Objective:To develop an evidence-based nursing practice program based on the Stetler model to extend the lifespan of arteriovenous fistulas (AVF) in maintenance hemodialysis (MHD) patients and evaluate its effectiveness.Methods:An evidence-based nursing practice program was constructed based on the best evidence for extending AVF lifespan in MHD patients. Expert group discussions were held to determine review indicators. A baseline survey was conducted among 100 MHD patients and 42 nurses using covenience sompling at the Blood Purification Center of Wuxi People's Hospital from January to December 2023, to identify barriers and facilitators. In the following year, from January to December 2024, the evidence-based practice was implemented at the same hospital, and the AVF surgical intervention rate and AVF self-management levels of MHD patients were compared before and after the practice.Results:No statistically significant difference was found in the AVF surgery rates before and after the evidence-based practice ( P>0.05) . However, after implementing the evidence-based practice, MHD patients' scores on the AVF self-management scale significantly improved, and the difference was statistically significant ( P<0.01) . Conclusions:The evidence-based practice based on the Stetler model is beneficial for standardizing the clinical management of AVF, reducing the need for surgical interventions, and improving MHD patients' ability to self-manage their AVF.

Objective To translate English version of the Questionnaire evaluating the Disability of Upper Extremities in daily activities among patients undergoing maintenance Hemo-Dialysis (QDUE-HD) and test the reliability and validity of QDUE-HD. Methods After translation, back translation, 2 rounds of Delphi expert advice and pre-investigation, the initial questionnaire was formed. Two hundred and eleven patients undergoing hemo-dialysis from 4 hospitals located in Suzhou, Wuxi, Shanghai and Zhejiang were investigated using the initial questionnaire. And reliability and validity of QDUE-HD were evaluated. Results Two common factors containing 10 items and 2 dimensions(i.e. gripping and upper and lower arm movements) were extracted by exploratory factor analysis. The rate of cumulative contribution was 69.473％. In addition, the content validity index, Cronbach α coefficients, half-fold coefficient and test-retest reliability coefficient of whole scale were 0.850, 0.906, 0.929 and 0.983, respectively. Conclusions The reliability and validity of QDUE-HD (Chinese version) are reliable and effective. This questionnaire is simple and convenient to use in clinic.

Objective To explore the results of non-compliance of fluid intake in maintence hemodialysis patients.Methods The qualitative phenomenological research method was adopted in this study.Self-structured in-depth interviews were conducted with 13 maintenance hemodialysis patients.Data were analyzed by content analysis.Result Four themes were extracted including allotrigeusia,have a smattering of fluid intake knowledge,self-condemned and guilty,muddling along.Conclusions Fuild restriction is the most difficuilt prescribed treatment schedule among maintence hemodialysis patients.Nurses should pay attention to those people and provide multilevel,continual,individual and comprehensive measures.
