1.Vitamin D and ferritin nutritional status among primary and secondary school students in the key nutrition monitoring counties of Xinjiang

Objective:
To analyze the relationship between vitamin D and ferritin nutritional status with physical indicators in primary and secondary school students in the key nutrition monitoring counties of Xinjiang, so as to provide reference for early prevention and intervention of students nutrition issues.
Methods:
From November to December 2023, 1 071 students aged 6-18 from key nutrition monitoring counties in the Xinjiang Nutrition Improvement Plan area were selected via stratified random sampling for physical and biochemical tests. The U test and Kruskal-Wallis H-test were used to compare the differences in physical indicators and the distribution of vitamin D and ferritin levels. Chi-square test was used to compare the prevalence of vitamin D and ferritin deficiencies among different groups of primary and secondary school students. Pearson correlation was used to analyze the relationship between the nutritional levels of vitamin D and ferritin and physical indicators.
Results:
The median vitamin D level was 14.7 (10.7, 19.0)ng/mL, with deficiency and insufficiency rates of 30.8% and 37.4% among primary and secondary school students in the key nutrition monitoring counties of Xinjiang Nutrition Improvement Plan area. Ferritin levels were 57.4 (37.7, 83.9)μg/L, with a deficiency rate of 5.7%. Males, primary school students, and rural residents had higher vitamin D and ferritin levels than females, secondary school students, and urban residents ( U =-11.35, -6.88, -4.52; -3.94, -9.17, -5.23, P <0.05). Vitamin D deficiency was more prevalent in females, secondary school students, and urban students ( χ 2=97.52, 49.01, 21.89, P <0.05), while ferritin deficiency was higher in primary school students and urban areas ( χ 2=34.11, 5.63, P <0.05). Significant differences in body mass index (BMI) and waist circumstance (WC) were observed across vitamin D/ferritin statuses ( U/H =35.47, 22.82; -4.19 , -5.36, P <0.05). Vitamin D and ferritin levels negatively correlated with age, BMI, and WC but positively with waist-to-height ratio (WHtR) ( r = -0.31, -0.19, -0.19, 0.20; -0.32, -0.13, -0.21, 0.08, P <0.05).
Conclusions
Vitamin D and ferritin levels in primary and secondary school students in key nutritional monitoring counties in Xinjiang are correlated with age, BMI, WC and WHtR, and there are nutrient deficiencies. Targeted measures are recommended to improve nutritional status and physical health.
2.Expression of polybromo 1 in prostate cancer tissues and its relationship with CD8+T lymphocyte infiltration

Objective To investigate the expression of polybromo 1(PBRM1)in prostate canc-er tissues and its correlation with CD8+T lymphocyte infiltration.Methods A total of 124 prostate-cancer patients were enrolled as observation group,and 80 patients with benign prostate tumors during the same period were selected as control group.Immunohistochemical staining(IHC),real-time fluo-rescent quantitative polymerase chain reaction(qRT-PCR),and western blot were used to detect the expression of PBRM1 in the tumor tissues of both groups and the adjacent normal tissues of the obser-vation group(more than 1 cm from the tumor edge).Flow cytometry was employed to detect the infil-tration of CD8+and CD4+T lymphocytes in the tissues.Results The positive expression rate of PBRM1 in the tumor tissues of the observation group was 59.68%,which was significantly lower than 85.00%in the control group and 81.45%in the adjacent normal tissues of the observation group(P<0.001).The relative expression levels of PBRM1 mRNA and PBRM1 protein,as well as the percentage of CD8+T lymphocyte infiltration in the tumor tissues of the observation group were all lower than those in the tumor tissues of the control group and the adjacent normal tissues of the obser-vation group(P<0.05).Pearson correlation analysis revealed a positive correlation between the rel-ative expression level of PBRM1 protein and the percentage of CD8+T lymphocyte infiltration in the tumor tissues(r=0.856,P<0.001).In the observation group,patients with tumors having a maxi-mum diameter ≥3 cm,poor differentiation,clinical stage Ⅲ to Ⅳ,Gleason score ≥8,and prostate-specific antigen(PSA)≥20 ng/mL had significantly lower relative expression levels of PBRM1 protein compared to those with tumors having a maximum diameter<3 cm,moderate to well differentiation,clinical stage Ⅰ to Ⅱ,Gleason score<8,and PSA<20 ng/mL,and the differences were statistically sig-nificant(P<0.05).Conclusion Downregulation of PBRM1 expression in prostate cancer tissues may be associated with an increase in tumor malignancy and a decrease in CD8+T lymphocyte infil-tration,thereby enhancing the tumor's immune escape ability.
3.Study on Preoperative Serum Exosomes miR-193a and miR-208b Levels Expression and Their Prognostic Predictive Value in Patients Undergoing Radical Gastrectomy for Gastric Cancer

