1.Research on classification of Korotkoff sounds phases based on deep learning

Objective To recognize the different phases of Korotkoff sounds through deep learning technology, so as to improve the accuracy of blood pressure measurement in different populations. Methods A classification model of the Korotkoff sounds phases was designed, which fused attention mechanism (Attention), residual network (ResNet) and bidirectional long short-term memory (BiLSTM). First, a single Korotkoff sound signal was extracted from the whole Korotkoff sounds signals beat by beat, and each Korotkoff sound signal was converted into a Mel spectrogram. Then, the local feature extraction of Mel spectrogram was processed by using the Attention mechanism and ResNet network, and BiLSTM network was used to deal with the temporal relations between features, and full-connection layer network was applied in reducing the dimension of features. Finally, the classification was completed by SoftMax function. The dataset used in this study was collected from 44 volunteers (24 females, 20 males with an average age of 36 years), and the model performance was verified using 10-fold cross-validation. Results The classification accuracy of the established model for the 5 types of Korotkoff sounds phases was 93.4%, which was higher than that of other models. Conclusion This study proves that the deep learning method can accurately classify Korotkoff sounds phases, which lays a strong technical foundation for the subsequent design of automatic blood pressure measurement methods based on the classification of the Korotkoff sounds phases.
2.Clinical significance of combined detection of serum SAA,CRP,PCT and TAP in early diagnosis and treatment of severe acute pancreatitis

Objective To analyze the changes of serum amyloid A (SAA) ,C reactive protein (CRP) ,procalcitonin(PCT) and trypsin activated peptide(TAP) in the patients with acute pancreatitis (AP) ,and to evaluate the above 4 indexes combined detection in the diagnosis and treatment of AP .Methods The levels of SAA ,CRP ,PCT and TAP were detected in 21 cases of severe AP (SAP) and 49 cases of mild AP(MAP) .The detection results were compared with those in the healthy control group (n=50) .Re‐sults The levels of SAA ,CRP ,PCT and TAP had statistical differences between the patients with acute stage of SAP and MAP groups with the healthy control group(P<0 .01);the levels of SAA ,CRP ,PCT and TAP in the SPA group were significantly high‐er than those in the MAP group with statistical difference(P<0 .01) .The diagnostic efficiency of 4‐index combined detection was higher than that of single index detection .Conclusion The combined detection of SAA ,CRP ,PCT and TAP is conducive to early diagnosis and disease condition judgement of AP ,and has an important significance to the diagnosis and treatment of SAP .
3.Effects of hyperbaric oxygen on serum levels of antioxidant in neonates with hypoxic ischemic encephalopathy

Objective To explore the effects of hyperbaric oxygen ( HBO) therapy on the serum levels of malondialdehyde (MDA), superoxide dismutase (SOD) , neuron specific enolase (NSE) and myelin basic protein (MBP) in neonates with hypoxic ischemic encephalopathy (HIE).Methods Ninety neonates with HIE were randomly divided into the HBO group ( n=45 ) and the conventional treatment group ( n =45 ) . Another 45 normal neonates borns at the same time were chosen as the control group .Serum levels of MDA , SOD, NSE and MBP were detected with enzyme immunoassay and chemical colorimetric assay , both before and after treatment.Changes in the scores of NBNA were evaluated both before and after treatment , and the relationship between NBNA and the above data was analyzed .Results Before HBO therapy , the MDA, NSE and MBP levels in the neonates with HIE were obviously higher than those of the control group ( P<0.05) [(4.48 ±0.94)nmol/L、(10.02 ±3.10)μg/L、(4.58 ±2.17)μg/L], and the level of SOD was noticeably lower that of the control group (P<0.05)[(137.65 ±34.54)μU/L].Following HBO therapy, the levels of SOD for the HBO group and the routine treatment group were increased , while the levels of MDA , NSE and MBP were decreased , and the scores of NBNA were elevated .Statistical significance could be noted , when compared with those before treatment (P<0.05).More significant changes in the above parameters could be noted in the moderate and severe patients of the HBO group , and statistical significance could be found , when compared with those of the routine treatment group (P<0.01).Results showed that the levels of MBP and NBNA were well correlated .Conclusions HBO therapy could improve the antioxidant capacity of the neonates with HIE and decrease lesion induced by brain tissue lipid peroxidation .Combined detection of MDA , SOD, NSE and MBP was beneficial to the assessment and judgment of the patients'conditions .

