1.Calcitriol reverses sepsis-induced immunosuppression via VDR/Ca2+/pyroptosis signaling pathway

OBJECTIVE To investigate the effects of calcitriol on sepsis-induced immunosuppression and its potential mechanism. METHODS A sepsis-induced immunosuppression mice model was established using cecal ligation and puncture (CLP). The 7-day survival rate, serum levels of tumor necrosis factor- α (TNF- α), interleukin-6 (IL-6), and IL-1β were determined in sham operation group, CLP group and calcitriol group (1 μg/kg); the morphological changes of lung tissue in mice were observed. Lipopolysaccharide (LPS) tolerance macrophage models (representing sepsis-induced immunosuppression) were established using mice macrophage cell line RAW264.7 cells. The levels of TNF-α and IL-6 in cell supernatants as well as mRNA expressions of IL-1β, nucleotide-binding domain leucine-rich repeat and pyrin domain-containing receptor 3 (NLRP3), IL-18 and caspase-1 were assessed in culture medium group, LPS group, LPS tolerance group, and calcitriol (5 μmol/L) group. The following parameters were measured: propidium iodide (PI)-positive cell ratio, caspase-1 activity, lactate dehydrogenase (LDH) release, and Ca2+ levels. RESULTS Compared with CLP group, 7-day survival rate and serum levels of TNF-α, IL-6 and IL-1β were increased significantly in calcitriol group (P<0.05). Additionally, pulmonary tissue damage was markedly attenuated in calcitriol group. Compared with LPS tolerance group, the levels of TNF-α and IL-6 in cell supernatants, mRNA expressions of IL- 1β, NLRP3, IL-18 and caspase-1, PI-positive cell ratio, caspase-1 activity, LDH release, and Ca2+ levels were all increased significantly in calcitriol group (P<0.05). CONCLUSIONS Calcitriol can reverse sepsis-induced immunosuppression, and the mechanism of action may be E-mail:yarfwy@163.com achieved by the binding of calcitriol to vitamin D receptor,which promotes the release of Ca2+ from the endoplasmic reticulum, thereby driving the NLRP3/caspase-1-mediated pyroptosis pathway.
2.Clinical value of contrast-enhanced ultrasound combined with magnetic resonance imaging in the diagno-sis of parotid gland tumors

Objective:To explore the clinical value of combination of contrast-enhanced ultrasound(CEUS)and magnetic reso-nance imaging(MRI)in the diagnosis of parotid gland tumors.Methods:167 patients with parotid gland tumors were diagnosed by CEUS and MRI respectively and in combination.With postoperative pathological diagnosis result as the gold standard,the diagnos-tic value of CEUS combined with MRI(CEUS+MRI)was analyzed.Results:171 tumors were confirmed by postoperative pathology in 167 patients,including 143 benign tumors(83.63%)and 28 malignant tumors(16.37%).The benign tumors were mainly pleo-morphic adenoma(65 tumors,45.45%),Warthin tumor(40 tumors,27.97%)and basal cell adenoma(13 tumors,9.09%).Among the malignant tumors,mucoepidermoid carcinoma(7 tumors,25.00%),adenoid cystic carcinoma(6 tumors,21.43%)and acinic cell carcinoma(3 tumors,10.71%)were the most common.CEUS showed 39 cases of malignant parotid gland tumors and 132 cases of benign parotid gland tumors.The sensitivity,specificity and accuracy rate of CEUS were 57.14%,83.92%and 79.53%respectively.The MRI ADC values were manifested as pleomorphic adenoma>malignant tumor>Warthin tumor(P<0.05).MRI showed 53 cases of malignant parotid gland tumors and 118 cases of benign parotid gland tumors,and the sensitivity,specifici-ty and accuracy rate of MRI diagnosis were 67.86%,76.22%and 74.85%respectively.CEUS+MRI revealed 33 cases of malig-nant parotid gland tumors and 138 cases of benign parotid gland tumors,the sensitivity,specificity and accuracy rate of CEUS+MRI diagnosis were 78.57%,92.31%and 90.06%respectively.The specificity and accuracy rate of CEUS+MRI were significantly higher than those of single examination(x2=4.806,13.951,7.348,13.664,allP<0.05).Conclusion:Both CEUS and MRI have certain clinical value in the diagnosis of parotid gland tumors,and the specificity and accuracy rate of CEUS+MRI are significantly higher than those of single examination.
3.Study on Predictive Parameters of Liver Fibrosis Risk in Patients with Metabolic Dysfunction-associated Fatty Liver Disease Complicated with Chronic Hepatitis B