Objective To investigate the expression levels and prognostic value of preoperative serum exosomes micro RNA(miR)-193a and micro RNA(miR)-208b in patients with gastric cancer(GC).Methods A total of 132 patients who underwent GC radical gastrectomy in the First Affiliated Hospital of Xinjiang Medical University from March 2018 to March 2020 were regarded as the GC group,while 132 healthy individuals who underwent physical examination were selected as the control group.Their clinical and pathological data were collected and compared.The relative expression levels of miR-193a and miR-208b in serum exosomes were detected using quantitative real-time polymerase chain reaction(qRT-PCR)method.Pearson method was used to analyze the correlation between miR-193a and miR-208b.The correlation between the expression of miR-193a and miR-208b in preoperative serum exosomes of GC patients and postoperative prognosis was analyzed using Kaplan-Meier method.Univariate and multivariate COX regression were applied to analyze the influencing factors of prognosis.Results The expression level of miR-208b in the serum exosomes of the GC group was higher than that of the control group(1.77±0.14 vs 1.02±0.01),while the expression level of miR-193a was lower than that of the control group(0.52±0.06 vs 1.01±0.01),and the differences were statistically significant(t=92.551,61.392,all P<0.05).The expression levels of miR-193a and miR-208b in GC patients before surgery were negatively correlated(r=-0.409,P<0.05).The low expression rate of miR-193a and high expression rate of miR-208b in patients with TNM stage Ⅰ+Ⅱ,no lymph node metastasis,and no distant metastasis were lower than those in patients with TNM stage Ⅲ+Ⅳ,lymph node metastasis,and distant metastasis,and the differences were statistically significant(χ2=5.008,4.397;7.142,4.688;4.407,5.189,all P<0.05).The 3-year cumulative survival rate of patients with low expression of miR-193a(30.43%)was lower than that of patients with high expression(60.32%)(χ2=17.861,P<0.001),while the 3-year cumulative survival rate of patients with high expression of miR-208b(27.14%)was lower than that of patients with low expression(64.52%)(χ2=16.340,P<0.001).The independent prognostic factors included serum levels of exosomes miR-193a(HR=0.493,95%CI:0.323~0.753)and miR-208b(HR=2.697,95%CI:1.382~5.262)(all P<0.05).Conclusion The preoperative serum miR-193a level in the exosomes was decreased and miR-208b level was increased,and their expression levels were related to the prognosis of patients undergoing GC radical gastrectomy.
4.Construction a nomogram model for predicting stress urinary incontinence in young and middle-aged women

Objective:To construct a nomogram model for predicting stress urinary incontinence (SUI) in young and middle-aged women.Methods:Using a sampling survey method, 1 000 questionnaires were distributed to young and middle-aged women in 2 streets of Urumqi community from May 2021 to October 2023 to investigate their basic situation, lifestyle habits and gynecological related information. The International Urinary Incontinence Advisory Committee urinary incontinence questionnaire was used to diagnose SUI, and the patients were divided into SUI group and control group based on the results. Seven hundred and eighty-six questionnaires were collected. The survey results of the two groups were analyzed, and a nomogram model for predicting the occurrence of SUI in young and middle-aged women was constructed and validated.Results:Among the 786 young and middle-aged women, there were 147 cases in the SUI group and 639 cases in the control group. The age, body mass index (BMI), and the incidences of diabetes, chronic constipation, delivery history, macrosomia delivery history, pelvic floor dysfunction in SUI group were significantly higher than those in control group: (44.51 ± 8.20) years vs. (38.60 ± 12.35) years, (27.31 ± 4.53) kg/m 2 vs. (24.28 ± 4.38) kg/m 2, 13.61% (20/147) vs. 3.44% (22/639), 19.05% (28/147) vs. 5.01% (32/639), 90.48% (133/147) vs. 75.90% (485/639), 17.01% (25/147) vs. 3.44% (22/639) and 11.56% (17/147) vs. 3.29% (21/639), and there were statistical differences ( P<0.01). Multivariate Logistic regression analysis result showed that age>44 years, BMI≥30 kg/m 2, diabetes, chronic constipation, delivery history, macrosomia delivery history and pelvic floor dysfunction were independent risk factors for SUI in young and middle-aged women ( RR = 1.511, 2.543, 4.636, 4.293, 2.526, 6.220 and 5.834; 95% CI 1.007 to 2.268, 1.661 to 3.894, 2.281 to 9.422, 2.339 to 7.881, 1.374 to 4.643, 3.205 to 12.071 and 2.641 to 12.888; P<0.05 or <0.01). The age, BMI, diabetes, chronic constipation, delivery history, macrosomia delivery history and pelvic floor dysfunction were used as predictors to construct a nomogram model for predicting the SUI in young and middle-aged women. The 550 cases were randomly selected from the dataset as the training set and the remaining 236 cases as the validation set. The receiver operating characteristic curve was drawn, and the result showed that the area under the training set curve was 0.818 (95% CI 0.773 to 0.862), and the area under the validation set curve was 0.826 (95% CI 0.764 to 0.889); the Hosmer-Lemeshow goodness of fit test in validation set that result showed that the nomogram model had high reliability ( χ2 = 8.48, P>0.05). Conclusions:The incidence of SUI in young and middle-aged women is high. The age >44 years, BMI≥30 kg/m 2, diabetes, chronic constipation, delivery history, macrosomia delivery history and pelvic floor dysfunction are independent risk factors for SUI in young and middle-aged women. The nomogram model based on related risk factors has high predictive value and credibility.
5.The relationship between PM 2.5 exposure concentration in different environments during pregnancy and iodine nutritional level of pregnant women

