1.Predictive study of serum 25-hydroxyvitamin D and blood lipid metabolism indexes in occurrence of osteoporosis in type 2 diabetes mellitus

Objective To explore the predictive effect of serum 25-hydroxyvitamin D3 [25(OH)D3] and blood lipid metabolism indexes on the occurrence of osteoporosis in type 2 diabetes mellitus (T2DM). Methods Totally 98 patients with T2DM in the hospital from January 2022 to January 2024 were classified into osteoporosis group (38 cases) and non-osteoporosis group (60 cases) by means of concurrent osteoporosis status. The levels of serum 25(OH)D3 and blood lipid metabolism indexes [high density lipoprotein (HDL), total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL), VLDL] were measured in study subjects. The association of serum 25(OH)D3 and blood lipid metabolism indexes with osteoporosis was explored by Logistic regression analysis. The predictive value of serum 25(OH)D3 and blood lipid metabolism indexes on osteoporosis was analyzed by receiver operating characteristic curve (ROC). Results Serum 25(OH)D3 and HDL levels in the osteoporosis group were lower while TG and LDL levels were higher than those in the non-osteoporosis group (P<0.05). The differences in the levels of TC and VLDL were insignificant between groups (P>0.05). After logistic regression analysis, the levels of serum 25(OH)D3, HDL, TG and LDL were closely related to the occurrence of osteoporosis (P<0.05). ROC curve indicated that the area under the curve (AUC), sensitivity and specificity of combined prediction of osteoporosis by serum 25(OH)D3, HDL, TG, and LDL were 0.943, 92.11% and 85.00%, and the efficiency of combined prediction was better than that of each index alone (P<0.05). Conclusion The levels of serum 25(OH)D3, HDL, TG and LDL in T2DM are closely related to osteoporosis. Early combined monitoring of the indicators can provide reference value for clinical prediction of osteoporosis occurrence in patients with T2DM.
2.Correlation analysis of inflammatory markers (NLR/PLR/SII) with the severity of intrauterine adhesions

ObjectiveTo investigate the correlation between neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), systemic immune-inflammation index (SII) and the severity of intrauterine adhesions (IUA). MethodsThe retrospective study included 380 patients who underwent transcervical resection of adhesions (TCRA) from December 2019 to March 2025. Based on the American Fertility Society (AFS) classification, patients were divided into mild (n=61), moderate (n=225), and severe (n=94) groups. NLR, PLR, and SII were calculated from preoperative blood tests. Statistical analyses included Kruskal-Wallis test and ordinal Logistic regression. ResultsNLR, PLR, and SII were significantly higher in the severe IUA group compared to the mild group (P<0.05), with SII showing the strongest predictive ability (OR=1.004, P=0.001). The number of intrauterine procedures was an independent risk factor (OR=1.27/level, P=0.016). The predictive model [Logit(P)=-0.676+0.241×operation times+0.004×SII] effectively identified severe IUA cases. ConclusionInflammatory markers (particularly SII) are correlated with IUA severity and may serve as non-invasive tools for clinical assessment.
3.Impact of Nutritional Support on Antitumor Efficacy in the Era of Immunotherapy

Despite breakthroughs in immunotherapy for solid tumors, significant variations in treatment efficacy persist. Up to 80% of cancer patients suffer from malnutrition, which leads to: lymphoid atrophy and reduced T-cell reserves; deficiency of substrates required for T-cell activation and expansion; concurrent inflammation hindering T-cell infiltration into tumors; and cachexia accelerating PD-1 antibody clearance. Clinical studies confirm that severe malnutrition significantly impairs immune responses and increases the risk of treatment toxicity. Therefore, implementing standardized nutritional therapy is crucial for optimizing the reserve, activation, expansion, and infiltration capacity of immune cells, thereby providing a sound immune system foundation for immunotherapy. Immunonutrition therapy, by enhancing immunonutrients such as arginine, omega-3 polyunsaturated fatty acids, and nucleotides, reduces the secretion of pro-inflammatory mediators and promotes T-cell activation and proliferation. This enhances anti-tumor immune responses, prolongs survival, and advances cancer treatment towards multimodal combination and precision approaches.
4.Effect of Ligustilide on Neutrophil Extracellular Traps in Rats with Cerebral Ischemia-reperfusion Injury

