1.Correlation of levels of serum Visfatin, CXCL12 and Sirt1 with carotid atherosclerosis in patients with T2DM

Objective To investigate the correlation between serum Visfatin, CXC chemokine 12 (CXCL12) and silent information regulator 1 (Sirt1) levels and carotid atherosclerosis (CAS) in patients with type 2 diabetes mellitus (type 2 diabetes mellitus ,T2DM). Methods Four hundred and ninety-five patients with T2DM in the hospital from July 2021 to June 2023 were selected as the observation group, and 50 healthy volunteers were included in the control group. The levels of serum Visfatin, CXCL12 and Sirt1 were detected, and the above levels were compared between groups. The patients in the observation group were divided into simple T2DM group and T2DM with CAS group by means of the results of carotid ultrasound examination, and the clinical data were compared. Multivariate Logistic regression analysis was performed to analyze the factors affecting the occurrence of CAS in T2DM patients. Spearman correlation analysis of serum Visfatin, CXCL12 and Sirt1 levels and severity of CAS was analyzed by Spearman correlation analysis. Results Compared with the control group, the levels of serum Visfatin and CXCL12 in the observation group were higher (t=14.524, t=11.536, all P<0.05) while the level of Sirt1 was lower (t=21.912, P<0.05). There were statistical differences in age, body mass index (BMI), FBG, HbAlc, FINS, TG, LDL-C, Visfatin, CXCL12 and Sirt1 between T2DM with CAS group and simple T2DM group (P<0.05). Multivariate analysis suggested that age (OR=2.155), FBG (OR=2.563), HbAlc (OR=2.472), FINS (OR=0.438), TG (OR=2.492), LDL-C (OR=2.445), Visfatin (OR=2.404), CXCL12 (OR=2.214) and Sirt1 (OR=0.398) were the influencing factors of CAS in patients with T2DM (P<0.05). The levels of serum Visfatin and CXCL12 were positively correlated with the severity of CAS (r=0.574, r=0.530, P<0.05), and the level of Sirt1 was negatively correlated with the severity of CAS (r=-0.621, P<0.05). Conclusion Serum Visfatin, CXCL12 and Sirt1 in T2DM patients are related to the occurrence and severity of CAS. Visfatin, CXCL12 and Sirt1 may be involved in the occurrence and development of CAS in T2DM patients.
2.Pyronaridine combined with diminazene aceturate inhibits Babesia in vitro and in vivo

Objective: To evaluate the combination therapy of pyronaridine tetraphosphate and diminazene aceturate against Babesia in vitro and in vivo. Methods: Bioinformatic analysis was performed using atom pair fingerprints. An in vitro combination test was performed against Babesia bovis and Theileria equi. Moreover, the in vivo chemotherapeutic efficacy of pyronaridine tetraphosphate in combination with diminazene aceturate was investigated against the growth of Babesia microti in mice using a fluorescence inhibitory assay. Results: Pyronaridine tetraphosphate and diminazene aceturate exhibited nearly similar molecular weights. The in vitro combination of pyronaridine tetraphosphate and diminazene aceturate was synergistic on Babesia bovis and additive on Theileria equi. In addition, 5 mg/kg pyronaridine tetraphosphate combined with 10 mg/kg diminazene aceturate inhibited Babesia microti growth significantly compared with those observed after treatment with 25 mg/kg diminazene aceturate alone from day 6 post treatment to day 12 post treatment. The combination therapy also normalized the hematological parameters of infected mice. Conclusions: An oral dose of pyronaridine tetraphosphate combined with a subcutaneous dose of diminazene aceturate inhibits Babesia in vitro and in mice, suggesting it might be a new paradigm for the treatment of babesiosis.
3.Association between prosthesis use to the physical functioning and quality of life among adult unilateral lower limb amputees: An analytical cross-sectional study