Objective To explore the influencing factors of liver fibrosis in patients with metabolic dysfunction-associated fatty liver disease(MAFLD)complicated with chronic hepatitis B(CHB),and to find clinically convenient predictive parameters for assessing liver fibrosis risk.Methods A retrospective analysis was conducted on 221 cases of MAFLD with CHB diagnosed and treated at the Second Hospital of Lanzhou University between January 2015 and August 2022.Patients were divided into low-risk(FIB-4<1.30,n=84),medium-risk(1.30≤FIB-4≤2.67,n=94)and high-risk(FIB-4>2.67,n=43)liver fibrosis groups based on the Fibrosis-4(FIB-4)index.General clinical data,laboratory indicators and composite indicators were compared among the three groups.Variables were screened using forward stepwise regression,and binary logistic regression analysis was performed to determine independent predictors of liver fibrosis.A prediction model was constructed and evaluated using receiver operating characteristic(ROC)curve analysis.Results Age,AST and Triglyceride-glucose index(TyG)were independent risk factors for liver fibrosis in patients with MAFLD combined with CHB,while platelet-to-lymphocyte ratio(PLR)was an independent protective factor(all P<0.05).ROC curve analysis showed that the area under the curve(AUC)for age,AST,TyG,PLR,age-AST and AST-TyG in predicting liver fibrosis were 0.668,0.764,0.680,0.738,0.856 and 0.805,respectively.At the optimal cut-off values,the sensitivities were 86.0%,67.4%,93.0%,62.8%,86.0%and 79.1%,and the specificities were 43.3%,82.0%,51.7%,86.0%,71.3%and 70.2%.Conclusion Age,AST,TyG and PLR are influencing factors of liver fibrosis in MAFLD combined with CHB patients.The parameters established based on these factors may predict the risk of liver fibrosis,and combined prediction can improve predictive efficacy.

5.Improvement effect and mechanism of petroleum ether extract of Saposhnikovia divaricata on rheumatoid arthritis rats by regulating neutrophil extracellular traps

OBJECTIVE To explore the improvement effect and mechanism of petroleum ether extract of Saposhnikovia divaricata on rheumatoid arthritis (RA) rats by regulating neutrophil extracellular traps (NETs). METHODS Establishment of rat RA model using bovine type Ⅱ collagen and Freund’s complete adjuvant. The model rats were randomly divided into model group and low-dose, middle-dose and high-dose groups (55, 110, 220 mg/kg) of petroleum ether extract of S. divaricata; the normal group without modeling was also established, with 10 rats in each group. Each group was given corresponding drugs or constant volume of 2% Tween-80 solution intragastrically, once a day, for consecutive 28 days. The toe swelling degree in rats was observed, and the arthritis index (AI) was scored. The serum levels of interleukin-1β (IL-1β), IL-10, IL-17, tumor necrosis factor-α (TNF-α), 25-hydroxyvitamin D3 [25(OH)D3], myeloperoxidase (MPO), neutrophil elastase (NE) and NETs in rats were detected. The histopathological changes in ankle joint were observed. The expression of citrullinated histone H3 (CitH3) in ankle joint as well as the expressions of cytochrome P450 24A1 (CYP24A1), cytochrome P450 27B1 (CYP27B1),vitamin D receptor (VDR) and peptidyl arginine deiminase 4 疾病。E-mail:zhongyao626@126.com (PAD4) in synovium were all determined. RESULTS Compared with the model group, the toe swelling degree and AI score in the middle-dose and high-dose groups of petroleum ether extract of S. divaricata decreased significantly from day 14 to day 28 after administration (P<0.05 or P<0.01). The serum levels of IL-1β, IL-17, TNF-α, MPO, NE and NETs decreased significantly, while the levels of IL-10 and 25(OH)D3 increased significantly (P<0.05 or P<0.01). The widened ankle joint space and the improved structure were found; the expression of CitH3 in ankle joint, and the expressions of CYP24A1 and PAD4 in synovium were down-regulated significantly, while the expressions of CYP27B1 and VDR were up-regulated significantly in synovium (P<0.05 or P<0.01). CONCLUSIONS The petroleum ether extract of S. divaricata may inhibit the production of NETs and improve the symptoms of RA by regulating the vitamin D system.
6.Evaluation of therapeutic effect of calycosin on microgravity-induced muscle atrophy based on real-time shear wave elastography technology