ObjectiveTo explore the possible mechanisms by which ligustilide (LIG) exerts neuroprotective effects on ischemic stroke (IS) by inhibiting the release of neutrophil extracellular traps (NETs), promoting blood-brain barrier repair, and alleviating post-ischemic neuroinflammation, thereby providing a new direction for IS treatment. MethodsA middle cerebral artery occlusion (MCAO) model was established in rats. The rats were divided into the sham operation (Sham) group, model (Model) group, low- and high-dose LIG groups (20, 40 mg·kg-1), and the NET inhibitor CI-amidine group (CI-amidine, 10 mg·kg-1). Drug treatments were administered for 3 days. Neurological injury after ischemia was evaluated by 2,3,5-triphenyltetrazolium chloride (TTC) staining, neurological deficit scoring, and brain index measurement. Flow cytometry and Western blot were used to analyze changes in neutrophil expression. Immunofluorescence was used to observe the fluorescence intensity of the NET marker citrullinated histone H3 (H3Cit). Western blot was performed to detect the expression of blood-brain barrier tight junction-related proteins and inflammatory factors, including interleukin-18 (IL-18) and interleukin-1β (IL-1β). ResultsCompared with the Sham group, the Model group exhibited significant brain tissue injury (P<0.05), significantly increased neutrophil numbers and NET expression (P<0.05), significantly impaired blood-brain barrier permeability (P<0.05), and significantly increased expression of inflammatory factors (P<0.05). Compared with the Model group, both low- and high-dose LIG significantly alleviated brain tissue injury in rats (P<0.01), inhibited neutrophil numbers and NET expression (P<0.01), reduced blood-brain barrier damage (P<0.01), and suppressed the expression of inflammatory factors IL-18 and IL-1β (P<0.01), thereby ultimately exerting a neuroprotective effect. ConclusionThe neuroprotective effect of LIG in rats with cerebral ischemia-reperfusion injury may be related to inhibition of neutrophils and the NETs induced by them.

6.Quality assessment of blood components prepared by an automatic whole blood processing system

Objective: To compare the Reveos automated blood processing system with the current method, and to evaluate the feasibility and validity of using the Reveos system for blood component preparation. Methods: Forty units of 400 mL whole blood samples were divided into two groups: 2C group (for two-component preparation) and 3C group (for three-component preparation). Each group was further divided into a Reveos subgroup and a control subgroup. Blood components were prepared using the Reveos system and the current centrifugation method respectively. The 2C group yielded suspended red blood cells and plasma, while the 3C group yielded suspended red blood cells, plasma, and platelets. Key quality indicators for red blood cells, plasma, and platelets were measured before and after separation. Inter-group differences were analyzed using SPSS 25.0. Results: The trend of changes in the main performance indicators of red blood cells, plasma, and platelets before and after separation was generally consistent between the Reveos group and the control group, with no significant differences for most performance indicators. The Reveos system outperformed the current method in several aspects: in the 3C group, the hematocrit (Hct) was significantly higher in the Reveos group than in the control group [(62.82%±1.64%) vs (53.62%±3.04)%, P<0.001]; the white blood cell count in red blood cell suspensions was significantly lower than that in the control group [(3.37±1.42)×10
/L vs (8.42±2.30)×10
/L, P<0.001]; plasma yield was 27.5% higher than that in the control group [(183.90±17.37) mL vs (144.28±20.53) mL, P<0.001]; and the platelet activation rate was significantly lower than that in the control group [(21.97±14.25)% vs (34.73±11.92)%, P=0.044]. Conclusion: The Reveos system demonstrates good consistency with the current method in preparing blood components, and outperforms the current method in terms of leukocyte reduction and red blood cell concentration.
7.The role of functional near-infrared spectroscopy in predicting postoperative delirium in elderly patients undergoing perioperative joint replacement surgery

