1.Construction and application of a system for rational drug use for prescriptions from internet hospitals and external prescriptions and medical insurance fund control based on a pre-review prescription system

OBJECTIVE To ensure the safety of patients’ drug use and control the risk of medical insurance expenditure by upgrading the pre-prescription review system to conduct pre-review on prescriptions from internet hospitals and external prescriptions, as well as to review the payment methods of drugs (including in-hospital and external drug dispensing). METHODS The data interfaces of prescriptions from internet hospitals and external prescriptions were integrated to achieve real-time rational drug use intervention. Additionally, an intelligent review project for payment method was added to precisely intervene in the medical insurance payment methods of drugs. The effect of the system upgrade was evaluated by comparing the qualification rates of prescriptions from internet hospitals and external prescriptions and the suspected amounts of drug violations from January to April 2025 (before the system upgrade) and May to August 2025 (after the system upgrade). RESULTS After the upgrade of the pre-prescription review system, the qualification rates of prescriptions from internet hospitals and external prescriptions increased by 3.5% [95% confidence interval (CI)=0.3%-6.7%, P =0.037 ] ; the suspected amounts of drug violations decreased to 52.9% of the pre-upgrade level (95%CI=31.6%-88.5%, P =0.026), and the average monthly sequential decrease was 29.5% (95%CI=12.2%-43.4%, P =0.012). Moreover, the addition of the intelligent review project for payment methods promoted the management of off-label drug use in our hospital. After the upgrade, a total of 79 filling valid applications for off-label drug use were received and archived. CONCLUSIONS The upgrade of the pre-prescription review system effectively improves the review qualification rates of prescriptions from internet hospitals and external prescriptions and the accuracy of medical insurance payment for drugs, and strengthens the supervision of off-label drug use, achieving dual guarantees of clinical rationality and medical insurance compliance.
2.Analysis of clinical characteristics and risk factors for infection in patients with multiple myeloma treated with bortezomib

OBJECTIVE To study the clinical characteristics and potential risk factors for infection in patients with multiple myeloma (MM) following treatment with bortezomib. METHODS Clinical data were retrospectively collected from MM patients who received bortezomib-based treatment regimens at the Department of Hematology, the Second Affiliated Hospital of Soochow University, from October 2021 to February 2025. The collected data primarily included demographic characteristics, disease characteristics of MM, treatment regimens, occurrence of infections and corresponding management measures, and prophylactic medication use. Univariate and multivariate Logistic regression analyses were conducted to identify potential risk factors for MM complicated with infection. RESULTS Among the 284 MM patients treated with bortezomib, 132 patients (46.5%) experienced at least one infection. The predominant types of infections were respiratory tract infections and gastrointestinal infections. Univariate analysis showed that age at initial diagnosis, pathological classification, and grade of myelosuppression were influencing factors for infection in MM patients ( P <0.05). Further analysis of influencing factors for the two main types of infections revealed that sex, age at initial diagnosis, pathological classification, treatment regimen, and smoking history were influencing factor s for respiratory tract infections in MM patients ( P <0.05); BMI, pathological classification, treatment regimen, and grade of myelosuppression were influencing factors for gastrointestinal infections in MM patients ( P <0.05). Multivariate Logistic regression analysis indicated that age≥70 years and the presence of grade Ⅳ myelosuppression before treatment were risk factors for infection in MM patients, while the IgG-λ type was a protective factor against infection ( P <0.05). CONCLUSIONS The incidence of infection is relatively high in MM patients receiving bortezomib-based treatment regimens, with respiratory and gastrointestinal infections being the most common. Age at initial diagnosis, grade of myelosuppression, and pathological classification are influencing factors for infection in MM patients.
3.Evaluation of the ERCP introductory training model and the teaching effect for gastroenterology specialists

Objective To explore the methods of continuing education for advanced endoscope operations by diges-tive specialists through the establishment and teaching effect evaluation of the ERCP(endoscope retrograde cholan-giopancreatography)introductory training mode.Methods A total of 26 trainees from 3 sessions of the ERCP intro-ductory training courses at Peking Union Medical College Hospital from September 2023 to September 2024 were in-cluded.The teaching effects of the training courses and its 5 modules were subjectively and objectively evaluated by questionnaires,on-site tests and evaluations by senior ERCP operators.Results Through the ERCP introductory training courses,the trainees'self-evaluated proficiency in duodenoscope structure(pre-training:2.4±2.4,post-training:8.2±1.5,P<0.001),duodenoscope operation(pre-training:1.2±2.2,post-training:6.6±1.8,P<0.001),papillary cannulation(pre-training:0.5±1.3,post-training:5.4±1.8,P<0.001),intra-bile duct operation(pre-training:0.2±0.6,post-training:4.9±2.1,P<0.001),and identification of intra-bile duct lesions(pre-training:1.7±2.1,post-training:6.0±2.0,P<0.001)was significantly improved.The accuracy rate of the trainees'theoretical tests and picture recognition before training was 37.2%and then increased up to 62.8%after training.Before training,all trainees were considered by senior operators as not ready to start ERCP training on real patients,while after training,69.2%(18/26)of the trainees were considered ready to start ERCP training on real patients.Conclusions The multi-module ERCP introductory training courses have a significant effect in terms of laying a foundation for trainees to start ERCP training on patients and of providing a reference for the con-tinuing education mode of advanced endoscope operations for digestive specialists in China.
4.Biomarkers affecting the progression of mild to moderate cognitive impairment after stroke:a non-targeted metabolomics analysis

