1.Clinical Significance of Unipolar Electrogram and Bipolar Electrogram in Guiding Radiofrequency Ablation of Outflow Tract Ventricular Premature Contraction

Objectives:We evaluated the clinical significance of unipolar electrogram and bipolar electrogram in guiding radiofrequency ablation for outflow tract ventricular premature contractions(PVC).Methods:Data were collected from 78 patients who underwent successful radiofrequency ablation of PVC.Pre-procedure electrocardiogram showed a LBBB morphology on inferior leads.In the included patients,the right-side ablation was performed at first,and if the ablation failed,the left-side ablation was then performed.According to the location of the successful ablation target,they were divided into unilateral ablation successful group and bilateral ablation successful group.The differences in the earliest bipolar local activation to QRS(LATBi),unipolar-dV/dTmax to QRS(LATUni),and local activation time were compared between the two groups.Furthermore,the electrogram characteristics predicting bilateral ablation were explored.Results:Patients were divided into unilateral ablation group(n=57)and bilateral ablation group(n=21)according to the ablation site.There were statistically significant differences in LATBi(25[21,30]ms vs.14[10,24]ms),LATUni(7[0,17]ms vs.-23[-41,-11]ms),and ΔLATBi-Uni(15[11,25]ms vs.44[25,56]ms)between the unilateral ablation group and the bilateral ablation group(all P<0.05).Multivariate regression analysis suggested that LATBi,LATUni,and ΔLATBi-Uni were independent predictors of the need for bilateral ablation.There was no significant difference between left and right LATBi,LATUni and ΔLATBi-Uni in the bilateral ablation group.LATUni≤-1 was the most accurate parameter for predicting the need for bilateral ablation(AUC=0.941;specificity 80.7%;sensitivity 100%).Conclusions:LATUni≤-1ms is a useful indicator for predicting the need for bilateral PVC ablation in this patient cohort.
2.Advances in the assessment of the effectiveness of immediate restorative techniques with full arch implants

The full-arch immediate implant restoration technique is one of the mainstream treatment options for edentulous patients.With the changes in disease patterns,health concepts,and the emphasis on the"patient-centered"philosophy,restoring function is no longer the sole goal of treatment for patients with tooth loss.Other factors such as patient expectations,improvements in quality of life,aesthetics,low economic burden,and convenience of maintenance are also important indicators for evaluating treatment outcomes.Pa-tient-reported outcomes provide a new perspective for assessing the effectiveness of full-arch immediate implant restoration.This study reviews the application and research progress of patient-reported outcomes in full-arch immediate implant restoration techniques,sys-tematically summarizing four evaluation indicators(oral health-related quality of life,quality of life,patient satisfaction,and health economic evaluation),in order to provide reference for clinical decision-making and humanistic care pathways in full-arch immediate implant restoration techniques.

5.Mechanism of Yiyuan Qiwei Pills in Treatment of Diabetes Mellitus-induced Erectile Dysfunction

ObjectiveTo observe the therapeutic effect of Yiyuan Qiwei pills (YYQW) on diabetes mellitus-induced induced erectile dysfunction (DMED) in rats and explore its regulation on the nitric oxide (NO)-cyclic guanosine monophosphate (cGMP) signaling pathway. MethodFifty-five healthy SD male rats of clean grade aged 2-3 months underwent intraperitoneal injection of streptozotocin (STZ) to induce the DMED model, and another 10 healthy SD male rats of clean grade aged 2-3 months were assigned to the control group. The model rats were randomly divided into a model group, a sildenafil group (5 mg·kg-1, ig), and low-, medium-, and high-dose YYQW groups (1.5, 3.0, 6.0 g·kg-1, ig). The rats in the model group and the control group were given normal saline by gavage at 10 mL·kg-1, once a day for two months. After intervention, the penile erectile function of rats in each group was measured by a pressure detection system. The pathological changes and ultrastructure of penile corpus cavernosum were observed by hematoxylin-eosin (HE) staining and transmission electron microscopy, respectively. The level of NO in the corpus cavernosum was detected by nitrate reductase. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of cGMP and advanced glycation end products (AGEs). Real-time quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expression of endothelial nitric oxide synthase (eNOS), neurogenic nitric oxide synthase (nNOS), total nitric oxide synthase (NOS), and phosphodiesterase type5 (PDE5) in rat penile tissues. The expression of above proteins was detected by Western blot. ResultCompared with the control group, the model group showed decreased intracavernous pressure (ICP), NO, and cGMP levels, reduced mRNA and protein expression of nNOS and NOS, and increased PDE5 mRNA and protein expression (P<0.05). Compared with the model group, the sildenafil group and the YYQW groups displayed increased ICP, NO, and cGMP levels, elevated mRNA and protein expression levels of nNOS and NOS, and reduced PDE5 mRNA and protein expression levels (P<0.05). There were no pathological changes in the tissues and cell ultrastructure of the corpus cavernosum in the control group, while serious pathological changes were observed in the model group. Additionally, the sildenafil group and the YYQW groups were superior to the model group, the optimal effect was observed in the high-dose YYQW group. ConclusionYYQW can improve the penile erectile function of DMED rats and reduce the pathological damage of corpus cavernosum. The mechanism may be related to the promotion of nNOS and NOS expression, the inhibition of PDE5 expression, and the activation of the NO/cGMP signaling pathway.
6.Distinctive Roles of Wnt Signaling in Chondrogenic Differentiation of BMSCs under Coupling of Pressure and Platelet-Rich Fibrin

