1.Screening of IgG N-glycosylation markers associated with ankylosing spondylitis

Objective:To evaluate the potential of IgG N-glycans as diagnostic biomarker for ankylosing spondylitis (AS) by comparing and analyzing the IgG N-glycan profiles with AS and healthy controls.Methods:A 1∶1 matched case-control study design was adopted, 81 AS patients who visited the Department of Rheumatology and Immunology at Taian City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University between July 2020 and June 2021 were recruited. These patients were matched with 81 healthy individuals undergoing routine physical checkup. The levels of IgG N-glycosylation in human plasma were quantitatively measured using ultrahigh-performance liquid chromatography. Binomial logistic regression analysis was performed to identify IgG N-glycan biomarkers associated with AS.Results:A total of 14 primary glycans and 13 derived traits showed statistically significant differences between the AS case group and the control group. Binomial logistic regression analysis showed that glycan peak 4, agalactosylated glycans, fucosylated glycans, and fucosylated agalactosylated glycans were positively associated with AS[ OR(95% CI)=1.12(1.01, 1.42), 1.21(1.03, 1.43), 1.48(1.08, 2.03), and 1.27(1.04, 1.55); P=0.036, 0.022, 0.039, 0.020, respectively]. In terms of diagnostic performance, the single glycan GP4 exhibited the largest area under the ROC curve, with an AUC (95% CI) 0.751 (0.677, 0.826), while the combined glycan indicators (GP4+G0+F+FG0) achieved an AUC (95% CI) 0.768(0.697, 0.840). Conclusion:IgG N-glycans have the potentials to serve as candidate biomarkers for AS, and warrants further investigation.
2.Effect of moxibustion on central insulin resistance related proteins in diabetic rats with cognitive decline.

OBJECTIVE:
To investigate the effect of moxibustion on central insulin resistance related proteins of the rats suffering from diabetic cognitive decline, and analyze the underlying mechanism of moxibustion for cognition improvement.
METHODS:
Using the intraperitoneal injection of STZ combined with a high-fat diet, the rat model of diabetic cognitive decline were prepared. Twenty successfully-modeled rats were assigned randomly into a model group and a moxibustion group, 10 rats in each one. Besides, a blank group was set up with 10 rats collected. In the moxibustion group, suspending moxibustion was applied to "Baihui" (GV20), "Shenting" (GV24) and "Dazhui" (GV14) at the same time, 20 min in each intervention, once a day, and 6 interventions were delivered weekly and the duration of treatment was consecutive 4 weeks. The random blood glucose was measured using glucometer, and the learning-memory ability was detected by water maze test. HE staining was used to observe the morphology of neurons in the hippocampal tissue, real-time PCR assay was to detect mRNA expression of insulin receptor substrate 1 (IRS1), phosphatidylinositol 3-kinase (PI3K) and protein kinase B (AKT) in the hippocampal tissue. The Western blot method was employed to detect the protein expression of IRS1, PI3K, AKT, phosphorylated IRS1 (p-IRS1), phosphorylated PI3K (p-PI3K) and phosphorylated AKT (p-AKT) in the hippocampal tissue, and the ratio of p-IRS1/IRS1, p-PI3K/PI3K and p-AKT/AKT was calculated separately. The immunofluorescence intensity of p-IRS1, p-PI3K, and p-AKT was measured using immunofluorescence.
RESULTS:
Compared with the blank group, the rats of the model group exhibited higher random blood glucose (P<0.001), longer escape latency (P<0.001), severe pathological damage in the hippocampus, lower mRNA expression of IRS1, PI3K, and AKT (P<0.001), reduced ratio of p-IRS1/IRS1, p-PI3K/PI3K and p-AKT/AKT (P<0.001), and declined immunofluorescence intensity of p-IRS1, p-PI3K, and p-AKT in the hippocampal tissue (P<0.001). In comparison with the model group, for the rats of the moxibustion group, the random blood glucose decreased (P<0.05), the escape latency was shortened (P<0.01), the hippocampal pathological damage was attenuated, the mRNA expression of IRS1, PI3K and AKT increased (P<0.01), the ratio of p-IRS1/IRS1, p-PI3K/PI3K and p-AKT/AKT was elevated (P<0.01, P<0.05), and the immunofluorescence intensity of p-IRS1, p-PI3K, and p-AKT in the hippocampal tissue was strengthened (P<0.01, P<0.05).
CONCLUSION
In diabetic rats experiencing cognitive decline, moxibustion can enhance the learning-memory ability, which may be attributed to modulating the protein expression of IRS1, PI3K, and AKT, and their phosphorylation, activating insulin signal transduction, and reducing central insulin resistance.

