BACKGROUND
Medication compliance contributes to preventing significant morbidities, such as stroke, among hypertensive patients.
OBJECTIVEThis study aimed to examine the factors affecting hypertensive patients' medication compliance in Sambag II, Cebu City.
METHODSThis study is an observational cross-sectional study. The study was conducted in Barangay Sambag II of Cebu City and involved 170 random, voluntary, self-reporting hypertensive patients. The Tao Yamane formula was used to determine the sample size. An interviewer-guided survey questionnaire was utilized to collect the data for the pilot study. The data were analyzed using Cronbach’s Alpha Test for internal consistency and reliability.
RESULTSThree factors were shown to influence compliance with antihypertensive medications. These are the source of medication, access to medication, and symptoms experienced before intake of anti-hypertensive medications. A factor that significantly affects compliance is the source of medication, wherein hypertensive patients prefer medications from retail pharmacies compared to the free medications supplied by the local health center.
CONCLUSIONResidents of Sambag II, Cebu City preferred medications from retail pharmacies over the free medications provided by the local health center. Local health units may use this information to implement information drugs regarding the efficacy of medications provided by government agencies. Further studies are recommended to use subgroup analysis on factors influencing compliance and non-compliance to anti-hypertensive medicines.

3.Determination of toxicity and hypoglycemic effect in alloxan-induced diabetic mice of Manihot esculenta Crantz (Fam. Euphorbiaceae) aqueous crude leaf extract and its fractions

OBJECTIVE
Manihot esculenta (Crantz) leaves have been used for the management of diabetes based on cited ethnopharmacological studies. However, scientific evidence to support its efficacy is lacking. The aim of the study was to assess the cytotoxicity of the aqueous leaf extract and fractions of Manihot esculenta and its potential hypoglycemic effect on male Swiss albino mice.
METHODSThe cytotoxicity assay was necessary to screen which extract and fractions will be used for the in vivo hypoglycemic study.
Phytochemical screening identified the composition of the aqueous crude extract and its fractions. The phytochemical results showed the presence of alkaloids, anthraquinone glycosides, carbohydrates, phenolic glycosides, saponins, and flavonoids.
Cytotoxicity was screened using CytoTox 96® (Promega), a NonRadioactive Cytotoxicity Assay on liver cancer (HepG2) and normal kidney (HK-2) cell lines. Five samples were tagged as highly cytotoxic and were flagged for further assays. These samples were DCM fraction (100 ppm), n-Hexane fraction (1 and 10 ppm), Ethyl acetate fraction (1 ppm), and Aqueous fraction (100 ppm).
RESULTSThe hypoglycemic activity was examined in alloxan-induced diabetic mice using in vivo hypoglycemic study. The aqueous crude extract at dose levels of 200 mg/kg and 300 mg/kg body weight showed significant reduction in blood glucose levels compared to the diabetic control but not exceeding the results in the metformin treatment group. A p-value of 0.05 set a priori was used to consider whether the intervention had a statistically significant difference compared to the diabetic control.
CONCLUSIONThe effectiveness of aqueous crude extract in reduction of blood glucose in mice may be attributed to the synergistic effects of phytochemicals present, especially the alkaloids which were retained in the extract but were undetectable in the fractionated samples. The results of this study also support the findings of existing ethnopharmacological studies on M. esculenta leaf extract as a hypoglycemic agent.

Locally advanced low rectal cancers pose a challenge for surgeons as reported local recurrence remain high despite the performance of Total Mesorectal Excision and the provision of neoadjuvant treatment. Extralevator Abdominoperineal Excision offers better oncologic margins with reported decreased recurrence rates when compared to the conventional technique. The improvement in oncologic outcomes, however, comes at the cost of producing larger perineal defects—and with this comes concerns related to coverage. Presented here is a case of a patient with a locally advanced low rectal cancer with gluteal extension where a Vertical Rectus Abdominis Myocutaneous flap was utilized as a means of perineal reconstruction.

5.Lasting experiences of COVID-19 isolation in Filipino adults: An interpretative phenomenological analysis.

