1.Urinary microalbumin to creatinine ratio and α1-microglobulin to creatinine ratio and their influencing factors in people aged ≥40 years old in 10 counties of Shanxi province

Objective:To investigate urinary microalbumin to creatinine ratio (ACR) and α1-microglobulin to creatinine ratio (MCR) of people aged 40 years old and above in Shanxi province, and analyze the influencing factors of abnormal ACR and MCR, and to provide evidence for the prevention and control of chronic kidney diseases.Methods:It was a cross-sectional study. The data came from a screening study of chronic kidney diseases conducted by Shanxi Provincial People's Hospital from April to November 2019, involving aged 40 years old and above from 10 counties (Ningwu county, Yu county, Yangqu county, Lin county, Shouyang county, Zezhou county, Huozhou city, Hejin city, Linyi county and Ruicheng county) in Shanxi province. The related data were collected through questionnaire surveys, physical examinations, and blood and urine sample collection. Urinary α1-microglobulin, creatinine, and microalbuminuria were measured. Urinary ACR and MCR were calculated using urinary creatinine correction. ACR abnormality was defined as ≥30 mg/g, and MCR abnormality was defined as >23 mg/g. Covariate analysis was used to control confounding factors, and adjusted urinary ACR and MCR of 10 counties were calculated. Spearman correlation analysis and chi-square test were performed to analyze the factors associated with abnormal urinary ACR and MCR. Logistic regression analysis model was used to identify the influencing factors of abnormal urinary ACR and MCR.Results:A total of 12 285 residents were enrolled in the study, including 5 206 males (42.4%) and 7 079 females (57.6%). The median age was 58.0 (51.0, 66.0) years old. The median urinary ACR was 7.5 (4.5, 15.7) mg/g, and the median urinary MCR was 10.2 (6.4, 16.2) mg/g. A total of 1 572 individuals (12.80%) had urinary ACR abnormality and 1 450 individuals (11.80%) had urinary MCR abnormality. Yangqu county, Yuxian county, and Ningwu county had higher urinary ACR with (35.58±3.04) mg/g, (34.08±4.50) mg/g and (32.09±3.19) mg/g, respectively. The urinary MCR was generally similar among the 10 counties and Yangqu county had higher urinary MCR with (13.86±0.41) mg/g. In addition to Yu county, female individuals had higher urinary ACR compared to males in other counties, whereas female individuals had lower urinary MCR compared to males in 10 counties. Multivariate logistic regression analysis results showed that elevated triglyceride, fasting blood glucose, glycated hemoglobin, systolic blood pressure, diastolic blood pressure, age, body mass index and gender were independent influencing factors of abnormal urinary ACR and MCR (all P<0.05). Elevated blood homocysteine and low educational level were independent influencing factors of urinary MCR abnormality (both P<0.05). Conclusions:There are differences of gender and region in urinary ACR and MCR among individuals aged 40 years old and above in the 10 counties of Shanxi province. Triglyceride, fasting blood glucose, glycated hemoglobin, systolic blood pressure, diastolic blood pressure, age, gender, and body mass index are independent related factors of abnormal urinary ACR and MCR. Blood homocysteine and education level are independent related factors of abnormal urinary MCR.
2.Promotion effects of trans-regional review system on rational drug use in second-grade general hospital

OBJECTIVE To explore the role of the establishment of the trans-regional review system on rational drug use in second-grade general hospital ,and to promote the rational drug use. METHODS With the administrative support of Jiuquan Health Commission,5 second-grade general hospitals in Jiuquan Medical Association jointly established Jiuquan Rational Drug Use Review Training Center . A trans-regional review system was established to carry out cross-review of prescriptions and medical orders among 5 second-grade general hospitals. Totally 1 500 prescriptions and 900 medical records were collected from 5 second-grade general hospitals before (April to June ,2020)and after (July to September ,2020)the implementation of the project. The changes of rational drug use indicators ,the results of prescriptions and medical order review were investigated before and after the implementation of the project. RESULTS After the implementation of the project ,except for one hospital ,the proportion of drugs in other hospitals decreased to varying degrees with the highest decline rate of 22.56% . Compared with before the implementation of the project ,reasonable rate of outpatient and emergency prescription review increased by 5.72% averagely and the reasonable rate of medical order review increased by 10.10%(P<0.05). The average utilization rate of antibiotics in outpatients decreased by 14.45%,the average utilization rate of antibiotics in inpatients decreased by 7.98%,and the average use intensity of antibiotics decreased by 25.19%. CONCLUSIONS Through the establishment of trans-regional review system ,medical institutions can be forced to pay more attention to prescription review ,effectively improve the prescription review of pharmacists in medical institutions,and promote the rational use of drugs in medical institutions in the region. However ,there are still some problems , such as incomplete system coverage ,insufficient work experience ,lagging of informatization ,uneven pharmacist level ,and insufficient assessment and supervision of administrative functional departments.
3.Effect of increased mitophagy of chondrocytes in tibial growth plate of young rats with chronic renal failure on apoptosis