4.Application of discharge preparation services in patients undergoing uniportal video-assisted thoracoscopic surgery for lung cancer

Objective:To explore the effect of discharge preparation services in patients undergoing uniportal video-assisted thoracoscopic surgery for lung cancer.Methods:A total of 192 lung cancer patients who underwent uniportal video-assisted thoracoscopic surgery between April 2021 and May 2023 were selected using a convenience sampling method. The 97 patients admitted from April 2021 to April 2022 were designated as the control group and received routine care, while the 95 patients admitted from May 2022 to May 2023 were designated as the observation group and received discharge preparation services. Postoperative recovery indicators (e.g., time to first meal, time to first ambulation), the Readiness for Hospital Discharge Scale (RHDS), the Chinese version of the Functional Assessment of Cancer Therapy-Lung (FACT-L), and the nursing service satisfaction questionnaire were used to evaluate the intervention's effects.Results:The observation group had significantly shorter times to first meal, first ambulation, and drain removal, as well as lower complication rates compared to the control group, with statistically significant differences ( P<0.05). After the intervention, the RHDS, FACT-L, and nursing service satisfaction questionnaire scores were significantly higher in the observation group compared to the control group, with statistically significant differences ( P<0.05) . Conclusions:The application of discharge preparation services improves postoperative recovery, discharge readiness, quality of life, and satisfaction with nursing services in patients undergoing lung cancer surgery.
5.Transmission chains of local epidemic of COVID-19 caused by 2019-nCoV Delta variant in Zhenhai district, Ningbo

Objective:To investigate the local epidemic of COVID-19 caused by 2019-nCoV Delta variant in Zhenhai district of Ningbo, identify the transmission chain and provide reference for the prevention and control of COVID-19 epidemic.Methods:The incidence data of COVID-19 in Zhenhai from 6 to 18 December, 2021 were collected in field investigation. Field epidemiological investigation was conducted to understand the epidemiological characteristics of COVID-19 cases and analyze the transmission chains.Results:The first case might be infected with 2019-nCoV through direct or indirect exposure when passing through a medium-risk area, then a family cluster was caused, and the epidemic spread through close contacts of family members with others such as work, daily life, and moxibustion. The epidemic lasted for 14 days, and 74 confirmed COVID-19 cases were reported. The median incubation period was 4.0(3.0,5.8)d. All the cases were in a chain of transmission for more than 6 generations, and the intergenerational interval was 3.5(2.0,5.3)d. The gene sequencing result indicated that the pathogen was Delta AY.4 variant of 2019-nCoV. Both the epidemiological investigation and the gene sequencing results supported that the local COVID-19 epidemic in Zhenhai was associated with the COVID-19 epidemic in Shanghai.Conclusions:The transmission chain of this epidemic was clear. Delta AY.4 variant has obvious characteristic to cause case clusters in families, places with poor ventilation, and residential communities. It is suggested to strengthen the health management in key areas and key populations, and increase the frequency of nucleic acid testing.
6.Size estimation of MSM population in Pudong New Area using new media technology