ObjectiveTo explore the role of neuroimaging features monitored by functional near-infrared spectroscopy (fNIRS) in postoperative delirium (POD) in elderly patients undergoing joint replacement during perioperative period, and to provide a basis for early clinical prediction. MethodsA total of 105 elderly patients who underwent joint replacement under general anesthesia were included. The Mini-Mental State Examination (MMSE) scale was used to evaluate the patient's cognition one day before the operation. Before the start of the surgery, fNIRS was used to monitor the changes of cerebral blood oxygen saturation when the patient performed the task state. The 3-minute delirium diagnostic scale (3D-CAM scale) was used to evaluate the occurrence of POD at 24, 48 and 72 h after operation. Brain network analysis was performed and Logistic regression analysis was used to explore the relationship between fNIRS monitoring data and POD in elderly patients undergoing joint replacement surgery during preoperative task state. The receiver operating characteristic curve (ROC curve) was constructed to evaluate the diagnostic efficacy, and the Hosmer-Lemeshow goodness-of-fit test was used to test the goodness of fit of the model. ResultsAmong 105 patients, 100 cases were effectively analyzed, of which 20 cases (20%) had POD. Brain network analysis showed that the r value of functional connectivity correlation coefficient in POD group (0.069±0.118) was lower than that in non-POD group (0.073±0.084). The low channel connectivity of right primary somatosensory cortex-right primary motor cortex (RS1-RM1) and left anterior pole-right Broca's triangle (LFP-RBA44) was an important factor affecting the occurrence of POD (P < 0.05). Based on this result, the area under the ROC curve was 0.797 and 0.784, respectively. The results of Hosmer-Lemeshow goodness-of-fit test showed that the model fitted well (all P>0.5). ConclusionThe neuroimaging features extracted from the cerebral oxygen saturation data monitored by fNIRS are significantly correlated with the risk of POD in elderly patients undergoing joint replacement during perioperative period. Among them, the low connectivity of preoperative RS1-RM1 and LFP-RBA44 brain network channels is an important influencing factor of POD occurrence. Predicting the occurrence of POD based on fNIRS is conducive to the early intervention and risk reduction of perioperative complications, improving medical quality and promoting precision medical practice.
8.Effect of Zhinao Capsules on Mild and Moderate Sleep Disorders in Alzheimer's Disease Patients with Syndrome of Spleen-kidney Deficiency and Combined Phlegm and Stasis

ObjectiveTo observe the clinical efficacy and safety of Zhinao capsules in the treatment of mild and moderate sleep disorders in Alzheimer's disease patients with the syndrome of spleen-kidney deficiency and combined phlegm and stasis. MethodsA randomized, double-blind, placebo, controlled clinical trial design was adopted, and 60 Alzheimer's disease patients aged 50-85 years with mild and moderate sleep disorders due to spleen-kidney deficiency and combined phlegm and stasis were included. According to the randomized numble table method, the patients were assigned into a treatment group and a control group (30 in each group, with 29 patients in the treatment group and 28 patients in the control group finally completing the trial). The treatment group was treated with donepezil hydrochloride tablets combined with Zhinao capsules, and the control group was treated with donepezil hydrochloride tablets combined with the simulant of Zhinao capsules for 12 weeks. The Pittsburgh sleep quality index (PSQI), smart bracelet sleep monitoring, epworth sleeping scale (ESS), Mini-Mental State Examination (MMSE), Activities of Daily Living (ADL), and traditional Chinese medicine symptoms were scored. Furthermore, the brain-derived neurotrophic factor (BDNF) and gamma-aminobutyric acid (GABA) levels were measured. ResultsAfter 12 weeks of treatment, the total response rate of clinical efficacy assessment in the treatment group reached 89.7%, which was significantly higher than that (21.4%) in the control group (Z=-5.14, P<0.01). After treatment, the TCM symptom scores declined in both groups (P<0.01), and the scores in the treatment group were lower than those in the control group (P<0.01). After treatment, the treatment group showed decreased total score and seven component scores of the PSQI scale (P<0.05, P<0.01), and the scores in the treatment group were lower than those in the control group (P<0.05, P<0.01). After treatment, the treatment group showed increases in the duration of deep sleep, light sleep, rapid eye movement sleep, and total sleep at night and decreases in the time to fall asleep, the number of awakenings from sleep at night, and the time for sporadic daytime naps (P<0.01). The control group did not show significant changes in the duration of light sleep, time to fall asleep, and number of awakenings from sleep at night. Moreover, the treatment group outperformed the control group in terms of the indicators above (P<0.05, P<0.01). After treatment, the treatment group showed decreased scores of ESS, ADL and its subscales physical self-maintenance scale (PSMS), and Instrumental Activities of daily living scale (IADLs) and increased score of MMSE and levels of BDNF and GABA (P<0.05, P<0.01). There was no significant difference in PSMS between the two groups, and the treatment group was significantly better than the control group in terms of the rest of the above indicators (P<0.05, P<0.01). Patients in neither groups complained of uncomfortable symptoms. ConclusionZhinao capsules have significant clinical efficacy in treating patients with mild and moderate sleep disorders in Alzheimer's disease patients with the syndrome of spleen-kidney deficiency and combined phlegm and stasis. Zhinao capsules can significantly reduce PSQI, ESS, and ADL scores, increase the MMSE score and the expression of BDNF and GABA, effectively optimize the structure of sleep, improve the intelligence, and enhance the ability of daily life, without causing uncomfortable symptoms. The mechanism may be related to the increase in the expression of sleep/wake-related neurotransmitters.
9.Effectiveness and safety of coblopasvir hydrochloride combined with sofosbuvir in treatment of chronic hepatitis C