Objective:To explore the level of tibial growth plate chondrocyte mitophagy in young rats with chronic renal failure (CRF) and its effect on chondrocyte apoptosis.Methods:Male 4-week-old Sprague-Dawley rats were randomly divided into two groups according to random number table method: normal control group ( n=20, intragastric administration with distilled water) and CRF group ( n=20, given adenine suspension 150 mg·kg -1·d -1). All the young rats were sacrificed after continuous gavage for 6 weeks. The length of tibia was measured on X ray film, the width of tibia growth plate was measured and compared on histological section, and the apoptosis rate of chondrocytes in growth plate was detected by terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling (TUNEL) assay. The growth plate chondrocytes of two groups were isolated and cultured to the third generation in vitro, and the apoptosis rate of chondrocytes was detected by TUNEL assay. The co-localization of mitochondria and autophagy lysosomes in chondrocytes was observed by double fluorescence staining. Western blotting was used to detect the level of mitochondrial marker protein translocate of the outer mitochondrial membrane-20 (Tom-20) and autophagy marker light chain-3 protein (LC-3). The mitophagy of growth plate chondrocytes was observed by transmission electron microscope. Results:Compared with the normal control group, the tibia length of CRF group was shorter [(27.32±5.81) mm vs (35.43±3.61) mm, t=5.226, P<0.001], and the relative width of growth plate in histological section was narrower (0.56±0.19 vs 1.00±0.21, t=6.744, P<0.001). The apoptosis rate of chondrocytes in growth plate in CRF group was higher than that in the normal control group (17.2%±4.8% vs 5.1%±3.4%, t=6.505, P<0.001). The apoptosis rate of chondrocytes cultured in vitro in CRF group was higher than that in the normal control group (11.8%±6.2% vs 3.1%±1.2%, t=4.357, P<0.001). The result of double influorescence staining showed that there was co-localization between mitochondria and autophagy lysosomes in CRF group. Western blotting results showed that the levels of LC-3 protein ( t=8.944, P<0.001) and Tom-20 protein ( t=6.708, P<0.001) in CRF group were lower than those in the normal control group. Conclusion:The level of tibial growth plate chondrocyte mitophagy in young rats with CRF increases, which will lead to a decrease in the number of mitochondria, an increase in the apoptosis and a decrease in the number of chondrocytes, and eventually lead to dysplasia of tibia.
4.Role of the NLRP3 inflammatory signaling pathway in promoting neointimal hyperplasia associated with chronic renal disease

