1.Study on the effects of different tidal volume ventilation on respiratory mechanics and hemodynamics in children with severe pneumonia

Objective To investigate the effect of ventilation with different tidal volumes on respiratory mechanics and haemodynamics in children with severe pneumonia.Methods A total of 104 children with severe pneumonia were divided into the control group and the observation group using a random number table method,with 52 cases in each group.Both groups underwent mechanical ventilation treatment.The control group was treated with conventional tidal volume of 10-12 mL/kg,while the observation group was given a low tidal volume of 6-8 mL/kg.The respiratory mechanics indicators,hemodynamic indicators and inflammatory factor indicators before and after treatment were compared between the two groups.Results After treatment,the airway resistance(AR)and airway closing pressure(AOP)were lower in the observation group than those of the control group(P<0.05),and pulmonary dynamic compliance(Cdyn)was higher in the observation group than that of the control group(P<0.05).There were no significant differences in heart rate(HR),mean arterial pressure(MAP)and central venous pressure(CVP)after treatment between the two groups(P<0.05).After treatment,the levels of interleukin(IL)-6,IL-8 and tumor necrosis factor-α(TNF-α)were all lower in the observation group than those in the control group(P<0.05).Conclusion Small tidal volume mechanical ventilation is effective in improving the respiratory mechanics of children with severe pneumonia without causing large fluctuations in their hemodynamics.At the same time,it can reduce the level of inflammatory factors in the organism.

The prevention and treatment of esophageal cancer constitute an important component of China's cancer control efforts.As the earliest established cancer prevention and control field site in China,the Linzhou Esophageal Cancer Prevention and Treatment Site has developed a sys-tematic prevention and control strategy encompassing tumor registration,etiological research,early diagnosis and treatment,and other key areas,providing valuable experience for refining China's cancer prevention system.Based on over six decades of practical experience at the Linzhou site,this paper reviews its historical development,the construction of its prevention-control system,and emerging challenges in the new era,while exploring its reference significance for China's na-tionwide cancer prevention and control initiatives.
3.Two different fixation methods for vertically unstable femoral neck fractures: mechanical study and finite element analysis

Objective:To compare the changes in fracture displacement under different vertical loadings between the 2 different internal fixation modalities for vertically unstable femoral neck fractures of Pauwels type Ⅲ by a mechanical study and a finite element analysis.Methods:Twelve biomimic bones were transversely dissected from 10 cm below the lesser trochanter of the femur to create femoral neck fracture models with a Pauwels angle of 70° using a swing saw. The models were equally divided into 2 groups ( n=6): group A was fixed with 3 cannulated screws after fracture reduction (scheme A), and group B with 3 cannulated screws plus a self-designed anteromedial support plate after fracture reduction (scheme B). Continuous vertical force was applied using a mechanical testing machine. Changes in displacement were recorded and load-displacement curves were plotted. One volunteer (female, 28 years old, 168 cm in height and 65 kg in weight) was selected for finite element analysis of her CT images of both lower limbs to examine the maximum displacement and the maximum Mises stress in scheme A and scheme B respectively. Objective To investigate the correlation between different TCM syndrome types and RNF180/Septin9 gene methylation in patients with chronic gastritis.Methods Hospitalized cases diagnosed with chronic gastritis from March 2022 to July 2024 were retrieved through the information system of Wangjing Hospital,China Academy of Medical Sciences.Information such as general conditions,pathological findings and RNF180/Septin9 gene methylation detection results were collected.A total of 441 patients with chronic gastritis were finally collected according to the attrition criteria,and were divided into 5 types:liver-stomach disharmony syndrome,spleen-stomach damp-heat syndrome,spleen-stomach weakness syndrome,stomach-collateral stasis syndrome and stomach-yin deficiency syndrome.SPSS 25.0 was used to analyze the correlation between TCM syndrome types of chronic gastritis and methylation of RNF180/Septin9 gene.Results The majority of 441 patients with chronic gastritis were spleen-stomach damp-heat syndrome.The results of statistical analysis showed that there were differences in gender,smoking and drinking history and age distribution among different TCM syndrome types(P<0.05).The positive rate of RNF180/Septin9 gene methylation in stomach-collateral stasis syndrome was significantly higher than that in other syndrome types(P<0.01).Correlation analysis further showed that spleen-stomach damp-heat syndrome and stomach-collateral stasis syndrome were positively correlated with RNF180/Septin9 gene methylation(P<0.05),and the correlation of stomach-collateral stasis syndrome was particularly significant(P<0.01).Conclusion Spleen-stomach damp-heat syndrome and stomach-collateral stasis syndrome are positively correlated with RNF180/Septin9 gene methylation in patients with chronic gastritis.Pathological products such as blood stasis and damp-pathogenic bacteria can increase the risk of"inflammatory-cancer"transformation,and its prognosis is worse than other syndrome types.Timely intervention and regular examination should be conducted to achieve early diagnosis and treatment.

10.Cryptotanshinone promotes brown fat activity by AMPK activation to inhibit obesity

BACKGROUND/OBJECTIVES:
Activating brown adipose tissue (BAT) and browning of white adipose tissue (WAT) can protect against obesity and obesity-related metabolic conditions.Cryptotanshinone (CT) regulates lipid metabolism and significantly ameliorates insulin resistance. Adenosine-5'-monophosphate (AMP)-activated protein kinase (AMPK), a receptor for cellular energy metabolism, is believed to regulate brown fat activity in humans.MATERIALS/METHODS: The in vivo study included high-fat-fed obese mice administered orally 200/400 mg/kg/d CT. They were evaluated through weight measurement, the intraperitoneal glucose tolerance test (IPGTT), intraperitoneal insulin tolerance test (IPITT), cold stimulation test, serum lipid (total cholesterol, triglycerides, and low-density lipoprotein) measurement, hematoxylin and eosin staining, and immunohistochemistry.Furthermore, the in vitro study investigated primary adipose mesenchymal stem cells (MSCs) with incubation of CT and AMPK agonists (acadesine)/inhibitor (Compound C).Cells were evaluated using Oil Red O staining, Alizarin red staining, flow cytometry, and immunofluorescence staining to identify and observe the osteogenic versus adipogenic differentiation. Quantitative real-time polymerase chain reaction and the Western blot were used to observe related gene expression.
RESULTS:
In the diet-induced obesity mouse model mice CT suppressed body weight, food intake, glucose levels in the IPGTT and IPTT, serum lipids, the volume of adipose tissue, and increased thermogenesis, uncoupling protein 1, and the AMPK pathway expression. In the in vitro study, CT prevented the formation of lipid droplets from MSCs while activating brown genes and the AMPK pathway. AMPK activator enhanced CT’s effects, while the AMPK inhibitor reversed the effects of CT.
CONCLUSION
CT promotes adipose tissue browning to increase body thermogenesis and reduce obesity by activating the AMPK pathway. This study provides an experimental foundation for the use of CT in obesity treatment.