Introduction:
A prosthesis is an artificial assistive device designed to replace a missing body part (e.g., limb), secondary to a disease, injury or congenital deformities. Prostheses are often used to restore
functional capacity while improving the quality of life (QoL).
Methods:
An analytical cross-sectional study was conducted among adults aged 19 to 64 years who were
permanent residents in Luzon who had undergone unilateral lower leg amputation. This epidemiologic
study employed the Modified Barthel Index for activities of daily living (ADLs), the Frenchay Activities
Index for instrumental activities of daily living (IADLs), and the WHOQOL-BREF for QoL. Descriptive
and analytical statistics of the responses of the Lower Limb Amputees (LLA) were done. Prevalence odds
ratio (POR) was calculated, and statistical significance was determined.
Results:
Among 165 LLA respondents, only 47.88% used prostheses. Unilateral LLA who exhibited
greater independence in ADLs (POR=19.22), more actively performed IADLs (POR=5.51), and had good
QoL (POR=3.83) were more likely to have been using prosthesis. All these findings were statistically
significant.
Conclusion
This study revealed a statistically significant association between prosthesis use to the
physical functioning and QoL among adult unilateral LLAs. It showed that prosthetic use was likely linked
with improved performance in ADLs and IADLs, and better QoL compared to those without.

WSWs have the potential to reliably and continuously screen for AFib and detect it in a timely manner.The inconclusive results produced by WSWs are a significant problem. Once the inconclusive results are rectified, WSWs may be used for widespread screening of AFib in those people who are at high risk of developing AFib.

Objective:
Pain is one of the most common nonmotor symptoms in Parkinson’s disease (PD), with variable characteristics among populations. This multicenter Egyptian study aimed to translate and validate the King’s Parkinson’s Disease Pain Scale (KPPS) and Questionnaire (KPPQ) into Arabic versions and to investigate the pain characteristics in Egyptian people with PD (PWP).
Methods:
A total of 192 PWP and 100 sex- and age-matched controls were evaluated by the KPPS-Arabic and KPPQ-Arabic. Both tools were assessed for test–retest reliability, floor or ceiling effects, construct validity and convert validity. PWP were also assessed by the Movement Disorders Society-Unified Parkinson’s Disease Rating Scale (MDS-UPDRS), Hoehn and Yahr scale (H&Y), Non-Motor Symptom Scale (NMSS), PD Questionnaire-39, and Non-Motor Fluctuation Assessment (NoMoFA).
Results:
The KPPS-Arabic and KPPQ-Arabic showed inter- and intrarater consistency and high validity, with an acceptable ceiling effect. A total of 188 PWP (97.9%) reported at least 1 type of pain (p < 0.001). The severity and prevalence of all pain domains in the KPPS-Arabic were significantly higher among PWP than among controls (p < 0.001). Fluctuation-related and musculoskeletal pains were the most common (81.3% and 80.7%, respectively). In the PD group, the total and domains of KPPS-Arabic were significantly correlated to the MDS-UPDRS total score and the scores of Parts I, II, III, postural instability gait disorder, axial, and H&Y but not with age or age of onset. The predictors of KPPS-Arabic scores included the total MDS-UPDRS, the part III-OFF, disease duration, the total NMSS, and the NoMoFA scores.
Conclusion
The current multicenter study provided validated Arabic versions of the KPPS and KPPQ, which exhibited high reliability and validity, and demonstrated a high prevalence and severity of pain within Egyptian PWP and characterized its determinants.