OBJECTIVES
To explore the lasting lived experiences of Filipino adults who were isolated due to COVID-19 as late adolescents and examine how these experiences shaped their health consciousness
METHODSThis qualitative study employed an interpretative phenomenological analysis using Colaizzi’s method. Six participants aged 18 and above, who were aged 17–19 during their COVID-19 diagnosis and isolation, were recruited. One-onone semi-structured interviews were conducted and thematically analyzed.
RESULTSFour main themes emerged: (1) Sources of Stress including academic disruptions, isolation, symptoms and healthcare inefficiencies; (2) Attitudes Toward the Experience ranging from fear to calm acceptance and social responsibility; (3) Coping Strategies such as external support and personal mechanisms like spirituality and hobbies; and (4) Impact of COVID-19 which encompassed interpersonal growth, normalization of illness and varying degrees of health behavior change.
CONCLUSIONWhile the experience of isolation during adolescence posed significant stress, it also offered opportunities for personal and interpersonal growth. Most participants exhibited increased health consciousness, though behavior change was limited. These insights highlight the value of integrating health behavior interventions for young adults shaped by pandemic experiences.

Background:
Ardisia serrata (Aunasin) is an endemic Philippine plant of the family Primulaceae, with several studiesshowing the genus Ardisia as having potential antibacterial, antiangiogenic, cytotoxic, and antipyretic properties.
Objective:
This study aims to determine the antibacterial and antibiofilm-forming activity of Ardisia serrata ethanolic and aqueous extracts on Escherichia coli, Methicillin-Sensitive Staphylococcus aureus (MSSA), and Methicillin-Resistant Staphylococcus aureus (MRSA).
Methods:
This is an experimental study testing the activity against bacterial strains of E. coli, MSSA, and MRSA using ethanolic and aqueous extracts of A. serrata leaves. Microtiter susceptibility and biofilm inhibition assays were done with two-fold dilutions of the extract against the selected strains using spectrophotometry with optical density (OD) at 600 nm and 595 nm, respectively, to quantify bacterial growth and biofilm inhibition. The bacterial susceptibility and biofilm inhibition activity was reported as percent inhibition (PI). Minimum inhibitory concentration (MIC), and minimum biofilm inhibition concentration (MBIC) values were obtained using logarithmic regression of the PI values.
Results:
A. serrata ethanolic extracts showed weak growth inhibitory activity against MSSA and MRSA with minimum inhibitory concentration (MIC) values of 2.6192 and 3.2988 mg/mL, respectively, but no biofilm inhibition activity was noted, while the aqueous extracts exhibited negligible biofilm inhibition activity against MSSA and MRSA with minimum biofilm inhibition concentration (MBIC) values of 13.5972 and 8964.82 mg/mL, respectively, and with no growth inhibition activity. Both ethanolic and aqueous extracts showed no growth inhibition and biofilm inhibition activities against E. coli.
Conclusion
Staphylococcus aureus is susceptible to the bioactivity of the leaf extracts of A. serrata and has potential to be used as an antibacterial in the treatment of infectious diseases.

Background:
Severe and critical COVID-19 disease is characterized by hyperinflammation involving pro-inflammatory cytokines, particularly IL-6. Tocilizumab is a monoclonal antibody that blocks IL-6 receptors.
Objectives:
This study evaluated the efficacy of tocilizumab in Filipino patients with severe to critical COVID-19 disease.
Methods:
This phase 3 randomized double-blind trial, included patients hospitalized for severe or critical COVID-19 in a 1:1 ratio to receive either tocilizumab plus local standard of care or placebo plus standard of care. Patients were eligible for a repeat IV infusion within 24-48 hours if they deteriorated or did not improve. Treatment success or clinical improvement was defined as at least two categories of improvement from baseline in the WHO 7-point Ordinal Scale of patient status, in an intention-to-treat manner.
Results:
Forty-nine (49) patients were randomized in the tocilizumab arm and 49 in the placebo arm. There was no significant difference in age, comorbidities, COVID-19 severity, need for mechanical ventilation, presence of acute respiratory distress syndrome, or biomarker levels between groups. Use of adjunctive therapy was similar between groups, with corticosteroid used in 91.8% in tocilizumab group and 81.6% in the placebo group, while remdesivir was used in 98% of participants in both groups. There was no significant difference between groups in terms of treatment success in both the intention-to-treat analysis (relative risk=1.05, 95% CI: 0.85-1.30) and per-protocol analysis (relative risk=0.98, 95% CI: 0.80 to 1.21). There was no significant difference in time to improvement of at least two categories relative to baseline on the 7-point Ordinal Scale of clinical status.
Conclusion
The use of tocilizumab on top of standard of care in the management of patients with severe to critical COVID-19 did not result in significant improvement as defined by the WHO 7-point Ordinal Scale of patient status, nor in significant improvement in incidence of mechanical ventilation, incidence of ICU admission, length of ICU stay, and mortality rate.

