1.Comparison Study of Cannulated Compression Countersunk Head Screws and Anatomical Locking Compression Hook Plate for Fractures of Proximal Fifth Metatarsal Bone at Lawrence Zone Ⅰ

Objective To compare the clinical effect between cannulated compression countersunk head screws(CS)and anatomical locking compression hook plate(LCP)for fractures of proximal fifth metatarsal bone at Lawrence zone Ⅰ.Methods A total of 60 patients with fractures of proximal fifth metatarsal bone at Lawrence zone Ⅰ from May 2021 to May 2023 were retrospectively analyzed.Of them,20 patients were treated by internal fixation with cannulated compression countersunk head screws(CS group),and other 40 patients were treated by internal fixation with anatomical locking compression hook plate(LCP group).The postoperative therapeutic effects were evaluated by using the Visual Analogue Scale(VAS)and the American Orthopaedic Foot and Ankle Society(AOFAS)Midfoot Scale.Incidences of postoperative complications were also recorded.Results The operations were successfully completed in all the 60 patients.The operation time in the CS group was shorter than that in the LCP group[37(15-74)min vs.50.5(28-102)min,P=0.002].The VAS score in the CS group immediately after surgery was lower than that in the LCP group[3(2-5)vs.4(2-5),P=0.004],and there was no significant difference between the two groups at 3,6,and 12 months after surgery and at the last follow-up(P>0.05).At the 3rd month after operation,the AOFAS score in the CS group was better than that in the LCP group[52(23-62)vs.49(23-62),P=0.038],and there was no significant difference between the two groups at 6 and 12 months after surgery and at the last follow-up(P>0.05).In the LCP group,there were 1 case of superficial wound infection,3 cases of long-term chronic pain,6 cases of foreign body sensation,5 cases of removal of the internal fixation by a second operation after one year postoperatively.In the CS group,there was only 1 case of foreign body sensation.Conclusions Both cannulated compression countersunk head screws and anatomical locking compression hook plate can effectively fix fractures of proximal fifth metatarsal bone at Lawrence zone Ⅰ.Compared with anatomical locking compression hook plate,cannulated compression countersunk head screws can shorten operation time,reduce postoperative pain,and facilitate early functional rehabilitation.

3.A cross-sectional study of mood disorder in Inner Mongolia Autonomous Region

Objective:To describe the prevalence and distributions of mood disorder in Inner Mongolia Au-tonomous Region,and analyze the related risk factors.Methods:The multistage stratified sampling method with un-equal probability was used to select permanent residents aged 18 years and over in Inner Mongolia Autonomous Re-gion.The Composite International Diagnostic Interview 3.0 was used as a diagnostic tool.Mood disorders were di-agnosed according to the Diagnostic and Statistical Manual of Mental Disorders Fourth Edition(DSM-Ⅳ).Single and multivariate analyses were used to investigate the related factors of mood disorders.Results:Totally,12 315 community residents were interviewed in the survey.The weighted 12-month prevalence and lifetime prevalence of mood disorder were 5.4%and 8.7%,respectively.Weighted 12-month prevalence of depressive disorder was 4.9%,and that of bipolar disorder was 0.3%.Among all subtypes of mood disorder,the 12-month prevalence rate of major depressive disorder(3.1%)was the highest.Multivariate logistic regression analysis showed that female,unmarried,separated or divorced,unemployment,family history,other mental disorders,sleep disorders and chronic diseases(OR=1.56,2.80,2.07,1.42,13.46,7.97,3.23,2.13)were risk factors of mood disorder,while aged 65 years and over(OR=0.44)was protective factor of mood disorders.The lifetime consultation rate in patients with mood disorders was 15.5%,the rate of psychiatric consultation was 3.7%,the rate of medication was 1.8%.Con-clusion:It indicates that female residents and people who are unmarried,separated and divorced,unemployed,with family history,suffering from other mental disorders,suffering from sleep disorders,and suffering from chronic dis-eases may be high risk groups of mood disorders,and the utilization rate of health services is rather low in Inner Mongolia Autonomous Region.
4.Mid-to Long-term Outcomes of Headless Compression Screws Versus Anatomical Plate Fixation for Radial Head and Neck Fractures With≤3 Fragments

