1.Study on the Hemodynamic Effects of Various Bypass Grafting Techniques for the Left Anterior Descending Artery on the Native Artery and Graft Vessels

Objectives:To evaluate the hemodynamic impact of different bypass grafting methods on the left anterior descending(LAD)artery using computational fluid dynamics,providing a theoretical basis for optimizing coronary artery bypass grafting.Methods:A patient-specific 3D coronary model was constructed using SimVascular software,including an in-situ internal mammary artery graft(model 1)and three free grafts of varying diameters(model 2:2 mm internal mammary artery free graft;model 3 and model 4:3 mm and 4 mm great saphenous vein free grafts).The hemodynamic parameters of the native artery and grafts were compared among the four bypass grafting models.Results:All four bypass grafting models supply sufficient blood flow for the severely stenosed LAD.LAD flow was slightly lower in the model 1.Free internal mammary artery grafts showed higher flow and wall shear stress.4 mm great saphenous vein free grafts presented with maximum flow but lowest shear stress,3 mm great saphenous vein free grafts offered better hemodynamic stability.Conclusions:Computational fluid dynamics analysis indicates that free internal mammary grafts could provide superior hemodynamics,while larger grafts result in lower shear stress,which might affect long-term graft patency.These findings thus provide experimental evidences useful for optimizing LAD bypass strategies.
2.Investigation of Proteomic Mechanisms of Luteolin's Inhibition on Growth of Colorectal Cancer SW620 Cells

With the continuous rise in the incidence of colorectal cancer and the trend towards younger patient population,the existing treatment options,while able to prolong survival,are difficult to avoid significant side effects.It is imperative to develop new treatment strategies.Luteolin(LUT),as a natural herbal active ingredient,has been proved to have broad-spectrum anti-tumor effects in studies of multiple cancer types.However,the mechanism of LUT action in colorectal cancer has not been systematically elucidated.In this study,for the first time,the molecular mechanism of LUT on colorectal cancer SW620 cells from the perspective of proteomics-glycoproteomics co-regulation was revealed.Proteomic analysis identified 472 differentially expressed proteins.Functional enrichment analysis showed that down-regulated proteins were mainly involved in oxidative stress response,mRNA processing,RNA splicing,and actin filament organization among key biological processes,involving oxidative phosphorylation and peroxisome pathways.Up-regulated proteins were mainly involved in DNA replication,protein folding,and rRNA metabolism,closely related to DNA replication and protein processing pathways in the endoplasmic reticulum.At the level of glycoproteomics,231 differentially expressed intact N-glycopeptides were identified.Functional enrichment analysis of corresponding glycoproteins indicateed that LUT might exert biological effects by regulating biological processes such as nuclear organization,nuclear membrane organization,and Fc receptor-mediated signaling pathways,as well as endoplasmic reticulum protein processing and N-glycan biosynthesis pathways.Analysis of key interaction networks revealed 5 core target proteins namely RPS15A,WDR43,FBL,UTP18,and UTP11.The loss of these proteins had been confirmed to inhibit the proliferation and migration of various tumor cells.Notably,altered glycosylation modifications of the lysosome-associated membrane proteins LAMP1 and LAMP2 suggested that LUT might affect tumor metastatic potential by regulating organelle dynamics.It was found that LUT could inhibit the malignant phenotype of colon cancer cells through a dual mechanism of specifically regulating protein expression networks and glycosylation modification patterns,providing new molecular targets and theoretical basis for precise treatment of colorectal cancer based on natural products.
3.Training methods and effects of individualized eyebrow shape design before eyebrow transplantation