Objective To explore the use of new media technology to estimate the size of the online MSM population in Pudong New Area, to understand the frequency of new media software use, HIV testing and high-risk behavior, and to provide a basis for AIDS prevention and control. Methods The online MSM registration information in Pudong New Area was collected through a gay dating APP. At five time points on March 28 and April 11, 2019, MSM who logged into the gay dating APP were counted. The MSM were confirmed by comparing the login name and registration information. The capture-recapture (C-R) method was used to calculate the MSM population size. Meanwhile, a questionnaire survey was conducted on the online MSM. Results The MSM population size in Pudong New Area was estimated at 13 174 [95% CI: 9,672~16,676] through the C-R method. The proportion of multiple sexual partners accounted for 72.5%. According to the ordered multi-classification logistics analysis, age, education level, experience of attending a talk on AIDS, and experience of receiving AIDS publicity and education in the VCT clinic were the influencing factors of AIDS awareness. Conclusion New media technology provided new technical means for estimating the size of MSM population. The MSM population’s awareness of AIDS risk and prevention still needs to be strengthened. It is worthy of further studies on the role of new media technology in AIDS prevention and health education in the future.
7.Observation on the therapeutic effect of hyperbaric oxygen on acute encephalopathy and myocardial damage secondary to carbon monoxide poisoning in 105 cases

Objective:To observe and study the therapeutic effect of hyperbaric oxygen (HBO) on acute encephalopathy and myocardial damage secondary to carbon monoxide (CO) poisoning.Methods:A total of 451 cases of CO poisoning admitted to the Department of Hyperbaric Oxygen of Central Hospital of Zibo Mining Group Co., Ltd. were retrospectively analyzed, including 105 cases of acute encephalopathy with secondary myocardial damage of different degrees. After dehydration, nerve nutrition, circulation improvement, waking up, organ protection, anti-infection, and other supportive symptomatic treatments, HBO was given for 2-3 courses. The myocardial enzyme levels of carboxyhemoglobin (COHb), cardiac troponin (cTnI), creatine kinase (CK), creatine kinase myocardial isoenzyme (CK-MB), and lactate dehydrogenase (LDH) were measured in all patients before and after treatment; the dynamic abnormal changes were monitored by 12-lead electrocardiograph (EKG); and the cerebrospinal fluid pressure was measured by lumbar puncture.Results:The blood biochemical test results after CO poisoning in 105 patients were as follows: the level of COHb was 40.3%~54.7%, with an average of (44.5±7.9)%; myocardial enzymes such as cTnI, CK, CK-MB, and LDH were significantly increased with 5-10 times of the normal values. The EKG of 105 patients showed significant abnormalities. The intracranial pressure was from 185 to 230 cmH 2O, with an average of (210.6±22.9) cmH 2O. After 2-3 courses of combined treatment, the level of COHb in 105 patients was 1.3%~4.7%, with an average of (3.2±0.6)%; the myocardial enzymes such as cTnI, CK, CK-MB and LDH returned to normal values. The intracranial pressure ranged from 160 to 178 cmH 2O, with an average of (166.4±11.9) cmH 2O. After treatment, 95 (90.5%) patients’ EKG gradually returned to normal; among the 10 patients (9.5%) with abnormal EKG, 5 patients had insufficient myocardial blood supply, 3 patients had atrial premature beat, and 2 patients had ventricular premature beat. There were significant differences in these indicators before and after treatment ( P<0.05, or P<0.01). Conclusion:CO poisoning is prone to secondary acute encephalopathy and myocardial damage. HBO combined therapy is highly effective for treating secondary acute encephalopathy and myocardial damage caused by CO poisoning, therefore it can provide a reference for clinical treatment.
8.Study on the trend of vital signs in patients with granulomatous infection of malignant hematological diseases