Objective To compare the mid-to long-term clinical outcomes between screw and plate fixation for radial head and neck fractures with ≤ 3 fragments.Methods This retrospective study included 70 cases of comminuted radial head and neck fractures treated between January 2013 and December 2022.Fracture patterns included 44 cases with 2 fragments and 26 cases with 3 fragments.Patients were divided into screw fixation group(n=31,headless compression screws)and plate fixation group(n=39,anatomic plate).A comparison was conducted in the Mayo Elbow Performance Score(MEPS),range of motion of the elbow and forearm,complications,and reasons for re-operation.Results One case in each of the two groups required radial head arthroplasty due to non-union of the fracture(failure of internal fixation),while the remaining 68 cases met the criteria of fracture union.Follow-up duration ranged for 20-135 months(mean,66.7 months).Among them,14 cases were less than 36 months,18 cases were between 36-60 months,and 36 cases were over 60 months.At the last follow-up,the MEPS score was 85-100 points,with a median of 100 points,and there was no significant difference between the two groups(P=0.762).The screw fixation group demonstrated superior supination to the plate fixation group(78.5°±5.0° vs.76.2°±4.6°,t=2.010,P=0.049),while flexion,extension,and pronation showed comparable outcomes between the two groups(P>0.05).Compared to the plate fixation group,the screw fixation group had significantly lower overall complication rates[12.9%(4/31)vs.46.2%(18/39),P=0.003],reduced re-operation rate[6.5%(2/31)vs.30.8%(12/39),P=0.012]and re-operation rate due to hardware irritation[3.3%(1/31)vs.25.6%(10/39),P=0.026].Conclusions Both fixation methods provide satisfactory outcomes for radial head and neck fractures with 3 fragments.Screw fixation demonstrates a reduced postoperative reoperation rate.
5.Application of BOPPPS combined with scenario simulation in cultivating humanistic care ability of undergraduate nursing interns

Objective:To explore the application effect of BOPPPS (Bridge-in, Objectives, Pre-assessment, Participatory learning, Post-assessment, and Summary) combined with scenario simulation in cultivating the humanistic care ability of undergraduate nursing interns.Methods:Undergraduate nursing students who were interning at the First Affiliated Hospital of Zhengzhou University were selected by convenience sampling. The 46 interns from June to December 2023 were set as the control group, and the 46 interns from June to December 2024 were set as the experimental group. The control group adopted the traditional lecture-based teaching method, while the experimental group adopted BOPPPS combined with scenario simulation. Theoretical and practical scores, humanistic care ability, empathy ability, and critical thinking ability of the two groups were compared.Results:After the intervention, the theoretical and practical scores, as well as humanistic care ability, empathy ability, and critical thinking ability scores of the experimental group were all higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:BOPPPS combined with scenario simulation plays a positive role in improving theoretical and practical scores, humanistic care ability, empathy ability, and critical thinking ability of undergraduate nursing interns, and it is worthy of further promotion and application.
6.Immune checkpoint inhibitors-associated myocarditis in 7 patients with bladder cancer

Objective To explore the clinical characteristics and treatment of immune checkpoint inhibitors(ICIs)-associated myocarditis in patients with bladder cancer(BCa).Methods Clinical and follow-up data of 213 BCa patients treated with ICIs in our hospital during Jan.2020 and May 2024 were collected.The data of 7 patients(3.3%)who developed ICIs-associated myocarditis were analyzed.Results The cohort included 2 females and 5 males(median age:72 years).Four patients were asymptomatic,while 3 presented with chest tightness,dyspnea,or orthopnea.All patients showed significantly elevated high-sensitivity troponin T.Only 2 patients had markedly increased N-terminal pro-B-type natriuretic peptide.Electrocardiograms were normal in 4 patients,while 2 patients exhibited significantly reduced left ventricular global longitudinal strain on echocardiography,with cardiac magnetic resonance confirming acute myocarditis.All patients discontinued ICIs and received first-line methylprednisolone upon diagnosis.Two patients showed no improvement after 5 days of treatment and received second-line therapy.One patient received intravenous immunoglobulin and infliximab without response,but improved after third-line tofacitinib.One patient developed acute respiratory failure after intravenous immunoglobulin administration and was then transferred to ICU,and died of multiple organ failure after 10 days.Conclusion ICIs-associated myocarditis is a relatively rare but clinically distinct immune-related adverse reaction during BCa treatment.Methylprednisolone is the first-line therapy,while critically ill and steroid-resistant patients often require early combined immunosuppressants based on individualized multidisciplinary discussion.
7.Exploration on the Clinical Experience of Wang Yan'gang in Treating Gastroesophageal Reflux Disease Based on Data Mining

Objective To explore the clinical experience of Professor Wang Yan'gang in treating gastroesophageal reflux disease(GERD)using data mining methods.Methods SPSS 23.0 was used to statistically analyze the frequency,properties,flavors,and meridian tropism of Chinese herbal medicines in the prescriptions of effective GERD cases treated by Professor Wang Yan'gang in outpatient clinics from January 2020 to December 2022.Cluster analysis,factor analysis,and association rule analysis were performed on the relevant drugs.Results A total of 294 outpatient prescriptions for the treatment of GERD were collected,including 174 Chinese herbal medicines.The top 10 frequently-used drugs were Sepiae Endoconcha(Haipiaoxiao),Fritillariae Thunbergii Bulbus(Zhebeimu),Scutellariae Radix(Huangqin),Coptidis Rhizoma(Huanglian),Pinelliae Rhizoma Praeparatum Cum Alumine(Qingbanxia),Arecae Semen Tostum(Chaobinglang),Artemisiae Scopariae Herba(Yinchen),Gypsum Fibrosum(Shigao),Aurantii Fructus Immaturus(Zhishi),and Magnoliae Officinalis Cortex(Houpo).After cluster analysis,factor analysis,and association rule analysis,16 commonly-used herb pairs,such as Huangqin-Huanglian,Citri Reticulatae Pericarpium(Chenpi)-Zhuru(Bambusae Caulis in Taenia),Haipiaoxiao-Zhebeimu,Shichangpu(Acori Tatarinowii Rhizoma)-Yujin(Curcumae Radix),and Zhishi-Houpo were identified,and 9 herb groups such as"Huangqin,Huanglian,Yinchen"and"Chenpi,Zhuru,Qingbanxia"were obtained.Factor analysis was performed on 22 drugs with a usage frequency more than 110 times,and then 6 common factors were extracted after principal component analysis.Conclusion In differentiating and treating GERD,Professor Wang Yan'gang suggests that stagnated heat injuring yin should be taken as the core pathogenesis,and therapy of clearing heat and nourishing yin should be used as the core treatment method.Additionally,auxiliary methods such as soothing the throat,regulating the liver and stomach,and nourishing the heart and spirit can be adopted according to the accompanying symptoms during the disease progression.
8.Real world clinical data analysis of fuzuloparib for the treatment of ovarian epithelial cancer patients

