Background:
s/Aims: Non-invasive models stratifying clinically significant portal hypertension (CSPH) are limited. Herein, we developed a new non-invasive model for predicting CSPH in patients with compensated cirrhosis and investigated whether carvedilol can prevent hepatic decompensation in patients with high-risk CSPH stratified using the new model.
Methods:
Non-invasive risk factors of CSPH were identified via systematic review and meta-analysis of studies involving patients with hepatic venous pressure gradient (HVPG). A new non-invasive model was validated for various performance aspects in three cohorts, i.e., a multicenter HVPG cohort, a follow-up cohort, and a carvediloltreating cohort.
Results:
In the meta-analysis with six studies (n=819), liver stiffness measurement and platelet count were identified as independent risk factors for CSPH and were used to develop the new “CSPH risk” model. In the HVPG cohort (n=151), the new model accurately predicted CSPH with cutoff values of 0 and –0.68 for ruling in and out CSPH, respectively. In the follow-up cohort (n=1,102), the cumulative incidences of decompensation events significantly differed using the cutoff values of <–0.68 (low-risk), –0.68 to 0 (medium-risk), and >0 (high-risk). In the carvediloltreated cohort, patients with high-risk CSPH treated with carvedilol (n=81) had lower rates of decompensation events than non-selective beta-blockers untreated patients with high-risk CSPH (n=613 before propensity score matching [PSM], n=162 after PSM).
Conclusions
Treatment with carvedilol significantly reduces the risk of hepatic decompensation in patients with high-risk CSPH stratified by the new model.

Objective To investigate the predictive value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)radiomics combined with Magee equation 3(ME3)for pathological response following neoadjuvant chemotherapy(NAC)in patients with hormone receptor-positive/human epidermal growth factor receptor 2-negative(HR+/HER2-)breast cancer. Methods A ret-rospective analysis was performed on 325 breast cancer patients diagnosed with HR+/HER2-in two hospitals.Patients from the first hospital were randomly divided into training and internal validation sets at a ratio of 7∶3.Patients from the second hospital served as external validation set.The volume of interest(VOI)of breast cancer was delineated on DCE-MRI images. Then the rele-vant radiomics features were extracted and selected, and the Radiomics score(Radscore)was calculated. The statistically significant clinical and pathological features and Radscore were incorporated into a multivariate analysis. The combined radiomics-clinical model and nomogram were established, and the prediction performance was evaluated by using the receiver operating characteristic(ROC)curve, calibration curve, and decision curve analysis(DCA).Results In the training set, seven radiomics features were selected to construct the radiomics model.ME3 score emerged as the independent factor for pathological complete response(pCR)(P<0.001).By integrating ME3 score and Radscore,a combined radiomics-clinical model was developed. This model demonstrated robust predictive accuracy, with area under the curve(AUC)of 0.88[95%confidence interval(CI)0.80-0.95],0.90(95%CI 0.82-0.98),and 0.82(95%CI 0.70-0.93)in the training, internal validation, and external validation sets, respectively. The nomogram construc-ted from the combined model exhibited excellent discrimination and calibration, with P-values of 0.455,0.312 and 0.062 in the train-ing,internal validation, and external validation sets. Conclusion DCE-MRI radiomics combined with ME3 can be used to predict the pathological response of NAC in HR+/HER2-breast cancer.

The rise of nanomedicine has opened up new avenues for drug delivery and tumor treatment.Nanocarriers,in particular, have become core tools in the field of drug delivery.Anti-cancer peptides(ACPs)can achieve anti-tumor effects by inducing tumor cell apoptosis, inhibiting angiogenesis, and regulating the immune microenvironment.Their low toxic side effects and drug resistance make them im-portant in the field of cancer treatment. However,ACPs face limitations such as poor stability, short in vivo half-life,and low targeted delivery efficiency, which seriously limit their therapeutic effects and fur-ther development.Nanocarriers provide an efficient, flexible and precise solution for the delivery of ACPs due to their advantages of increasing drug solubility, changing drug distribution in the body and improving drug targeting. This review systematically summarizes the structural characteristics and biological proper-ties of four types of nanocarriers, including liposome nanocarriers, polymer nanocarriers, inorganic nano-carriers,and self-assembled peptides, and focuses on analyzing the application examples of nanocarriers in enhancing the stability of ACPs,improving targeted delivery efficiency, improving bioavailability, and overcoming drug-resistant tumor treatment. We also summarize in detail the structural pros and cons of these four carriers. Finally,we look forward to the development direction of research on the combination of nanocarriers and ACPs,in order to provide a theoretical basis for the clinical application of ACPs.

Objective To investigate the effect of electroacupuncture(EA)on the behaviors of lipopolysaccharide(LPS)-induced depression-like mice and explore the potential molecular mechanism of the antidepressant effect of EA using transcriptomics sequencing technology. Methods Thirty-six male specific pathogen-free-grade C57BL/6J mice were divided into the normal, the model, and the EA groups using the random number table method, with 12 mice per group. The EA group received a daily 20-minute EA at the"Baihui"and"Yintang"acupoints for 7 days. After 7 days of EA treatment,1.5 mg/kg LPS was injected intraperitoneally into the model and EA groups, whereas the normal group received an injection of an equal volume of saline solution.Depressive behaviors were assessed using the open-field test,tail-suspension test, and other relevant tests.Pathologic morphology of the hippocampus tissue was determined using hematoxylin-eosin staining.Transcriptomics technology was used to identify differentially expressed genes in the hippocampus, and bioinformatics analysis was performed.Real-time fluorescence quantitative PCR was performed to detect the mRNA expressions of hematopoietic cell kinase(Hck),adhesion G protein-coupled receptor E1(Adgre1),Toll-like receptor 2(Tlr2),and Gm49037 in the hippocampus. Results Compared to the normal group, mice in the model group exhibited a decrease in total travel distance, central travel distance, center area time(P<0.01),as well as an increase in tail-suspension immobility time(P<0.05).Hippocampal neurons were loosely arranged, and some neurons had blurred boundaries.Compared to the model group, mice in the EA group showed an increase in open-field total travel distance, central travel distance, center area time(P<0.05),as well as a decrease in tail-suspension immobility time(P<0.01);Additionally,hippocampal neurons were more tightly arranged, exhibiting clearer morphology and structure.Transcriptomic analysis revealed that 779 differentially expressed genes were identified in the model group compared to the normal group, whereas 172 genes differed between the EA and model groups.Nineteen genes were upregulated in the model group but downregulated in the EA group, and ten genes were down-regulated in the model group but up-regulated in the EA group.Gene Ontology Functional Analysis indicated that these genes primarily function in immune responses,Toll-like receptor activity, cell surface receptor signaling pathways, and interleukin-10 production.Kyoto Encyclopedia of Genes and Genomes pathway analysis revealed enrichment in inflammation-related pathways, including Toll-like receptor signaling pathway and tumor necrosis factor signaling pathway.Compared to the normal group,Hck,Adgre1,and Tlr2 mRNA expressions were elevated in the model group(P<0.01),whereas there was a decrease tendency for Gm49037 mRNA expression and there was no significant difference(P>0.05);compared to the model group,Hck,Adgre1,and Tlr2 mRNA expressions were decreased in the EA group(P<0.05),whereas there was an increase tendency for Gm49037 mRNA expression and there was no significant difference(P>0.05).Conclusion EA alleviated depressive-like behavior and pathological damage in the hippocampus in LPS-induced mice, which may be related to attenuating hippocampal inflammatory pathways, and Hck,Adgre1,and Tlr2 genes may be the potential targets for its effect.
