1.Expert consensus on artificial intelligence methods based on multimodal data for predicting adverse drug reactions of antineoplastics

OBJECTIVE To provide guidance on the scientific and standardized application of artificial intelligence (AI) and multimodal fusion techniques in constructing predictive models for adverse drug reactions (ADR) of antineoplastics, thereby enhancing the safety of antineoplastic therapy. METHODS Jointly initiated by the Clinical Pharmacy and Translational Pharmacy Society of the Liaoning Provincial Life Sciences Association and other professional societies, and organized by the Fourth Affiliated Hospital of China Medical University, the writing group conducted a comprehensive search of domestic and international databases using keywords such as “multimodal data” “artificial intelligence” “adverse drug reactions”“antineoplastics” and “models” to collect guidelines, expert consensuses, and relevant guidance literature. Based on this search, and combined with China’s practical experience in applying AI to oncology pharmaceutical care, a consensus framework and preliminary recommendations were drafted. Following evaluation and discussion by an external expert panel, the writing group incorporated the assessment results and formulated recommendations based on those endorsed by 90% or more of the experts. RESULTS &CONCLUSIONS After multiple rounds of review, discussion and revision, this consensus outlines the process for constructing an ADR prediction model of antineoplastics, encompassing “defining research objectives-data preparation-selection of algorithmic frameworks-design of multimodal data fusion strategies-model training and validation-interpretability analysis-clinical deployment-publication of results”. It also formulates 18 recommendations regarding core aspects such as data preparation, algorithm framwork selection, multimodal data fusion strategy design, model validation and interpretability analysis, and clinical deployment. These recommendations are intended for use by physicians in the field of oncology, clinical pharmacists, researchers, and AI technology developers to assist in making optimal clinical treatment decisions.
2.Survey on the Actual Work of Registered Sales Clerks of Pharmaceuticals and Their Awareness of Changes in the Pharmaceutical Sales System

Objective: The “One-half Rule” for OTC drug sales has been eliminated, and furthermore, “Remote-controlled sales” of OTC medicines by qualified sellers is under discussion. The number of registered sales clerks is greater than the number of pharmacists selling OTC drugs. Although this is a result of the different employment status of pharmacists and registered sales clerks due to their salary structure and role in the store, it can be said that registered sales clerks are the main people to deal with the general public when they purchase OTC drugs. Therefore, we conducted a survey of registered sales clerks in order to understand their actual work and their awareness of the revision of the system, and to estimate the impact on customers and patients.Methods: The questionnaire was administered at an external training session held by the Tokyo Metropolitan Druggist Association from April 16 to 18, 2022.Results: Valid responses were obtained from 319 participants. Responses were obtained for the ratio of work as a registered sales clerk before becoming a qualified, after qualification (present), and what they think would be an ideal work ratio. The percentage of “pharmaceutical consultation and sales” increased from 7.7 to 15.9, and finally 26.4% before, at present, and ideally, respectively. It was clear that the acquisition of the certification increased the percentage of specialized tasks and that the respondents would like to further increase this percentage. Awareness of the “One-half Rule” and “Remote-controlled sales” was 43.6 and 23.8%, respectively.Conclusion: It was inferred that the introduction of “Remote-controlled sales” would improve convenience but dilute interpersonal work, which would hinder the operation, maintenance, and management of the store, including courteous customer service that cannot be handled through a screen. Therefore, from this study, we believe that registered sales clerks express doubt about the introduction of “Remote-controlled sales”.
3.Expert consensus on thrombolytic therapy for ST-segment elevation myocardial infarction