Objective:To investigate the role and mechanism of Nod-like receptor protein 3 (NLRP3) in chronic kidney disease (CKD)-related neointimal hyperplasia (NH) of vessels.Methods:Wild type C57BL/6J male mice were randomly divided into normal control group ( n=6) and experimental group ( n=18), by removal of 5/6 kidney and ligation of left common carotid artery to establish a NH model. After established successfully, the mice in NH experimental group were randomly divided into NH model group, NLRP3 inhibitor group, and drug control group ( n=6/group). C57BL/6J male mice with NLRP3 gene knockout group did not do any treatment after the establishment of NH model. After 3 weeks of feeding, the blood and vascular tissue samples of mice were collected. The pathological changes of vascular tissue samples in mice were observed by hematoxylin-eosin staining. The expressions and localization of NLRP3-related protein were observed by immunofluorescence staining. The expression of NLRP3 mRNA in vascular tissue was detected by quantitative real-time PCR. The activity of caspase-1 in vascular tissue was measured by colorimetric method. Human aortic smooth muscle cells (HASMCs) were treated with 10% uremic serum to simulate the body's internal environment during the uremic phase. NLRP3 small interfering RNA (siRNA) was transfected or NLRP3 inhibitor glibenclamide was added to the cell cultures. The expression of NLRP3 mRNA in HASMCs was detected by quantitative real-time PCR. The activity of caspase-1 in HASMCs was detected by colorimetric method. Results:Compared with the control group, the levels of serum creatinine and blood urea nitrogen were significantly increased in the NH model group (both P<0.01). The vascular histopathology showed that vascular intima thickened, vascular smooth muscle cells proliferated and hypertrophied, nuclei were deeply stained, and cells arranged disorderly and migrated to vascular intima in the experimental group. Quantitative analysis showed that the ratio of neointima to lumen increased significantly in the NH model group than that in control group ( P<0.01). Compared with the control group, the immunofluorescence staining of vascular tissue showed that the expressions of NLRP3, caspase-1, IL-18, IL-1β and proliferating cell nuclear antigen (PCNA) protein in the NH model group increased (all P<0.01), while the expression of α-SMA decreased ( P<0.01). NLRP3 was mainly located in vascular smooth muscle cells (VSMCs). VSMCs showed a synthetic phenotype. Compared with the NH model group, the expression of NLRP3, caspase-1, IL-18, IL-1β and PCNA protein in the NLRP3 inhibitor group and NLRP3 gene knockout group decreased (all P<0.01), the expression of α-SMA increased ( P<0.01), and the pathological changes of blood vessels alleviated. Compared with healthy serum group, the expression of NLRP3, IL-18, and IL-1β and bromodeoxyuridine (BrdU) uptake in uremic serum-stimulated group were increased (all P<0.01). After transfection of NLRP3 siRNA and addition of glibenclamide, the expression of NLRP3, IL-18, and IL-1β in VSMCs in uremic serum-stimulated group decreased, and BrdU intake decreased (all P<0.01). Conclusions:NLRP3 inflammatory bodies play an important role in promoting CKD-related neointimal hyperplasia of vessels, and glibenclamide can effectively reduce neointimal hyperplasia.
5.Ethical Problems and Countermeasures of Medical Support at Chinese Second-Grade Peacekeeping Hospital in Democratic Republic of the Congo

The particularity and complexity of medical support in the United Nations peacekeeping operations bring various ethical problems to medical staff of the second-grade peacekeeping hospitals during their work.This paper analyzed the ethical dilemma in terms of the relevant provisions of the United Nations,the medical humanitarianism principle,lack of medical resources and the principle of fairness,emergency treatment and informed consent,cultural differences and the principle of optimal health,occupational protection requirements and respect to the human rights.In addition,it discussed the countermeasures from personnel selection,the laws and regulations,the training of medical norms and foreign customs,the concept of “patient-centered”,and the humanitarian treatment,providing a theoretical basis for solving the problems of medical support in peacekeeping operations.

7.Influence of social support and coping style on chronic post-traumatic stress disorder after floods

Objective To explore the long-term prognosis and influence of social support and coping style of patients with post-traumatic stress disorder (PTSD) after suffering from floods.Methods Patients suffered PTSD due to Dongting lake flood in 1998 were selected through cluster random sampling.PTSD scale civilian version (PCL-C) was used to examine and diagnose the participants in this study.PTSD was then evaluated by the social support rating scale (SSRS) and the simple coping style questionnaire (SCSQ).Results Among all the 120 subjects,14(11.67%) of them were diagnosed as having PTSD.Compared with the rehabilitation group,scores on subjective support,objective support,total social support and positive coping,total of coping style from the nonrehabilitation group all appeared significant low (P<0.05).Data from the multivariate logistic regression showed that social support (OR=0.281,95%CI:0.117-0.678) and coping style (OR=0.293,95% CI:0.128-0.672) were protective factors of the chronic PTSD after the floods while disaster experience (OR=1.626,95%CI:1.118-2.365) appeared as a risk factor.Conclusion Chronic PTSD developed after the floods called for attention.Better social support,positive coping style could significantly improve the long-term prognosis of patients with PTSD after the floods.
8.Changes in expressions of lipoxin A4 and its receptor in rats with fat embolism syndrome

