Objective:To explore the application effect of phased target teaching combined with case teaching method in neurosurgery intern nurses.Methods:A convenient sampling method was used to select 96 intern nurses in the neurosurgery department of Shandong Provincial Hospital Affiliated to Shandong First Medical University from January 2022 to December 2023. The participants were divided into a control group and an observation group. The control group received conventional teaching, while the observation group was given phased target teaching combined with case teaching method based on the control group's curriculum. The clinical work ability, critical thinking ability, teaching performance, teaching satisfaction and cooperation of the intern nurses in both groups were compared.Results:After the intervention, the observation group scored higher in clinical work ability, critical thinking ability, teaching satisfaction and cooperation, teaching performance compared to the control group ( P<0.05) . Conclusions:The application of phased target teaching combined with case teaching method for neurosurgery intern nurses can improve their clinical work ability, critical thinking ability, academic performance, teaching satisfaction and cooperation. This method is worth recommending.

3.Predictive value of combined IAA, ICA, and GADA tests for pregnancy outcome in patients with gestational diabetes mellitus

Objective:To analyze the predictive value of combined insulin autoantibody (IAA), islet cell antibody (ICA), and serum glutamic acid decarboxylase antibody (GADA) testing for pregnancy outcome in patients with gestational diabetes mellitus (GDM) .Methods:One hundred and twenty patients with GDM were retrospectively selected for the study, and were divided into a poor pregnancy outcome group and a normal pregnancy outcome group based on the pregnancy outcome at follow-up to delivery, which were 37 and 83 cases, respectively. The occurrence of adverse pregnancy outcomes in the study population was counted, and univariate and multivariate analyses of adverse pregnancy outcomes in GDM were performed.Results:Adverse pregnancy outcomes were seen in 37 of the 120 study subjects, including 12, 13 and 9 cases of macrosomia, caesarean section and neonatal hypoglycaemia, respectively, and 1 case of premature rupture of the foetal hairs, placenta previa and neonatal asphyxia. The percentage of IAA, ICA, and GADA positivity was higher in the adverse pregnancy outcome group than in the normal pregnancy outcome group ( P<0.05). Multifactorial regression analysis showed that IAA ( OR=3.180, 95% CI 1.394-7.258), ICA ( OR=3.459, 95% CI 1.592-7.517), and GADA ( OR=3.219, 95% CI 1.508-6.872) positivity were independent risk factors for adverse pregnancy outcomes in GDM ( P<0.05). The AUC values for the combined IAA, ICA, and GADA for the detection of adverse pregnancy outcomes in GDM were higher than those for IAA, ICA, and GADA alone ( Z=2.607, 2.600, and 2.527, P<0.05) . Conclusion:Positive IAA, ICA, and GADA are risk factors for adverse pregnancy outcomes in patients with GDM, and combined testing has a higher predictive value.

5.Construction andeffect evaluation of precise oral nutrition guidance program for postoperative patients with colorectal cancer undergoing accelerated rehabilitation

Objective:To construct and evaluate the effect of precise oral nutrition guidance program for colorectal cancer patients with accelerated rehabilitation after operation.Methods:This study was a quasi experimental study. A total of 116 patients with colorectal cancer from department of Colorectal Surgery, Jiangsu Cancer Hospital from March to Deamber, 2021 were selected by convenience sampling method. They were divided into two groups according to their admission time, the experimental group (58 cases), was given precise oral nutrition guidance program after colorectal cancer surgery based. In the control group, 58 cases were treated with the conventional dietary guidance for rapid rehabilitation. The gastrointestinal tolerance rate, the compliance rate of oral nutritional supplements, the first defecation time after operation, albumin, total albumin, prealbumin and other nutritional indexes were compared between the two groups before and after intervention.Results:Finally, 111 cases were included, induding 56 in the experimental group and 55 in the control group. The compliance rate of oral nutritional supplements after intervention in the experimental group was 73.2% (41/56), higher than that in the control group 40.0%(22/55), and the difference between the two groups was statistically significant ( χ2 = 12.47, P<0.05). The first defecation time after operation in the experimental group was (29.51 ± 5.52) h, while in the control group was (61.48 ± 8.39) h, the difference between the two groups was significant ( t = -12.06, P<0.05). The postoperative enteral nutrition tolerance rate of the experimental group was 91.1% (51/56), which was higher than the 69.1%(38/55) in the control group, the difference between the two groups was statistically significant ( χ2 = 8.44, P<0.05). The serum total protein, albumin and prealbumin levels of the experimental group at discharge were (66.56 ± 5.15), (46.78 ± 7.62) g/L and (276.07 ± 53.57) mg/L, compared with the (60.27 ± 4.37), (39.12 ± 6.41) g/L and (230.14 ± 44.58) mg/L in the control group, the differences were statistically significant ( t = 6.66, 5.02, 4.91, all P<0.05). Conclusions:The implementation of precise oral nutrition guidance program is conducive to guiding the clinical practice of oral nutritional supplements in patients with colorectal cancer after surgery, improving patient compliance, reducing gastrointestinal intolerance, promoting intestinal function recovery, and improving nutritional status of patients.
6.Expression of serum inflammatory cytokines in patients with thyroid tumor and their correlation with thyroid hormones

