Background:
Coronavirus disease 2019 (COVID-19)-associated mucormycosis of the maxilla emerged as a significant concern in India during the second wave of the pandemic, necessitating surgical interventions such as maxillectomy. This study investigated the quality of life following prosthodontic rehabilitation with interim obturators in patients who underwent bilateral total maxillectomy due to COVID-19-associated mucormycosis of the jaws.
Methods:
The study was conducted using questionnaire-based interviews administered by a single investigator employing the Oral Health Impact Profile-14 (OHIP-14) and Obturator Functioning Scale (OFS) questionnaires. Responses were evaluated on a Likert scale. All statistical analyses were performed using SPSS version 21.0 for Windows. A significance level of 5% was applied to all tests. The Wilcoxon signed-rank and chi-square tests were utilized to compare categorical and quantitative variables across groups. Responses to the questionnaires were recorded on numerical Likert scales ranging from 1 to 5 for OFS and 0 to 4 for OHIP-14. The mean OHIP-14 and OFS scores were compared between the pre-rehabilitation and post-rehabilitation stages.
Results:
A comparison between the pre-rehabilitation and post-rehabilitation scores from the OHIP-14 questionnaire revealed a significant difference (p= 0.001). The OFS results indicated significant improvements across all domains following the use of obturators.
Conclusion
Interim obturators play a vital role in improving speech, swallowing, and mastication during the recovery period for patients who have undergone bilateral total maxillectomy. Despite the altered anatomy resulting from the resection, patients adapted effectively and exhibited improvements in their social, psychological, and mental health.

In addition, it has bronchodilating, sedative, and amnestic properties, preserving airway reflexes and sympathetic nervous system tone. Since the discovery of ketamine, it has been a major topic of discussion due to controversies regarding its usage in particular sets of patients. In the past 50 years, despite its potential benefits, it is not commonly used because of concerns of “emergence phenomenon,” its use as a substance of abuse, and its systemic side effects. Since 2012, three World Health Organization reviews on ketamine have addressed its international control.Researchers have been studying this wonder drug for a decade worldwide. Many myths of ketamine regarding emergence phenomenon and its use in traumatic brain injury and open eye injury have been disproved in recent times. It is becoming popular in pre-hospital settings, critical care, emergency medicine, low-dose acute pain services, and adjuvant in regional anesthesia techniques. This review highlights the current consensus on the various applications of ketamine in the literature.

9.Complex Obstetric Perineal Injury Reconstruction Using Antropyloric Valve Transposition

Despite significant advancements in the field of medicine, management of complex obstetric perineal injuries remains a challenge. Although several surgical techniques have been described, no techniques have provided satisfactory long-term results. Recently, a perineal transposed antropyloric valve has been used for anorectal reconstruction in patients with damaged or excised anal sphincters. We describe this technique in the case of complex obstetric perineal trauma with extensive tissue loss, presenting with end stage fecal incontinence. The functional outcome after this procedure was evaluated. The patient tolerated the surgery well, and there were no procedure-related upper gastrointestinal disturbances. Short-term functional outcomes were encouraging. At the 36-month follow-up, the patient’s neoanal resting and squeeze pressures were 50 and 70 mmHg, respectively. The postoperative St. Mark’s incontinence score was 7. Perineal antropyloric valve transposition is feasible and can be successfully applied in the management of end-stage fecal incontinence associated with complex obstetric perineal injury.