Objective:To explore the training method for personalized eyebrow shape design before eyebrow transplantation surgery and to evaluate its effectiveness.Methods:Thirty physicians with similar foundational knowledge in facial aesthetic design and equivalent educational backgrounds were prospectively selected from the Second Hospital of Lanzhou University and Beijing Biliansheng Medical Beauty Clinic from January to December 2023. All the physicians were right-handed. Based on prior experience in eyebrow transplantation surgery, they were divided into two groups: an observation group [ n=20; 10 males, 10 females; aged (27.8±3.9) years] without experience in personalized eyebrow transplantation surgery, and a control group [ n=10; 5 males, 5 females; aged (29.6±4.0) years] with at least 2 years of experience in personalized eyebrow transplantation surgery. All the physicians received 2 hours of theoretical instruction on personalized eyebrow shape design and 8 hours of simulated operation training. Differences in personalized eyebrow design scores between the two groups were compared before and after training. After the assessment, physicians in the observation group performed eyebrow shape design on 20 eyebrow transplantation patients with randomly assigned face shapes, and patient satisfaction was evaluated. Results:Before training, personalized eyebrow design scores were 1.16±0.29 for the observation group and 3.75±0.22 for the control group, showing a statistically significant difference ( P<0.001). After operation training, the scores were 4.51±0.11 for the observation group and 4.89±0.08 for the control group, with no statistically significant difference ( P>0.05). The difference between pre- and post-training scores within the observation group was statistically significant ( P<0.05). Physicians in the observation group designed eyebrow shapes for male and female patients with various face shapes. Immediately after surgery, 80% (16/20) of patients were very satisfied, and 20% (4/20) were satisfied. Conclusion:After training of combined theoretical lectures, simulated practice, and hands-on patient design, physicians in the observation group demonstrate improved ability in personalized eyebrow shape design and achieve high patient satisfaction.
4.Risk signal mining of adverse reactions to triazole antifungal drugs: a comparative study on domestic and foreign adverse drug reaction/event reports

Objective:To detect adverse reaction risk signals of triazole antifungal agents and provide evidences for their safe use in clinic.Methods:Adverse reaction/event reports with fluconazole, itraconazole, voriconazole, posaconazole, or isavuconazonium as the primary suspect drug were collected from the data in National Adverse Drug Reaction Monitoring System of China reported by Shandong Province from January 2004 to June 2024 and the US Food and Drug Administration Adverse Event Reporting System (FAERS) database from the first quarter of 2004 to the second quarter of 2023. Adverse reaction/event terms were standardized using the preferred term (PT) and system organ class in Medical Dictionary for Regulatory Activities 24.0. Risk signals were detected using the reporting odds ratio (ROR) method and the Bayesian confidence propagation neural network (BCPNN) algorithm. A PT was defined as an adverse reaction risk signal if the number of reports was ≥3, the lower limit of the 95% confidence interval ( CI) for ROR was >2, and the lower limit of the 95% CI for the information component ( IC) was >0. Descriptive statistical analysis was performed. Results:A total of 3 988 reports with the above 5 antifungal drugs as the primary suspect drug were collected from data in National Adverse Drug Reaction Monitoring System of China reported by Shandong Province, 822 (20.6%) of which were serious cases. Voriconazole, fluconazole, itraconazole, posaconazole, and isavuconazonium was the primary suspect drug in 1 852, 1 395, 703, 27, and 11 cases among the 3 988 reports, and in 591 (31.9%), 149 (10.7%), 59 (8.4%), 18 (66.7%), and 5 (5/11) serious cases among the 822 serious case reports, respectively. A total of 20 066 reports with the above 5 drugs as the primary suspect drug were collected in FAERS database, 9 635 (48.0%) of which were serious cases. Voriconazole, fluconazole, itraconazole, posaconazole, and isavuconazonium was the primary suspect drug in 7 758, 6 180, 2 869, 1 796, and 1 463 cases among the 20 066 reports, and in 4 295 (55.4%), 2 806 (45.4%), 1 191 (41.5%), 828 (46.1%), and 515 (35.2%) serious cases among the 9 635 serious case reports, respectively. Based on the data reported by Shandong Province and in FAERS database, 18 and 207 risk signals of adverse reaction not mentioned in the labels were identified, respectively, and 5 of them were identified in both databases, including fluconazole-induced renal impairment and voriconazole-induced oliguria, delirium, psychiatric disorders, and rhabdomyolysis. In the data reported by Shandong Province and in FAERS database, 13 and 189 reports of muscle-related disorders (rhabdomyolysis, myopathy, and myositis) were identified respectively, involving voriconazole (in 8 and 62 cases), itraconazole (in 4 and 74 cases), and fluconazole (in 1 and 53 cases).Conclusions:Renal impairment induced by fluconazole and oliguria, delirium, psychiatric disorders, and rhabdomyolysis induced by voriconazole are risk signals of adverse reaction not mentioned in the labels for triazole antifungal agents. Voriconazole, itraconazole, and fluconazole may also cause muscle-related disorders, warranting vigilance in clinical practice.
5.Establishment and assessment of deep vein thrombosis model in rats in a plateau hypoxic environment