Background:
and Purpose Outcomes following mechanical thrombectomy (MT) are strongly correlated with successful recanalization, traditionally defined as modified Thrombolysis in Cerebral Infarction (mTICI) ≥2b. This retrospective cohort study aimed to compare the outcomes of patients with low Alberta Stroke Program Early Computed Tomography Score (ASPECTS; 2–5) who achieved mTICI 2b versus those who achieved mTICI 2c/3 after MT.
Methods:
This study utilized data from the Stroke Thrombectomy and Aneurysm Registry (STAR), which combined databases from 32 thrombectomy-capable stroke centers between 2013 and 2023. The study included only patients with low ASPECTS who achieved mTICI 2b, 2c, or 3 after MT for internal carotid artery or middle cerebral artery (M1) stroke.
Results:
Of the 10,229 patients who underwent MT, 234 met the inclusion criteria. Of those, 98 (41.9%) achieved mTICI 2b, and 136 (58.1%) achieved mTICI 2c/3. There were no significant differences in baseline characteristics between the two groups. The 90-day favorable outcome (modified Rankin Scale score: 0–3) was significantly better in the mTICI 2c/3 group than in the mTICI 2b group (adjusted odds ratio 2.35; 95% confidence interval [CI] 1.18–4.81; P=0.02). Binomial logistic regression revealed that achieving mTICI 2c/3 was significantly associated with higher odds of a favorable 90-day outcome (odds ratio 2.14; 95% CI 1.07–4.41; P=0.04).
Conclusion
In patients with low ASPECTS, achieving an mTICI 2c/3 score after MT is associated with a more favorable 90-day outcome. These findings suggest that mTICI 2c/3 is a better target for MT than mTICI 2b in patients with low ASPECTS.

Background:
and Purpose The management of acute ischemic stroke (AIS) due to distal medium vessel occlusion (DMVO) remains uncertain, particularly in comparing the effectiveness of intravenous thrombolysis (IVT) plus mechanical thrombectomy (MT) versus IVT alone. This study aimed to evaluate the safety and efficacy in DMVO patients treated with either MT-IVT or IVT alone.
Methods:
This multinational study analyzed data from 37 centers across North America, Asia, and Europe. Patients with AIS due to DMVO were included, with data collected from September 2017 to July 2023. The primary outcome was functional independence, with secondary outcomes including mortality and safety measures such as types of intracerebral hemorrhage.
Results:
The study involved 1,057 patients before matching, and 640 patients post-matching. Functional outcomes at 90 days showed no significant difference between groups in achieving good functional recovery (modified Rankin Scale 0–1 and 0–2), with adjusted odds ratios (OR) of 1.21 (95% confidence interval [CI] 0.81 to 1.79; P=0.35) and 1.00 (95% CI 0.66 to 1.51; P>0.99), respectively. Mortality rates at 90 days were similar between the two groups (OR 0.75, 95% CI 0.44 to 1.29; P=0.30). The incidence of symptomatic intracerebral hemorrhage was comparable, but any type of intracranial hemorrhage was significantly higher in the MT-IVT group (OR 0.43, 95% CI 0.29 to 0.63; P<0.001).
Conclusion
The results of this study indicate that while MT-IVT and IVT alone show similar functional and mortality outcomes in DMVO patients, MT-IVT presents a higher risk of hemorrhagic complications, thus MT-IVT may not routinely offer additional benefits over IVT alone for all DMVO stroke patients. Further prospective randomized trials are needed to identify patient subgroups most likely to benefit from MT-IVT treatment in DMVO.

Purpose:
This study aimed to evaluate the anatomic circle around the impacted lower third molar to show, document,and correlate essential findings that should be included in the routine radiographic assessment protocol as clinically meaningful factors in overall case evaluation and treatment planning.
Materials and Methods:
Cone-beam computed tomographic images of impacted lower third molars were selected according to specific inclusion criteria. Impacted teeth were classified according to their position before assessment. The adjacent second molars were assessed for distal caries, distal bone loss, and root resorption. The fourth finding was the presence of a retromolar canal distal to the impaction. Communication with the dentist responsible for each case was done to determine whether these findings were detected or undetected by them before communication.
Results:
Statistically significant correlations were found between impaction position, distal bone loss, and detected distal caries associated with the adjacent second molar. The greatest percentage of undetected findings was found in the evaluation of distal bone status, followed by missed detection of the retromolar canal.
Conclusion
The radiographic assessment protocol for impacted third molars should consider a step-by-step evaluation for second molars, and clinicians should be aware of the high prevalence of second molar affection in horizontal and mesioangular impactions. They also should search for the retromolar canal due to its associated clinical considerations.(Imaging Sci Dent 2023; 53: 137-44)
9.Molecular prevalence of Anaplasma marginale in ruminants and Rhipicephalus ticks in northern Pakistan