9.Exploratory Study of Epidermis, Basement Membrane Zone, Upper Dermis Alterations and Wnt Pathway Activation in Melasma Compared to Adjacent and Retroauricular Skin

BACKGROUND:
Melasma is a chronic acquired focal hypermelanosis which pathogenesis has not been fully elucidated. Classical pathophysiologic studies have analysed the affected and perilesional areas, but little is known about the status of sun-protected skin, which is subjected to the same endogenous and genetic factors.
OBJECTIVE:
To assess the histological characteristics of melasma compared to adjacent and retroauricular skin.
METHODS:
Skin samples were collected from 10 female from: melasma, perilesional area and retroauricular. The samples were stained (haematoxylin-eosin, periodic acid-Schiff, Fontana-Masson, picrosirius red, toluidine blue and Verhoeff), immunolabelled for CD34 and Wnt1. The data from the skin sites were analysed simultaneously by a multivariate model.
RESULTS:
Melasma skin exhibited noteworthy stratum corneum compaction, greater collagen heterogeneity, solar elastosis, higher number of mast cells, basement membrane zone (BMZ) damage, Wnt1 expression, pendulum melanocytes, higher cellularity and vascular proliferation at the superficial dermis. Stratum corneum compaction, collagen heterogeneity and BMZ abnormalities were variables associated to melasma that not follow a continuum through retroauricular to adjacent skin. Mast cell count was the variable that disclosed correlation with the most other abnormalities as well as had the greater contribution in the multivariate model.
CONCLUSION
In addition to melanocyte hyperactivity, melasma skin exhibits alterations in the epidermal barrier, upper dermis and BMZ, which differ from the adjacent sun-exposed skin and retroauricular skin, indicating a distinct phenotype, rather than a mere extension of photoageing or intrinsic ageing. Mast cells appear to play a central role in the physiopathology of melasma.
10.Immunohistochemical profile, disease-free survival, and pattern of recurrence among non-metastatic breast cancer patients of the Philippine General Hospital during the first 5 years of implementation of the Department of Health-Breast Cancer Medicine Access Program.

Background: Breast cancer is the third leading cause of cancer mortality in the Philippines. The Department of Health (DOH) started the Breast Cancer Medicine Access Program (BCMAP) in 2011, providing chemotherapeutic and hormonal drugs for Stage I-IIIB Breast Cancer patients.
Objectives: This study determined the 5-year disease-free survival and patterns of recurrence of patients enrolled in the Breast Cancer Medicine Access Program (BCMAP) of the Philippine General Hospital.
Methodology: This is a retrospective cohort study of patients enrolled in BCMAP from January 2012 to December 2016. Kaplan-Meier survival analysis was used to determine the disease-free survival. Cox-Mantel Log Rank Test and Cox Proportional Hazards were used to determine factors that influenced survival.
Results and Conclusion: Of the 1,680 patients enrolled in the study period, 231 did not finish their treatment. The most common molecular subtype was Luminal A, and majority had High Risk St. Gallen Category. The most common site of recurrence was the bone. Only 612 patients were included in the analysis of survival due to incomplete data. Median disease-free survival had not yet been reached, but those who did have recurrence, did so in a median time of 17 months. Survival was found to be significantly influenced by co-morbidities, lymphovascular invasion, ER and PR statuses, and molecular subtypes. Even though a lot of patients benefitted from the BCMAP, lacking data and a significant number of patients lost to follow-up limited the analysis of outcomes. Complete data collection and stronger follow-up is recommended.