Objective To establish a rat model of venous thrombosis in a plateau hypobaric hypoxic environment and to investigate the effect of this environment on venous thrombosis.Methods A total of 144 healthy male SD rats were assigned randomly to four groups(n=36 rats per group):a plains sham operation(A)group,plains operation(B)group,plateau altitude 6000 m+sham operation(C)group,and plateau altitude 6000 m+surgery(D)group.Rats in A and B groups were maintained in a plains normoxic environment,while rats in C and D groups C and D were subjected to a plateau environment.Rats in the surgical groups underwent quantitative constriction to incompletely obstruct the inferior vena cava blood flow.Each group was further divided into subgroups based on time:1,3,5,7,14,and 21 d(n=6 rats per group).Regular vascular ultrasound monitoring was conducted,and blood samples were taken for whole blood viscosity testing and the assessment of inflammatory indicators,including endothelin-1(ET-1),interleukin-6(IL-6)and tissue factor(TF).Coagulation function was evaluated through the activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(FIB)and D-dimer.After the observation period,the experimental animals were sacrificed and the limbs were removed.Thrombus samples were stained with hematoxylin/eosin(HE),and the thrombus wet mass was measured.Results The thrombosis incidence was significantly higher in the plateau D group than in B group,accompanied by a marked increase in blood viscosity and hematocrit(P<0.01).Additionally,levels of ET-1,IL-6,and TF were significantly elevated(P<0.05),indicating a coagulation disorder.Conclusions A plateau hypoxic environment model can be successfully simulated by quantitative coarctation of the inferior vena cava,combined with a specialized environmental chamber.The findings of this study suggest that a plateau hypoxic environment promotes venous thrombosis.
6.Clinical value of transcranial color-coded duplex sonography in elderly patients with acute ischemic stroke after endovascular therapy

Objective To investigate the intracranial hemodynamic parameters with transcranial color-coded duplex sonography(TCCS)in elderly patients with acute ischemic stroke(AIS)after endovascular therapy,and analyze their association with functional outcomes.Methods A total of 360 elderly AIS patients undergoing endovascular therapy in our hospital from March 1,2020 to January 31,2024 were prospectively included,and according to the score of modified Rankin scale(mRS)at 90 d,they were divided into a poor function group(mRS score 3-6,145 cases)and good function group(mRS score≤2,215 cases).TCCS was conducted on all the patients in 12 h after endovascular therapy to evaluate intracranial blood flow status.Peak systolic velocity(PSV),end-diastolic velocity(EDV),mean blood flow velocity(MFV),pulsatility index(PI),MFV ratio and PSV ratio,adjusted PSV ratio and adjusted MFV ratio were recorded in all pa-tients.Receiver operator characteristic(ROC)curve analysis was employed to analyze the value of TCCS parameters in predicting poor functional outcomes,and the AUC values and cut-off values were also analyzed.Results The PSV ratio,MFV ratio,adjusted PSV ratio and adjusted MFV ra-tio were significantly higher in the poor function group than the good function group(P<0.01).The predictive performance of PSV ratio and MFV ratio were not good enough for poor functional outcome in the AIS patients(AUC<0.750),while the adjusted PSV ratio and adjusted MFV ratio showed better predictive performance(P<0.01).Multivariate logistic regression analysis showed that NIHSS score at admission ≥14.00,ASPECT score at admission≥8.00,adjusted PSV ratio>1.39 and adjusted MFV ratio>1.40 were independent risk factors for poor functional outcomes in AIS patients(P<0.01).ROC curve analysis indicated that combined NIHSS score at admission+ASPECT score at admission showed poor value in predicting poor functional outcome in the AIS(AUC=0.780,95%CI:0.734-0.822),the predictive performance of combination of above two scores+adjusted PSV ratio+adjusted MFV ratio was significantly improved(AUC=0.976,95%CI:0.955-0.989),with higher sensitivity and specificity(Z=8.261,P<0.01).Conclusion Early TCCS detection in elderly AIS patients after endovascular treatment is an effective method for identifying poor functional outcomes at 90 d,with the advantages of non invasiveness and easy op-eration.And,better predictive performance is obtained when adjusted PSV ratio and adjusted MFV ratio are combined with conventional predictors.