Objective:To evaluate the safety and effectiveness of fuzuloparib for the treatment of ovarian epithelial cancer patients in the real world setting.Methods:A retrospective analysis was conducted on the baseline data of 4 620 ovarian cancer patients who had received fuzuloparib monotherapy or combination therapy. Another 224 ovarian cancer patients who were willing to receive fuzuloparib monotherapy or combination therapy were prospectively enrolled, and their baseline characteristics, drug effectiveness, and safety data were analyzed.Results:(1) Among the 4 620 patients in the retrospective cohort, the median age of patients was 60 years; tumor types: 89.8% (4 149/4 620) had ovarian cancer. Among patients with clearly documented information, the vast majority had a histological type of serous carcinoma (82.9%, 3 770/4 546) and International Federation of Gynecology and Obstetrics (FIGO) staging of Ⅲ-Ⅳ (90.9%, 1 537/1 691). (2) Among the 224 patients in the prospective cohort, the median age of patients was 57 years; tumor types: 83.9% (188/224) had ovarian cancer. Among patients with clearly documented records, the predominant pathologic type was serous carcinoma (91.9%, 193/210), and FIGO stage was Ⅲ-Ⅳ in 79.9% (139/174). (3) Among the 224 prospective patients: 84 patients received first-line fluzoparib maintenance therapy, 92 patients received fluzoparib maintenance therapy after platinum-sensitive recurrence, 23 patients received direct fluzoparib treatment after platinum-sensitive recurrence, 19 patients received direct fluzoparib treatment after platinum-resistant recurrence. The median follow-up durations were 8.5, 8.7, 7.9, and 6.7 months, respectively. The median durations of fluzoparib treatment were 6.7, 4.8, 3.1, and 1.9 months, respectively. The median progression-free survival (PFS) times were not reached during follow-up, 12.6 months, not reached during follow-up, and 4.8 months, respectively. The 1-year PFS rates were 84.1%, 55.0%, 69.8%, and 45.5%, respectively. The remaining 6 patients received other fluzoparib regimens. (4) Among the 224 patients in the prospective dataset, 205 had safety data recorded. Of these, 127 patients (62.0%, 127/205) experienced treatment-related adverse events, with common events including anemia (24.4%, 50/205), thrombocytopenia (21.0%, 43/205), and leukopenia (19.5%, 40/205). Among the 205 patients, 43 (21.0%, 43/205) experienced grade 3 or higher treatment-related adverse events, with common events including anemia (8.3%, 17/205) and thrombocytopenia (8.3%, 17/205).Conclusions:The effectiveness of fuzuloparib in clinical application is generally consistent with other drugs in the same class, with good safety. This study provids new clinical evidence for the treatment of ovarian cancer with fuzuloparib.

10.Metformin upregulates ABCA1 expression via inhibiting ubiquitin-proteasome system

Aim To explore the potential mechanism of metformin on ATP-binding cassette transport A1(ABCA1)expression.Methods J774A.1 macrophages were treated with metformin and cycloheximide,and ABCA1 expression was determined by Western blot.His-tagged ABCA1 and HA-tagged Ub plasmids were co-transferred into HEK293 cells and stimulated with metformin.Co-immunoprecipitation(Co-IP)was used to test the binding ability of ABCA1 and ubiquitin.Candidate E3 ubiquitin-protein ligases(CE3)of ABCA1 were identified through Co-IP-based pro-teomics.The MIB1 plasmid was constructed and transferred into HEK293 cells,and Western blot was used to determine the effect of metformin and MIB1 on ABCA1 expression.Results Metformin increased the expression of ABCA1 in J774A.1 cells(P<0.01),and inhibited ABCA1 degradation(P<0.05).Metformin disrupted the binding of ABCA1 to ubiquitin(P<0.05).The proteins regulated by metformin in ABCA1 expression were primarily enriched in pathways re-lated to cell development,inflammation and immune defense.Metformin may upregulate ABCA1 protein expression via MIB1(P<0.05).Conclusion Metformin inhibits the degradation of ABCA1 by blocking the ubiquitin-proteasome system(UPS),and MIB1 might act as a candidate E3 ubiquitin-protein ligase(CE3)for ABCA1.