Objective To investigate the change in expressions of lipoxin A4 (LXA4) and lipoxin A4 receptor (ALX) in rats with fat embolism syndrome(FES).Metbods Sixty healthy male SD rats were assigned to control group and FES group which was subgrouped at l,6,12 and 24 h according to the random number table,with 12 rats each.Allogeneic perinephric fat (0.706 ml/kg) was injected to rat caudal veins in FES group.Instead isotonic saline in an equal volume was given to rats in control group.Lung samples were harvested from each group to detect pathological morphology,concentration of total protein and LXA4 in bronchoalveolar lavage fluid (BALF),lung weight to dry ratio (W/D),and activity of myeloperoxidase (MPO) and ALX mRNA.Additional 40 SD rats were divided into control group,FES 24-hour group,BML-1 11 + FES 24-hour group,and Boc-2 + FES 24-hour group according to the random number table,with 10 rats each.Pathology of lung tissue was observed using microscopy and expression of lung MPO mRNA was detected.Results Lung tissues in FES group were seriously injured compared with control group.Total protein concentration in BALF was (71.12 ± 11.05) μg/ml in FES 12-hour group,significantly increased compared to (29.82 ± 0.64) μg/ml in control group (P < 0.05).LXA4 concentration in BALF was (2.72 ± 0.24) ng/ml in FES 24-hour group,significantly higher than (0.69 ±0.05)ng/ml in control group (P < 0.05).Lung W/D value was 9.13 ±0.83 and 9.60 ±0.86 respectively in FES 6-hour and 12-hour groups,higher than 3.09 ±0.10 in control group (P <0.05).Activity of MPO in lung tissue was (0.74± 0.07)U/g and (0.53 ±0.08)U/g respectively in FES 6-hour and 12-hour groups,significantly higher than (0.19 ± 0.03) U/g in control group (P < 0.05).Expression of ALX mRNA was 3.99 ± 1.09 in FES 24-hour group,significantly higher than 1.00 ±0.21 in control group (P <0.05).Expression of MPO mRNA was lower in BML-111 + FES 24-hour group (0.69-0.08) and was higher in Boc-2 + FES 24-hour group (2.05-0.14),when compared to 1.52 ±-0.07 in FES 24-hour group (P<0.05).Conclusion LXA4 mainly involves in the resolution of inflammation in FES rats,which may be achieved at least in part by binding to ALX.
9.Optimization of triple plasmids transfection into HEK293 cells mediated by polyethylenimine.

In the present study, packaging system composed of pAAV-CMV-GFP, pAAV-RC and pHelper were transfected into human embryonic kidney 293 cells (HEK293 cells) mediated by polyethyleneimine (PEI) to explore an optimal transfection condition. Different total plasmid DNA dosages (1, 2, 3, 4, 5, 6 μg) and different PEI/Plasmid ratios (1:1, 3:1, 5:1, 7:1) were tested with detection of green fluorescence protein (GFP) with ImagePro Plus6. 0 Software. Then transfection efficiency of the optimized transfection system was further observed for different time periods(12, 24, 36, 48, 60, 72 h). The results showed that total plasmid dosage of 4 μg/well with PEI/plasmid ratio of 3 : 1-5 : 1 was an efficient transfection condition. Transfection efficiency-time curve was an S-shaped curve. Transfection efficiency reached a plateau at 60 h after transfection. The optimized conditions for PEI-mediated transfection at the optimal time result in enhanced transfection efficiency of triple plasmid into HEK293 cells.

10.A case of iatrogenic ilio-iliac arteriovenous fistula initially misdiagnosed as deep venous thrombosis

Iliac arteriovenous ifstula (AVF) usually manifests in a wide range of symptoms similar to typical deep venous thrombosis (DVT), which often lead to delayed diagnosis or misdiagnosis. We reported a 51-year old woman who was performed lumbar discectomy and showed a progressive abdominal distention, dyspnea, and swollen left leg. She was initially diagnosed as deep vein thrombosis and the ifnal diagnosis was arteriovenous ifstula. hTe ifstula was successfully sealed by an endovascular covered stent. No further recurrence was found atfer a half year’s follow-up. hTis article summarized the experience regarding iliac arteriovenous ifstula misdiagnosed, and discussed the differential diagnosis between arteriovenous ifstula and pulmonary thromboembolism causedby deep vein thrombosis.