5.Clinical significance of the changes of plasma homocysteine,endothelin,mononitrogen monoxide,folic acid and vitamine B_(12) in acute cerebral infarction

Objective To detect the fluctuations of homoeysteine(Hcy),endothelin(ET),mononitrogen monoxide(NO),folic acid(FA) and vitamine Bi2 (VitB_(12)) in the plasma of patients suffered from acute cerebral infarction and to investigate the clinical significance.Methods 140 blood specimens from cerebral infarction patients and 61 specimens from healthy individuals were obtained in this study and a series of items of Hey,ET,NO,FA and VitB_(12) were detected.Results The Hey [(17.15±4.63) μmol/L]and ET [(32.62±18.76) μmol/L]were increased in acute cerebral infarction(ACI) group compared with that in control group[(10.12±2.62) μmol/L,(20.08±13.38) μmol/L respectively]and the difference was significant (P<0.01).The levels of NO [(39.82±7.26) μmol/L],FA [(7.54±4.01) ng/ml]and VitB_(12) [(247.70±119.85) pg/ml]were decreased compared with control group [(60.22±6.28) μmol/L,(10.08±3.86) ng/ml,(411.52±194.58) pg/ml respectively]and the differences significant (P<0.01).Gender and hypertension affected the values of Hey.Conclusions Hcy and ET were increased while NO,FA and VitB_(12) were decreased in acute cerebral infarction.Hcy had positive correlation with ET and negative correlation with FA and VitB_(12).
6.Clinical value of combined detection of CA72-4,CA19-9 in gastric juice in diagnosis of gastric carcinoma

Objective To explore the possibility of united test of CA72-4, CA19-9 in gastric juice in diagnosis of gastric carcinoma.Methods The gastric juices were respectively obtained by liquid collector made by ourselves through gastreecope in study group(30 cases of gastric cancer) and control groups(32 cases of gastratrophia,30 cases of gastric ulcer,and 30 cases of chronic superficial gastritis).The gastric juice content of CA72-4, CA19-9 were determined synchronously with ES-300 auto-enzymoimmuno-analyzer.Results The gastric juice contents of CA72-4,CA19-9 were (83.54±143.70) U/ml and (47.05±29.64) U/ml, and the positive rates were 76.66% and 43.33% in study group,respectively,and all higher than those of control groups(P <0.01 ).The level of CA72-4 was high in two of 32 patients with gastratrophia,and one was diagnosed as gastric carcinoma after nine months follow-up.Conclusion The combined detection of CA72-4 and CA19-9 provide main evidence for the diagnosis of gastric cancer and diagnostric significance of advanced gastric cancer.
7.Plasma endothelin in acute cerebral infarction and its change after nimodipine treatment

AIM: To explore the role of plasma endothelin (ET) in patients with acute cerebral infarction (ACI) and its change after nimodipine treatment.METHODS:Sixty-six patients with ACI were randomly divided into 2 groups according to blood pressure:hypertension ACI group 35 patients[M 20,F 15;age (65± s 11) a] and pure ACI group 31 patients[M 17,F 14; age (62±10) a].Plasma ET was measured by radioimmunoassay and compared with 27 healthy individuals.Among them,42 patients(hypertension ACI group 25 patients,pure ACI group 17 patients) with ACI were given nimodipine 4 mg, iv,gtt, qd, for 2 wk and measured ET again. RESULTS:Plasma ET in 2 groups were (144±42) ng*L-1 and (72±35) ng*L-1 respectively, and that were (94±55) ng*L-1 and (60±37) ng*L-1 respectively after treatment with nimodipine, but was still higher than that in healthy individuals (P<0.01). CONCLUSION: The level of plasma ET was associated with blood pressure and the severity of disease. ET is closely related with ACI. Nimodipine reduces the plasma ET while nerve function default degree is effectively improved.