Objective:To investigate whether there is an correlation between exposure to fine particulate matter (PM 2.5) in different environments during pregnancy and iodine nutritional levels of pregnant women. Methods:From March to August 2023, a case-control study method was used to select Midong District of Urumqi City with severe PM 2.5 pollution and Bole City of Bortala Mongolian Autonomous Prefecture with higher air quality index as the survey sites. Pregnant women before 28 weeks of pregnancy who voluntarily participated in this study and met the inclusion and exclusion criteria, were selected as the study subjects at the above locations. The basic information of the study subjects was investigated in the form of questionnaires. Whole blood, saliva, and urine samples were collected from the study subjects to determine serum iodine, salivary iodine, urinary iodine, and urinary creatinine levels. The average daily PM 2.5 exposure concentrations of pregnant women in outdoor, indoor, and multiple environments were calculated in Midong District and Bole City, respectively. The correlation between PM 2.5 exposure concentrations in different environments during pregnancy and iodine nutrition level of pregnant women was analyzed through a generalized linear model. Results:Midong District included 157 pregnant women, and Bole City included 145 pregnant women. There were statistically significant differences in the outdoor daily average PM 2.5 exposure concentration, indoor daily average PM 2.5 exposure concentration, daily maximum PM 2.5 exposure concentration in multiple environments, and daily minimum PM 2.5 exposure concentration in multiple environments among pregnant women between the two regions ( P < 0.05). The serum iodine [ M ( Q1, Q3): 83.99 (71.93, 93.39), 86.93 (76.90, 98.31) μg/L] and salivary iodine levels [76.90 (44.93, 140.36), 50.22 (24.12, 82.77) μg/L] of pregnant women in the two regions were compared, and the differences were statistically significant ( P < 0.05). The overall indoor and outdoor daily average PM 2.5 exposure concentrations of pregnant women in Midong District and Bole City were positively correlated with salivary iodine levels ( P < 0.001), and the daily minimum PM 2.5 exposure concentration in multiple environments was positively correlated with serum iodine levels ( P = 0.019). Conclusion:There are differences in iodine nutrition levels among pregnant women in different PM 2.5 exposure areas, and exposure to PM 2.5 during pregnancy may lead to fluctuations in iodine nutrition levels among pregnant women.

7.Effect of preoperative 25 hydroxyvitamin D deficiency on posoperative parathyroid function in patients with papillary thyroid cancer