ObjectiveTo investigate the effectiveness and safety of coblopasvir hydrochloride combined with sofosbuvir (CLP/SOF) in the treatment of patients with chronic hepatitis C (CHC) and those with genotype 3b in terms of virologic response rate, changes in liver and renal function parameters, and adverse events, and to provide a reference for clinical diagnosis and treatment. MethodsThe patients with CHC who were admitted to Maanshan Fourth People’s Hospital from March 1, 2023 to July 31, 2024 were enrolled and treated with CLP/SOF for a course of 12 weeks. Follow-up assessments were conducted at weeks 4, 8, and 12 of treatment and at 12 weeks after the end of treatment, and related data were collected, including HCV RNA, routine blood test results, and liver and renal function parameters. Sustained virologic response at 12 weeks after treatment (SVR12) and safety profile were evaluated. The Friedman test was used for comparison of liver and renal function parameters, routine blood test results, and liver fibrosis score at different time points. ResultsA total of 107 CHC patients treated with CLP/SOF were enrolled, with an age of 22 — 92 years, and there were 55 male patients. The predominant genotypes were 1b (46.73%) and 3b (28.97%). Comorbidities mainly included liver cirrhosis (15 patients), fatty liver disease (27 patients), syphilis (15 patients), HBV infection (8 patients), and HIV infection (2 patients). The overall SVR12 rate was 100% (107/107), with an SVR12 rate of 100% for subgroups with different genotypes and those with liver cirrhosis, fatty liver disease, syphilis, HBV infection, or HIV infection. The patients with HCV genotype 3b comorbid with liver cirrhosis, fatty liver disease, syphilis, HBV infection, hypertension or diabetes also achieved an SVR12 rate of 100%. After CLP/SOF treatment, the patients showed significant reductions in the levels of the liver function parameters alanine aminotransferase, aspartate aminotransferase, total bilirubin, and hemoglobin (all P<0.001) and significant reductions in the liver fibrosis scores fibrosis-4 and aspartate aminotransferase-to-platelet ratio index (both P<0.001). The median creatinine level was 66 μmol/L before treatment, which decreased to 52 μmol/L at week 12 of treatment (P<0.001). No adverse events were reported. ConclusionPatients with CHC achieve a high SVR12 rate and significant improvements in liver function parameters and liver fibrosis markers after treatment with the CLP/SOF regimen, and this regimen has no significant impact on renal function and shows a favorable safety profile. Therefore, it holds promise for clinical application.
10.The development trajectory of sleep disturbance in patients with hepatolenticular degeneration and its relationship with depression:A cross-lagged regression analysis

Objective To investigate the changing trend of sleep disturbance in patients with hepatolenticular degeneration and the bidirectional relationship between sleep disturbance and depression through a cross-lagged regression analysis. Methods A total of 80 patients who met the diagnostic criteria for WD from January to June 2024 were enrolled in this longitudinal study and were followed up for 3 months.Pittsburgh Sleep Quality Index (PSQI) was used to assess sleep quality, and Beck Depression Inventory (BDI) was used to assess the severity of depression, at the time of enrollment (T0), at 1 month after enrollment (T1),and at 2 months after enrollment (T3). An unconditional growth model was used to analyze the trajectory of sleep disturbance, and a cross-lagged regression model was used to investigate the temporal relationship between sleep disturbance and depression. Results From T0 to T2, there were significant increases in PSQI and BDI scores in all WD patients(P<0.05). The variance of the intercept factor (the initial status of sleep disturbance) was estimated at 10.83(P<0.01),and the variance of the slope factor (the rate of change in sleep disturbance) was 1.20 (P<0.01),with a significant negative correlation between the intercept and the slope (r=-0.25,P<0.01). The correlation analysis of PSQI and BDI scores across the three time points revealed a positive correlation between PSQI and BDI scores (r∈[0.19,0.96],P<0.01).The cross-lagged model analysis showed that sleep disturbance significantly predicted subsequent depression (P<0.01),with standardized regression coefficients (β) of 0.392 and 0.347, respectively; meanwhile, depression also significantly predicted subsequent sleep disturbance (P<0.01), with β of 0.273 and 0.372, respectively. These findings suggested a bidirectional predictive relationship between sleep disturbance and depression in WD patients over time. Conclusion There is a bidirectional relationship between sleep disturbance and depression in patients with WD, and depression has a more pronounced influence on sleep disturbance. Therefore, clinical interventions should focus on both sleep and psychological state, and combined management should be performed to improve the effect of disease control.