Objective:To explore the expression of serum inflammatory factors in patients with thyroid tumor and their correlation with thyroid hormones.Methods:A total of 92 patients with thyroid tumors (48 cases of thyroid cancer and 44 cases of thyroid adenoma) admitted to Department of Endocrinology in Linyi Central Hospital in Shandong Province from Jan. 2020 to Oct. 2021 were enrolled. 50 healthy volunteers who received physical examination in the hospital during the same period were enrolled in the control group. The serum inflammatory factors [tumor necrosis factor-α (TNF-α) , interleukin-6 (IL-6) , interleukin-17 (IL-17) ] and thyroid hormone expression levels [thyroid stimulating hormone (TSH) , free thyroxin T4 (FT4) , free triiodothyronine (FT3) ] of the three groups were detected. Analysis of variance was used for multi-group comparison, independent sample t test was performed for comparison between groups, Spearman correlation analysis and Logistic regression analysis were made for the risk factors of thyroid cancer. The expression of serum inflammation and thyroid hormones in patients with different stages of thyroid cancer was observed. Results:Serum TNF-α, IL-17, IL-6 from high to low were in the thyroid cancer group (74.61±7.94 ng/L, 68.65±7.05 ng/L, 20.52±2.84 ng/L) , thyroid adenoma group (26.97±3.42 ng/L, 46.31±5.31 ng/L, 13.61±1.58 ng/L) , control group (18.82±2.63 ng/L, 34.52±4.02 ng/L, 8.97±1.06 ng/L) ( F=1596.271, 468.602, 423.351, all P<0.001) ; Serum TSH levels from high to low were in the thyroid cancer group (8.64±1.34 mU/L) , the thyroid adenoma group (5.21±1.02 mU/L) , the control group (3.94±0.85 mU/L) ( F=242.182, P=0.000) . There was no significant difference in serum FT4 or FT3 levels among the three groups ( P=0.753, 0.634) . Correlation analysis indicated that serum TNF-α, IL-17, and IL-6 were positively correlated with the expression level of serum TSH ( r=0.936, 0.726, 759, all P<0.05) . The expression level of TNF- α, IL-17, IL-6 and TSH was significantly higher in patients of stage III and IV than that in patients of stage I and II ( t=2.541, 4.394, 6.390, 4.962, P=0.015, P<0.001, P<0.001, P<0.001) . Multivariate Logistic regression analysis suggested serum TNF-α, IL-17, IL-6 and TSH were all risk factors for thyroid cancer. Conclusions:Serum inflammatory factors and some thyroid hormones (TSH) are generally highly expressed in patients with thyroid tumors, the expression levels of serum inflammatory factors are correlated with the expression of TSH. There are statistically significant differences in the expression levels of serum inflammatory factors and TSH between patients with thyroid cancer of different stages, serum inflammatory factors and TSH are also involved in the occurrence and development of thyroid cancer, and are risk factors for thyroid cancer.
7.Application of PDCA Cycle Management Method in Improving Colostomy Bags Replacement Skills of Colostomy Patients