BACKGROUND:
Although newly formed constructs of feasible pressure-preadjusted bone marrow mesenchymal stem cells (BMSCs) and platelet-rich fibrin (PRF) showed biomechanical flexibility and superior capacity for cartilage regeneration, it is still not very clear how BMSCs and seed cells feel mechanical stimuli and convert them into biological signals, and the difference in signal transduction underlying mechanical and chemical cues is also unclear.
METHODS:
To determine whether mechanical stimulation (hydrostatic pressure) and chemical cues (platelet-rich fibrin, PRF) activate canonical or noncanonical Wnt signaling in BMSCs, BMSCs cocultured with PRF were subjected to hydrostatic pressure loading, and the activation of the Wnt signaling molecules and expression of cartilage-associated proteins and genes were determined by western blotting and polymerase chain reaction (PCR). Inhibitors of canonical or noncanonical Wnt signaling, XVX-939 or L690,330, were adopted to investigate the role of Wnt signaling molecules in mechanically promoted chondrogenic differentiation of BMSCs.
RESULTS:
Hydrostatic pressure of 120 kPa activated both Wnt/b-catenin signaling and Wnt/Ca2+ signaling, with the the maximum promotion effect at 60 min. PRF exerted no synergistic effect on Wnt/b-catenin signaling activation. However, the growth factors released by PRF might reverse the promotion effects of pressure on Wnt/Ca2+ signaling. Real-time PCR and Western blotting results showed that pressure could activate the expression of Col-II, Sox9, and aggrecan in BMSCs cocultured with PRF. Blocking experiment found a positive role of Wnt/b-catenin signaling, and a negative role of Wnt/ Ca2+ signaling in chondrogenic differentiation of the BMSCs. Mutual inhibition exists between canonical and noncanonical Wnt signaling in BMSCs under pressure.
CONCLUSION
Wnt signaling participates in the pressure-promoted chondrogenesis of the BMSCs co-cultured with PRF, with canonical and noncanonical pathways playing distinct roles during the process.
7.Osteogenic effects of antihypertensive drug benidipine on mouse MC3T3-E1 cells in vitro.

Hypertension is a prevalent systemic disease in the elderly, who can suffer from several pathological skeletal conditions simultaneously, including osteoporosis. Benidipine (BD), which is widely used to treat hypertension, has been proved to have a beneficial effect on bone metabolism. In order to confirm the osteogenic effects of BD, we investigated its osteogenic function using mouse MC3T3-E1 preosteoblast cells in vitro. The proliferative ability of MC3T3-E1 cells was significantly associated with the concentration of BD, as measured by methylthiazolyldiphenyl-tetrazolium bromide (MTT) assay and cell cycle assay. With BD treatment, the osteogenic differentiation and maturation of MC3T3-E1 cells were increased, as established by the alkaline phosphatase (ALP) activity test, matrix mineralized nodules formation, osteogenic genetic test, and protein expression analyses. Moreover, our data showed that the BMP2/Smad pathway could be the partial mechanism for the promotion of osteogenesis by BD, while BD might suppress the possible function of osteoclasts through the OPG/RANKL/RANK (receptor activator of nuclear factor-κB (NF-κB)) pathway. The hypothesis that BD bears a considerable potential in further research on its dual therapeutic effect on hypertensive patients with poor skeletal conditions was proved within the limitations of the present study.
8.Advances in research on Chiari malformation