Objective:To investigate the death of patients with granulocytopenia complicated with infection after chemotherapy and the changes of vital signs before emergency treatment, and to analyze the prognosis of different vital signs on patients' prognosis and emergency treatment.Methods:This study used a case-control study method to select 211 patients with hematologic malignancies who met the inclusion criteria in two tertiary hospitals in Weifang City. The vital signs of patients were collected and the vital signs were analyzed using SPSS 17.0 software. And the statistical significance and predictive value in the emergency response group.Results:The heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were significantly different between the survival group (112 cases) and the death group (99 cases)( t values were 11.038-177.102, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, and blood oxygenation saturation and urine volume were 0.547, 0.495, 0.294, 0.899, 0.988, and 0.827, respectively. The highest predictive efficiency (higher level) was observed with the change of blood oxygen saturation, and the corresponding optimal cutoff point. 0.91; between the emergency treatment group (103 cases) and the non-emergency treatment group(108 cases), the difference in heart rate, respiratory rate and oxygen saturation between the two groups was statistically significant ( t values were 5.247, 8.001, 9.066, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were 0.581, 0.732, 0.813, 0.346, 0.102, and 0.543, respectively. Among them, the predicted value of respiratory frequency change was the highest (medium level), which was the best corresponding. The cutoff point was 27.5. Conclusions:Patients with granulocytic infection after malignant hematologic disease will have abnormal changes in vital signs before death and emergency treatment. However, different vital signs have different effects on predicting disease changes, and should focus on respiratory rate and oxygen saturation. Changes, when the respiratory rate exceeds 27 beats/min, the probability that the patient needs to implement emergency treatment such as rescue will increase. If the condition is not effectively controlled, the blood oxygen saturation is lower than 0.91, the risk of death of the patient is greatly increased.

10.Effect of peer support education on the survival quality of urbanized type 2 diabetes patients in rural areas

Objective To explore the effect of peer support education on self-management ability and survival quality of urbanized type 2 diabetes patients in rural areas. Methods From January to September 2015, a total of 117 patients with type 2 diabetes were selected from three large-scale living communities of Fengxian District.Then,they were randomly divided into observation group(58 cases)and control group (59 cases) using random number method, respectively. And the same health education contents were conducted for the patients in both two groups.The patients in control group were educated by the diabetic nurse in classroom. Additionally, the peer support education was conducted for these patients in observation group for 6 months on this basis.The diabetes patient's self-management behavior scale and 36-item Short-Form Health Survey Questionnaire(SF-36)were used to assess the patients before intervention and 6 months later,respectively.Besides,the level of glycosylated hemoglobin was measured by the automatic biochemical analyzer. The self- management ability and survival quality and glycometabolism level were assessed as well. Results 6 months later after intervention, the survival quality score in observation group was as following:physical function(PF) 73.6±11.5,role physical(RP) 86.7 ± 14.5, body pain (BP) 76.3 ± 12.3, general health (GH) 64.5 ± 13.6, vitality (VT) 67.5 ± 12.3, social function (SF) 87.1 ± 16.2, role emotional (RE) 81.1 ± 14.2, mental health (MH) 68.9 ± 12.6, while that in control group was as following:physical function(PF)51.3±10.6,role physical(RP)62.1±12.1,body pain (BP)51.3±10.8,general health(GH)51.1±11.6,vitality(VT)56.4±12.1,social function(SF)70.1±14.1, role emotional (RE) 71.2 ± 13.9, mental health (MH) 56.1 ± 10.8 (t=2.316-6.365, P<0.05). The self-management score in observation group was following:diet management 5.79±0.98,exercise management 5.41 ± 0.57, blood glucose monitoring 4.01 ± 0.90, foot care 5.26 ± 0.89, drug administration 4.47 ± 0.90, smoking 0.86 ± 0.34, while that in control group was following: diet management 3.62 ± 0.99, exercise management 3.66 ± 0.94, blood glucose monitoring 1.90 ± 0.84, foot care 2.54 ± 0.62, drug administration 3.73 ± 0.80, smoking 0.76 ± 0.42 (t=4.659-19.137, P of rest was less than 0.01 except that of smoking equaled to 0.172 and t=1.375). Conclusions Peer support education can improve the quality of life, enhance self-management ability, control the level of glucose metabolism and has a strong operability. Therefore,it is worthy of promotion.
