Histoid leprosy is a clinical entity with characteristics clinical, immunologic and bacteriologic findings. It has been reported in multibacillary patients who has been on inadequate dapsone monotherapy and has been documented to arise de novo. A 30-year-old man presented with multiple yellowish-brown and erythematous well-defined nodules and papules, and a few erythematous sharply demarcated plaques. Histologic findings were consistent with lepromatous leprosy, histoid variant. The patient was started on a 12-month multidrug therapy regimen for multibacillary leprosy.