Objective:To investigate the effect of 25 hydroxyvitamin D [25 (OH) D] on postoperative parathyroid function in patients with papillary thyroid carcinoma (PTC) .Methods:426 patients who diagnosed as PTC and underwent at least total thyroidectomy plus central lymph node dissection (including lateral lymph node dissection) in Center of Thyroid and Parathyroid Surgery of West China Hospital, Sichuan University, from Jun. 2017 to Sep. 2019 were retrospectively analyzed. Among them, 313 were women and 113 were men, they were divided into vitamin D deficient (VDD) group ( n=297) and normal group ( n=129) according to preoperative 25 (OH) D levels ≤ 20ng/ml and > 20ng/ml. SPSS 23.0 statistical software was used for data processing. Pearson χ2 test or Fisher exact test were used to compare the incidence of clinical hypoparathyroidism, biochemical temporary and biochemical permanent hypoparathyroidism between the two groups. Univariate and multivariate logistic regression analysis were used to analyze the related factors of postoperative hypoparathyroidism between the two groups. Results:Preoperative 25 (OH) D level was significantly correlated with postoperative biochemical temporary hypoparathyroidism ( P=0.04, OR=1.11) . Compared with patients with adequate preoperative vitamin D, preoperative vitamin D deficiency increased the risk of biochemical transient hypoparathyroidism by 6.67 times ( P=0.03;OR=6.67) . There was no significant correlation between preoperative vitamin D deficiency and early postoperative clinical hypoparathyroidism (2.7% vs 3.9%, P=0.55) . Conclusion:Correction of vitamin D deficiency before operation may reduce the incidence of postoperative temporary hypoparathyroidism caused by 25 (OH) D deficiency.
8.Predictive value of parathyroid hormone decline rate for permanent hypoparathyroidism after thyroidectomy

Objective:To investigate the value of the decrease rate of parathyroid hormone (PTH) in predicting permanent hypoparathyroidism (PHP) in one month after operation of papillary thyroid carcinoma (PTC) .Methods:The clinical data of 322 patients with PTC who underwent at least total thyroidectomy and unilateral central lymph node dissection in the Thyroid Surgery Center of West China Hospital of Sichuan University from Jan. 2013 to Jun. 2018 were retrospectively analyzed. There were 257 women and 65 men. According to the time when PTH returned to normal, they were divided into 307 cases in the temporary hypoparathyroidism (THP) group and 15 cases in the PHP group. The decrease rate of PTH and the incidence of hypoparathyroidism in one month after operation were counted, the clinical characteristics and operation information were observed, and the risk factors of PHP were analyzed. SPSS 23.0 statistical software was used for data processing. χ 2 test and t test were used for comparison between THP and PHP. The related factors were analyzed by multivariate logistic regression. Results:307 cases of THP and 15 cases of PHP were included. Multivariate logistic analysis showed that age ≥ 55 years old ( OR, 35.193; 95% CI, 2.936 - 421.870; P=0.005) and PTH before operation closer to normal lower limit ( OR, 2.349; 95% CI, 1.161 - 4.755; P = 0.018) were independent risk factors for PHP. Patients with PHP had a higher rate of PTH decline in 1 month after operation ( OR, 373.439; 95% CI, 26.568 - 5248.983; P=0.000) . The ROC curve showed that the sensitivity and specificity of predicting PHP were 86.7% and 94.8% respectively. Conclusions:The age of patients with PTC ≥ 55 years old and PTH before operation closer to normal lower limit are independent risk factors for the occurrence of PHP. The decline rate of PTH in one month after operation which more than 73.2% can predict PHP.
9.Analysis of correlation factors of contralateral central lymph node metastasis in unilateral papillary thyroid carcinoma with lateral cervical lymph node metastasis

Objective:To examine the correlation factors of contralateral central lymph node metastasis in unilateral papillary thyroid carcinoma with lateral cervical lymph node metastasis.Methods:The clinical data of patients with unilateral papillary thyroid carcinoma who underwent total thyroidectomy and bilateral central lymph node dissection and ipsilateral cervical lymph node dissection from June 2016 to June 2018 at Department of Thyroid Surgery, West China Hospital, Sichuan University were analyzed retrospectively. A total of 317 patients, including 87 males and 230 females, aged (41.4±12.1) years (range: 16 to 75 years), were enrolled in this study. The risk factors of contralateral central lymph node metastasis were analyzed by χ 2 test and Spearman correlation analysis. Results:There were 116, 69, 269, and 181 cases of pretracheal lymph node, prelaryngeal lymph node, ipsilateral central lymph node and contralateral central lymph node metastasis, respectively, and 16 cases of skipping metastasis. Univariate analysis showed that contralateral central lymph node metastasis was associated with gender, maximum tumor diameter, capsule invasion, pretracheal lymph node metastasis, prelaryngeal lymph node metastasis, and ipsilateral central lymph node metastasis (all P<0.05). Spearman correlation analysis showed that male ( r s=0.162, P=0.004), maximum tumor diameter>10 mm ( r s=0.184, P=0.001), capsule invasion ( r s=0.135, P=0.016), pretracheal lymph node metastasis ( r s=0.394, P<0.01), prelaryngeal lymph node metastasis ( r s=0.272, P<0.01) and ipsilateral central lymph node metastasis ( r s=0.203, P<0.01) were independent correlation factors for contralateral central lymph node metastasis.
