhibition of proliferation and the regulation of histone acetylation modification in Jurkat cells treated by sodium valproate(VPA). Methods Jurkat cells were treated with VPA.Cell proliferation was determined by CCK-8 assay, and cell cycle were analyzed by flow cytometry (FCM). mRNA of HDAC1 was detected by semi-quantitative RT-PCR, and protein expression of HDAC1 and acetylation of histone H3, H4 was examined by Western blotting. Results VPA inhibited the proliferation of Jurkat cells in concentration-and time-dependent manners. After exposure to VPA in different concentrations for 48h,cell cycle was arrested obviously at G0/G1 phase (P <0.05), and with increasing concentration, the percentage of G0/G1 phase cells was increased and that of S phase were decreased. HDAC1 mRNA expression were inhibited with the increasing concentration of VPA. The protein level of HDAC1 was down-regulated, while acetylation of histone H3、H4 was up-regulated in Jurkat cells by VPA. Conclusion VPA can inhibit proliferation of Jurkat cells and induce G0/G1 phase arrest. The mechanism may be that VPA increase acetylation of histone H3/H4 by inhibiting expressions of HDAC1 gene.

Objective To study the effects of cryopreservation on the immunogenicity of human umbilical vein endothelial cells(HUVEC).Methods HUVEC were isolated ex vivo and cryopreserved.Lymphocyte stimulation index(SI)was analyzed by MTT in lymphoeyte-endothelial cell co-culture.Both HLA-ABC and HLA-DR antigen expression on fresh or cryopreserved HUVEC,and the effects of IFN-γ treatment on HLA antigen expression in both groups were determined by using flow cytometry.Results No difference in SI was observed between fresh prepared and cryopreserved HUVEC(1.716±0.181 vs 1.686±0.145,P>0.05).The percentage of HLA-ABC expression was(96.6±1.9)%and(96.0±1.4)%in fresh and cryopreserved HUVEC(P>0.05),and the mean intensity for HLA-ABC expression was 84.1±5.7 and 82.4±4.8 in fresh and cryopreserved HUVEC(P>0.05),respectively.However,no HLA-DR expression was observed in both groups.When treated with IFN-γ,HLA-ABC expression was significantly up-regulated,and HLA-DR expression was induced in a dose-dependent manner.No significant difference was found in the HLA-ABC expression between fresh and cryopreserved HUVEC(P>0.05),while the HLA-DR expression in cryopreserved HUVEC was remarkably lower than in fresh HUVEC with the increase of IFN-γ(P<0.01).Conclusion The immunogenicity of HUVEC remains stable by cryopreservation without IFN-γtreatment or treated with low concentration of IFN-γ(≤50 U/ml).However,the HLA-DR expression in HUVEC was remarkably reduced in eryopreserved cells treated with a high concentration of IFN-γ(≥100 U/ml).These data indieated that the effects of cryopreservation on immunogenicity of HUVEC may result from the decreased responses of HLA-DR expression by the stimulation of IFN-γ treatment.
9.Measurement of hone mineral density in the proximal humeral region

Objective To establish a method of measuring bone mineral density (BMD) in the proximal humeral region, and explore the relationship between proximal humeral BMD level and age and body mass index (BMI).Methods A scanning procedure was developed, using a shoulder & forearm position device.Firstly, 30 subjects were examined on the proximal humerus with 2 successive scan sessions to de-termine the short-term precision.Secondly, scanning of the right proximal humerus BMD in 92 healthy women was done.Results Short term precision revealed that RMS SD was 0.011 g/cm2, and RMS CV (coefficient of variation) on BMD measurement was 2.4%.Proximal humeral BMD values decreased with age.No correlation existed between the proximal humeral BMD and BMI.Conclusions The method of measuring BMD over the proximal humeral region is reasonable, and the short term precision of the result is acceptable.Proximal humeral BMD tends to decrease with age.Since the shoulder is subject to the least in-fluence of weight and stress loading, the proximal humeral BMD scan might be an early stage indicator of osteoporosis.
10.Bcl-2 Antisense Oligodeoxynucleotides Increase Apoptosis of Lung Cancer Cells Induced by Radiation

Objective To study the effects of Bcl-2 antisense oligodeoxynucleotides(ASODN)on the apoptosis of lung cancer cells induced by radiation in vitro.Methods NCI-H446 lung cancer cell strains were divided into 5 groups:control simple radiation,lipofectin plus radiation,nonsense sqnence radiation and ASODN plus radiation.The cells cultured in five groups were collected at 6h,12h,24h,48h and 72h,with Wright-Giemsa stain,morphology analysis for which was done;the mRNA expression for p53、bcl-2 and PTEN gene was examined by RT-PCR half quantivity and DNA-ploid of the cells in five groups was detected by flow cyfometric method.Results Cell proliferation is obviously restrained and conformation is changed too with the shape crimpled and adherence function decreased obviously after irradiated for 10 Gy dose by the linac;p53 and PTEN expression clearly increased for the combination of Bcl-2 ASODN and bcl-2 mRNA expression clearly decreased.The apoptosis rate after 72 hours among control,pure radiation,lipofectin+radiation,nonsense+radiation and ASODN +radiation grouop is 0.14?0.09,13.17?2.47,11.84?1.76,13.72?1.4,21.26?2.97 respectively,the difference between ASODN combined with radiation grouop and other 4 groups are significant(P