Anaplasma marginale is the most prevalent tick-borne haemoparasite of cattle and causes huge
economic losses to the dairy industry worldwide. This study aimed to determine the occurrence of A.
marginale infection in blood and tick samples collected from livestock animals in the districts located
in Khyber Pakhtunkhwa (KPK), Pakistan. A total of 184 blood and 370 tick samples were included in this
study. It has never been reported that sheep, goats, and cattle in Tank, Ghulam Khan, Birmil and Miran
Shah areas were infected with A. marginale. All samples of blood and ticks were collected through
random sampling from March 2021 to January 2022 from cattle, sheep and goats and screened through
PCR for anaplasmosis by using primer pairs of Anaplasma spp. Three hundred and seventy ticks were
collected from infested hosts (120/184, 64.21%). Among the four morphologically identified tick species,
the highest occurrence was recorded for Rhipicephalus sanguineus (n=138, 37.29%), followed by
Rhipicephalus microplus (n=131, 35.4%), Rhipicephalus annulatus (n=40, 10.81%), Hyalomma anatolicum
(n=31, 8.37%), and Hyalomma marginatum (n=30, 8.1%). The occurrence of female tick was highest
(n=160, 43.24%), followed by nymphs (n=140, 37.38%) and males ticks (n=70, 18.9%). Among these ticks,
A. marginale was detected in female ticks of R. microplus, and R. sanguineus. Molecular identification
of A. marginale was confirmed in 120 out of 184 blood samples and 6 out of 74 tick samples. Overall,
occurrence of A. marginale in blood and tick samples was found to be 65.21% and 8.1% respectively.
Species-wise occurrence in blood samples of goats were 71.11% followed by sheep 68.31% and cattle
50%. Specie-wise occurrence of A. marginale in tick samples of cattle were 12.5% followed by goats
6.89%. The obtained sequence showed similarity with A. marginale reported from Kenya and USA. We
report the first PCR based detection of A. marginale infection in blood samples and in R. sanguineus
ticks of goats simultaneously.
10.How the Commonwealth of the Northern Mariana Islands stalled COVID-19 for 22 months and managed its first significant community transmission

Objective: The Commonwealth of the Northern Mariana Islands (CNMI) is a remote Pacific island territory with a population of 47 329 that successfully prevented the significant introduction of coronavirus disease (COVID-19) until late 2021. This study documents how the response to the introduction of COVID-19 in CNMI in 2021 was conducted with limited resources without overwhelming local clinical capacity or compromising health service delivery for the population.
Methods: Data from COVID-19 case investigations, contact tracing, the Commonwealth’s immunization registry and whole genome sequencing were collated and analysed as part of this study.
Results: Between 26 March 2020 and 31 December 2021, 3281 cases and 14 deaths due to COVID-19 were reported in CNMI (case fatality rate, 0.4%). While notification rates were highest among younger age groups, hospitalization and mortality rates were disproportionately greater among those aged >50 years and among the unvaccinated. The first widespread community transmission in CNMI was detected in October 2021, with genomic epidemiology and contact tracing data indicating a single introduction event involving the AY.25 lineage and subsequent rapid community spread. Vaccination coverage was high before widespread transmission occurred in October 2021 and increased further over the study period.
Discussion: Robust preparedness and strong leadership generated resilience within the public health sector such that COVID-19 did not overwhelm CNMI’s health system as it did in other jurisdictions and countries around the world. At no point was hospital capacity exceeded, and all patients received adequate care without the need for health-care rationing.