Thrombolytic therapy plays a vital role in reperfusion strategy for ST-segment elevation myocardial infarction(STEMI)patients.Previous Chinese expert consensus focused on pre-hospital thrombolysis therapy in STEMI patients was published in 2018 which tremendously promoted pre-hospital thrombolysis therapy.In recent years,plenty of research progress and results of clinical trials have been released in following areas such as STEMI reperfusion strategy,anti-platelet/anti-coagulants therapy,contemporary facilitated percutaneous coronary intervention,and so on.Meanwhile,the COVID-19 pandemic makes timely reperfusion for STEMI patients even more challenging.Therefore,this consensus has been proposed aiming to summarize recent progress in STEMI thrombolysis therapy and to provide practical & evidence-based instructions conforming to national conditions.We hope this consensus will contribute to the emergency management and long-term prognosis of STEMI patients.
4.Analysis of the Marketing Approval of Pediatric Drugs in the United States and Its Enlightenment to Pedia tric Drug Use Guarantee in China

OBJECTIVE:To provide sugge stions for improving the variety of pediatric drugs and ensuring the safety of pediatric drug use in China. METHODS :The historical evolution of laws and regulations on the marketing approval of pediatric drugs in the United States and the implementation results of relevant policies were summarized. Combined with the current situation of the development of pediatric drugs in China ,some suggestions were put forward to ensure the accessibility and safety of pediatric drug use in China. RESULTS & CONCLUSIONS :Since 1994,the United States had issued a series of laws and regulations to encourage the development of pediatric drugs. At present ,the marketing approval of pediatric drugs were mainly based on the two laws of Best Pediatric Drug Act(BPCA)and Pediatric Research Equality Act (PREA). From 1998 to 2019,the amount of supplements of pediatric drug information in drug instructions of the United States showed a fluctuating growth. As of April 2020,854 kinds of drugs had been modified in pediatric instructions ,792 of which had been carried out post marketing pediatric clinical research ,and the problem of incomplete pediatric instructions had also been greatly improved. At present ,China’s policies on pediatric drugs mainly included encouraging R&D innovation ,giving priority to review and approval ,and strengthening R&D technical guidance. Although certain achievements had been made ,there were still some problems ,such as imperfect policies and regulations ,and great difficulties carrying out pediatric drug clinical trials. It is suggested that our country should draw lessons from the American regulations on pediatric drugs ,pediatric research and the catalogue of pediatric drugs ,and establish a system and catalogue of ped iatric drug use suitable for China ’s national conditions ,so as to improve the effectiveness ,safety and accessibilityof pediatric drugs.

6.Status of Responses to Foreign Language Inquiries When Selling OTC Drug

Objective: Along with the increase in the number of foreign visitors to Japan, the number of inquiries in foreign languages at the time of OTC drug sales has increased. To clarify the current status of responses to foreign language inquiries when selling OTC drug, we surveyed the frequency of foreign language response, languages used, trouble experienced, and preparation useful in responding to foreign language inquiries when selling OTC drug.Methods: Our survey was conducted with 694 registered salesclerks of drugs at an external training seminar in Tokyo, Japan.Results: Of the 649 respondents, 337 (51.9%) had experience in responding to inquiries in a foreign language when selling OTC drug. The languages used were as follows: ”English,” 86.4%; ”Chinese,” 70.0%; ”Korean,” 25.8%. Of the respondents who had experience in responding to inquiries in a foreign language when selling OTC drug, 107 (31.8%) responded ”I experienced trouble because I could not understand what was being said,” and 228 (67.7%) responded ”I could not respond and experienced trouble.” When asked about preparation/tools useful for selling OTC drug, 434 (66.9%) responded ”multilingual written correspondence table,” 359 (55.3%) responded ”smartphone or tablet‐based correspondence multilingual table,” 299 (46.1%) responded ”marks and illustrations,” and 253 (39.0%) responded ”I would like to be taught how to respond in a foreign language (simple conversation).”Conclusions: Our results revealed that when selling OTC drug, response to inquiries in a foreign language occurred frequently and trouble was experienced in listening comprehension and speaking a foreign language. Therefore, urgent countermeasures such as preparing a multilingual correspondence table of symptoms, usage, and dosage, and lectures on methods of responding through simple foreign language conversations are necessary. In addition, efforts such as creation of multilingual drug package inserts by pharmaceutical companies were considered to be an effective measure.