Chiari malformation, also known as cerebellar tonsillar hernia, is a congenital developmental anomaly in which the cerebellar tonsils extend downward and protrude into the cervical spinal canal through the foramen magnum. It is a common craniocervical junction malformation. Its pathogenesis is complex and clinical manifestations are diverse. Also Chiari malformation is accompanied by many complications. Its pathogenesis remains not clear; therefore, its treatment is controversial. In recent years, new understanding and great progress have been made in the pathogenesis, diagnosis and treatment of Chiari malformation. In this paper, we discuss the classification, pathogenesis and diagnosis of Chiari malformation. In addition, we summarize the operation of Chiari with the hope to provide some help for further basic and clinical research.
9.Application of multi-mode electrophysiological monitoring in highly selective posterior rhizotomy for patients with spastic cerebral palsy

【Objective】 To explore the application of multi-mode electrophysiological monitoring technology in highly selective posterior rhizotomy (SPR) of patients with spastic cerebral palsy. 【Methods】 We retrospectively collected data on patients with systolic cerebral palsy who underwent SPR in our hospital from January 2019 to December 2019. Bispectral index (BIS), somatosensory evoked potentials (SEP) and electromyography during surgery of all the patients were measured. We used EMG to monitor the depth of the anus, spinal cord function, and the stimulation response of multiple muscle groups, and recorded and summarized abnormal intraoperative electrophysiological monitoring, surgical treatment and complications during displacement and discharge, and relief and exercise functional recovery six months after the surgery. 【Results】 The intraoperative BIS parameters of 18 patients were 60-75, the EMG amplitude was abnormal in 1 case (5.6%), and the SEP amplitude decreased in 1 case (5.6%). The preoperative muscle tension of both lower extremities was 3.6 on average, and the postoperative muscle tension was 1.4, with a decrease by 2.2 grades. There were 16 cases (88.9%) in long-term follow-up, among which 12 cases (75%) had complete relief of spasm, 4 cases (25%) had significant improvement, gait function improvement rate was 100%, and no complications occurred. 【Conclusion】 Multi-mode electrophysiological monitoring technology is an effective method for SPR to increase the success rate of surgery. It can increase the accuracy of the ratio of posterior root resection of spinal nerves, reduce the risk of spinal cord damage, and improve the poor prognosis.
10.Application of multimodal intraoperative neurophysiological monitoring in cervical spine surgery

【Objective】 To explore the application of multi-mode electrophysiological monitoring technology in cervical spine surgery. 【Methods】 We retrospectively collected data of patients who received cervical spine surgery in Department of Neurosurgery of our hospital in January 2018 to December 2019. All patients were guided by the bispectral index (BIS) to control the depth of anesthesia. High cervical spine lesions were treated with somatosensory evoked potentials (SEP), motor evoked potentials (MEP), brainstem auditory evoked potentials(BAEP)and electromyography (EMG) monitoring. We selected SEP, MEP and EMG combined monitoring for low cervical nerve and spinal cord injury, recorded and summarized the abnormalities of intraoperative electrophysiological monitoring, surgical efficacy and complications at the time of discharge and 6 months after surgery. 【Results】 The intraoperative BIS parameters of 112 patients were 45-60, the MEP amplitude decreased in 9 cases (8.0%), the SEP amplitude decreased in 4 cases (3.6%), the EMG amplitude was abnormal in 5 cases (4.5%), and the BAEP amplitude decreased in 4 cases (3.6%); no operation was terminated due to significant abnormalities in monitoring. Compared with 44 cases (39.3%) before operation, 112 patients had little change in SEP and MEP amplitudes, 35 cases (31.3%) had a slight increase in SEP amplitude, 2 cases (1.8%) had a slight decrease, 24 cases (21.4%) had slightly increased MEP amplitude, while 7 cases (6.3%) had slightly decreased one. There were no new neurological disorders at the time of postoperative discharge and 6 months after surgery. 【Conclusion】 Multi-mode electrophysiological monitoring shows the recovery of damaged nerve electrophysiological activity immediately after the operation, which provides an objective basis for the recovery of nerve function, and can effectively prevent and reduce iatrogenic spinal cord and nerve function damage, and improve the efficacy of surgery.