Objective:To investigate the protective effect of calycosin on microgravity-induced muscle atrophy by using real-time shear wave elastography (RT-SWE).Methods:The potential key active compound calycosin of anti-muscular atrophy in Astragali Radix was screened by systematic pharmacology. Eighteen healthy male Sprague-Dawley rats were divided into the control group [0.5% carboxymethyl cellulose-Na (CMC-Na) gavage], the hind limb unloading (HLU)+CMC-Na group (HLU+0.5% CMC-Na gavage), and the HLU+calycosin group (HLU+calycosin gavage) according to the method of random number table, with 6 rats in each group. After 28 d of continuous administration, the organ index, the toxicity of liver and kidney, the wet weight of soleus muscle and the ratio of muscle weight to body weight was measured, respectively. The non-invasive RT-SWE was used to evaluate the thickness and elastic modulus of rectus femoris in each group and the one-way analysis of variance was used to compare the differences among groups.Results:There was no significant difference in organ index, liver and kidney toxicity among different groups of rats (all P>0.05). There were significant differences in the weight of soleus muscle and the ratio of muscle weight to body weight among different groups of rats ( F=60.66, 56.44, both P<0.001). Compared with the HLU+CMC-Na group, the weight of soleus muscle and the ratio of muscle weight to body weight in the HLU+calycosin group increased, and the differences were significant (both P<0.01). The thickness and elastic modulus of rectus femoris of rats in different groups were significantly different ( F=35.47, 14.68, both P<0.001). Compared with the HLU+CMC-Na group, the muscle thickness and elastic modulus of rats in HLU+calycosin group were increased, and the differences were significant (both P<0.01). Conclusions:The treatment of calycosin has no side effects on rats. It can improve the thickness and elastic modulus of rectus femoris, and effectively prevent microgravity-induced muscle atrophy, which may provide a new candidate drug for astronaut muscular atrophy.

8.Boosting 5-ALA-based photodynamic therapy by a liposomal nanomedicine through intracellular iron ion regulation.

5-Aminolevulinic acid (5-ALA) has been approved for clinical photodynamic therapy (PDT) due to its negligible photosensitive toxicity. However, the curative effect of 5-ALA is restricted by intracellular biotransformation inactivation of 5-ALA and potential DNA repair of tumor cells. Inspired by the crucial function of iron ions in 5-ALA transformation and DNA repair, a liposomal nanomedicine (MFLs@5-ALA/DFO) with intracellular iron ion regulation property was developed for boosting the PDT of 5-ALA, which was prepared by co-encapsulating 5-ALA and DFO (deferoxamine, a special iron chelator) into the membrane fusion liposomes (MFLs). MFLs@5-ALA/DFO showed an improved pharmaceutical behavior and rapidly fused with tumor cell membrane for 5-ALA and DFO co-delivery. MFLs@5-ALA/DFO could efficiently reduce iron ion, thus blocking the biotransformation of photosensitive protoporphyrin IX (PpIX) to heme, realizing significant accumulation of photosensitivity. Meanwhile, the activity of DNA repair enzyme was also inhibited with the reduction of iron ion, resulting in the aggravated DNA damage in tumor cells. Our findings showed MFLs@5-ALA/DFO had potential to be applied for enhanced PDT of 5-ALA.

10.Neutropenia and fever induced by esomeprazole sodium for injection

A 58-year-old female patient with sour regurgitation, pyrosis, nausea, and vomiting received an IV infusion of esomeprazole sodium for injection 40 mg once daily.Before treatment, her white blood cell (WBC) count was 4.5×109/L and neutrophil (NEUT) count was 2.8×109/L.On day 2 of medication, her WBC was 3.4 ×109/L and NEUT 1.7×109/L.On day 7, the patient′s WBC was 2.1×109/L and NEUT 0.1×109/L.On day 10, her WBC was 1.6 ×109/L and NEUT 0×109/L, she developed chillness and fever (38.4 ℃).On day 11, her temperature reached 40 ℃ with throat discomfort.She was diagnosed with neutropenia combined with upper respiratory infection due to esomeprazole sodium for injection.Esomeprazole sodium for injection was stopped and meropenem for anti-infection and recombinant human granulocyte colony-stimulating factor for increasing neutrophil were given at the same day.On day 3 of drug withdrawal, her temperature descended to 36.4 ℃.On day 4, her WBC was 2.3×109/L and NEUT 0.8×109/L.On day 10, her WBC increased to 8.0 ×109/L and NEUT 4.9×109/L.