Objective To study the clinical efficacy of the use of PDCA cycle management method in colostomy bags replacement skills of colostomy patients in our gastrointestinal surgery department. Methods 81 colostomy patients from January to May 2017 were selected as control group with general colostomy nursing management method and 69 colostomy patients from June to October 2017 were selected as experimental group with PDCA cycle management method. Patients' colostomy bags replacement qualified rate and nurses' ARC procedure qualified rate are compared between control group and experimental group. Results The incidence of problems from experimental group (5. 7%) was significantly lower than that from control group (17. 5%) with statistically significant difference (x~ 2 = 66. 74, P<0. 01); Patients' colostomy bags replacement qualified rate from experimental group (84. 1%) was significantly higher than that from control group (60%) with statistically significant difference (x ~2 = 11. 05, P<0. 01); Nurses' ARC procedure qualified rate & excellent rate in October (100%& 75%) were significantly higher than those in June (75% & 10%). Conclusion PDCA cycle management method can significantly improve colostomy bags replacement skills of colostomy patients, improve nurses' professional skills, improve nursing quality in our gastrointestinal surgery department.
8.Microarray gene expression profiling of articular chondrocyte in patients with osteoarthritis

ObjectiveTo investigate the gene expression profiling of articular chondrocyte and the function and pathway of the differentially expressed genes in patients with osteoarthritis (OA) by using gene microarray technique.MethodsThree patients with OA,three patients with rheumatoid arthritis(RA) and three traumatic controls without arthritis were selected and were divided into OA versus RA and OA versus the traumatic control groups,and their articular cartilage cells were cultivated.The gene expression profiling was performed by human genome oligonucleotide microarray technique.The differences of gene expression of the articular chondrocyte in OA versus RA group and OA versus the traumatic control group were compared respectively by two class unpaired test using significant analysis of microarray software.The function and pathway of these differentially expressed genes were analyzed by using the database of Molecule Annotation System.ResultsThe number of differentially expressed genes was 145 when OA compared with the traumatic control,in which 70 were up-regulated and 75 were down-regulated; and the number was 281 when OA were compared with RA,in which 94 were up-regnlated and 187 were down-regulated.The Gene Onto-logy (GO) functions of the differentially expressed genes of OA in each group were related to pathological and immune courses including cellular process,physiological process,cell division,biological regulation and cell signal transduction.The statistically significant pathways of these genes in each group included apoptosis pathway,cell cycle pathway,P53 signaling pathway,Fas signaling pathway,NO pathway,apeptotic cascade pathway,focal adhesion pathway, ECM-receptor interaction pathway, tight junction pathway, adhesive bonding pathway,actin cytoskeleton regulation pathway,bone remodeling pathway,chondroitin sulfate biosynthesis pathway,Jak-STAT signaling pathway,Wnt signaling pathway,Toll recepor signaling pathway,cell adhesion molecules pathway,T cell receptor signaling pathway,and s o on (P<0.05).They displayed not only marked dif-ferences in GO function and gene pathway of differentially expressed genes between OA versus RA group and OA versus the traumatic control group,but also displayed the significant overlapping of the differentially expressed genes and pathways between the two groups.ConclusionThe differentially expressed genes and pathways of articular chondrocytes involve apoptosis,extracellular matrix and cytokines in OA,which contri-bute to further study of early warning genes of OA.
9.Study on the hyperuricemia and its influencing factors in elderly male people aged 90 years and above

ObjectiveTo explore the prevalence and clinical features of hyperuricemia and its influencing factors in elderly male people aged 90 years and above. MethodsOne hundred elderly male people aged 90 years and above who underwent routine health examination in our hospital in 2007 were selected in the study. Serum uric acid level was examined by uricase-peroxidase method, and all patients were divided into hyperuricemia group and control group according to the serum uric acid level. Clinical and biochemical indications were compared between the two groups, and logistic regression was used to analyze the influencing factors of hyperuricemia in elderly people. ResultsThe serum uric acid level was increased in 20% of the elderly people, and the prevalence of gouty arthritis was 1%. The levels of blood urea nitrogen and serum creatinine were higher in hyperuricemia group than in control group[(10. 98±4.29) mmol/L vs. (6. 87± 1.86) mmol/L, (125.2±25.9)μmol/L vs. (93. 4±19. 8)μmol/L;both P<0.05)3. The patients in hyperuricemia group had a higher prevalence of hypertension and hypertriglyceridemia, and a higher proportion of diuretic application than patients in control group(P<0. 05). Logistic regression analysis showed that serum uric acid level had the most remarkable correlation with serum creatinine(OR= 1. 969), followed by fasting blood glucose (OR= 1. 310)and blood urea nitrogen(OR = 1.161). There was negative correlation between serum uric acid level and plasma cholesterol level(OR = 0. 802). ConclusionsThe prevalence of hyperuricemia is high in elderly people aged 90 years and above, while the incidence of gouty arthritis is low. Renal function impairment, metabolic syndrome and thiazide diuretic are the major factors for hyperuricemia.