BACKGROUND:Cognitive impairment is the most common complication after stroke,and its severity is closely related to the patient's prognosis.The prognosis of patients can be significantly improved if the severity of their cognitive impairment is recognized and targeted early.OBJECTIVE:To initially explore potential biomarkers affecting the progression of post-stroke cognitive impairment,thereby providing a richer and unique reference for the study of their pathophysiological mechanisms.METHODS:Using ultra performance liquid chromatography-mass spectrometry,non-targeted metabolomics analysis was conducted on serum samples from patients with mild and moderate post-stroke cognitive impairment to identify differential metabolites between the two groups.To further validate the diagnostic efficacy of the differential metabolites,the receiver operating characteristic curve analysis was used to evaluate their accuracy and sensitivity in distinguishing disease severity.In addition,pathway analysis was conducted on the differential metabolites.RESULTS AND CONCLUSION:(1)There were significant differences in metabolic profiles between patients with mild and moderate post-stroke cognitive impairment,and 9 differential metabolites were screened by the receiver operating characteristic curve.(2)Differential metabolite pathway analysis revealed that the metabolic pathways affecting disease progression in patients with mild-to-moderate post-stroke cognitive impairment included tryptophan metabolism,D-amino acid metabolism,biotin metabolism,retinol metabolism,aminoacyl-tRNA biosynthesis,lysine degradation,protein digestion and uptake,pyrimidine metabolism,cysteine and methionine metabolism,ABC transporter proteins,amino acid biosynthesis,and 2-oxocarboxylic acid metabolism.To conclude,9 potential biomarkers affecting disease progression in patients with mild-to-moderate post-stroke cognitive impairment have been identified,involving 12 metabolic pathways including tryptophan metabolism,D-amino acid metabolism and retinol metabolism.

To summarize the research progress and to discuss current drug supplement status of radiation preventive and curative drugs,which would provide fresh thoughts for nuclear emergency rescue and therapy.Classification of clinically available and under-development radiation prevention and treatment drugs has been presented in terms of symptomatic and causative prevention and treatment drugs.We analyzed the necessity and challenges of establishing a supply system for radiation prevention and treatment and put forward counter measures and thinking prospects.Clinical stockpile of symptomatic drugs is relatively sufficient.Allopathic drugs are widely developed,however,the barriers between research and clinical translation and the inadequate market supply system pose greater demands and challenges to the provision of hospital radiation allopathic drugs.

7.Establishment of a closed-loop management system for the whole-process traceability of outpatient drugs based on internet of things and blockchain technology

OBJECTIVE To establish a closed-loop management system for the whole-process traceability of outpatient drugs based on internet of things(IoT)and blockchain technology,and evaluate its implementation effects.METHODS A closed-loop management system for the whole-process traceability of outpatient drugs covering the entire drug lifecycle was designed using drug traceability codes integrated with IoT and blockchain technology.System effectiveness was evaluated from three dimensions:work efficiency,medication management quality and data safety by comparing indicators such as the acceptance time of incoming drugs and the number of collected drug traceability codes before the system implementation(October to December 2024)and after the system implementation(January to March 2025).RESULTS A closed-loop management system for the whole-process traceability of outpatient drugs,centered around the drug traceability code management system,was successfully established.The acceptance time for incoming drugs was shortened from(4.65±0.26)h before implementation to(0.34±0.08)h after implementation(P<0.05).The number of collected drug traceability codes increased from 419 018 to 1 236 522,and the coverage rate of traceability codes rose from 28.36%to 89.88%(P<0.05).The time pharmacists spent on drug expiry management per week decreased from(128.40±19.20)min to(0.56±0.13)min(P<0.05),and the dispensing time for a single prescription(excluding a part of injections and repackaged drugs)was reduced from(143.25±17.67)s to(15.24±10.08)s(P<0.05).The time for drug return was reduced from 129.90(122.32,137.00)s to 104.36(89.91,117.33)s(P<0.05);the number of drug dispensing errors decreased from 2 cases to 0 cases.After the system was launched,there were no data security incidents in our outpatient pharmacy.CONCLUSIONS The constructed closed-loop management system for the whole-process traceability of outpatient drugs can significantly enhance drug traceability accuracy and drug management quality,improve pharmacist work efficiency,and reduce drug management risks,thus providing a feasible solution for the digital transformation of hospital pharmaceutical services.

9.Practice of PIVAS operation cost-benefit management in a hospital based on lean Six Sigma management

OBJECTIVE To explore the practice and application effect of lean Six Sigma (LSS) management in the cost-benefit management of PIVAS operation in a tertiary comprehensive hospital (hereinafter referred to as "S Hospital"),providing reference for the operation and management of PIVAS in hospitals.METHODS The five steps (define,measure,analyze,improve and control,i.e.DMAIC) of LSS management were implemented for PIVAS operation cost-benefit of S Hospital,and lean management was implemented for its cost-benefit management elements (human resource cost,medical and health material cost,and all-in-one parenteral nutrition preparation income).Several intervention measures including personnel training and performance assessment,refined management system of consumables,and doctor's advice package of full parenteral nutrition were developed.Finally,the overall improvement effect was evaluated by the total benefit,total cost and net benefit of PIVAS.The effects of human resource allocation optimization and improvement were evaluated by the work efficiency,work quality,job satisfaction,turnover rate and accumulated rest days.The effects of consumables cost management were evaluated by the amount of medical and health materials cost.The improvement effects of all-in-one parenteral nutrition preparation income were evaluated by the profit amount,quantity and the proportion of single bottle of parenteral nutrition.RESULTS After implementing DMAIC in S Hospital,the total benefit of PIVAS was increased from (471366.50±9201.5) yuan/month to (479679.50±14320.14) yuan/month (P>0.05),the total cost was decreased from (305878.88±3201.75) yuan/month to (294610.59±5007.33) yuan/month (P<0.05),and the net benefit of PIVAS was increased by 11.83% compared with that before the improvement.The work efficiency,work quality and job satisfaction of employees were significantly improved,the accumulated rest days were significantly reduced,and the turnover rate of third-party employees was reduced from 15.0% before the improvement to 7.5% after the improvement.The cost of medical and health materials significantly decreased from (67826.42±2812.76) yuan/month before improvement to (56384.33±4607.67) yuan/month after improvement (P<0.05).The quantity of all-in-one parenteral nutrition was significantly increased from (1263.75±135.83)group/month before improvement to (2061.25±89.04) group/month after improvement (P<0.05),and the proportion of users of single bottle of parenteral nutrition in total users decreased from 93.25% before improvement to 58.75% after improvement.The profit of all-in-one parenteral nutrition was 63.18% higher than that before implementing DMAIC.CONCLUSIONS The implementation of PIVAS operation cost-benefit management based on DMAIC is conducive to strengthening the cost control of PIVAS and promoting the healthy development of PIVAS.
10.The Efficacy of Combined Endoscopic Ultrasound Fine-needle Aspiration and Endoscopic Retrograde Cholangiopancreatography in Same Session for the Diagnosis and Management of Pancreatic Carcinoma with Obstructive Jaundice

To explore the application value of endoscopic ultrasound fine-needle aspiration (EUS-FNA) and endoscopic retrograde cholangiopancreatography (ERCP) in the diagnosis and treatment ofpatients with pancreatic cancer combined with obstructive jaundice. Clinical data of patients hospitalized in the Department of Gastroenterology of Peking Union Medical College Hospital who underwent ERCP biliary stent drainage for pancreatic cancer combined with obstructive jaundice from January 1, 2023 to February 26, 2024 were retrospectively collected. They were categorized into the fusion technology group and the simple ERCP group according to whether EUS-FNA was performed in the same endoscopic unit. The differences in pathologic diagnosis rate, ERCP drainage success rate, postoperative complication rate and patients' single hospitalization time were compared between the two groups. A total of 161 patients with pancreatic cancer combined with obstructive jaundice who underwent ERCP biliary stent drainage meeting the inclusion and exclusion criteria were enrolled, of which 80 were in the fusion technique group and 81 were in the simple ERCP group. The pathological diagnosis rate in the fusion technique group was higher than that in the simple ERCP group[92.50%(74/80) The fusion of EUS-FNA and ERCP enhances the efficiency of diagnosis and treatment for pancreatic carcinoma, warranting widespread adoption and further research.
